OBJECTIVE: The objective of this article is to review the prospect of treating migraine with sphenopalatine ganglion (SPG) neurostimulation.
BACKGROUND: Fuelled by preliminary studies showing a beneficial effect in cluster headache patients, the potential of treating migraine with neurostimulation has gained increasing interest within recent years, as current treatment strategies often fail to provide adequate relief from this debilitating headache. Common migraine symptoms include lacrimation, nasal congestion, and conjunctival injection, all parasympathetic manifestations. In addition, stud

The trigeminal nerve is the largest cranial nerve with both a sensory and motor function. MRI is an excellent technique to evaluate the trigeminal nerve. We report the MRI findings and illustrate the normal and diseased trigeminal nerve at Meckel's cave, the cavernous sinus, the skull base foramina, the pterygopalatine fossa and the peripheral branches.

A 2-year-old, male Weimaraner presented with acute-onset nonaxial exophthalmos with dorsal deviation of the left globe. Periorbital swelling, pain and a profuse purulent ocular discharge were present on the left side. A draining sinus tract was present in the left ventral conjunctival fornix and another in the left pterygopalatine fossa. The right eye was normal on complete ophthalmic examination. Orbital ultrasonography revealed large, double, linear, parallel echogenic bands with shadowing present in the ventrolateral aspect of the left orbit suggestive of a foreign body. Deformation of the

We present the case of a progressive enophthalmos, 23 years after an orbital floor fracture treated conservatively. Imaging demonstrated orbital fat herniation into the pterygopalatine fossa and a collapse of the maxillary sinus walls. We hypothesise that some "silent sinus syndrome" might be caused by an aspiration phenomenon resulting from masticatory muscle movements.

BACKGROUND: Juvenile nasopharyngeal angiofibroma is a rare and highly vascular tumor that affects adolescent males. Although histologically benign, its locally aggressive growth and propensity for significant bleeding make surgical management challenging. Endoscopic techniques have increasingly become the preferred treatment approach.
OBJECTIVE: To evaluate the outcomes of endoscopic surgical treatment using extended medial maxillectomy with wide exposure of the pterygopalatine fossa and to assess factors potentially associated with recurrence in pediatric patients with JNA.
METHODS: A retro

We reviewed the CT findings of four cases of Wegener's granulomatosis that presented as inflammatory masses in the sinonasal cavity or orbit. In the present study, an infiltrative nature and homogeneous texture with contrast enhancement were typical of the masses. In addition, the masses were frequently accompanied by infiltration of the pterygopalatine fossa or destruction of adjacent bone. However, no pathognomonic findings were observed.

Juvenile nasopharyngeal angiofibroma (JNA) is a highly vascular benign tumor that originates in the sphenopalatine foramen and often spreads to adjacent compartments.1 Microsurgical resection with preoperative embolization remains the treatment of choice.2 We present a case of a large JNA involving multiple compartments. The patient is a 20-yr-old male who presented with long-term right nasal congestion. The MRI demonstrated a large enhancing mass that extended from the right nasal cavity and nasopharynx into the right pterygopalatine fossa (PPF), infratemporal fossa (ITF), and parapharyngeal

BACKGROUND AND OBJECTIVES: Maxillary nerve block has traditionally been performed by using external anatomic landmarks. However, the classic approach to the nerve may be confounded because of anatomic variability. We describe a technique for the block using the suprazygomatic route guided by computed tomography (CT).
CASE REPORT: A 90-year-old woman had a 30-year history of episodic pain in her right maxillary region. We attempted to block the maxillary nerve with classic technique, but we could not identify the nerve by eliciting paresthesia. In addition, bleeding was noted after repeated at

The aim of the study was to present consecutive stages of the extended subtemporal approach (ESA). Seven simulations of ESA were performed on non-fixed human cadavers without any known pathologies in the head and neck. The consecutive stages of the procedure were documented with photographs and schemes. The starting point for ESA is osteotomy of the zygomatic arch and craniectomy including the greater wing of the sphenoid bone. Dislocation or removal of subtemporal fossa contents allows one to penetrate its inside and related structures. Additional widening of inspection allows osteotomy of th

INTRODUCTION: The maxillary division of the trigeminal nerve is a purely sensory nerve that originates in the cranial cavity from the trigeminal ganglion. It leaves the cranial cavity through the foramen rotundum to enter the pterygopalatine fossa. The maxillary nerve enters the infraorbital fissure and gives off its zygomatic and posterior superior alveolar branches.
METHOD: The cranial cavity, infratemporal, and pterygopalatine fossae of 20 formalin-embalmed human cadavers of South Indian origin were dissected bilaterally to expose the maxillary nerve, and its morphology and morphometric di
