Journal of hand therapy : official journal of the American Society of Hand Therapists2001eng
Because of the clinical impression that traditional forearm goniometry has several potential sources of error and is therefore inherently unreliable, a new method using a newly designed goniometer was compared with traditional forearm passive range-of-motion goniometry. The new method utilized an offset face with a plumbline design and a patient-held tubular handle. Thirty orthopedic patients (31 wrists) were measured by three experienced hand therapists in a test-retest protocol to determine the reliability of both the traditional and new methods. Both the intraclass correlation coefficients

BACKGROUND: Foot orthosis (FO) is one of the most important treatments for pressure relief and gait rehabilitation in the foot misalignment population. The previous design conception was primarily focused on arch support and pressure reduction, but recent studies also questioned their efficiency. METHODS: Three bionic fish bone FOs based on the COP characteristics of healthy people were designed and manufactured. The orthoses were tested in 30 volunteers through a series of experiments including both static standing and dynamic gait, and evaluated through finite element (FE) simulations with a

BACKGROUND: We aimed to biomechanically evaluate the distal pronator quadratus and compare two locations of distal transection on the strength of the subsequent repair.
METHODS: Eighteen fresh-frozen cadaveric specimens were dissected to the pronator quadratus muscle. Specimens were randomly allocated for transection of the pronator quadratus at the myotendinous junction (red group) or parallel to the myotendinous junction at the midsection of the distal tendinous zone (white group). For both groups, repair of the muscle was performed using two figure-of-8 sutures. The radius and ulna were po

The pronator teres muscle is a major dynamic stabilizer of elbow valgus stress during throwing. This study aims to investigate pronator teres muscle activation during breaking ball pitching in baseball pitchers. Twelve male college baseball players with more than eight years of baseball experience were included in this study. A wireless surface electromyography (EMG) system was used to measure the activation of the forearm muscles and record EMG data during fastball and curveball pitching. Peak pronator teres muscle activation during curveball pitching was greater than that during fastball pit

The diagnostic process that allows pronator syndrome to be differentiated reliably from carpal tunnel syndrome remains a challenge for clinicians, as evidenced by the most common cause of pronator syndrome misdiagnosis: carpal tunnel syndrome. Pronator syndrome can be caused by compression of the median nerve as it passes through the anatomical structures of the forearm, while carpal tunnel syndrome refers to one particular topographic area within which compression occurs, the carpal tunnel. The present narrative review is a complex clinical comparison of the two syndromes with their anatomica

Distal radius fractures are the most common type of upper extremity fractures, and their incidence is increasing. There has been a rise in the surgical treatment of distal radius fractures, primarily with volar locking plate fixation. Although this procedure is commonly done among orthopaedic surgeons, the role of pronator quadratus repair after fixation remains controversial. The pronator quadratus serves as a secondary forearm pronator and a dynamic stabilizer of the distal radioulnar joint. Aside from a functional role, repair of the pronator quadratus has been proposed to serve as a biolog

BACKGROUND: Injuries to the thenar muscle mass or the thenar branch of the median nerve and resulting loss of thumb opposition lead to a massive impairment of hand function. For decades, reconstructive approaches were based on tendon transfers. To broaden the reconstructive repertoire, we present the free functional pronator quadratus flap as a viable alternative for functional reconstruction and provide a specification for its indication. We demonstrate our surgical technique to a single incision reconstruction using the free functional pronator quadratus flap. Based on a series of three pati

PURPOSE: Fractures of distal radius are one of the common orthopaedic injuries. Placing the plate on volar surface requires release of underlying pronator quadratus (PQ) muscle. No consensus is present in the literature about the repair or not of the PQ. The purpose of this study was to evaluate the influence of PQ repair on functional outcome and complications.
METHODS: Retrospectively 83 patients of distal radius fractures managed with volar plating between 2014 and 2016 were evaluated. Demographic data, operative notes and physical therapy records were retrieved. Patients were divided into

