Comparison of universal screening in major lynch-associated tumors: a systematic review of literature.

Lynch syndrome (LS) is associated with an increased lifetime risk of several cancers including colorectal (CRC), endometrial (EC), ovarian (OC), urinary (UT) and sebaceous tumors (ST). The benefit for universal screening in CRC and EC is well known. However, this benefit in other major lynch-associated tumors is unclear. We performed a systematic review of all published articles in the MEDLINE database between 2005 to 2017 to identify studies performing universal screening for LS in unselected CRC, EC, OC, UT and ST. All cases with MSI-H (instability in two or more markers) or missing one or m

The Role of Genetic Testing in Pediatric Renal Diseases: Diagnostic, Prognostic, and Social Implications.

Pediatric renal diseases vary widely and are linked to high morbidity and mortality; hence, early diagnosis is vital. Presently, genetic testing is being incorporated into the standard of care for children and their families with kidney disease, primarily as a diagnostic tool. In the present review, we aim to collect all potential evidence from relevant studies that reported the role of genetic testing in pediatric renal disease diagnostic, prognostic, and social implications. We have conducted both electronic and manual searches within PubMed, the Cochrane Library, Web of Science, and Scopus

The sensitivity and specificity of platelet autoantibody testing in immune thrombocytopenia: a systematic review and meta-analysis of a diagnostic test.

Essentials The diagnosis of ITP is based on a platelet count < 100 × 109  L-1 and exclusion of other causes. There are no standard tests or biomarkers to diagnose ITP. The sensitivity of platelet autoantibody testing is low (53%). The specificity is high (> 90%). A positive autoantibody test can be useful to rule in ITP but a negative does not rule out ITP. SUMMARY: Background Immune thrombocytopenia (ITP) is an autoimmune disorder characterized by a low platelet count and an increased risk of bleeding. The sensitivity and specificity of platelet autoantibody tests is variable and their utilit

Accuracy of antigen tests for meningococcal meningitis in cerebrospinal fluid: A diagnostic meta-analysis.

OBJECTIVES: Neisseria meningitidis is one of the major pathogens of meningitis in children worldwide and causes invasive meningococcal disease (IMD), which is a critical illness that mainly presents as meningitis and/or septicemia in children. Identification of N. meningitidis in cerebrospinal fluid (CSF) is the gold standard for the diagnosis of meningococcal meningitis, but antigen tests have advantages such as timely results, relatively low cost, and convenience. Yet, the diagnostic accuracy of antigen tests remains uncertain. Therefore, the aim of this meta-analysis was to evaluate the dia

A novel strategy for community screening of SARS-CoV-2 (COVID-19): Sample pooling method.

The rapid global spread of the coronavirus disease (COVID-19) has inflicted significant health and socioeconomic burden on affected countries. As positive cases continued to rise in Malaysia, public health laboratories experienced an overwhelming demand for COVID-19 screening. The confirmation of positive cases of COVID-19 has solely been based on the detection of the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) using real-time reverse transcription polymerase chain reaction (qRT-PCR). In efforts to increase the cost-effectiveness and efficiency of COVID-19 screening, we evalua

Clinical Test Performance of a Rapid Point-of-Care Syphilis Treponemal Antibody Test: A Systematic Review and Meta-analysis.

We reviewed relevant syphilis diagnostic literature and conducted a meta-analysis to address the question, "What is the sensitivity and specificity of the Syphilis Health Check, a rapid qualitative test for the detection of human antibodies to Treponema pallidum." The Syphilis Health Check is the only rapid syphilis test currently cleared by the Food and Drug Administration (FDA). We conducted a systematic review and a meta-analysis using Bayesian bivariate random-effects and fixed-effect models to create pooled estimates of sensitivity and specificity of the Syphilis Health Check. PCR-based detection of cytomegalovirus in neonates and children, with emphasis on congenital and neonatal-screening cohorts: A systematic review and meta-analysis.

BACKGROUND: Cytomegalovirus (CMV) is one of the commoner infections of childhood and the leading non-genetic cause of sensorineural hearing loss. Diagnosis in children is not straightforward. Serology is hard to interpret in early life and viral culture is slow and often insensitive, and neither method suits testing at scale. CMV detection using PCR is easier, but sample type, format, and age affect accuracy. We reviewed how well PCR-based CMV testing performs in neonates and children, and how much the available studies differ from one another. METHODS: The Cochrane Library, Web of Science, S

Impact of antithrombotic drugs on the accuracy of fecal occult blood testing for advanced colorectal neoplasia screening: a meta-analysis and systematic review.

