Bocca's Technique Hemipharyngo-Total-Laryngectomy for Right Pyriform Sinus Squamous Cell Carcinoma.

This report describes the step-by-step surgical technique of Bocca's hemipharyngo-total laryngectomy for right pyriform sinus squamous cell carcinoma in an elderly patient with significant comorbidities. The procedure allowed oncologically radical resection with immediate neohypopharyngeal reconstruction using the preserved hemilaryngeal mucosa, avoiding flap reconstruction. Bocca's technique represents a valuable option in carefully selected cases, providing adequate functional outcomes while reducing operative complexity.

Jejunum free flap for head and neck reconstruction, a step-by-step surgical technique videos.

Circular pharyngolaryngectomy for oncologic resection requires a tubular reconstruction. Different options can be proposed to the patient: digestive free flap, fasciocutaneous flap, or musculocutaneous flap. The jejunum free flap is a tubular flap commonly used in esophageal and pharyngeal reconstruction with good functional outcomes and an acceptable rate of complications. Reconstruction with a jejunum free flap is an ideal choice. Patients at Gustave Roussy Institute (Villejuif, France) were offered a jejunum flap free flap for all circular pharyngolaryngectomies. The surgical technique is e

The ideal flap for reconstruction of circumferential pharyngeal defects: A systematic review and meta-analysis of surgical outcomes.

BACKGROUND: There is a lack of consensus regarding the superiority of the common free flaps for the reconstruction of circumferential pharyngeal defects. METHODS: A systematic literature search was conducted to identify studies reporting the complications of circumferential pharyngeal reconstruction between 2005 and 2020. Anterolateral thigh free flaps (ALTFF), jejunal free flaps (JFF), and radial forearm free flaps (RFFF) were compared. Various complications were compared by meta-analysis. Primary endpoints were fistula and stricture rates. RESULTS: Forty studies were included (2230 patient

Assessment of swallowing function after circumferential pharyngolaryngectomy. A multicenter study by the GETTEC group.

OBJECTIVE: Circumferential pharyngolaryngectomy is performed for advanced pharyngeal tumor or in a context of postradiation recurrence. Several free or pedicle flaps have been described for pharyngeal defect reconstruction, with choice at the surgeon's discretion. The aim of this study was to evaluate long-term swallowing function according to the type of flap used for reconstruction. MATERIAL AND METHOD: A multicenter retrospective study was conducted from January to September 2016 within the French GETTEC head and neck tumor study group. All patients in remission after circumferential phary

Transoral robotic resection of recurrent nasopharyngeal carcinoma.

Transoral robotic nasopharyngectomy was carried out for a patient who developed recurrent nasopharyngeal cancer after radiotherapy. The tumor in the left lateral wall of the nasopharynx was exposed after splitting the soft palate and a curative resection, including the medial crus of the eustachian tube was carried out under the three-dimensional 0° camera. The operative procedure was uncomplicated, blood loss was minimal and the patient recovered smoothly. The camera of the surgical robot provided superb visualization of the operative field and the maneuverability of the robotic arms with the

Current treatment options for recurrent nasopharyngeal cancer.

Loco-regional control rate of nasopharyngeal carcinoma (NPC) has improved significantly in the past decade. However, local recurrence still represents a major cause of mortality and morbidity in advanced stages, and management of local failure remains a challenging issue in NPC. The best salvage treatment for local recurrent NPC remains to be determined. The options include brachytherapy, external radiotherapy, stereotactic radiosurgery, and nasopharyngectomy, either alone or in different combinations. In this article we will discuss the different options for salvage of locally recurrent NPC.

[Surgery of the parapharyngeal adenomas].

Pleomorphic adenomas are the most common tumors of the parapharyngeal space (PPS) and often grow to an enormous size before becoming visible or symptomatic. CT and MRI scans are important in both diagnosis and surgical treatment. There are basically four different surgical approaches (transoral-transpalatal, transcervical-submandibular, transparotid und transmandibular) to the anterior PPS. Wide and direct exposure of the PPS is provided by a midline mandibulotomy with transection of the floor of the mouth. Owing to its morbidity this approach is reserved exclusively for extended tumors. The t

Partial laryngopharyngectomy for piriform sinus carcinoma. Technique and preliminary results.

