The current classification systems provide broad guidelines for treating the acquired adult flat foot. They essentially place the acquired flat foot into categories of no deformity, flexible deformity, or rigid deformity, as well as the condition of the PTT; however, there are many more variables that need to be taken into account to provide optimal care. As our knowledge and experience grows in treating the adult flat foot, more options will be available. The physician treating this condition must stay up-to-date and must not rely solely on the existing classification systems to direct care.

Tendon and ligament disorders are common indications for ankle surgery. Numerous surgeries and related modifications, ranging from simple repairs and debridement to complex reconstructions with tendon transfers or graft, are available. Knowledge of postoperative imaging appearances of common ankle surgeries is critical to identify complications and aid assessment of clinical treatment failure. This comprehensive review article discusses the surgical indications, techniques, imaging appearances, and complications-specifically those related to minimally invasive or arthroscopic surgery-for vario

We conducted magnetic resonance imaging of the posterior tibial (PT) and flexor digitorum longus (FDL) muscle bellies in 12 patients undergoing surgical treatment for unilateral posterior tibial tendon (PTT) dysfunction. All patients had atrophy of the PT muscle compared to the normal leg (mean 10.7%, p = 0.008). In those patients with a complete rupture of PTT there was replacement of the PT muscle by fatty infiltration. Conversely, the FDL muscle showed a compensatory hypertrophy (mean 17.2%, p < 0.002). We support the use of FDL as an appropriate tendon for augmentation of PTT in stage II d

BACKGROUND: A supple flatfoot deformity caused by posterior tibial tendon (PTT) dysfunction may include a supination deformity of the forefoot that usually is not sufficiently corrected by the commonly suggested treatment options. The use of a partial anterior tibial tendon (ATT) graft that is rerouted through the first cuneiform to the proximal stump of the PTT may restore plantarflexion power of the first ray (Cobb procedure).
METHODS: Twenty-two consecutive patients with stage II PTT dysfunction and a supple supination deformity of the forefoot were included. A clinical examination, a subj

BACKGROUND: Overactivity of anterior and/or posterior tibial tendon may be a causative factor of spastic varus foot deformity. The prevalence of their dysfunction has been reported with not well defined results. Although gait analysis and dynamic electromyography provide useful information for the assessment of the patients, they are not available in every hospital. The purpose of the current study is to identify the causative muscle producing the deformity and apply the most suitable technique for its correction.
METHODS: We retrospectively evaluated 48 consecutive ambulant patients (52 feet

BACKGROUND: Historical concept of flatfoot as posterior tibial tendon dysfunction (PTTD) has been questioned. Recently, the consensus group published a new classification system and recommended renaming PTTD to Progressive Collapsing Foot Deformity (PCFD). The new PCFD classification could be effective in providing comprehensive information on the deformity. To date, there has been no study reporting intra- and interobserver reliability and the frequency of each class in PCFD classification.
METHODS: This was a single-center, retrospective study conducted from prospectively collected registry

This case report describes posterior tibial tendon (PTT) tendinopathy, valgus deformity with tenosynovitis, and osteopenia at the medial malleolus as the primary symptoms of a young patient with celiac disease (CD) without gastrointestinal symptoms. CD is an autoimmune condition that is a chronic inflammatory disorder of the small intestine triggered by ingestion of gluten in individuals with a particular genetic background. Without typical gastrointestinal symptoms, CD patients are often misdiagnosed or undiagnosed. The patient was diagnosed with CD by duodenal biopsy. He underwent a surgical

PURPOSE: Posterior tibial tendon (PTT) is particularly vulnerable and its insufficiency is recognized as the main cause of adult acquired flatfoot. Some patients have a predisposition without clinically recognized cause, suggesting that individual characteristics play an important role in tendinopathy. The objective of the present study is to investigate the association of -519 (rs1144393) matrix metalloproteinase-1 (MMP-1) polymorphism and the -1607 (rs1799750) and -519 MMP-1 haplotypes and risk of PTT dysfunction.
METHODS: The test group included 50 females who presented PTT dysfunction Gra

Approximately 50% of all sports injuries are secondary to overuse and result from repetitive microtrauma that causes local tissue damage. Injuries are most likely with changes in mode, intensity, or duration of training and can accumulate before symptoms appear. Intrinsic factors contributing to injuries are individual bio-mechanical abnormalities such as malalignments, muscle imbalance, inflexibility, weakness, and instability. Contributing extrinsic (avoidable) factors include poor technique, improper equipment, and improper changes in duration or frequency of activity. Injuries are often re