BACKGROUND: The current meta-analysis was conducted to determine whether antithrombotic drug use would affect the diagnostic accuracy of fecal occult blood testing for advanced colorectal neoplasia. METHODS: Articles published between 2000 and 2019 were systematically retrieved and screened according to the inclusion and exclusion criteria by two reviewers independently. Pooled analyses were conducted with a fixed-effect model if no apparent heterogeneity (I2 ≥ 50%) was found between studies; otherwise, the random effects model would be used. Sensitivity analysis and subgroup analysis were al

Mycoplasma genitalium infection and female reproductive tract disease: a meta-analysis.

To determine the association between Mycoplasma genitalium infection and female reproductive tract syndromes through meta-analysis, English-language, peer-reviewed studies were identified via PubMed, Embase, Biosis, Cochrane Library, and reference review. Two reviewers independently extracted data. Random-effects models were employed to calculate summary estimates, between-study heterogeneity was evaluated using I(2) statistics, publication bias was assessed via funnel plots and the Begg and Egger tests, and methodologic quality was rated. Mycoplasma genitalium infection was significantly asso

Meta-analysis on the performance of p16/Ki-67 dual immunostaining in detecting high-grade cervical intraepithelial neoplasm.

AIM: Although routine screening contributes to substantial reductions in cervical cancer morbidity and mortality, the low specificity of HPV detection and limited sensitivity of cervical cytology necessitates the application of more optimized markers, such as the newly-introduced p16/Ki-67 dual-staining method. Here we reviewed several studies to evaluate the performance of this method in cervical cancer screening. METHODS: An electronic database search was performed on PubMed, Web of Science, CNKI and Wanfang Database for studies assessing p16/Ki-67 dual immunostaining in the diagnosis of hi

When pooling fails: low-intensity Angiostrongylus cantonensis infections in Theba pisana result in false-negativity.

Pooling of gastropod samples is frequently used in surveys of the neurotropic nematode Angiostrongylus cantonensis to reduce cost and processing time. Here we highlight a potential limitation of this technique using the snail Theba pisana as an example. Pooled samples tested negative using a high-sensitivity qPCR assay, whereas subsequent individual testing revealed positive cases. This pattern is consistent with a dilution effect, where low parasite DNA concentrations could lead to false-negative results. These findings suggest that pooling may reduce diagnostic sensitivity in regions where i

Seroprevalence of Dirofilaria immitis in traveling dogs: implications for transboundary pathogen movement.

BACKGROUND: Dirofilaria spp. are parasitic filarial nematodes endemic worldwide that infect dogs but are also zoonotic. Traveling dogs may serve as carriers of zoonotic parasites, including Dirofilaria immitis, introducing them to new regions. However, the risks of importation from endemic areas are unknown. METHODS: To study seroprevalence in travelling dogs, we first established the signal-to-positive (S/P) cutoffs for the DiroCHEK™ ELISA assay (Zoetis) by spectrophotometry at three wavelengths (490, 620, or 650 nm). We also validated the assay for testing serum pools using known D. immitis

Diagnostic accuracy of serum (1-3)-β-D-glucan for Pneumocystis jirovecii pneumonia: a systematic review and meta-analysis.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) can be a life-threatening opportunistic infection in immunocompromised hosts. The diagnosis can be challenging, often requiring semi-invasive respiratory sampling. The serum 1,3-β-D-glucan (BDG) assay has been proposed as a minimally invasive test for the presumptive diagnosis of PJP. METHOD: We carried out a systematic review and meta-analysis using articles in the English language published between January 1960 and September 2019. We estimated the pooled sensitivity and specificity of BDG testing using a bivariate random effects approach an

Cost-effective and efficient diagnosis of bovine theileriosis in a low-income setting.

Sample pooling combined with conventional polymerase chain reaction (cPCR) is a rapid and cost-effective approach for herd-level detection of Theileria spp., particularly in resource-limited settings. Bovine theileriosis is a tick-borne protozoan disease causing major economic losses to livestock production worldwide. In Sri Lanka, particularly in Jaffna, Theileria infections threaten cattle productivity, highlighting the need for affordable and sensitive surveillance tools. This study evaluated pooled cPCR for herd-level detection of Theileria spp. in cattle from three farms in Jaffna. Sixty
