A 33-year-old man who had undergone total colectomy for refractory ulcerative colitis (UC) at 22 years of age was referred for severe pouchitis with anal stricture. The remission of pouchitis was achieved with vedolizumab (VDZ) treatment. However, as diarrhea improved, obstructive defecation symptoms became apparent due to the stricture. Repeated anal bougie dilation was unsuccessful. A radial incision and cutting were thus performed, and the defecation symptoms improved without recurrence for 1 year. This is the first case report demonstrating the effectiveness of a radial incision and cuttin

BACKGROUND: Complications such as intermittent pouchitis (IP) and chronic inflammatory disorders of the pouch (CP) are common after ileal pouch-anal anastomosis (IPAA), particularly among patients with both ulcerative colitis and primary sclerosing cholangitis (PSC). There are no standardized approaches for prophylaxis against these complications for patients with PSC undergoing IPAA. We compared outcomes among patients with PSC and IPAA who received antibiotic or immunosuppressive therapies to those who did not.
METHODS: We designed a retrospective cohort study that included patients with ul

INTRODUCTION: There are limited data on the impact of glucagon-like peptide-1 receptor agonist (GLP-1RA) on the risk of recurrent pouchitis after ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC).
METHODS: A retrospective cohort study was conducted in the TriNetX Research Network in adult patients with obesity and IPAA for UC with a history of pouchitis in whom a GLP-1RA was initiated compared to those patients in whom a GLP-1RA was not initiated. The primary outcome was the risk of recurrent pouchitis within 12 months. The secondary outcomes were mean number of antibiotic presc

BACKGROUND: Recurrent or chronic antibiotic therapy is a therapeutic hallmark of chronic antibiotic-dependent pouchitis (CADP) or Crohn's-like disease of the pouch. Antibiotics alter the gut microbiome, which may increase the risk of Clostridioides difficile infection (CDI). The aim of this study was to determine the prevalence of CDI in patients with CADP and Crohn's-like disease of the pouch.
METHODS: We conducted a retrospective cohort study of patients with CADP or Crohn's-like disease of the pouch at a tertiary academic medical center. The primary outcome was prevalence of CDI. Secondary

A female patient in her late 20s, with a history of ulcerative colitis since adolescence, underwent ileoanal canal anastomosis for ulcerative colitis. She developed diarrhea, rectal pain, and pyoderma gangrenosum and underwent resection of the remaining rectum and ileal pouch-anal anastomosis. One year postoperatively, she developed ileocolitis, left sacroiliitis, and right dorsal foot pain, which were diagnosed as extraintestinal manifestations, and adalimumab was effective. The patient should be introduced to biologic agents at an earlier stage, considering the possibility of flare-ups or th

BACKGROUND: Pouchitis is an inflammatory condition that affects the ileal pouch during ileal pouch-anal anastomosis surgery. Despite its clinical significance, precise immunological mechanisms underlying pouchitis remain unclear. This study aimed to investigate the lymphocyte profile in the ileal pouch of patients with pouchitis compared to those with familial adenomatous polyposis (FAP) and ulcerative colitis without pouchitis using flow cytometry and immunohistochemical techniques.
METHODS: We prospectively analyzed endoscopic biopsy specimens from the ileal pouches of 15 patients and categ

BACKGROUND: Pouchitis is a prevalent complication following ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC) and significantly impacts patient outcomes. The identification of risk factors and development of predictive models would help to guide perioperative management.
METHODS: This multicentre retrospective study analysed data from 1136 UC patients who underwent IPAA between 2005 and 2019 as part of the Clinical Outcome of Surgery for Ulcerative Colitis (COSUC) study. Potential risk factors, including demographic, clinical and surgical variables, were assessed using univariab

BACKGROUND AND AIMS: To investigate if treatment with non-pooled, multidonor faecal microbiota transplantation [FMT] for 4 weeks was superior to placebo to induce clinical remission in patients with chronic pouchitis.
METHODS: The study was a randomised, double-blinded, placebo-controlled study with a 4-week intervention period and 12-month follow-up. Eligible patients with chronic pouchitis were recruited from five Danish hospitals. Participants were randomised to non-pooled, multidonor FMT derived from four faecal donors, or placebo. Treatment was delivered daily by enema for 2 weeks, follo

BACKGROUND: Clinical, nonspecific pouchitis is common after restorative proctocolectomy for ulcerative colitis, but its cause is unknown. A possible lack of protection for the ileal mucosa in its role as a reservoir for colonic-type bacteria may be the missing piece in defining the causes of pouchitis.
OBJECTIVE: The study aimed to review the causes of pouchitis and introduce the hypothesis that inadequate mucus protection in the pouch, combined with a predisposition to abnormal inflammation, is the most common cause of nonspecific pouchitis.

