Monomicrobial Klebsiella pneumoniae Necrotizing Fasciitis With Liver Abscess: A Case Report and Literature Review.

BACKGROUND: Necrotizing fasciitis is one of the most life-threatening soft tissue infections and usually follows a major or minor trauma. Published reports regarding single microorganism liver abscess caused by Klebsiella pneumoniae in Taiwan indicate a septic metastasis rate about 12%. Although a hematogenous origin of necrotizing fasciitis from a liver abscess is rare in Taiwan, it remains possible in our clinical practice. CASE PRESENTATION: A 75-year-old man with a history of diabetes and liver abscess had a status of postdrainage 5 years earlier. The patient presented with elevated body

A Case of Bronchopleural Fistula and Hydropneumothorax in a Patient With Necrotizing Pneumonia Complicated by Mycobacterium avium Complex.

A bronchopulmonary fistula is a pathological connection that develops between the bronchi and the pleural cavity as a result of etiologies including surgery, infection, blunt or penetrating trauma, radiation, chemotherapy, and chronic obstructive pulmonary disease sequela. Diagnosis and treatment are challenging for intensivists. We present a case report of bronchopulmonary fistula resulting in hydropneumothorax caused by necrotizing pneumonia and complicated by mycobacterium avium complex that resolved spontaneously. The aim of this case report is to discuss the presentation and treatment of

Necrotizing fasciitis due to penicillin-resistant Streptococcus pneumoniae: case report and review of the literature.

Necrotizing fasciitis (NF) is a life-threatening infection involving rapid necrosis of subcutaneous and fascial tissues. Streptococcus pneumoniae (SPN) soft tissue infection is exceedingly uncommon, reported primarily in patients with immunosuppression or other underlying conditions. We report a case of NF and septic shock in a healthy 32-year-old man, whose only predisposing factor was antecedent blunt trauma. Pathological examination and culture of the extensive tissue debridement were positive only for SPN. The serotype 9V isolate was penicillin (PCN)-resistant (MIC=2.0), and closely-relate

Safety and long-term outcomes of surgical intervention in pediatrics with necrotizing pneumonia: a retrospective cohort study.

UNLABELLED: This study aimed to analyze the evolving etiology, clinical features, surgical outcomes, and long-term prognosis of pediatric necrotizing pneumonia (NP). Data were collected from 398 children diagnosed with NP between January 2017 and June 2025, among whom 42 (10.55%) required surgical intervention due to failed medical therapy. Pathogens were identified in 95.2% of cases, with mycoplasma pneumoniae being the most common (52.50%). Preoperative complications were present in 83.33% of patients. The mean operative time was 140.71 ± 49.84 min, intraoperative blood loss was 119.79 ± 49.

Fatal Influenza B-MRSA Coinfection in a Healthy Adolescent: Necrotizing Pneumonia, Cytokine Storm, and Multi-Organ Failure.

BACKGROUND: Influenza B usually causes mild illness in children. Severe and fatal cases can occur when complicated by secondary Staphylococcus aureus (S. aureus) pneumonia, including community-acquired methicillin-resistant Staphylococcus aureus (MRSA). We present a rare, rapidly progressive fatal case in an adolescent with no known medical history to highlight diagnostic and therapeutic pitfalls. CASE PRESENTATION: A 16-year-old boy with no known underlying conditions (unvaccinated for influenza) presented critically ill at "Sf. Ioan" Clinical Emergency Pediatric Hospital in Galați after one

Community Acquired Staphylococcus Aureus Necrotizing Pneumonia and Guillain Barre Syndrome: An Unusual Presentation in An Adolescent Patient.

Community-acquired Staphylococcus aureus (SA) pneumonia can present with multiple complications but has not been reported earlier to present as or lead to Guillain Barre syndrome (GBS). However, there are few case reports of GBS following SA infective endocarditis, polymyositis, and meningitis. We report an unusual presentation of GBS most probably secondary to community-acquired SA necrotizing pneumonia in a young immunocompetent adult. The clinical course, challenges in the management, and unfortunate death of the patient due to an unforeseen complication have been discussed. This report add

Inhalation of Burkholderia thailandensis results in lethal necrotizing pneumonia in mice: a surrogate model for pneumonic melioidosis.

Burkholderia thailandensis is closely related to B. pseudomallei, the causative agent of melioidosis, a lethal tropical disease. Burkholderia thailandensis is largely avirulent in humans and, unlike B. pseudomallei, does not require strict biocontainment conditions for study. Because it may be a useful research surrogate for B. pseudomallei, we developed a murine model of airborne B. thailandensis infection. In both C57BL/6 and BALB/c mice, deposition of 103 colony-forming units (CFU)/lung or less of B. thailandensis was non-lethal and infection was readily controlled. Compared with C57BL/6 mi

Subacute anterior spontaneous spinal epidural hematoma at the thoracolumbar junction: A diagnostic confounder following necrotizing staphylococcal pneumonia.

Spontaneous spinal epidural hematoma (SSEH) is a rare cause of acute spinal cord compression, accounting for less than 1% of spinal space-occupying lesions. Its ventral localization is exceptionally uncommon due to the firm adherence of the dura to the posterior longitudinal ligament. We report the case of a 61-year-old woman with type 1 diabetes and a remote history of necrotizing Staphylococcus aureus pneumonia (treated 5 months prior) who presented with a 1-week history of progressive paraparesis and marked biological inflammatory markers (Leukocytes: 19,800/mm³; Eosinophils: 6800/mm³). Necrotizing Pneumonia Caused by PVL-Negative Methicillin-Susceptible Staphylococcus aureus Complicated by Refractory Pneumothorax: A Case Report.

