The significance of multidisciplinary team meetings in diagnosing and managing childhood interstitial lung disease within the RespiRare network.

INTRODUCTION: Childhood Interstitial Lung Disease (chILD) represents a rare and severe group of diseases for which the etiologic workup, classification, and management remain a challenge for most pediatric pulmonologists. In France in 2018, the RespiRare network established the first multidisciplinary team meetings (MDTm) dedicated to chILD. This study aims to investigate the impact of MDTm in chILD diagnosis and management as well as user satisfaction. METHODS: The MDTm took place on a monthly basis through video conferences. The participants consisted of a quorum and included pediatric pulm

Cryobiopsy in the Diagnosis of Pulmonary Vein Stenosis After Catheter Ablation of Atrial Fibrillation: A Case Report.

Pulmonary vein stenosis-a complication of atrial fibrillation ablation unfamiliar to most pulmonologists-was identified in a 42-year-old man who presented with haemoptysis. Initial bronchoscopy at a previously visited hospital revealed a central airway lesion, but biopsy was declined because of its haemorrhagic nature. Repeat bronchoscopy at our hospital showed oedematous changes and vascular proliferation in the left main bronchus, suggesting adenoid cystic carcinoma. However, cryobiopsy excluded malignancy and suggested a vascular malformation with thickened, irregularly dilated muscular ves

Ergonomics of bronchoscopy: good advice or a pain in the neck?

Interventional pulmonologists require a unique set of skills including precise motor abilities and physical endurance, but surprisingly the application of ergonomic principles in the field of bronchoscopy remains limited. This is particularly intriguing when considering the significant impact that poor ergonomics can have on diagnostic aptitude, income potential and overall health. It is therefore imperative to provide comprehensive education to physicians regarding the significance of ergonomics in their work, especially considering the introduction of advanced diagnostic and therapeutic proc

Indications, procedural standards, and training requirements for flexible nasolaryngoscopy in pulmonology - a Delphi study protocol.

Flexible nasolaryngoscopy (FNL) is a well-established, minimally invasive procedure for assessing the nasal passages, nasopharynx, and larynx. Its role in pulmonology is expanding, particularly for evaluating vocal cord dysfunction/inducible laryngeal obstruction, chronic cough, and other upper airway conditions relevant to the "united airways" model of care. However, no consensus-based guidance currently exists on the indications, procedural standards, or training requirements for FNL performed by pulmonologists. This protocol describes a modified two-round Delphi study that will survey an in

Comorbidity of eosinophilic chronic rhinosinusitis in chronic eosinophilic pneumonia.

We aimed to elucidate details of comorbid chronic rhinosinusitis (CRS) in chronic eosinophilic pneumonia (CEP) under the collaboration between otolaryngologists and pulmonologists in a prospective study. The CEP diagnosis was performed by pulmonologists based on clinical symptoms, laboratory findings, and/or eosinophilia detected in bronchoalveolar lavage. All patients were referred to otolaryngologists before undergoing oral corticosteroid treatment for CEP. Ten CEP cases visited to otolaryngologists. All cases showed bilateral sinonasal inflammation in computed tomography (CT), indicating co

Chronic obstructive pulmonary disease features in patients visiting primary respiratory clinic and tertiary hospital in Japan.

Pulmonologists in primary care clinics are positioned between those in tertiary hospitals and general practitioners in clinics and are anticipated to promote hospital-clinic collaboration for patients with chronic obstructive pulmonary disease (COPD). However, the clinical features of patients in primary respiratory clinics are unclear. This multicenter study cross-sectionally compared the characteristics of patients with COPD in a respiratory clinic (n = 69) with those in a university hospital (n = 124). More patients visited the clinic for symptom relief without a referral, whereas more visi

Respiratory disorders in the Middle East: a review.

The spectrum of pulmonary pathology in the Middle East is as versatile as its civilizations and cultures. In this review, we outline the key challenges confronting pulmonologists in the Middle East. We shed light on the diverse conditions commonly encountered in the region, from the centuries-old illnesses of tuberculosis, to contemporary problems such as lung complications from chemical warfare. We specifically highlighted unique aspects related to respiratory illnesses in the Middle East, for example, climate factors in the desert region, cultural habits, for example, water-pipe smoking and

Anesthesia and anesthesiologist concerns for bronchial thermoplasty.