This paper presents a technique for partial laryngopharyngectomy followed by a one-stage reconstruction and its preliminary results. This surgery is indicated for carefully selected cases in which the lesion is confined to the ipsilateral piriform sinus, aryepiglottic fold, arytenoid eminence and paraglottic space at the level of the false fold. The hyoid bone, thyroid ala, arytenoid cartilage, epiglottis, aryepiglottic fold, arytenoid eminence and false fold are removed on the affected side. Reconstruction is performed with the use of a pectoralis major myocutaneous (PMMC) flap. The surgery w

Avoiding stenosis in the tubed greater pectoral flap in pharyngeal repair.

Replacement of circumferential defects of the pharynx with skin-lined flaps rolled into a tube had the problem of anastomotic stricture formation. To prevent and correct stenosis at the mucocutaneous junction, an interdigitating anastomotic line produced by making vertical slits on the skin edge and the esophageal stump edge is used. It allows for maintenance of the lumen despite suture line contraction.

Paratracheal lymph node involvement in advanced cancer of the larynx, hypopharynx, and cervical esophagus.

OBJECTIVES/HYPOTHESIS: The presence of nodal metastatic disease in head and neck cancer is the foremost prognostic factor. Although neck dissection is the surgical gold standard for the treatment of cervical lymphatic spread, the paratracheal nodal group is not routinely included in the dissection. The study determined the nodal yield, presence of metastases, and prognostic importance of paratracheal nodes in patients with advanced carcinoma of the upper aerodigestive tract. STUDY DESIGN: Prospective histological and survival analysis. METHODS: Over a 4-year period (October 1994-June 1998),

Neck dissection with simultaneous carotid endarterectomy.

BACKGROUND: Patients with metastatic neck disease from upper aerodigestive tract carcinomas have an extensive history of tobacco and alcohol abuse. These patients are predisposed to develop atherosclerotic vascular disease. OBJECTIVE: An increased incidence and severity of carotid stenosis in patients receiving radiotherapy for head and neck cancers is known. Management of patients with severe carotid stenosis who require surgical treatment of their neck disease has not been described. The authors describe our experience with simultaneous carotid endarterectomy and neck dissection. STUDY DES

Analysis of treatment results of hypopharyngeal cancer.

Sixty-three patients with squamous cell carcinoma of the hypopharynx were treated at the University of Tokyo between 1985 and 1993. Twelve patients were treated with surgery alone, 16 with preoperative irradiation, 26 with postoperative irradiation, and nine with irradiation alone. A tumor dose of 50 Gy (25 fractions/5 weeks) was used in postoperative irradiation, 40-60 Gy in preoperative irradiation, and 70 Gy in irradiation alone. When "number of indications," including, for example, partial pharyngectomy, positive or close (< or = 5 mm) margin, and extracapsular extension, was used for mult

Durable Complete Response Following Nivolumab Discontinuation in Human Papillomavirus (HPV)-Positive Oropharyngeal Carcinoma Complicated by Bullous Pemphigoid: A Case Report.

Immune checkpoint inhibitors have improved outcomes in recurrent/metastatic head and neck squamous cell carcinoma (HNSCC), although durable complete responses (CRs) remain uncommon. The optimal duration of immunotherapy is not yet established. We report the case of a 69-year-old female smoker diagnosed in November 2015 with human papillomavirus (HPV)16-positive squamous cell carcinoma of the oropharynx (base of tongue), staged cT2N2bM0. Following induction chemotherapy with docetaxel, cisplatin, and 5-fluorouracil, and concurrent chemoradiotherapy with cisplatin, persistent disease was confirm

Oncological outcome after free jejunal flap reconstruction for carcinoma of the hypopharynx.

It has been a common practice among the oncologist to reduce the dosage of adjuvant radiotherapy for patients after free jejunal flap reconstruction. The current aims to study potential risk of radiation to the visceral flap and the subsequent oncological outcome. Between 1996 and 2010, consecutive patients with carcinoma of the hypopharynx requiring laryngectomy, circumferential pharyngectomy and post-operative irradiation were recruited. Ninety-six patients were recruited. TNM tumor staging at presentation was: stage II (40.6%), stage III (34.4%) and stage IV (25.0%). Median follow-up period

Flap selection for circumferential pharyngeal reconstruction.

PURPOSE OF REVIEW: Six systematic reviews and meta-analyses (2019-2025) have compared reconstructive options for circumferential pharyngeal defects following total laryngopharyngectomy. This narrative review synthesises their findings, examines where they conflict and why their conclusions diverge, and weighs factors that fall below systematic-review level but affect flap selection. RECENT FINDINGS: Free jejunal flap (FJF) demonstrates the lowest fistula rate (8-11%); in the largest network meta-analysis (1357 patients), FJF had a 93% probability of ranking first for fistula avoidance.
