Eye movement abnormalities in essential tremor may indicate cerebellar dysfunction.

Experimental and clinical data indicate that the cerebellum is involved in the pathophysiology of advanced stages of essential tremor (ET). The aim of this study was to determine whether a dysfunction also affects cerebellar structures involved in eye movement control. Eye movements of 14 patients with ET and 11 age-matched control subjects were recorded using the scleral search-coil technique. Vestibular function was assessed by electro-oculography. Eight ET patients had clinical evidence of intention tremor (ET(IT)); six had a predominantly postural tremor (ET(PT)) without intention tremor.

Saccadic dysmetria is similar in patients with a lateral medullary lesion and in monkeys with a lesion of the deep cerebellar nucleus.

Some of the clinical hallmarks of lateral medullary infarction--Wallenberg's syndrome--are saccadic dysmetria, smooth pursuit deficit, and lateropulsion of the body. Similar movement disorders are seen in monkeys after local unilateral injection of GABAergic drugs in the caudal fastigial nucleus of monkeys. These include an ipsilateral saccadic hypermetria and a contralateral saccadic hypometria as well as cogwheel smooth pursuit eye movements toward the contralateral side and an ipsiversive lateropulsion of the body. It was previously suggested that the lateral medullary infarction causes a l

A common mechanism modulates saccade timing during pursuit and fixation.

Smooth pursuit is punctuated by catch-up saccades, which are thought to automatically correct sensory errors in retinal position and velocity. Recent studies have shown that the timing of catch-up saccades is susceptible to cognitive modulation, as is the timing of fixational microsaccades. Are the timing of catchup and microsaccades thus modulated by the same mechanism? Here, we test directly whether pursuit catch-up saccades and fixational microsaccades exhibit the same temporal pattern of task-related bursts and subsidence. Observers pursued a linear array of 15 alphanumeric characters that

Efficient saccade enhances performance in clinical dynamic visual acuity test.

PURPOSE: To investigate the eye movement pattern across different speeds during clinical dynamic visual acuity (DVA) tests and their relationship with dynamic visual performance. METHODS: Twenty young adults with normal or corrected-to-normal visual acuity underwent clinical DVA tests at 20, 40, 60, and 80 degrees per second (dps). Eye movement was recorded using a table-mounted eye tracker. Smooth pursuit- and saccade-related parameters were calculated from the gaze trace, and the relationships between eye movement parameters and dynamic visual performance were analyzed. RESULTS: DVA was si

Visual pursuit over textured backgrounds in different depth planes.

Several investigators have reported that voluntary pursuit of a moving object is less smooth when it moves over a textured background compared with when it moves over a dark background. Furthermore, when a person fixates a stationary target on a moving background, microdrifts of the eyes occur in the direction of motion of the background. These two facts suggest that OKN is not completely switched off during voluntary pursuit or fixation. Howard and Simpson (1989) found that optokinetic nystagmus (OKN) is only weakly evoked by moving stimuli which are out of the plane of convergence. This led

Eye movement abnormalities in spinocerebellar ataxia type 17 (SCA17).

BACKGROUND: Spinocerebellar ataxia type 17 (SCA17) is associated with an expansion of CAG/CAA trinucleotide repeats in the gene encoding the TATA-binding protein. In this quantitative characterization of eye movements in SCA17 mutation carriers, we investigated whether eye movement abnormalities originate from multiple lesion sites as suggested by their phenotypic heterogeneity. METHODS: Eye movements (saccades, smooth pursuit) of 15 SCA17 mutation carriers (mean age 36.9 years, range 20 to 54 years; mean disease duration 7.3 years, range 0 to 20 years; 2 clinically unaffected, 13 affected) w

Purkinje cells of the cerebellar dorsal vermis: simple-spike activity during pursuit and passive whole-body rotation.