Bronchial thermoplasty (BT) is an upcoming treatment for patients with asthma refractory to traditional pharmacotherapy. BT is an invasive procedure which carries a risk of coughing, wheezing, bronchospasm, and laryngospasm during and after the procedure. Some of these complications can be minimized using better anesthetic techniques during BT. We hereby report a case of a 63-year-old female with poorly controlled asthma posted for BT done under general anesthesia (GA) with supraglottic device. GA provides better working conditions for pulmonologists when compared with sedation. But still ther

[Thoracic drainage tubes].

The invasive procedures handled by the pulmonologist in the diagnosis of lung disease have greatly expanded over the last decade (transbronchial, percutaneous or thoracoscopic pulmonary biopsy and needle aspiration). The growing number of intensive care units with facilities in mechanical ventilation and hemodynamic support has also modified the approach of the critically ill patient. Pneumothorax and hemothorax are well-known complications related to these diagnostic and therapeutic techniques. Other areas of interest and up-to-date are diagnostic thoracoscopy in pleural disease, chemical ple

Evaluation and management of exercise-induced dyspnea in otherwise healthy adolescents and young adults: A critical review.

Dyspnea on exertion in otherwise healthy adolescents and young adults is often brought to the attention of a pediatric pulmonologist when it interferes with an athletic activity. Assessment of the cause and management has been controversial. Skilled pediatric pulmonologists may suspect the cause from a careful history, but a more definite diagnosis is needed to provide the most appropriate management. Suspecting that laryngeal obstruction is the major cause, continuous laryngoscopy during exercise has been proposed. However, that method tends to over diagnose laryngeal obstruction and does not

Geographical, institutional and workplace variability in genetic evaluation, screening practices, multidisciplinary care, and lung transplantation management for familial interstitial lung disease.

BACKGROUND: Advances in genetics have improved the understanding of risk factors for interstitial lung diseases (ILDs). However, geographical and institutional differences in genetic evaluation may affect clinical practice. We aimed to investigate how such variability influences access to genetic testing and screening, awareness of telomerase mutation-related symptoms, and the management of lung transplantation in familial ILD. METHODS: An expert panel of pulmonologists working in the field of ILD developed an international survey targeting pulmonologists involved in ILD care. The survey comp

Prevalence of Asthma Characteristics in COPD Patients in a Dutch Well-Established Asthma/COPD Service for Primary Care.

PURPOSE: Primary care COPD guidelines indicate that COPD patients with asthma characteristics should be treated as having asthma. This study aims to describe the prevalence of asthma characteristics in patients with a pulmonologist-confirmed working diagnosis of COPD or ACO. PATIENTS AND METHODS: This retrospective cross-sectional study used real-life data (collected between 2007 and 2017) from a Dutch asthma/COPD-service, a structured web-based system in which pulmonologists support general practitioners in their diagnosis of patients with suspicion of obstructive lung disease. The prevalenc

Transesophageal approach to lung, adrenal biopsy and fiducial placement using endoscopic ultrasonography (EUS): An interventional pulmonology experience. Initial experience of the UCSF-FRETOC (fresno tracheobronchial & oesophageal center) study group.

BACKGROUND: Routine lung cancer surveillance has resulted in early detection of pulmonary nodules and masses. Combined endobronchial ultrasound (EBUS) and trans-esophageal endoscopic ultrasound (EUS) are approved methods for sampling lymph nodes or masses. Furthermore, EUS allows for adrenal sampling as part of staging, and can assist with fiducial placement for stereotactic body radiation therapy (SBRT). OBJECTIVES: Promote use of EUS by interventional pulmonologists in the United States when diagnosing and staging lung cancer or when placing fiducials. METHODS: All patients undergoing EUS

The "Flolan Rash": An Impressively Asymptomatic and Non-Inflammatory Eruption Following Long-Term Epoprostenol Therapy.

Epoprostenol (Flolan) is a last-resort intravenous medication for the treatment of severe pulmonary arterial hypertension (PAH). Cutaneous adverse events of Flolan are well-known by pulmonologists, though lacking in dermatologic literature. We report a near erythrodermic appearing, yet asymptomatic eruption lasting 10 years in a woman with end-stage PAH treated with long-term intravenous epoprostenol. Non-pruritic, blanching, erythematous papules coalescing to plaques surrounded by a hypopigmented halo encompassed her entire torso, as well as bilateral upper and lower extremities. Additional f
