Anorectal fissures: an under-recognized cause of low back pain? Case report.

INTRODUCTION: Low back pain is one of the most common complaints for which patients seek medical attention. The differential for such a complaint is wide and therefore requires a thoughtful and thorough work-up. Anorectal disorders are an often-overlooked cause of low back pain. CASE PRESENTATION: We discuss the case of a 46-year-old male with history of dull aching back pain in the midline at the lumbosacral junction. The pain extended to the left of midline, into the region of the sacroiliac joint. There was no radiation into the buttock or leg. Physical exam was unremarkable. Upon soliciti

Ultrasound compared with projection radiography for the detection of soft tissue foreign bodies - A technical note.

INTRODUCTION: Soft tissue foreign bodies (STFBs) present a diagnostic challenge depending on their composition. Untreated complications can arise, namely infection through to loss of function. General (projection) radiography is recommended as the first line imaging examination. However, some STFBs are radiolucent, leading to false negative radiographs. The aim of this in vitro study was to compare ultrasound with projection radiographs for the detection of a range of different types of STFB. METHOD: Ethical approval (for use of participants to evaluate images) was granted by the Higher Educa

A systematic review and meta-analysis of the efficacy of medical treatments for the management of solitary rectal ulcer syndrome.

BACKGROUND/AIM: Solitary rectal ulcer syndrome (SRUS) is a benign, poorly understood disorder that is difficult to manage. Medical interventions such as sucralfate, sulfasalzine, human fibrin, and a high fibre diet are reported as the first line of treatment. The aim of this study is to perform a systematic review and meta-analysis of the efficacy of medical treatments for SRUS. MATERIALS AND METHODS: Databases including PubMed, Cochrane, and Embase were searched for randomised clinical trials (RCT) and observational studies that evaluated medical treatments for SRUS. Two authors independentl

Allergy to laxative compound (Plantago ovata seed) among health care professionals.

BACKGROUND: The seeds of Plantago ovata (psyllium, ispaghula) used in the manufacture of bulk laxatives are known to be the cause of occupational allergy (rhinitis, asthma) in health care and pharmaceutical workers. OBJECTIVE: We studied the prevalence of P ovata seed allergy among health care workers in geriatric care homes and compared it with a group of health care professionals not exposed to P ovata seed. Cross reactivity with Plantago lanceolata pollen was also studied. METHODS: Two groups of health professionals were recruited: 58 health care workers from geriatric care homes who were

Dietary Protein Source Mediates Colitis Pathogenesis Through Bacterial Modulation of Bile Acids.

BACKGROUND & AIMS: Evidence-based dietary recommendations for individuals with inflammatory bowel diseases are limited. Red meat consumption is associated with increased inflammatory bowel disease incidence and relapse in patients, suggesting that switching to a plant-based diet may limit gut inflammation. However, the components and mechanisms underlying the differential effects of these diets remain poorly understood. This study investigated the differential impact of dietary protein source on experimental colitis and related mechanisms. METHODS: Isocaloric synthetic diets containing protei

Hypersensitivity reactions associated with nutraceuticals and dietary supplements: A narrative review.

The global use of nutraceuticals and dietary supplements-including vitamins, minerals, probiotics, and botanical extracts-has increased substantially, driven largely by consumer perceptions of safety and health benefits. However, their potential to cause hypersensitivity reactions (HSRs) is underrecognized. This review synthesizes published case reports, case series, systematic reviews, and pharmacovigilance data to characterize HSRs linked to these products, identify risk factors, and examine diagnostic and regulatory challenges. We classified reactions as immediate (< 1-6 h after administrat

Dietary management of functional constipation in disorders of gut-brain interaction - A scoping review.

BACKGROUND: Functional constipation is one of the most prevalent disorders of gut-brain interaction, but available dietary interventions haven't been comprehensively collated. AIMS: The aim of this scoping review was to synthesise the current evidence surrounding dietary-based interventions used to manage functional constipation in adults and to identify any gaps among published dietary management strategies that could warrant further research, namely a systematic review. METHODS: A comprehensive database search was conducted on MEDLINE, CINAHL and Web of Science capturing the notions of 'di

Efficacy and Safety of Over-the-Counter Therapies for Chronic Constipation: An Updated Systematic Review.

INTRODUCTION: Constipation is commonly treated with over-the-counter (OTC) products whose efficacy and safety remain unclear. We performed a systematic review of OTC therapies for chronic constipation and provide evidence-based recommendations. METHODS: We searched PubMed and Embase for randomized controlled trials of ≥4-week duration that evaluated OTC preparations between 2004 and 2020. Studies were scored using the US Preventive Services Task Force criteria (0-5 scale) including randomization, blinding, and withdrawals. The strengths of evidence were adjudicated within each therapeutic cat

Diarrhea in tube-fed hospitalized patients: Feeding formula is not the most common cause.