To track a slowly moving object during whole body rotation, smooth-pursuit and vestibularly induced eye movements must interact to maintain the accuracy of eye movements in space (i.e., gaze), and gaze movement signals must eventually be converted into eye movement signals in the orbit. To understand the role played by the cerebellar vermis in pursuit-vestibular interactions, in particular whether the output of the vermis codes gaze-velocity or eye-velocity, we examined simple-spike activity of 58 Purkinje (P-) cells in lobules VI-VII of head-stabilized Japanese monkeys that were trained to el

Ocular motor abnormalities in Wallenberg's lateral medullary syndrome.

The ocular motor abnormalities that commonly occur in Wallenberg's lateral medullary syndrome are often unappreciated. These abnormalities include signs of dysfunction of ocular alignment (skew deviation, ocular tilt reaction, and environmental tilt), various types of nystagmus, smooth pursuit and gaze-holding abnormalities (eye deviation, ipsipulsion or lateropulsion, and impaired contralateral pursuit), and saccadic abnormalities (ipsipulsion and torsipulsion). These impairments of ocular motor control and their proposed mechanisms are discussed.

Eye movement abnormalities in essential tremor.

Essential tremor (ET) is the most prevalent movement disorder, characterized mainly by an action tremor of the arms. Only a few studies published as yet have assessed oculomotor abnormalities in ET and their results are unequivocal. The aim of this study was to assess the oculomotor abnormalities in ET patients compared with the control group and to find the relationship between oculomotor abnormalities and clinical features of ET patients. We studied 50 ET patients and 42 matched by age and gender healthy controls. Saccadometer Advanced (Ober Consulting, Poland) was used to investigate reflex

External Dynamic Visual Noise Matched to the Appearance of Visual Snow Fails to Reproduce Key Perceptual Properties of the Phenomenon.

PURPOSE: Patients with visual snow (VS) perceive a disturbance resembling analog television noise while basic visual measures such as visual acuity remain unaffected. This study investigated whether external dynamic visual noise perceptually matched to VS (simulated VS) reproduces the motion percept of VS during smooth pursuit and whether the subjective awareness of low-contrast simulated VS differed between the VS participants and controls. METHODS: We compared 35 VS participants with 30 healthy controls. Participants adjusted the size, density, flicker rate, and luminance polarity of a high

Supranuclear eye movement dysfunction in mitochondrial myopathy with tRNA(LEU) mutation.

A patient with multiple neurological deficits and biopsy-proven mitochondrial myopathy with mutation of tRNA(LEU) at nucleotide 3243 was referred for eye movement evaluation. He had restricted range of voluntary motions in all directions and full range of eye movements on passive rotation of head while fixating a visual target. Eye movement recordings revealed decreased horizontal and vertical saccadic velocities and markedly decreased smooth pursuit gain in both directions. The vestibulo-ocular reflex showed gain abnormalities with many saccadic intrusions on the smooth reflex response. His b

Vermal infarct with pursuit eye movement disorders.

Severe deficits of foveal smooth pursuit and optokinetic nystagmus in all directions were electro-oculographically recorded in an 80 year old woman. Magnetic resonance imaging (MRI) showed an infarct involving the postero-inferior part of the vermis (lobules VI to X) and a portion of the left cerebellar hemisphere, with apparent preservation of the flocculus and the brainstem. The role of the vermal lesion in these pursuit eye movement disorders is discussed.

Association of acute vestibular/ocular motor screening scores to prolonged recovery in collegiate athletes following sport-related concussion.

INTRODUCTION: Researchers have yet to explore the association of acute (i.e., within 3 days) post-injury VOMS scores and length of time until clearance to return to play in collegiate athletes. OBJECTIVE: The purpose was to determine if individual Vestibular/Ocular Motor Screening (VOMS) component symptom scores (e.g., smooth pursuits, convergence, vestibular-ocular reflex) can predict clearance to return to activity from a sport-related concussion. METHODS: Pre-season demographic and medical history and post-injury VOMS testing were collected (n = 79) on collegiate student-athletes followin

Overlap of saccadic and pursuit eye movement systems in the brain stem reticular formation.

