Cutaneous metastasis of ovarian carcinoma with shadow cells mimicking a primary pilomatrical neoplasm.

Shadow cells are characteristic of pilomatricoma, although they have been described in other cutaneous and visceral neoplasms, particularly endometrioid adenocarcinomas of the female genital tract. We describe a metastasis of an ovarian endometrioid adenocarcinoma with shadow cells to the skin that was initially misinterpreted as a pilomatricoma. We compare the histology of the ovarian neoplasm to 21 pilomatricomas. This is the first reported case of a cutaneous metastasis of a visceral neoplasm mimicking a primary pilomatrical neoplasm.

Diagnostic imaging of benign and malignant neck masses in children-a pictorial review.

Neck masses are frequently encountered in pediatric medicine, and can present a diagnostic dilemma for the clinicians involved. There are several means by which neck masses in children can be subdivided, for example by age at presentation, anatomical location including compartments and fascia of the neck, their classical appearance when imaged, or by etiology. When imaging children the clinicians must be mindful of radiation exposure and as such ultrasound (US) is often attempted first. Cross sectional imaging can be helpful for problem solving with CT being particularly useful for assessing t

Basal Cell Carcinoma With Matrical Differentiation: A Case Report and Literature Review.

Basal cell carcinoma (BCC) with matrical differentiation is an extremely rare subtype of basal cell carcinoma. We present an example of BCC with matrical differentiation and review the relevant literature. A 75-year-old man presented with a rapidly enlarging nodule on his forehead, with the clinical diagnosis of squamous cell carcinoma. Given atypical matrical proliferation on initial biopsy, there was concern for pilomatrix carcinoma. However, a repeat biopsy confirmed a diagnosis of BCC with matrical differentiation. Immunohistochemically, the BCC lobules expressed BCL2 and BER-EP4, while th

Neurosis and true dermatosis: a case of ossified pilomatricoma developing within a self-inflicted ulcer.

Clinicians have a tendency to dismiss patients with psychiatric illness like skin picking disorder without assessing completely for organic disease. Patients with psychocutaneous disease have the potential to develop true dermatopathology and should always be examined thoroughly. We describe a case of skin picking disorder with underlying pilomatricoma. The patient met criteria for skin picking disorder and had been prescribed numerous topical treatments without efficacy by countless physicians over 18 years. In addition, a pilomatricoma was discovered within a self-inflicted ulcer. Pilomatric

Multiple Pilomatricomas: A Retrospective Study and Literature Review.

Pilomatricoma is usually a single lesion. However, some patients present with more than one, a fact that has been reported in association with several systemic diseases. The aim of this analysis is to determine the prevalence of multiple pilomatricomas (MP) in our institution as well as to describe their clinical features and associated comorbidities. This is a retrospective analysis reviewing patients with a histological diagnosis of pilomatricoma registered in the database of the Pathology Department of our institution, from January 2005 to March 2017. One hundred ninety-five patients with h

Pilomatricoma with a bullous appearance.

A 15-year-old Japanese girl had an asymptomatic nodule on the right thigh of seven months' duration. The clinical appearance was similar to that of a bulla. There was a history of blunt trauma from dog scratch to the skin over the tumor shortly before tumor growth. Histopathological findings were consistent with pilomatricoma. In the overlying dermis, the collagen bundles were compressed to the tissue surrounding the tumor and the large space was seen. Around the tumor, some dilated endothelium-lined vascular channels were found, which were identified as lymphatic vessels.

CTNNB1-Mutant High-Grade Endometrial Carcinoma: Clinical Outcome and Prevalence of Pilomatrix-Like and Corded Hyalinized Morphologies in an Institutional Case-Control Series.

The occurrence of somatic CTNNB1 pathogenic mutations has been recently recognized in high-grade endometrial carcinoma (HG-EMC) in association with pilomatrix-like (PM-L) morphology and an aggressive clinical course, highlighting the need for further study on the biologic and clinical importance of CTNNB1 alterations in HG-EMC. Thus, we document the clinical and pathologic characteristics of an institutional cohort of HG-EMC with and without CTNNB1 alterations. Of all the HG-EMCs subjected to next-generation sequencing in a 10-year period, those with pathogenic CTNNB1 alterations were selected

Pilomatricoma in childhood: a retrospective study from three European paediatric centres.

UNLABELLED: Pilomatricoma is characterised as a common, slowly growing benign cutaneous tumour that appears generally within the first decades of life. The clinical diagnosis is frequently missed, especially by the paediatrician unfamiliar with these tumours. We present the experience gained in three European tertiary care paediatric centres with the treatment of pilomatricoma and also current data on the aetiology, clinical presentation and management. A retrospective study was carried out in 83 patients suspected for pilomatricoma during a 7-year period (1996-2002) at the departments of Paed

Circular Excision and Purse-String Closure for Pediatric Facial Skin Lesions.

Standard resection of pediatric facial skin lesions consists of lenticular excision and linear closure. This one-stage procedure for circular lesions results in a linear scar 3 times longer than the diameter of the removed specimen. Circular excision and purse-string closure has been described for infantile hemangiomas to reduce the length of scar. The purpose of this study was to analyze the application of this technique for any type of circular facial skin lesion in the pediatric population. Records of consecutive pediatric patients with facial skin lesions treated with circular excision and

Basement membrane laminin and type IV collagen in various benign and malignant adnexal tumors of the skin: an immunohistochemical study.

