REM sleep behavior disorder, autonomic dysfunction and synuclein-related neurodegeneration: where do we stand?

INTRODUCTION: From newfound parasomnia to a marker of future synucleinopathy, since its first description in 1986, REM sleep behavior disorder (RBD) has been systematically tackled from virtually many viewpoints in basic, translational, and clinical studies. The time delay between RBD and synucleinopathy onset offers an exceptional window for observation and design of neuroprotective trials. In the last few years, research has focused on characterizing possible differences within RBD patients in order to draw potential profiles more or less susceptible to further neurodegeneration. Abnormal occipital event-related potentials in Parkinson's disease with concomitant REM sleep behavior disorder.

BACKGROUND: Rapid eye movement sleep behavior disorder is found in 33-46% of patients with Parkinson's disease and was shown to be associated with cognitive deficits. Our goal was to improve our understanding of the role of this sleep disorder in cerebral dysfunction occurring in Parkinson's disease using a visual cognitive task and event-related potentials. METHODS: Sixteen patients with Parkinson's disease and rapid eye movement sleep behavior disorder, 15 patients with Parkinson's disease without rapid eye movement sleep behavior disorder and 16 healthy control subjects were included. Relationship between (123)I-MIBG scintigrams and REM sleep behavior disorder in Parkinson's disease.

BACKGROUND: Uptake of (123)I-labeled meta-iodobenzylguanidine (MIBG) in myocardial scintigrams has been shown to be as low in patients with idiopathic RBD as in Parkinson's disease (PD) patients. AIM FOR STUDY: To clarify whether the existence of RBD accelerates autonomic dysfunction in PD, we investigated the association between MIBG scintigraphic findings and RBD measures among non-dementia PD patients. SUBJECTS & METHODS: We conducted clinical interviews to assess REM sleep behavior disorder (RBD) symptoms, and performed polysomnograms (PSG) recordings and MIBG scintigrams on 49 PD patient

Relationship between visual hallucinations and REM sleep behavior disorder in patients with Parkinson's disease.

OBJECTIVES: REM sleep behavior disorder (RBD) has been documented to precede or to co-occur with Parkinson's disease (PD). Parkinson's disease is one of the most common neurological conditions associated with visual hallucinations. Cognitive dysfunction is present in PD, even at the early stages of these diseases. In this study we aimed to investigate the relationship between visual hallucinations and RBD in patients with idiopathic Parkinson's disease (IPD). Additionally, we evaluated the association of the cognition and the pattern of cognitive impairment with VHs and RBD, effects of factors

Association of REM sleep behavior disorder and neurodegenerative disease may reflect an underlying synucleinopathy.

Our objective was to examine whether rapid eye movement (REM) sleep behavior disorder occurs in disproportionally greater frequency in multiple system atrophy (MSA), Parkinson's disease (PD), and dementia with Lewy bodies (DLB), collectively known as the synucleinopathies, compared to other nonsynucleinopathy neurodegenerative disorders. In study 1, we reviewed the clinical records of 398 consecutive patients evaluated at Mayo Clinic Rochester for parkinsonism and/or cognitive impairment. The frequency of suspected and polysomnogram (PSG)-confirmed REM sleep behavior disorder (RBD) among subje

Detecting isolated REM sleep behavior disorder at home using a lower-back wearable sensor.

Isolated REM Sleep Behavior Disorder (iRBD) is a strong predictor of neurodegenerative diseases, particularly synucleinopathies. Current diagnosis requires overnight video-polysomnography (vPSG) in sleep laboratories. Limited access to vPSG and differences in sleep habits result in diagnostic challenges. Here we aimed to evaluate the feasibility of identifying iRBD from a lumbar-mounted wearable sensor in the home setting and explored night-to-night variability. Seventy-three participants (15 iRBD, 58 controls) underwent vPSG, followed by six nights of wearing a lower-back inertial measurement

Dynamic thalamocortical functional connectivity disruptions in Parkinson's disease with probable REM sleep behavior disorder.

