Auditory attention in individuals with tinnitus.

INTRODUCTION: Tinnitus is characterized by the presence of a sound in the absence of external sound stimulus. In individuals with normal audiometry, it may be associated with auditory attention difficulty, especially in those who report high tinnitus annoyance. OBJECTIVE: To investigate auditory attention ability in individuals with tinnitus complaint. METHODS: Cross-sectional analytical observational study. We evaluated 30 volunteers with normal hearing (up to 25dBHL): 15 with tinnitus (test group) and 15 with no complaints (control group), aged between 18-40 years. The volunteers answered

Relationship between exposure to multiple noise sources and noise annoyance.

Relationships between exposure to noise [metric: day-night level (DNL) or day-evening-night level (DENL)] from a single source (aircraft, road traffic, or railways) and annoyance based on a large international dataset have been published earlier. Also for stationary sources relationships have been assessed. Here the annoyance equivalents model concerning noise annoyance from combined sources and the underlying assumptions are presented. The model first translates the noise from the individual sources into the equally annoying sound levels of a reference source, road traffic, and then sums thes

Neural processing of auditory looming in the human brain.

Acoustic intensity change, along with interaural, spectral, and reverberation information, is an important cue for the perception of auditory motion. Approaching sound sources produce increases in intensity, and receding sound sources produce corresponding decreases. Human listeners typically overestimate increasing compared to equivalent decreasing sound intensity and underestimate the time to contact of approaching sound sources. These characteristics could provide a selective advantage by increasing the margin of safety for response to looming objects. Here, we used dynamic intensity and fu

Auditory frequency discrimination in adult developmental dyslexics.

Developmental dyslexics reportedly discriminate auditory frequency poorly. A recent study found no such deficit. Unlike its predecessors, however, it employed multiple exposures per trial to the standard stimulus. To investigate whether this affects frequency discrimination in dyslexics, a traditional two-interval same-different paradigm (2I_1A_X) and a variant with six A-stimuli per trial (2I_6A_X) were used here. Frequency varied around 500 Hz; interstimulus interval (ISI) ranged between 0 and 1,000 msec. Under 2I_1A_X, dyslexics always had larger just noticeable differences (JNDs) than did

Binaural detection at high frequencies with time-delayed waveforms.

Recent research has demonstrated that the binaural system can utilize ongoing interaural time differences for lateralization at high frequencies as well as at low frequencies. The requirement is that the signal be complex so that the time difference appears as a delay in the envelope of the waveform at one ear. Reported here are several masking experiments that examine detection performance with time-delayed signals or maskers. In the first experiment, the signal was a 50-Hz band of noise centered at 4000 Hz that was time delayed by different amounts on different blocks of trials; the masker w

Beyond Auditory Sensory Processing Deficits: Lexical Tone Perception Deficits in Chinese Children With Developmental Dyslexia.

Increasing evidence suggests that children with developmental dyslexia exhibit a deficit not only at the segmental level of phonological processing but also, by extension, at the suprasegmental level. However, it remains unclear whether such a suprasegmental phonological processing deficit is due to a difficulty in processing acoustic cues of speech rhythm, such as rise time and intensity. This study set out to investigate to what extent suprasegmental phonological processing (i.e., Cantonese lexical tone perception) and rise time sensitivity could distinguish Chinese children with dyslexia fr

Neural Segregation of Concurrent Speech: Effects of Background Noise and Reverberation on Auditory Scene Analysis in the Ventral Cochlear Nucleus.

Concurrent complex sounds (e.g., two voices speaking at once) are perceptually disentangled into separate "auditory objects". This neural processing often occurs in the presence of acoustic-signal distortions from noise and reverberation (e.g., in a busy restaurant). A difference in periodicity between sounds is a strong segregation cue under quiet, anechoic conditions. However, noise and reverberation exert differential effects on speech intelligibility under "cocktail-party" listening conditions. Previous neurophysiological studies have concentrated on understanding auditory scene analysis u

Auditory dysfunction in patients with cerebrovascular disease.

Auditory dysfunction is a common clinical symptom that can induce profound effects on the quality of life of those affected. Cerebrovascular disease (CVD) is the most prevalent neurological disorder today, but it has generally been considered a rare cause of auditory dysfunction. However, a substantial proportion of patients with stroke might have auditory dysfunction that has been underestimated due to difficulties with evaluation. The present study reviews relationships between auditory dysfunction and types of CVD including cerebral infarction, intracerebral hemorrhage, subarachnoid hemorrh

Comparison of measures of frequency resolution and recruitment in patients undergoing neuro-otological investigation.

