BACKGROUND: Current research has emphasized the role of structural integration of personality and childhood experiences for the understanding of anxiety disorders. In this study, we examined the relationship between anxiety disorders (generalized anxiety disorder vs. panic disorder vs. phobic disorders), the level of structural integration of personality, and negative and protective childhood experiences at the beginning of outpatient psychodynamic psychotherapy treatment. Differences were characterized in comparison to patients with no anxiety disorders.
METHODS: The sample included a total

Psychiatry residency programs in the United States and Canada currently require trainees to demonstrate competency in psychodynamic therapy. Developed by Habib Davanloo, intensive short-term dynamic psychotherapy (ISTDP) is a brief psychodynamic therapy with emerging evidence to support its efficacy as an evidence-based, cost-effective treatment for various common psychiatric disorders and personality pathologies. Davanloo's metapsychology of the unconscious offers a way to understand the psychodynamic processes that maintain patients' psychiatric symptoms. Some key clinical processes unique t

OBJECTIVES: Patients with personality disorders (PDs) are often treated with non-manualized psychodynamic group therapy (PDT) lasting for several years. Non-manualized PDT often combines a variety of therapeutic approaches from different PDT traditions, including mentalization-based therapy. Currently, little is known about the effect of this long-term, costly treatment. This study investigated the extent to which patients with different PDs benefit from mentalization-oriented PDT as it is implemented in clinical practice in terms of symptom severity, interpersonal problems, and general functi

In this study, we aimed to examine how moment-to-moment interpersonal behaviors of warmth and dominance in patients and therapists, as well as interpersonal complementarity, are related to withdrawal and confrontation ruptures as sessions unfold. Sixteen psychotherapy sessions from eight independent therapeutic dyads were sampled for the highest level of alliance ruptures from a naturalistic psychotherapy data set featuring evidence-based psychodynamic psychotherapy for patients with interpersonal problems and personality pathology. Interpersonal behaviors, complementarity, and alliance ruptur

The aim of this introduction to this issue is to review and summarize significant new psychological perspectives and psychotherapeutic interventions in the treatment of eating disorder (EDs). In this issue, five novel therapeutic strategies for the treatment of EDs are presented. These include a combination of psychodynamic psychotherapy and neuropsychology, two recent modifications of cognitive behavioral therapy (the MANTRA, and the conceptualization of change in patients with ED in terms of sense of agency), a specific group movement therapy, and an innovative rehabilitation treatment. The paper gives an overview of the 4 randomized-controlled multi-center psychotherapy studies and the associated studies of the Eating Disorders Diagnostic and Treatment Network (EDNET). The multi-center trials include an outpatient treatment trial for AN comparing focal psychodynamic psychotherapy, cognitive-behavioral therapy, and treatment as usual, a trial comparing in- and day-patient treatment in adolescents with AN, and two internet-based relapse prevention trials for patients with AN and with BN after discharge from inpatient treatment. Associated studies are grouped around these core

Case report on the in-patient psychoanalytical psychotherapy of a 12 year old boy, who developed a nonorganic encopresis at the age of nine after his mother died. One focal issue is his denial of this loss and the beginning of a process of mourning due to the treatment. The Operationalized Psychodynamic Diagnostics (OPD) for Children and Adolescents are illustrated for this case.

BACKGROUND: Diagnostic interviews and questionnaires are commonly used in the assessment of adolescents referred to child and adolescent mental health services. Many of these rating scales are constructed for adults and focus on symptoms related to diagnosis. Psychodynamic Functioning Scales (PFS) focus on relational aspects and how the patients handle affects and solve problems, rather than manifest symptoms. As these aspects are considered important for mental health, the PFS were developed to assess change in adults, consistent with the relational and intrapsychic concepts of dynamic psycho

OBJECTIVE: The current study investigated bidirectional associations between defense mechanisms and therapeutic techniques in two different psychotherapies for panic disorder (PD). Identifying how technique use facilitates and is facilitated by change in defenses might guide adaptation and improvement of therapies.
METHOD: Patient defense mechanisms and use of therapeutic techniques were measured at early, mid, and late-treatment for 101 patients receiving panic-focused psychodynamic psychotherapy (PFPP) and cognitive behavioral therapy (CBT). Time-lagged associations between use of technique

Borderline personality disorder (BPD) is believed to be frequent among adolescents. While several prospective studies have assessed the use of mental health services among adults who suffer from BPD, few studies have provided adolescent data. This paper presents findings from the first assessment point of the European Research Network on Borderline Personality Disorder (EURNET BPD) study. In this study, we describe lifetime treatment utilization for 85 adolescents with BPD (Mean age: 16.3 years old). In line with adult findings, adolescents with BPD reported greater mental healthcare service u