As the symptom of rapid eye movement sleep behavior disorder (RBD) is paroxysmal during sleeping, the disruptions of dynamic functional connectivity (FC) between thalamic subnuclei and the cortex may play a critical role in Parkinson's disease (PD). A total of 35 PD patients with probable RBD (PD-pRBD) and 40 PD patients without probable RBD (PD-npRBD) and 41 healthy controls were enrolled. All participants underwent functional magnetic resonance imaging scan and clinical assessment. Altered dynamic FC between bilateral 14 thalamic nuclei and cortex was calculated. Although the RBDSQ demonstra

Physiological Signatures Derived from Wrist Accelerometry Improve Detection of Isolated REM Sleep Behavior Disorder.

Isolated REM sleep behavior disorder (iRBD) is the strongest prodromal marker of α-synucleinopathies, but polysomnography is impractical for population-scale screening and questionnaires lack sufficient specificity. Beyond nocturnal movement, recently published models can derive sleep staging and other physiological signatures from raw high-resolution wrist accelerometry alone. Across four cohorts (366 subjects: 95 iRBD, 271 controls; 5,804 nights), we extracted 876 features spanning sleep macrostructure, stage-probability dynamics, motor activity, and cardiorespiratory variation. Probable REM sleep behavior disorder marks a less levodopa-responsive phenotype in Parkinson's disease.

BACKGROUND: Probable REM sleep behavior disorder (pRBD) is a clinically relevant phenotype in Parkinson's disease (PD), commonly associated with greater non-motor burden and a more diffuse disease profile. However, its independent clinical correlates remain incompletely defined, particularly regarding acute dopaminergic responsiveness. OBJECTIVE: To identify independent clinical correlates of pRBD in a comprehensively phenotyped PD cohort, with particular attention to acute levodopa responsiveness. METHODS: In this retrospective cross-sectional study, 468 patients with PD underwent multidime

The Many Faces of REM Sleep Behavior Disorder. Providing Evidence for a New Lexicon.

BACKGROUND: People with idiopathic/isolated REM sleep behavior disorder (iRBD) are highly heterogeneous, showing mild motor, cognitive, and dysautonomia symptoms. The aim of this study is to unveil the clinical heterogeneity of iRBD with a specific reference to overlapping features with prodromal Parkinson's disease (pPD) and prodromal dementia with Lewy bodies (pDLB) labels. METHODS: People with a polysomnography-confirmed diagnosis of iRBD were enrolled and followed over time. At baseline, pPD and pDLB criteria were assessed. RESULTS: Among the 285 iRBD people (68.2 ± 7.6 years, 81% males)

EEG alpha peak frequency: cognitive impairment severity marker in isolated REM sleep behavior disorder.

Isolated REM sleep behavior disorder (iRBD) is a prodrome of α-synucleinopathies like Parkinson's disease (PD) and dementia with Lewy bodies (DLB). The best marker for prediction of lead time to phenoconversion is reduced striatal dopamine transporter (DaT)-binding, unavailable for most patients. This study investigated EEG alpha peak frequency (APF) slowing - an established marker of cognitive deterioration - as a severity marker in iRBD patients. In 320 patients clinically suspected of RBD 3-night-polysomnography was performed. After exclusion of 131 patients, mainly due to psychotropic medi

Association of early-onset psychiatric disorders with REM sleep behavior disorder - A retrospective study.

BACKGROUND: REM sleep behaviour disorder (RBD) is a known predictor of the subsequent development of neurodegenerative diseases, particularly Parkinson's disease and other alpha synucleinopathies. RBD has also been found to be common among children with other psychiatric disorders such as anxiety, depression, and ADHD. OBJECTIVE: This retrospective study aims to analyze the prevalence of early-onset psychiatric disorders among patients referred for RBD to our sleep laboratory. Our hypothesis is that early-onset psychiatric disorders are more common in patients with polysomnographically confir

Microstructural alterations of the hypothalamus in Parkinson's disease and probable REM sleep behavior disorder.

