Atypical Multifocal Presentation of Congenital Plunging Ranulas: A Radiological Approach to Diagnosis and Treatment.

Congenital plunging ranulas are pseudocystic lesions affecting the floor of the mouth. This case report presents a two-month-old female with an asymptomatic unilateral cheek swelling, diagnosed with a multifocal congenital plunging ranula. Magnetic resonance imaging, ultrasound, and fluoroscopy revealed an atypical, multiseptated submandibular lesion with multifocal sublingual salivary extensions. Needle aspiration of the submandibular lesions yielded mucoid salivary discharge, confirming the diagnosis. The swelling was significantly reduced and the patient continued to achieve all development

Hidden in Plain Sight: Neglected Congenital Ranula Presenting as Failure to Thrive in an Infant.

Congenital ranula is a rare mucous extravasation cyst arising from the sublingual gland, presenting early in life and potentially causing feeding difficulties and growth failure. We report a two-month-old infant with a large intraoral swelling present since birth, leading to poor weight gain and tongue displacement. Ultrasound confirmed a plunging ranula, and marsupialization was successfully performed. Post-surgery, the infant showed significant improvement in feeding and growth. This case highlights the importance of early oral cavity examination in neonates, as timely identification and int

Surgical management of a large plunging ranula: A case report and review of diagnostic challenges.

INTRODUCTION: Ranulas are mucous cysts that arise from the sublingual gland due to ductal obstruction or trauma. Plunging ranulas, a rare variant, extend into the neck and often present diagnostic challenges. Definitive surgical management is necessary to prevent recurrence and ensure successful outcomes. CASE PRESENTATION: A 20-year-old female presented with a slowly enlarging, painless mass in the floor of her mouth extending into the submandibular region and anterior neck. The mass had progressively increased in size over five years, affecting her appearance and mastication. MRI confirmed

Co-existence of Congenital Epidermoid Cyst and Ranula in a Newborn. Report of a Unique Case.

BACKGROUND: Congenital cystic swellings involving the floor of the mouth include various lesions such as developmental cysts (e.g., dermoid and epidermoid cysts), ranulas, vascular malformations etc. However, coexistence of such conditions, possibly with a cause-and-effect- relationship, is rare. The purpose of this case report is to present a rare case of a congenital epidermoid cyst associated with a mucous retention cyst in a newborn. METHODS: A 6-month-old female infant was referred to an Oral Medicine Clinic in Athens, Greece on October 2019 for evaluation of a swelling at the floor of t

A plunging ranula in a child with holoprosencephaly: a case of unique pathophysiology and difficult airway management.

A ranula is a pseudocyst that originates from the sublingual gland after trauma. Acute cases of ranulas that progress rapidly and cause respiratory distress are rare. Holoprosencephaly is a complex brain malformation caused by incomplete cleavage of the prosencephalon. Children with holoprosencephaly may experience upper airway obstruction due to the associated dentoalveolar malformations and oromotor dysfunctions. We present the case of an eight-year-old female patient with holoprosencephaly and a plunging ranula that manifested as an acute course due to difficult airway management. She requi

Are the patients with anatomic variation of the sublingual/Wharton's duct system predisposed to ranula formation?

OBJECTIVE: To evaluate ranula development according to anatomic variation of the ductal system of sublingual gland (SLG), especially the presence of Bartholin's duct. METHODS: The anatomic variation of SLG duct was prospectively investigated and compared between 55 consecutive patients with ranulas treated by SLG excision (group 1) and another 15 consecutive patients undergoing similar surgeries for other conditions (group 2). The ductal structures of SLGs and submandibular glands (SMG) were also compared between the pediatric patients and adult patients with ranulas. RESULTS: In 32 of 55 pa

Simultaneous bilateral ranula in an edentulous patient. Rare presentation with a brief review of the literature.

A ranula is a large mucocele located in the floor of the mouth. Ranulas is usually a mucous extravasation phenomenon due to injury to sublingual salivary gland. unilateral presentation has been reported in many cases but bilateral presentation and that too in an edentulous patient is quite uncommon. This is a case report of bilateral ranula in an edentulous patient and its management along with a brief review of pertinent literature.