Thirty benign and seven malignant adnexal tumors of the skin and one lymph node metastasis were stained for laminin and type IV collagen with rabbit antibodies against the human basement membrane (BM) proteins using the immunoperoxidase technique. Fifteen of the benign sweat gland, sebaceous gland, and hair follicle tumors showed a continuous and distinct BM around the tumor aggregates. The cylindromas and eccrine spiradenomas seemed to produce excessive amounts of BM material, part of which was seen as amorphic patches within the tumor cell clusters, whereas the trichofolliculomas, trichoepit

Pilomatrix Carcinoma in a Hodgkin Lymphoma Survivor: A Rare Second Primary Tumor.

Pilomatrix carcinoma is a rare, locally aggressive malignant adnexal tumor derived from hair follicle matrix cells. Although secondary malignancies are a well-known concern in Hodgkin lymphoma (HL) survivors, the occurrence of pilomatrix carcinoma in this setting is infrequent. We present the case of a 62-year-old man who was treated for Stage 1 Nodular Sclerosing Classical HL in 2018 with ABVD chemotherapy and radiotherapy. In January 2024, while in remission, he presented with a mass on the right posterior crus, distinct from the previous radiation field. Excision and histopathologic examina

What is the clinical utility of histological diagnosis between pilomatricoma and trichilemmal cyst in children?

Pilomatricoma and trichilemmal cyst are two benign tumors of hair follicles that can affect children. We performed a retrospective study to histologically differentiate the two tumors to determine if this has clinical utility. For 5 years, we conducted a retrospective study on the patients who underwent surgery and evaluated some parameters (site, recurrence and scar outcome) categorizing them based on their histological type. We treated 71 patients, with an average age of 7.6 years. Most of the lesions were localized on the scalp, head and neck. Histological examination diagnosed 55 pilomatri

Pilomatrix carcinoma with lymph node and pulmonary metastasis: report of a case arising on the knee.

A 58-year-old woman presented with a pilomatrix carcinoma on the right knee. The tumor developed at the site of a previous lesion that had been present since she was 30 years old. Histologic study showed the presence of basaloid cells with numerous atypical mitoses, shadow cells, and calcification. After several surgical excisions, the tumor mass infiltrated the subcutaneous tissue, muscle, and bone, resulting in inguinal lymph node and pulmonary metastasis. We also review the literature and comment on the histopathologic differences from other cutaneous tumors.

Bilobed flap in the reconstruction of inferior and/or lateral periorbital defects.

PURPOSE: To report the outcome of bilobed flap (BF) reconstruction of inferior and/or lateral periorbital defects following tumor excision. METHODS: Records of 20 patients who underwent inferior and/or lateral periorbital reconstruction with the BF were reviewed. The surgery was performed with local anesthesia in 18 patients and with general anesthesia in 2. Malignant tumors were excised together with a 4- to 5-mm margin of surrounding skin. Superolaterally based BFs were created from the malar or zygomatic region to cover the defects. RESULTS: Patients ranged in age from 48 to 86 years (mea

Does endometrial morular metaplasia represent odontogenic differentiation?

The nature of endometrial morular metaplasia (MorM) is still unknown. The nuclear β-catenin accumulation and the not rare ghost cell keratinization suggest a similarity with hard keratin-producing odontogenic and hair matrix tumors rather than with squamous differentiation. We aimed to compare MorM to hard keratin-producing tumors. Forty-one hard keratin-producing tumors, including 26 hair matrix tumors (20 pilomatrixomas and 6 pilomatrix carcinomas) and 15 odontogenic tumors (adamantinomatous craniopharyngiomas), were compared to 15 endometrioid carcinomas with MorM with or without squamous/k

Multimodal Ultrasound Evaluation of Bullous Pilomatricoma: A Subcutaneous Mass With Progressive Growth in the Upper Arm.

Bullous pilomatricoma (BP), a rare variant of pilomatricoma, accounts for approximately 2% of pilomatricoma cases. We present the case of a 19-year-old male with a painless, blister-like lesion on the left upper arm, diagnosed as BP based on histopathological examination. This report emphasizes the multimodal ultrasound features of BP, incorporating grayscale ultrasound, color Doppler flow imaging (CDFI), and shear wave elastography (SWE). Notably, to our knowledge, this is the first case to detail the SWE characteristics of BP, demonstrating a mean stiffness value of 52.0 kPa. Our findings in

Matrical Tumors: A Comparative Molecular Analysis of Six Cases With Histological Correlations.

Pilomatrical tumors include pilomatricoma, melanocytic matricoma, and pilomatrical carcinoma. Similar to the normal anagen hair follicle bulb, they may be associated with benign and, rarely, with atypical pigmented dendritic melanocytes. It has been recently suggested that the term "melanocytic matricoma" be replaced with "pilomatricoma with melanocytic hyperplasia" (PMMH). Occasional cases of PMMH show intermediate grades of histological atypia. Their clinical behavior is uncertain. Interestingly, we found in one of these atypical tumors a molecular characterization compatible with a malignan