BACKGROUND: Whether there is hypothalamic degeneration in Parkinson's disease (PD) and its association with clinical symptoms and pathophysiological changes remains controversial. OBJECTIVES: We aimed to quantify microstructural changes in hypothalamus using a novel deep learning-based tool in patients with PD and those with probable rapid-eye-movement sleep behavior disorder (pRBD). We further assessed whether these microstructural changes associated with clinical symptoms and free thyroxine (FT4) levels. METHODS: This study included 186 PD, 67 pRBD, and 179 healthy controls. GBA1 rs3115534 Is Associated with REM Sleep Behavior Disorder in Parkinson's Disease in Nigerians.

BACKGROUND: Rapid eye movement (REM) sleep behavior disorder (RBD) is an early feature of Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Damaging coding variants in Glucocerebrosidase (GBA1) are a genetic risk factor for RBD. Recently, a population-specific non-coding risk variant (rs3115534) was found to be associated with PD risk and earlier onset in individuals of African ancestry. OBJECTIVES: We aimed to investigate whether the GBA1 rs3115534 PD risk variant is associated with RBD in persons with PD. METHODS: We studied 709 persons with PD and 776 neurologically healthy co

Cardiovascular Risk Factors and Phenoconversion to Neurodegenerative Synucleinopathies in Idiopathic REM Sleep Behavior Disorder.

Several studies have suggested that atherosclerotic diseases and diabetes may be risk factors for α-synucleinopathies. This prospective cohort study evaluated whether cardiovascular diseases and metabolic risk factors alter the rate or type of phenoconversion from idiopathic/isolated REM sleep behavior disorder (iRBD) to parkinsonism or dementia. Polysomnography-confirmed iRBD patients recruited between 2004 and 2020 were followed annually. Baseline history of cardiovascular disorders, hypertension, hypercholesterolemia, and diabetes were compared among patients who developed outcomes versus t

REM sleep behavior disorder with predominant nightmares in a patient with ischemic pontine lesions.

UNLABELLED: Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia typically associated with synucleinopathy and evolving to neurodegenerative disorders. RBD is caused by impairment of brainstem nuclei controlling REM sleep muscle atonia. Rarely, focal lesions of the brainstem can cause secondary RBD. We present the case of a 74-year-old patient, previously evaluated at age 70 years for insomnia and periodic limb movements during sleep, who then rapidly developed unpleasant dreams with minor motor behavior, affecting his sleep quality. Polysomnography recorded REM sleep without

Articulatory undershoot of vowels in isolated REM sleep behavior disorder and early Parkinson's disease.

Imprecise vowels represent a common deficit associated with hypokinetic dysarthria resulting from a reduced articulatory range of motion in Parkinson's disease (PD). It is not yet unknown whether the vowel articulation impairment is already evident in the prodromal stages of synucleinopathy. We aimed to assess whether vowel articulation abnormalities are present in isolated rapid eye movement sleep behaviour disorder (iRBD) and early-stage PD. A total of 180 male participants, including 60 iRBD, 60 de-novo PD and 60 age-matched healthy controls performed reading of a standardized passage. The

REM sleep behavior disorder correlates with constipation in de novo Chinese Parkinson's disease patients.

BACKGROUND: Constipation, rapid eye movement sleep behavior disorder (RBD) and hyposmia are common prodromal symptoms of Parkinson's disease (PD), and they may represent two distinct types of disease origin, from the body or the brain. Our study aimed to compare the clinical characteristics of de novo PD patients with and without constipation and identify which prodromal symptoms were associated with constipation. METHODS: A total of 111 de novo, drug-naïve Chinese PD patients were consecutively enrolled from Jan 2017 to Sept 2021. Patients were classified into PD with and without constipatio
