Intravesical foreign body and vesicovaginal fistula: a rare complication of a neglected pessary.

A silicone Gellhorn pessary, 3 inches in diameter, neglected for several years in an elderly woman, eroded through the anterior vaginal wall, ultimately to lie entirely within the bladder. General anesthesia and bilateral deep full-length Schuchardt's incisions were required to remove it. A Latzko procedure was done at a later date to close the large vesicovaginal fistula; similar Schuhardt's incisions were again used.

Cervical Pessary Plus Progesterone for Twin Pregnancy with Short Cervix Compared to Unselected and Non-Treated Twin Pregnancy: A Historical Equivalence Cohort Study (EPM Twin Pessary Study).

OBJECTIVE: The present study aims to determine if the use of cervical pessary plus progesterone in short-cervix (≤ 25 mm) dichorionic-diamniotic (DC-DA) twin pregnancies is equivalent to the rate of preterm births (PBs) with no intervention in unselected DC-DA twin pregnancies. METHODS: A historical cohort study was performed between 2010 and 2018, including a total of 57 pregnant women with DC-DA twin pregnancies. The women admitted from 2010 to 2012 (n = 32) received no treatment, and were not selected by cervical length (Non-Treated group, NTG), whereas those admitted from 2013 to 2018 (n

Development of progesterone electrospun nanofibers to coat Arabin pessaries as a dual preventive and therapeutic approach for preterm labor.

Preterm labor is a growing health problem that causes newborn death, and safe and effective therapy is significantly needed. Arabin pessaries and progesterone are preventive and therapeutic approaches that can be applied to managing the short cervix; hence, reducing the risk of preterm labor. The main goal of current work is to fabricate a novel nanofiber formulation based on polycaprolactone (PCL) and loaded with progesterone to coat for Arabin pessaries to be used as dual preventive and therapeutic approaches for local vaginal delivery. Several important criteria were considered in this stud

Analysis of questions by patients with pelvic organ prolapse about vaginal pessaries resulting in a checklist for their physicians.

OBJECTIVE: International guidelines recommend vaginal pessaries as a first-choice treatment of symptomatic pelvic organ prolapse (POP). Gynecologists rarely receive systematic training or just do not take the time to communicate with their patients. We hypothesized that we could identify key deficits and limitations of counseling before or during pessary therapy from questions directed to manufacturers with the aim to improve and promote health literacy of women with POP. METHODS: We approached five manufacturers to provide anonymized inquiries related to pessary use. After exclusion of dupli

Differences in pessary support between successful and unsuccessful fit: an analysis in upright position.

BACKGROUND: Pessary treatment is a popular option for women with pelvic organ prolapse; however, success rates are limited, mostly due to dislodgement of the pessary. In literature, several support mechanisms are proposed such as support by the levator ani muscle and the uterus which acts as a lever to keep the pessary in place. To improve success rates of pessary therapy, the support mechanism should be further investigated. OBJECTIVE: The aim of this study is to identify ring-shaped pessary support mechanisms by comparing upright magnetic resonance imaging data of successful and unsuccessfu

Factors contributing to long-term vaginal pessary use: A mixed-methods study.

OBJECTIVE: One of the treatment options in pelvic organ prolapse (POP) is vaginal pessary therapy. For a well-informed decision, different aspects of the treatment option, including sustainable long-term outcomes, need to be discussed with the patient. However, long-term quality of life and psychological wellbeing of pessary use is unknown. This study explores the factors contributing to long-term pessary use. METHODS: A mixed-methods study was performed among patients with vaginal pessary use for at least 5 years. The quantitative part consisted of online, validated questionnaires on satisfa

A rare case report - Urosepsis as a result of a neglected and forgotten pessary for 10 years.

A pessary is a silicone ring placed on the vagina that can improve urinary incontinence. This study reports an 83-year-old patient with a pessary discovered inside her vagina after a prior urinary incontinence treatment. The prolonged presence of the pessary in the patient's body led to a complication called urosepsis. The presence of the pessary inside the patient's vagina for an extended period was not detected by the doctor who performed the gynecological checks, nor by the patient who had forgotten about it. The pessary remained inside her for 10 years until it was discovered accidently du

Prediction of Preterm Birth with the Uterocervical Angle in Singleton Pregnancies Treated with a Cervical Pessary.

BACKGROUND: Cervical pessaries are an established therapy option for patients at risk for spontaneous preterm birth (sPTB). The uterocervical angle (UCA) remains a promising sPTB predictor. However, its clinical significance has yet to be described in patients treated with a pessary. METHODS: This study analyzed data of patients treated with a pessary because of cervical shortening in singleton pregnancies. The patients were divided into 2 groups according to transvaginal ultrasound findings: the funneling group (n = 68) and the no-funneling group (n = 42). Moreover, we analyzed patients with