BACKGROUND AND AIM: Clostridium difficile-associated diarrhea (CDAD) and enteral nutrition (EN)-associated diarrhea are the most common recognized etiologies of nosocomial diarrhea. However, in clinical practice, the data regarding how each etiology contributes to the diarrheal episodes are limited. We identify the causes and factors associated with post-feeding diarrhea. METHODS: Using the data of patients enrolled in "Effect of Psyllium Fiber Supplementation on Diarrhea Incidence in Enteral Tube-Fed Patients: A Prospective, Randomized, and Controlled Trial", the randomized controlled trial

Recommendations for the Evaluation and Management of Inflammatory Bowel Disease With Irritable Bowel Syndrome-Like Symptoms: A Joint Rome Foundation and International Organization for the Study of IBD (IOIBD) Consensus.

BACKGROUND & AIMS: A substantial proportion of persons with inflammatory bowel disease (IBD) in remission continue to experience abdominal pain, altered bowel habits, and bloating that resemble irritable bowel syndrome (IBS). Lack of standardized definitions and evidence-based management strategies leads to diagnostic ambiguity and potentially unnecessary escalation of IBD therapy. A joint Rome Foundation/International Organization for the Study of Inflammatory Bowel Disease Working Team developed consensus recommendations on nomenclature, evaluation, and treatment of IBD with IBS-like symptom

Polyethylene glycol, unique among laxatives, suppresses aberrant crypt foci, by elimination of cells.

OBJECTIVE: Polyethylene glycol (PEG), an osmotic laxative, is a potent inhibitor of colon cancer in rats. In a search for the underling mechanisms, the hypothesis that fecal bulking and moisture decrease colon carcinogenesis was tested. We also investigated the PEG effects on crypt cells in vivo. MATERIAL AND METHODS: Fischer 344 rats (n=272) were injected with the colon carcinogen, azoxymethane. They were then randomized to a standard AIN76 diet containing one of 19 laxative agents (5% w/w in most cases): PEG 8000 and other PEG-like compounds, carboxymethylcellulose, polyvinylpyrrolidone, so

Probiotics, fibre and herbal medicinal products for functional and inflammatory bowel disorders.

UNLABELLED: Functional bowel disorders (FBD), mainly irritable bowel syndrome (IBS) and functional constipation (FC, also called chronic idiopathic constipation), are very common worldwide. Inflammatory bowel disease (IBD), including ulcerative colitis and Crohn's disease, although less common, has a strong impact on patients' quality of life, as well as being highly expensive for our healthcare. A definite cure for those disorders is still yet to come. Over the years, several therapeutic approaches complementary or alternative to traditional pharmacological treatments, including probiotics, p

Conservative treatment of anal fissure: an unselected, retrospective and continuous study.

Three hundred and ninety-three patients who had anal fissures were followed continuously for approximately five years. More than 44 per cent of them were cured nonsurgically within a four-to-eight-week period. There was an 8 per cent complication rate, consisting of abscesses and fistulas, necessitating surgical treatment. The recurrence rate following healing was 27 per cent, but a third of these patients had recurrent fistulas that healed in response to further conservative treatment. The authors feel that conservative treatment of anal fissure is justified unless there is advanced anal sten

Ispagula powder: an allergen in the work environment.

Bulk laxatives based on ispagula powder have given no known allergic side effects when administered perorally. Twenty-seven out of 64 workers in a pharmaceutical factory developed rhinitis, conjunctivitis or asthma when engaged in the packing of ispagula husks, an operation which gives rise to dust. The symptoms are accounted for by a type 1 allergy acquired from ispagula husks. This allergy can be demonstrated by intracutaneous and provocation tests. Occupational health measures were taken which reduced the dust content to a minimum; thereafter only one new case of sensitization was recorded

Type of Dietary Fiber Is Associated with Changes in Ileal and Hindgut Microbial Communities in Growing Pigs and Influences In Vitro Ileal and Hindgut Fermentation.

BACKGROUND: The degree of ileal organic matter (OM) fermentation appears to be comparable to hindgut fermentation in growing pigs. OBJECTIVES: This study aimed to determine if dietary fiber sources with known different total gastrointestinal tract (GIT) fermentability in humans affect ileal and hindgut microbial communities and ileal fermentation in growing pigs used as an animal model for human adults. METHODS: Male pigs (21 kg bodyweight; 9 wk old; PIC Camborough 46 × PIC boar 356L; n = 8/diet) were fed for 42 d a diet containing cellulose (CEL, low fermentability) as the sole fiber source

Efficacy and complications of polyethylene glycols for treatment of constipation in children: a meta-analysis.

Constipation is a common childhood complaint. In 90% to 95% of children, constipation is functional, which means that there is no objective evidence of an underlying pathological condition. Polyethylene glycol (PEG or macrogol) solution is an osmotic laxative agent that is absorbed in only trace amounts from the gastrointestinal tract and routinely used to treat chronic constipation in adults. Here, we report the results of a meta-analysis of PEG-based laxatives compared with lactulose, milk of magnesia (magnesium hydroxide), oral liquid paraffin (mineral oil), or acacia fiber, psyllium fiber,