Methicillin-susceptible Staphylococcus aureus (MSSA) necrotizing pneumonia can progress to severe disease, particularly when Panton-Valentine leukocidin (PVL) is involved. Here we report the case of a previously healthy 56-year-old man who developed severe necrotizing pneumonia due to MSSA despite the absence of major toxin genes including PVL. The patient reported a 10-day history of low-grade fever and was admitted for bilateral pneumonia with sepsis. Despite antibiotic therapy, his condition rapidly progressed, and chest computed tomography revealed extensive ground-glass opacities and mult

Endobronchial valves for severe air leak in critically ill children with necrotizing pneumonia requiring extracorporeal membrane oxygenation.

IntroductionAir leak is a common complication of necrotizing pneumonia in critically ill children. Severe air leaks impact effective oxygenation and ventilation, oftentimes necessitating multiple thoracostomy tubes and extracorporeal support. Endobronchial valves (EBV) are a minimally invasive intervention to control air leak that may expedite de-escalation of care for critically ill children with necrotizing pneumonia.MethodsA retrospective case series was conducted on patients at the Children's Hospital of Philadelphia who were hospitalized in the pediatric intensive care unit, required extr

Emerging Biomarkers of Illness Severity: Urinary Metabolites Associated with Sepsis and Necrotizing Methicillin-Resistant Staphylococcus aureus Pneumonia.

Our objective was to illustrate the potential of metabolomics to identify novel biomarkers of illness severity in a child with fatal necrotizing pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA). We present a case report with two control groups and a metabolomics analysis: an infant with fatal MRSA pneumonia, four children with influenza pneumonia (pneumonia control group), and seven healthy children with no known infections (healthy control group). Urine samples were collected from all children. Metabolites were identified and quantified using 1 H-nuclear magnetic resonan

[Necrotizing pneumonia associated with influenza A H1N1 infection in a child with mucopolysaccharidosis type II. Case report].

Mucopolysaccharidosis type II (MPS II) is an X-linked recessive disorder characterized by a deficiency of the enzyme iduronate-2-sulfatase leading to a multisystem involvement by tissue accumulation of glycosaminoglycans heparan and dermatan sulfate. We report a case of a 9-year-old boy diagnosed with mucopolysaccharidosis type II at 4 years of age, classified as severe variant. He presented necrotizing pneumonia associated with influenza A H1N1 infection, requiring mechanical ventilation for 1 month and 10 days. This case emphasizes the importance of healthcare workers to be aware of potentia

[Three cases of necrotizing pneumonia by pseudomonas aeruginosa infection in hematological malignancy, including dead and alive cases].

Pseudomonas aeruginosa (P. aeruginosa) is a common nosocomial pathogen that often causes pneumonia, especially in immunocompromised patients including cancer bearing-hosts. In cancer patients who have great risk of gram-negative bacteria leading to fatal infection, P. aeruginosa bacteremia easily results in septicemia with shock and life-threatening complications such as vital organ failure. Among those complications, necrotizing pneumonia is an infectious disease of lung caused by P. aeruginosa characterized by rapid cavitation and progressive clinical course, which is fatal not only in cance

Prolonged extracorporeal membrane oxygenation for pediatric necrotizing pneumonia due to Streptococcus pneumonia and influenza H1N1 co-infection: how long should we wait for native lung recovery?

Most children with severe respiratory failure require extracorporeal membrane oxygenation (ECMO) for 7-10 days. However, some may need prolonged duration ECMO (> 14 days). To date, no consensus exists on how long to wait for native lung recovery. Here we report the case of a 3-year-old boy who developed severe necrotizing pneumonia requiring venovenous (VV) ECMO after 19 days of mechanical ventilation. In the first 4 weeks of his ECMO run, he showed no lung aeration, requiring total extracorporeal support. However, after we started strategies for promoting lung recovery such as daily prone pos

Necrotizing Myositis Caused by Enterococcus faecium with Concurrent Klebsiella pneumoniae Bacteremia: A Rare Case Report.

We present a rare case of necrotizing myositis predominantly caused by Enterococcus faecium in a 68-year-old healthy man, with concurrent Klebsiella pneumoniae bacteremia. The patient rapidly progressed from diarrhea and fever to septic shock, multiple organ dysfunction, and compartment syndrome of the left leg. Blood cultures grew K. pneumoniae, while deep tissue cultures identified E. faecium as the main pathogen in necrotic muscle. Emergent fasciotomy, serial debridements, and targeted antibiotics (meropenem and linezolid/vancomycin) led to full recovery. This first reported case highlights

Pandemic Influenza Infection Promotes Streptococcus pneumoniae Infiltration, Necrotic Damage, and Proteomic Remodeling in the Heart.

For over a century, it has been reported that primary influenza infection promotes the development of a lethal form of bacterial pulmonary disease. More recently, pneumonia events caused by both viruses and bacteria have been directly associated with cardiac damage. Importantly, it is not known whether viral-bacterial synergy extends to extrapulmonary organs such as the heart. Using label-free quantitative proteomics and molecular approaches, we report that primary infection with pandemic influenza A virus leads to increased Streptococcus pneumoniae translocation to the myocardium, leading to