Recent physiological studies have suggested that there are several sites of interaction between the neural pathways that control saccadic eye movements and those that control visual pursuit movements. To address the question of saccade/pursuit interaction from a neuroanatomical point of view, we have studied the connections from the smooth and saccadic eye movement subregions of the frontal eye field (FEFsem and FEFsac, respectively) to the rostral interstitial nucleus of the medial longitudinal fasciculus (riMLF) in four Cebus apella monkeys. The riMLF has traditionally been considered to be

Repeatability, reliability, and stability of eye movement measurements in Parkinson's disease, cerebellar ataxia, and healthy adults.

INTRODUCTION: Eye movements have been proposed as biomarkers to track disease progression and treatment effects in neurological diseases. Before such variables are used in the clinic or in drug trials, properties such as measurement error must be documented. In this study, we assessed repeatability, reliability, and stability of fixation, smooth pursuit, and saccade measurements in patients with Parkinson's disease, cerebellar ataxia, and healthy adults. METHODS: Fixation, smooth pursuit, and saccade metrics were measured in 16 patients with Parkinson's disease, 16 patients with ataxia, and 2

Damage to the reticulotegmental nucleus of the pons in spinocerebellar ataxia type 1, 2, and 3.

BACKGROUND: The reticulotegmental nucleus of the pons (RTTG) is among the precerebellar nuclei of the human brainstem. Although it represents an important component of the oculomotor circuits crucial for the accuracy of horizontal saccades and the generation of horizontal smooth pursuits, the RTTG has never been considered in CAG repeat or polyglutamine diseases. METHODS: Thick serial sections through the RTTG of 10 patients with spinocerebellar ataxias (SCAs) assigned to the CAG repeat or polyglutamine diseases (2 SCA-1 patients, 4 SCA-2 patients, and 4 SCA-3 patients) were stained for neuro

Following Randolph Blake's furrow further.

In 1992, Randolph Blake, in collaboration with Robert Cormack and Eric Hiris, reported a strong deviation in perceived direction for a target moving over an oblique, static grating. Here we follow up on this effect, subsequently called the furrow illusion, to determine its origin. We find, unlike Cormack et al., that it is influenced by the luminance of the target and that it does not survive smooth pursuit of a moving fixation that stabilizes the target on the retina. We also introduce an inverted version of the furrow stimulus with the static grating visible only within the moving target rat

Eye movements in visual impairment.

This Special Issue describes the impact of visual impairment on visuomotor function. It includes contributions that examine gaze control in conditions associated with abnormal visual development such as amblyopia, dyslexia and neurofibromatosis as well as disorders associated with field loss later in life, such as macular degeneration and stroke. Specifically, the papers address both gaze holding (fixation), and gaze-following behavior (single saccades, sequences of saccades and smooth-pursuit) that characterize active vision in daily life and evaluate the influence of both pathological and si

Horizontal gaze nystagmus: a review of vision science and application issues.

The Horizontal Gaze Nystagmus (HGN) test is one component of the Standardized Field Sobriety Test battery. This article reviews the literature on smooth pursuit eye movement and gaze nystagmus with a focus on normative responses, the influence of alcohol on these behaviors, and stimulus conditions similar to those used in the HGN sobriety test. Factors such as age, stimulus and background conditions, medical conditions, prescription medications, and psychiatric disorder were found to affect the smooth pursuit phase of HGN. Much less literature is available for gaze nystagmus, but onset of nyst

Eye movement disorders after frontal eye field lesions in humans.

Eye movements were recorded electro-oculographically in three patients with a small ischemic lesion affecting the left frontal eye field (FEF) and in 12 control subjects. Reflexive visually guided saccades (gap and overlap tasks), antisaccades, predictive saccades, memory-guided saccades, smooth pursuit and optokinetic nystagmus (OKN) were studied in the three patients. Staircase saccades and double step saccades were also studied in one of the three patients. For both leftward and rightward saccades, latency in the overlap task (but not in the gap task) and that of correct antisaccades and of
