Functional improvement with digital prosthesis use after multiple digit amputations.

PURPOSE: Patients who sustain traumatic amputation of multiple fingers suffer both a functional and psychologic loss. Previous studies of prosthesis use for finger amputees have focused primarily on the psychologic benefits. Clinically our group noticed a functional improvement on hand function tests when patients with multiple digit amputations used a prosthesis. Given the expense of multiple finger prostheses we sought to determine if they led to a consistent functional improvement in these patients. METHODS: Ten consecutive patients performed a battery of hand function tests and rated thei

Functional and radiological outcomes of Bennett's fractures treated by Iselin's technique: About 29 cases with 6-year (2-14) follow-up.

INTRODUCTION: The percutaneous pinning method described by Iselin is one of the techniques used for treating Bennett fractures at the base of the thumb metacarpal. There is little published data on the medium- and long-term outcomes of this treatment, with most studies having a mean follow-up of 4 years. The primary objective of this study was to evaluate the functional and radiological outcomes of percutaneous pinning by Iselin's method with a mean follow-up of 6 years. The secondary objective was to look for evidence of radiographic carpometacarpal (CMC) arthritis. HYPOTHESES: 1) The functi

The Use of Stromal Vascular Fraction, Platelet-rich Plasma, and Stem Cells in the Treatment of Thumb Carpometacarpal Osteoarthritis.

BACKGROUND: Thumb function is integral to hand movement and overall hand function. Impairment, often caused by carpometacarpal (CMC) arthritis, reduces the quality of life. Here, we explored a novel approach using a mixture of the stromal vascular fraction, adipose-derived stem cells, and platelet-rich plasma to treat symptomatic trapeziometacarpal osteoarthritis. METHODS: Retrospective data from 30 hands of 19 patients classified as Eaton and Littler stages 1-4 were analyzed. Platelet-rich plasma and fat were collected under sterile conditions, followed by centrifugation, stem cell extractio

Long-term outcomes of proximal row carpectomy in patients younger than 45 years: A minimum 10-year follow-up study.

PURPOSE: Proximal row carpectomy is a well-established surgical procedure for treating various wrist pathologies that is frequently performed in patients over 45 years of age owing to its reliability and low complication rate. However, its effectiveness in younger patients with high functional demands remains debated, given the potential for subsequent failure. We aimed to assess the long-term outcomes of proximal row carpectomy in a population <45 years old, with an average follow-up of 15.55 years. METHODS: A retrospective study was conducted on 36 patients who underwent proximal row carpec

Anatomic Reconstruction of the Anterior Oblique and the Dorsoradial Ligaments for Painful Subluxating Carpometacarpal Joint of the Thumb.

Thumb carpometacarpal osteoarthritis (CMC OA) is a common pathology of the hand that is characterized by pain, loss of grip and pinch strength, and deformity. Although conservative management is often preferred in earlier stage of CMC OA, surgical techniques can be used when symptoms are not fully relieved, especially with subluxation. We report a case series of 26 patients (32 operations) with Eaton stage I and II CMC OA who underwent a novel surgical technique that anatomically restores the CMC joint with autologous double ligament reconstruction. All cases were retrospectively reviewed as a

Optimizing hand-function patient outcome measures for inclusion body myositis.

Inclusion body myositis is the most commonly acquired myopathy after the age of 45. The slowly progressive and heterogeneous disorder is a challenge for measuring clinical trial efficacy. One current method for measuring progression utilizes the Inclusion Body Myositis-Functional Rating Scale. We have found that the upper extremity domain scores in the Inclusion Body Myositis-Functional Rating Scale do not consistently change until there is extreme loss of grip and finger flexor strength. Therefore, we performed a cross-sectional observational study of 83 inclusion body myositis patients and 3

Great Toe Transplantation.

Despite being relatively uncommon in the general population, thumb amputations cause severe disability. More than 3,300 thumb amputations occurred in the United States. The thumb makes up around 40% of the function of the hand. Therefore, losing it would result in significant medical, hospital, and societal costs. Thumb reconstruction surgery's primary goal is to restore grip strength, including the range of motion, fine and tripod pinch, power grasp, strength, and sensibility, while secondary goals include restoring hand aesthetics. In cases of thumb replantation, like-for-like replacement is