Loudness recruitment and reduced frequency resolution both occur in cochlear types of hearing loss. One theory of loudness recruitment suggests that, as intensity is coded partly by spread of excitation across the nerve fibre array, recruitment is a direct consequence of the broad spread of excitation associated with poor frequency resolution. The present study investigated the relationship between these two quantities. The study involved a simple measure of frequency resolution (three-point psychoacoustical tuning curve, PTC) and conventional measures of recruitment obtained from patients und

Relationship between tinnitus pitch and edge of hearing loss in individuals with a narrow tinnitus bandwidth.

OBJECTIVE: Psychoacoustic measures of tinnitus, in particular dominant tinnitus pitch and its relationship to the shape of the audiogram, are important in determining and verifying pathophysiological mechanisms of the condition. Our previous study postulated that this relationship might vary between different groups of people with tinnitus. For a small subset of participants with narrow tinnitus bandwidth, pitch was associated with the audiometric edge, consistent with the tonotopic reorganization theory. The current study objective was to establish this relationship in an independent sample.

[Pathogenesis mechanism in ear fullness in otosclerosis].

To clarify the mechanism underlying ear fullness in otosclerosis, we studied the relationship between clinical features and examinations. Subjects were 116 otosclerosis patients (140 ears). The presence or absence of ear fullness was judged from a questionnaire in initial diagnosis or a chart description. Ear fullness was observed in 44 ears (31%) and absent in 96. The averaged air and bone conduction hearing levels (500 Hz-4 k Hz) in initial diagnosis were significantly lower in the group with ear fullness. The difference in averaged hearing was mainly apparent at 2 k Hz and 4 k Hz. The diffe

Dynamic range of the contralateral stapedius reflex in cochlear implant patients.

The dynamic range of the contralateral Stapedius reflex elicited by analog electrostimulation via cochlear implant was investigated in 8 subjects supplied with a Vienna prosthesis. The study comprised patients fitted with intracochlear devices as well as patients with extracochlear devices. Sinusoidal bursts of 125, 500, 1,000 and 2,000 Hz were used as stimulation signals. Only in one patient was saturation of reflex amplitude observed. The dynamic range of the Stapedius reflex was defined either by the difference in stimulus level for reflex saturation or individual uncomfortable loudness lev

Oral vibrotactile assessment procedures: modifications.

An ongoing process to develop oral vibrotactile instrumentation that can be standardized and used in practical assessment of the oral tactile sensory system was reported in The International Journal of Orofacial Myology in March, 1990, by Fucci, Petrosino, and Harris. The material presented in the present article reflects the current research and future direction that the development of this instrumentation is taking. Potential benefits of this instrumentation for individuals working in the area of orofacial myofunctional disorders are suggested.

[Value of in situ measurement for evaluating effective hearing aid amplification at high sound pressure levels].

A careful hearing aid fitting requires consideration of various characteristics of inner ear pathology. The recruitment phenomenon restricts the dynamic range of the impaired hearing and this increase the demands on the hearing aids for a satisfactory correction of the frequency characteristics as well as for an intensity reduction at higher sound pressure level SPL. For years data from coupler or KEMAR (Knowles Electronics Manikin of Acoustical Research) have served to estimate the real ear gain of a hearing aid at higher input levels. In previous papers we reported on some of the differences

Unanswered questions concerning the indication for cochlear implant.

In a discussion of the function of cochlear implant (CI), the following criteria should be mentioned: (1) If possible, all deaf persons (including the prelingually deaf and children) should benefit from CI. (2) Since the number of patients with late deafness is doubtlessly decreasing with better prevention and medical care (elimination of ototoxic damage, decrease of iatrogenic deafness), the percentage distribution of the deaf will be displaced in favor of the prelingually deaf. At present, the ratio of cases of late deafness to those of prelingual deafness among our CI candidates is 1:3. (3)

Cascades of two-pole-two-zero asymmetric resonators are good models of peripheral auditory function.

A cascade of two-pole-two-zero filter stages is a good model of the auditory periphery in two distinct ways. First, in the form of the pole-zero filter cascade, it acts as an auditory filter model that provides an excellent fit to data on human detection of tones in masking noise, with fewer fitting parameters than previously reported filter models such as the roex and gammachirp models. Second, when extended to the form of the cascade of asymmetric resonators with fast-acting compression, it serves as an efficient front-end filterbank for machine-hearing applications, including dynamic nonlin

Relation between frequency selectivity and speech perception following glycerol-induced cochlear changes.

Changes in auditory threshold, psychophysical tuning curves, and speech perception (in both quiet and noise) were monitored over a 3-hr period following the ingestion of glycerol. All listeners had sensorineural hearing loss secondary to Menière's disease. Findings were characterized by large intersubject variability and in general did not show a clear relation between changes in threshold, frequency resolution, and speech perception.
