Impact of decision support in electronic medical records on lipid management in primary care.

Electronic decision-support tools may help to improve management of hyperlipidemia and other chronic diseases. This study examined the impact of lipid management tools integrated into an electronic medical record (EMR) in primary care practices. This randomized controlled trial was conducted in a national network of physicians who use an outpatient EMR. Adult primary care physicians were randomized by office to receive an electronic form that was embedded in the EMR. The form contained prompts regarding suboptimal care based on Adult Treatment Panel-III (ATP-III) guidelines, as well as reporti

Influence of daily activity on changes in physical fitness for people with post-stroke hemiplegia.

To investigate the influence of daily activity on changes in the physical fitness of people with post-stroke (cerebrovascular disorders) hemiplegia, we evaluated the follow-up exercise load test of 30 ambulatory male patients with post-stroke hemiplegia. Between the times of the two tests, patients had no special supervised training. They were advised by their physicians to exercise according to the result of an exercise-loading test. We determined peak oxygen uptake and O2 consumption at the ventilatory threshold point. After 9.4 months, the mean peak oxygen uptake improved significantly from

The role of staging in urologic cancer. Physical assessment and risk factors.

Patients with urologic cancer may have improved survival if their diagnosis is made early. This task begins in the primary care physician's office through patient education on prevention and early detection procedures. A thorough history which provides for risk factor evaluation including life-style and environmental factors, a careful family history, past personal history, and finally, symptoms of current and past illness is the keystone of prevention and early diagnosis. The general physical examination as well as a specific examination of the scrotum and its contents, the penis, the prostat

An educational approach to improving physician breast cancer screening practices and counseling skills.

A multifaceted, individualized, physician education program designed to increase the breast cancer screening practices of community-based primary care physicians is described and the results are evaluated. Community-based surveys identified primary care providers with breast cancer screening educational needs who were assigned, using a factorial design, to an intervention or control condition. The sample included 154 control and 128 intervention physicians. The intervention consisted of a 1-2h in-office training program and/or self-study workbook. Self-reported overall breast cancer screening

Wolf-Parkinson-White alternans diagnosis unveiled by adenosine stress test.

The case of a 41-year-old woman who presented to her primary care physician with atypical chest pain was reported. An electrocardiogram (ECG) was performed in his office and the patient was told she had left bundle-branch block and an old infarct. The patient was very concerned and referred to cardiology for further evaluation/testing. An ECG at the cardiologist's office was normal. The cardiologist however suspected the ECG performed at the primary care physician office to be preexcitation (Wolf-Parkinson-White). During an adenosine nuclear stress test, intermittent preexcited beats occurred

Torture and war trauma survivors in primary care practice.

Close to 1 million refugees from around the world have entered the United States, fleeing repression, war, terrorism, and disease. It has been estimated that among these are thousands who have experienced torture. Many refugees and immigrants will appear in the offices of health care professionals with symptoms that may be related either directly or indirectly to torture. Both physical and psychological torture may result in long-term sequelae. Physical effects may be found in every organ system, but psychological effects are most commonly manifest in the symptoms of the post-traumatic stress

Promoting gynecologic cancer awareness at a critical juncture--where women and providers meet.

Given the absence of effective population-based screening tests for ovarian, uterine, vaginal, and vulvar cancers, early detection can depend on women and health care providers recognizing the potential significance of symptoms. In 2008, the Centers for Disease Control and Prevention's (CDC) Inside Knowledge campaign began distributing consumer education materials promoting awareness of gynecologic cancer symptoms. We investigated providers' in-office use of CDC gynecologic cancer materials and their recognition of the symptoms highlighted in the materials. We analyzed data from a national 201

Comparison of Automated Office Blood Pressure With Office and Out-Off-Office Measurement Techniques.

Automated office blood pressure (AOBP) has emerged as a valuable tool to assess patient's BP status, but the lack of strong evidence to establish a threshold value for hypertension diagnosis limits its use in clinical practice. We aimed at synthesizing the published literature through a meta-analysis of studies comparing AOBP with other BP measurement techniques and at analyzing the differences between AOBP and physician's office BP, nonphysician's office BP, daytime ambulatory BP monitoring, and home BP monitoring. We searched PubMed database for articles published up to April 2018; eligible

Rapid testing for streptococcal pharyngitis.

Within the past 2 years, new technology has become available to office laboratories that permits a 10-minute laboratory diagnosis of a group A streptococcal infection in patients who present with sore throats. The availability of the test result while the patient is in the office can be a great improvement compared with waiting 1 to 2 days for a throat culture report. In addition, these rapid tests may be easier for office staff to read than traditional agar plates. A physician who incorporates rapid testing for streptococcal infection in the office laboratory should perform appropriate qualit

The HITECH Act and electronic health records' limitation in coordinating care for children with complex chronic conditions.

While the HITECH Act was implemented to promote the use of electronic health records to improve the quality and coordination of healthcare, the limitations established to the setting of the hospital or physician's office affect the care coordination for those who utilize many health-related services outside these settings, including children with complex and chronic conditions. Incentive-based support or nationally supported electronic health record systems for allied and other healthcare professionals are necessary to see the full impact that electronic health records can have on care coordin

Drug prescribing for ambulatory patients 85 years of age and older.

Drug prescribing for ambulatory patients greater than or equal to 85 years of age was studied using data from the 1980 National Ambulatory Medical Care Survey (NAMCS) on office visits to physicians. NAMCS is a survey by the US National Center for Health Statistics, which collected information on office visits and extrapolated the results to the US population. Of the 575.7 million office visits by all ages, 6.8 million (1 per cent) were by persons greater than or equal to 85 years of age, and 64 per cent involved females. Ninety per cent of the total office visits of those greater than or equal