BACKGROUND: In extended upper-type lesions of the brachial plexus, nerve transfers and root grafting have improved the results of shoulder and elbow reconstruction. However, wrist extension reconstruction has received little attention.
METHODS: In 20 cadaveric upper limbs, we dissected the anterior interosseous nerve and extensor carpi radialis brevis motor branch. Four patients with upper-type lesions of the brachial plexus with paralysis of wrist and finger extension were operated on within 10 months of trauma and followed up for 12 months after surgery. The terminal division of the anterio

Between 1998 and 2007, a pronator quadratus pedicled bone graft was performed for 45 patients of ununited scaphoid fracture. One of them had bilateral ununited scaphoid fracture. There were 29 men and 16 women with a median age at operation of 24 (16-32) years. The affected side was the right side (dominant hand) in 32 patients whereas 13 patients had fracture of the nondominant left side. There had been 32 proximal pseudoarthrosis (through or proximal to the junction of the proximal and middle thirds of the bone) and 14 of the middle third of the scaphoid. The original fractures were caused b

BACKGROUND: Elbow valgus stability relies on both static and dynamic stabilizers. The medial collateral ligament, particularly the anterior bundle (AB), provides primary resistance to valgus stress, whereas the flexor-pronator mass (FPM) offers secondary, dynamic support. Combined disruption of these structures-especially following trauma-can result in significant elbow instability. This cadaveric study aimed to quantify radiographic changes in the medial ulnohumeral joint (UHJ) space and valgus alignment following sequential transection of the AB, posterior bundle (PB), and FPM.
METHODS: Twe

BACKGROUND: The aim of this study was to detail the morphology of the pronator quadratus muscle (PQM) and to study its morphometry. The objectives were to perform side-based, sex-based analyses.
METHODS: A total of 70 upper extremities (35 right and 35 left) from adult embalmed cadavers were included in this study. The quadrilateral-shaped pronator quadratus was pointed at its medio-superior angle, medio-inferior angle, latero-superior angle and latero-inferior angle. These points were used to measure the lateral length, medial length, proximal width and distal width via a digital Vernier cal

Background Pronator quadratus (PQ) acts as the pronator of the wrist and stabiliser of the distal radioulnar joint; it also provides a protective cover over the edge of the plate, preventing friction and subsequent rupture of flexor tendons. The repair of PQ is often difficult, and its durability is questionable; hence, preserving the PQ while volar plating distal radius fractures is advocated. Methods In this prospective randomised trial, 60 patients with a fracture of the distal end of the radius of AO-type (Arbeitsgemeinschaft für Osteosynthesefragen) 23 A2, A3, B1, B3, C1, and C2 were trea

BACKGROUND: Connective tissue serves a role beyond mere spatial filling. Furthermore, there is increasing evidence that connective tissue plays an important role in the pathogenesis of conditions such as carpal tunnel syndrome (CTS). According to our hypothesis, the median nerve (MN) is surrounded by a system of connective tissue distal to the pronator teres and extending up to, and including, the carpal tunnel.
METHODS: To visualize the connective tissue surrounding the median nerve, we dissected the forearms of 15 body donors from pronator teres to the carpal tunnel, created plastination sl

OBJECTIVE: The traditional volar approach requires the release of the pronator quadratus (PQ) muscle in the treatment of distal radius fractures. However, intraoperative repair of the PQ muscle often fails due to tissue injury and unstable muscle repair. This study compared the outcomes of different methods of sparing the PQ muscle combined with the volar plate in treating distal radius fractures.
METHODS: A total of 95 patients with distal radius fractures sparing the PQ muscle were enrolled with the brachioradialis (BR) splitting approach (group A, 33 people), the volar plating insertion PQ

CASE: A 57-year-old man presented with pain and paresthesia in both hands and was diagnosed with pronator teres syndrome. Surgical decompression of the left elbow and forearm revealed the median nerve in an unusual anatomic location, specifically running within the pronator teres muscle.
CONCLUSION: Anatomic anomalies of the pronator teres muscle and the path of the median nerve have been described. However, there are no reports of the median nerve entering and traveling within the pronator teres. Surgeons should be aware of this anomaly to avoid potential iatrogenic injury when performing an