RATIONALE AND OBJECTIVES: The optimal advanced imaging method for detection and characterization of posterior tibialis tendon (PTT) tears is unclear. The purpose of this study was to investigate the utility of ultrasonography (US) and MR imaging in the detection of surgically created PTT tears in cadavers.
MATERIALS AND METHODS: This was a prospective blinded study in which 16 fresh cadaveric foot and ankle specimens (3 men, 13 women; average age at death 83.9 years; age range 71-96 years) were scanned with both US and MR imaging before and after the surgical creation of 64 variable length lo

In this review article, the authors give an overview of the currently available soft tissue and bony procedures in the treatment of the adult acquired flexible flatfoot. Instead of starting from the classification for posterior tibial tendon dysfunction, described by Johnson and Storm, the authors address the flatfoot from a more anatomical point of view. Based on this, they will try to define a treatment algorithm.

BACKGROUND: Procedures utilized to address the flatfoot in this study included medializing calcaneal osteotomy, posterior tibial tendon reconstruction with flexor digitorum longus tendon transfer, and in patients with more severe deformity, lateral column lengthening. We evaluated patients age 50 and less at the time of surgery, who underwent surgical reconstruction for Stage 2 posterior tibial tendon dysfunction. Pre- and postoperative activity levels were evaluated to assess the effect of surgical reconstruction in the younger patient.
MATERIALS AND METHOD: Thirty-four feet in 30 patients (

BACKGROUND: The decision to offer surgery for Stage II posterior tibial tendon deficiency (PTTD) is a difficult one since orthotic treatment has been documented to be a viable alternative to surgery at this stage. Taking this into consideration we limited our treatment to bony realignment by a lengthening calcaneus Evans osteotomy and tendon balancing. The goal of the study was to clinically evaluate PTT functional recovery with this procedure.
METHOD: The patient population included 17 feet in 13 patients. Inclusion was limited to early Stage II PTTD flatfeet with grossly intact but deficien

Tibialis posterior dysfunction is a complex progressive condition caused primarily by injury to the tibialis posterior tendon, leading to acquired pes planus. The tibialis posterior is the most frequent ankle tendon to be injured, and the disorder commonly occurs in late middle-aged females. Degenerative, inflammatory, functional and post-traumatic aetiologies have all been proposed. Failure of the tibialis posterior tendon causes excessive load stress on the spring ligament and sinus tarsi ligaments. A wide spectrum of bony and soft-tissue abnormalities may be seen on plain radiographs, ultra

Posterior tibial tendon insufficiency is often associated with failure of the spring ligament and flatfoot deformity. Arch correction procedures involving bony realignment, such as lateral column lengthening or joint fusions, can predispose to arthritis. Soft tissue reconstruction may provide a more anatomical correction without these complications. The purpose of this investigation was to compare the ability of three different spring ligament reconstruction procedures to correct flatfoot deformity. A deformity model of 5 degrees - 15 degrees talonavicular abduction was created in 10 cadaver f

For treatment of a symptomatic flat foot in a child or an acquired flatfoot from posterior tibial dysfunction in the adult, an understanding of the biomechanics and function of the foot and leg is essential to achieve an excellent outcome. Evaluation of supramalleolar deformity directly influences the procedure choice and eventual success of the procedure. An understanding of the level of deformity also indicates the procedure to be used. Subtalar and midtarsal joint function, although related, have their own primary source of deformity. Correction through the subtalar and midtarsal joints mus

Isolated talonavicular fusions have most commonly been performed for pes valgus deformity, rheumatoid arthritis, and post-traumatic arthritis. Recent reports have advocated its use for posterior tibial tendon dysfunction. The isolated fusion of the talonavicular joint is an effective procedure to reduce rearfoot biomechanical faults. This allows the procedure to not only address local pathology but general rearfoot deformities as well. Complications with the procedure are rare.

OBJECTIVE: To review the progress in clinical and biomechanical study on soft tissue reconstruction of adult-acquired flatfoot deformity (AAFD).
METHODS: The recent original articles of soft tissue repair and tendon transfer for AAFD were extensively reviewed.
RESULTS: The soft tissue procedures for AAFD can be divided into two components: static restoration of medial column stability and dynamic reconstruction of the posterior tibial tendon. The most important static structure to be repaired for AAFD is the spring ligament. On the other hand, various methods can be used for dynamic reconstr