Submandibular cystic hygroma resembling a plunging ranula in a neonate. Review and report of a case.

Cystic hygromas are large lymphangiomas that are most often found in the posterior triangle of the neck and the axilla in children. They are most frequently found before age 2 and may be massive. After upper respiratory infection, they may become infected and enlarged, causing dysphagia and toxemia. The diagnosis can usually be made by history and physical examination and confirmed by biopsy. Treatment is by surgical excision of small lesions and staged debulking excisions in more severe cases. A patient with a cystic hygroma having many clinical characteristics of a plunging ranula is present

Cervical ranula arising from a projection of the sublingual gland through the mylohyoid muscle.

A case of cervical ranula is presented, which originated from a projection of the sublingual gland into the suprahyoid region. A diagnosis of cervical ranula could not be established preoperatively because there was no intraoral involvement of the ranula. A cervical approach was used, and the correct diagnosis was made during the operation.

Plunging Ranula: A Case Report of a Rare Late Complication After Tongue Cancer Surgery.

A plunging ranula is a benign cystic lesion in the neck formed due to mucin extravasated from a salivary gland, most commonly the sublingual gland. Ranulas have been described in association with congenital anomalies, trauma, diseases of the sublingual gland, and HIV; however, rarely, they may result as a complication of various oral and neck surgeries. Here, we report a rare case of plunging ranula that developed in an elderly male as a sequalae to surgery for tongue cancer. The patient had undergone a partial glossectomy with supra-omohyoid neck dissection for tongue carcinoma and nine month

Capnography-guided blind nasal intubation under conscious sedation for airway management in a plunging ranula.

A 22-year man with human immunodeficiency virus ((HIV)-positive status for excision of huge, painless swelling in the floor of the mouth was admitted. The swelling occupied the anterior part of the neck; intraoral part of the swelling was extended downward and anteriorly in the sublingual space deviating the tongue. Fiberoptic intubation has helped immensely in handling many difficult airway scenarios. In low-resource setting, often multiple-use fiberoptic is used due to which fear of risk of cross-contamination from blood, secretions, or suctioning is there. The challenges were to secure the

Congenital Sublingual Dermoid Cyst Mimicking a Recurrent Ranula in a Pediatric Patient: A Case Report.

In pediatrics, sublingual lesions are not a common disease. Due to the similarity in their clinical and radiological features, they present a diagnostic challenge. Despite the advancement in imaging techniques, the accurate preoperative diagnosis of sublingual lesions may fail. In this study, we present a case of a 13-year-old male referred to our surgical department with a recurrent painless mass in the floor of the mouth, which had been previously diagnosed as a ranula and treated at another medical facility. However, the patient experienced a recurrence six months later. Upon examination, a

Sclerotherapy of ranulas with OK-432 - a prospective, randomized, double-blinded placebo-controlled study.

BACKGROUND: Ranula is a rare benign cystic lesion in the floor of the mouth, which can herniate through the mylohyoid muscle and become a plunging ranula. Treatment for ranulas is currently surgical excision of the sublingual gland. Sclerotherapy with OK-432 is a well-established treatment of lymphatic malformations, but not yet thoroughly evaluated on ranulas. Objectives: To evaluate sclerotherapy of ranulas with OK-432 in a randomized double-blinded trial. MATERIALS AND METHODS: 20 patients with plunging or intraoral ranula were randomized to two double-blinded injections with OK-432 or sal

Bleomycin sclerotherapy following doxycycline lavage in the treatment of ranulas: A retrospective analysis and review of the literature.

OBJECTIVE: A ranula is a mucus-filled salivary pseudocyst that forms in the floor of the mouth, commonly arising from the sublingual or submandibular salivary glands following obstruction or trauma. Complete excision of the injured gland and removal of the cyst content is the first-choice therapy, but has the potential for complications related to injury to nearby structures. As such, minimally invasive approaches such as percutaneous sclerotherapy have been investigated. We aim to contribute to the literature by assessing the efficacy and safety of our technique through our experience with 18