Eight cases of fully recovered formerly schizophrenic (DSM-III-R) patients who had been treated with intensive psychodynamic psychotherapy were compared with 10 nonrecovered schizophrenic patients who also had been treated with intensive psychotherapy. There were no significant differences in chronicity, medication or type and amount of psychotherapy given. The most striking difference between the groups was the type and quality of auditory hallucinations. In the nonrecovered group the hallucinations were more dominating and often of the running commentary type, sometimes occurring even in cal

OBJECTIVES: Dispositional optimism predicts various beneficial outcomes in somatic health and treatment, but has been little studied in psychotherapy. This study investigated whether an optimistic disposition differentially predicts patients' ability to benefit from short-term versus long-term psychotherapy.
DESIGN: A total of 326 adult outpatients with mood and/or anxiety disorder were randomized into short-term (solution-focused or short-term psychodynamic) or long-term psychodynamic therapy and followed up for 3 years.
METHODS: Dispositional optimism was assessed by patients at baseline w

Extensive explorations in neuroscience, psychology, and psychotherapy increasingly recognized the embodied and relational foundations of selfhood, underscoring the need for an integrated framework spanning development, psychopathology, and therapeutic change. This narrative review synthesizes empirical and theoretical literature across neuroscience, embodiment research, predictive processing, developmental science, phenomenology, and psychodynamic theory, proposing a multidimensional neuropsychodynamic framework of embodied selfhood and its clinical implications. A central contribution is the

Within the SOPHO-Net-Project, mainly focusing on a randomized-controlled trial comparing short term cognitive and psychodynamic psychotherapy for social anxiety disorder, a subsample consisting of 88 patients from 3 of the 5 study sites was investigated to examine the relationship between outcome, initial attachment characteristics and negative indicators during the process. These negative indicators were assessed with the Vanderbilt Negative Indicators Scale which was applied to an early (3rd), a middle (9th) as well as a late therapy (22nd) session. The study shows that negative indicators a

Over the years, short term psychodynamic therapy (STPP) has been broadly researched in order to evaluate its efficacy in the treatment of major psychiatric disorders. In particular, a consistent number of studies focused on assessing clinical outcomes of the principal psychodynamic techniques in treating depressive disorders. We conducted a narrative review in which we aimed to evaluate the efficacy of STPP in monotherapy in major depressive disorder and to assess possible features that may correlate with its clinical use. Databases searched were PubMed, Ovid, Scopus, PsycINFO and Cochrane Lib

Body-focused repetitive behaviors, such as trichotillomania and skin-picking, are not well understood in clinical communities. Research and practice with a population exhibiting these behaviors have focused almost exclusively on the usefulness of cognitive-behavioral therapy in symptom reduction. The author specializes in treating patients with body-focused repetitive behaviors and suggests that a holistic, psychodynamic model of treatment for this population creates possibilities for deeper change. Group therapy can be an important component of this work, helping clients to overcome resistanc

BACKGROUND: Borderline personality disorder (BPD) is frequent (prevalence in Germany between 0.7% and 4.5%) [11] and is associated with a high level of psychological stress and frequent emergency inpatient admissions. The provision of disorder-specific outpatient psychotherapy is still insufficient also in Germany.
OBJECTIVE: This article provides an overview of the available data on the effectiveness of inpatient psychotherapy for BPD.
MATERIAL AND METHODS: A qualitative review on the effectiveness and therapy outcome predictors was conducted based on a literature search in PubMed.

OBJECTIVE: The objective of this study was to identify predictors of long-term weight loss after inpatient psychodynamic or behavioral psychotherapy of severely obese patients.
METHODS: In a longitudinal study, obese patients [body mass index (BMI)>or=35 kg/m(2)] were randomly assigned to behavioral or psychodynamic inpatient treatment. The average treatment duration was 7 weeks. Two hundred sixty-seven obese patients, mostly female (85%), with psychiatric and somatic comorbidity (age, 20-64 years; BMI=35-74 kg/m(2)) were examined with standardized self-report scales at intake, discharge, 1-y

INTRODUCTION: Alliance ruptures constitute a high risk of premature treatment termination and poor psychotherapy outcome. The Alliance-Focused Training (AFT) is a promising transtheoretical approach to enhance therapists' skills in dealing with alliance ruptures.
METHODS AND ANALYSIS: To evaluate the effectiveness of Modified AFT with doubling (MAFT-D), a randomised, patient and evaluator-blinded, multicentre trial was designed comparing MAFT-D (delivered to trainee therapists and supervisors) and psychotherapy training/treatment as usual (TAU) for therapists (n=120) and their patients with d