INTRODUCTION: Pouchitis and Crohn's-like disease of the pouch (CLDP) are common in patients who undergo ileal pouch anal anastomosis for ulcerative colitis. We conducted separate systematic reviews to evaluate the effectiveness of available interventions to prevent and treat pouchitis and CLDP.
METHODS: Through systematic literature reviews, we identified studies that evaluated the effectiveness of probiotics, antibiotics, 5-aminosalicylates, nonsystemic oral corticosteroids, and advanced therapies for prevention and treatment of pouchitis and CLDP for meta-analysis. Primary outcomes were occ

PURPOSE: The extent of preoperative small-bowel mucosal inflammation may be an important predictor of pouchitis after ileal pouch-anal anastomosis. This study examined the value of preoperative wireless capsule endoscopy in predicting outcome of ileal pouch-anal anastomosis in patients with ulcerative colitis or indeterminate colitis.
METHODS: Patients undergoing complete wireless capsule endoscopy before ileal pouch-anal anastomosis were identified. Findings on wireless capsule endoscopy were classified as positive (erosions, ulcers or erythema) or negative. Outcome was assessed prospectivel

BACKGROUND: Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is used to treat patients with ulcerative colitis or familial adenomatous polyposis who need colectomy. While this procedure substantially improves patient's quality of life and reduces cancer risk, it is associated with a variety of sequelae' including surgical complications, inflammatory disorders, and neoplasia. Pouchitis, cuffitis, and Crohn's disease of the pouch are the most common inflammatory disorders of the pouch and para-pouch.
OBJECTIVE: This study aimed to elaborate on the histopathology of common in

BACKGROUND AND AIMS: Ileoanal pouch patients frequently attribute pouch-related symptoms and pouchitis with diet. We aimed to assess perceived food intolerance and habitual dietary intake and their relationship with pouch indication, symptoms and current or history of pouchitis.
METHODS: In this cross-sectional study, patients with an ileoanal pouch completed a dietary intolerance and a food frequency questionnaire, that specifically quantifies habitual intake of FODMAPs. Perceived dietary intolerance rates, nutrient intake and diet quality, and their differences based on pouch indication, sy

BACKGROUND: The operation of choice for patients with chronic ulcerative colitis (CUC) is restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA). Pouchitis is the most common complication after IPAA. The incidence of Clostridium difficile infection (CDI) is higher in CUC patients than the general population and can lead to significant disease flares and higher rates of colectomy. We sought to determine the risk of pouchitis in patients with precolectomy CDI and 90-day postoperative IPAA complications.
METHODS: A retrospective case-control study was conducted on patients with CUC

BACKGROUND: Ileal pouch-anal anastomosis (IPAA) is associated with good long-term outcomes, yet pouchitis remains a common complication. Although antibiotics are first-line therapy, recurrent disease often leads to repeated use with declining efficacy. Dietary interventions have shown promise, but evidence and consensus remain limited. The aim of this study was to compare dietary intake between IPAA patients with chronic pouchitis, IPAA patients with a normally functioning pouch, and healthy controls.
METHODS: This cross-sectional study recruited participants between November 2017 and October

Mild arthritis/arthralgia is the most frequent extraintestinal manifestation in inflammatory bowel disease (IBD) and has been reported to occur in 10-35% of patients in different studies. A classification of peripheral arthropathy in relation to inflammatory bowel disease has recently been proposed. Type 1: pauciarticular, asymmetrical, preferably large joints, and related to IBD activity. Type 2: polyarticular, symmetrical, preferably small joints, and occurring independently of IBD activity. While this classification requires the presence of synovitis, arthralgia without swelling or other ob

Ileal pouch-anal anastomosis (IPAA) is the preferred surgical treatment of patients with ulcerative colitis and familial adenomatous polyposis. As the incidence of ulcerative colitis rises globally, increasing numbers of patients live long term with IPAA. Whereas most patients with IPAA achieve good functional outcomes, studies suggest that up to 80% will experience pouchitis during their lifetime, among other inflammatory and structural pouch-related complications. Patients with pouch disorders present with nonspecific gastrointestinal symptoms that may include increased stool frequency, urge
