DESIGN: Retrospective cohort study of prescriptions for midazolam, propofol, levomepromazine, phenobarbital, and morphine within 14 days before death, used as a proxy for palliative sedation and to gain insight into the occurrence. SETTING AND PARTICIPANTS: A total of 672 n

Trends over time in prescribing by English primary care nurses: a secondary analysis of a national prescription database.

BACKGROUND: A growing number of countries legislate for nurses to have medication prescribing authority although it is a contested issue. The UK is one of these countries, giving authority to nurses with additional qualifications since 1992 and incrementally widened the scope of nurse prescribing, most recently in 2006. The policy intention for primary care was to improve efficiency in service delivery through flexibility between medical and nursing roles. The extent to which this has occurred is uncertain. This study investigated nurses prescribing activities, over time, in English primary ca

Implementing Australia's new guidelines for appropriate psychotropic medication use in residential aged care: Applying the Consolidated Framework For Implementation Research.

OBJECTIVES: Accumulating evidence about psychotropic medication-related adverse events has had minimal apparent impact on overall rates of psychotropic medication use across Australian residential aged care facilities (RACFs). Australia's new Clinical Practice Guidelines for the Appropriate Use of Psychotropic Medications in People Living with Dementia and in Residential Aged Care were released in April 2023. This study aimed to identify contextual factors to inform strategies to implement the new Guidelines in Australian RACFs. METHODS: A qualitative study using semi-structured interviews wa

Postpartum and neonatal nursing care during the 2009 H1N1 influenza pandemic.

We describe select influenza infection control policies and practices related to postpartum and newborn care during the 2009 H1N1 pandemic. In an online survey of obstetric and neonatal nurses, significantly more nurses indicated a written hospital policy supporting each of the practices during versus before the pandemic. The two practices least often implemented were temporary separation of healthy newborns from ill mothers (37.7 percent) and testing newborns for influenza virus infection if signs of influenza were observed (31.4 percent). Presence of written hospital policies increased imple

Feasibility of a respiratory nurse specialist-led sleep disorder service in a district general hospital.

Aim The burden of referrals to investigate sleep disorders has escalated considerably. In order to address this, the feasibility of a nurse-led sleep disorder clinic was investigated. Methods Sleep referrals were stratified on the basis of information contained within the primary care referral, and appropriate home-based sleep studies arranged if obstructive sleep apnoea was considered the most likely working diagnosis. Selected patients were started on continuous positive airway pressure treatment if deemed appropriate. Education was almost exclusively provided by specialist nursing staff or

Supporting women's instinctive pushing behaviour during birth.

During birth women instinctively push in the most effective and safe way for themselves and their babies. However, midwives continue to direct women's pushing behaviours. The assumptions underpinning this practice must be examined and challenged. Midwives need to develop an evidence-based approach that acknowledges and reinforces women's innate ability to birth.

How clinical nurse and midwifery consultants optimise patient care in a tertiary referral hospital.

AIMS AND OBJECTIVES: To develop new insights into the phenomenon of clinical nurse/midwifery consultant clinical effectiveness in a tertiary referral hospital. BACKGROUND: International studies identify the pivotal role clinical nurse/midwifery consultants play in patient outcomes. There remains, however, a significant deficit in our knowledge of how these, or other advanced practice nurses and midwives, apply their extensive experience, ontological understandings and tacit knowledge to the enhancement of patient outcomes in complex healthcare environments. DESIGN: This study was underpinned

Implementation and evaluation of a nurse-led clinic for men referred with suspected prostate cancer.

BACKGROUND: Cancer diagnostic delays remain a challenge. Advanced nurse-led clinics represent an innovative approach to improving pathway efficiency while delivering person-centred care. Aims: To evaluate barriers and facilitators in establishing a nurse-led clinic for urgent suspected prostate cancer referrals and assess the impact on patient experience and services. METHODS: A hybrid effectiveness-implementation design and naturalistic case-study design. Data included routine data (pre-implementation, n=150; implementation, n=293), patient surveys (n=162), patient interviews (n=10), staff i

Clinical Outcomes of a Nurse-Delivered, Technology-Enabled HIV Outpatient Model.

We evaluated Klick, a nurse-led, digitally enabled model of HIV outpatient care, launched in 2020. Klick's smartphone app offers online booking, remote nurse-led consultations, and results. An audit of Klick nurse-led consultations was conducted against BHIVA monitoring guidelines, and nurses were interviewed about their experience. Of 40 Klick patients audited, 4 of 5 BHIVA standards were met: 100% had documented co-medications, smoking history, blood pressure, and viral load data, and 89% received a cardiovascular risk calculation (Targets 97%-90%-90%-90%-90%). Prescribing Practices of Nurse Practitioners and Physician Assistants Waivered to Prescribe Buprenorphine and the Barriers They Experience Prescribing Buprenorphine.

BACKGROUND: In 2016, the Comprehensive Addiction Recovery Act permitted nurse practitioners (NPs) and physician assistants (PAs) to obtain a waiver to prescribe buprenorphine to treat opioid use disorder (OUD), with the goal of increasing access to this treatment. This study's purpose was to describe the buprenorphine prescribing practices of NPs and PAs and compare the barriers rural and urban providers face delivering treatment. METHODS: From the October 2018 Drug Enforcement Administration list of providers with the waiver to prescribe buprenorphine, all rural NPs and PAs (1,057) and a ran
