Hypokalaemia after suxamethonium administration.

The effect of suxamethonium on serum potassium levels was studied in 20 black patients undergoing emergency surgery. All patients were resuscitated pre-operatively to achieve a normal pH (range 7.35-7.45), normal electrolyte values (potassium range 3.5-4.5 mmol/l), and an adequate circulating blood volume using the criteria of systolic blood pressure in excess of 100 mmHg and a central venous pressure maintained at more than 2 cm H2O. Suxamethonium (approximately 0.8 mg/kg) was administered during a standard rapid intubation sequence for the induction of anaesthesia. In the adequately resuscit

Suxamethonium-induced hyperkalaemia in a patient with a normal potassium level before rapid-sequence intubation.

The use of suxamethonium (succinylcholine) for rapid-sequence intubation may be limited by hyperkalaemia. Modest pre-induction hyperkalaemia is usually disregarded. We present a patient who underwent emergency surgery for a perforated peptic ulcer after being bedbound for 26 days because of a head injury. Serum potassium level was 4.0 mmol/L. The patient was intubated after injection of sedative and suxamethonium and, about 3 minutes later, developed ventricular arrhythmia. Blood tests during resuscitation showed a serum potassium level of 8.8 mmol/L. Immobilisation, denervation and intra-abdo

Analgesia and Sedation Related to the Use of Long-Acting Neuromuscular Blockers: A Narrative Review.

Rapid sequence intubation is a standard procedure in critical care transport (CCT). Owing to its numerous benefits and clinical uses, rocuronium has become the first-line neuromuscular blocking agent in rapid sequence intubation, supplanting succinylcholine in many transport services. The deferred resumption of muscle activity with rocuronium may delay administering analgesia or sedation after intubation, resulting in awareness while paralyzed. We undertook a narrative review to evaluate this hazard in the existing literature. Numerous studies have revealed that 10% to 50% of intubated emergen

Postintubation Sedation Practices Within Multiple Emergency Departments Across a Large Pediatric Health Care Organization.

OBJECTIVE: Children requiring rapid or standard sequence intubation are at risk of experiencing paralysis without adequate sedation when the duration of neuromuscular blockade exceeds the duration of sedation provided by the induction agent. The objective of this study was to evaluate the rate of appropriately timed postintubation sedation (PIS; defined as the administration of PIS before the clinical effects of the induction agent have dissipated) in patients requiring intubation across multiple emergency department/urgent care sites within a large pediatric health care organization. Rocuronium dosing for rapid sequence intubation: A retrospective analysis in ED and ICU settings.

BACKGROUND: Rocuronium dosing varies in clinical practice, and optimal dosing has yet to be identified. Several studies have investigated differing dosing strategies and have suggested doses ≥1.2 mg/kg may lead to greater intubation success. OBJECTIVES: This study aimed to compare the impact of rocuronium dosing on first attempt intubation success during rapid sequence intubation (RSI) with video laryngoscopy in the emergency department (ED) and intensive care units (ICU). METHODS: This was a single-center, retrospective chart review of adult patients undergoing RSI with rocuronium doses ≤1.

[Modified rapid sequence induction for Caesarian sections : case series on the use of rocuronium and sugammadex].

BACKGROUND: Aspiration is a feared complication of anesthesia and is accompanied by increased morbidity and mortality. Rapid sequence induction (RSI) describes the preferred procedure to perform endotracheal placement of the tubus in emergency cases of patients with an increased risk of aspiration of gastric contents. For more than 50 years RSI has consisted of the application of suxamethonium for neuromuscular blockade because of its fast onset and ultra short duration. Due to the serious side effects of suxamethonium attempts were made to find better alternative neuromuscular blocking drugs,

Videolaryngoscopic Intubation and Difficult Airway Classification (VIDIAC) accuracy in the prediction of first-pass nasal intubation using a video laryngoscope.

BACKGROUND: The Cormack-Lehane (CL) classification and the percentage of glottic opening (POGO) score are used for glottic views during direct laryngoscopy; however, no standard scoring exists for video-laryngoscopy. The Video Laryngoscopic Intubation and Difficult Airway Classification (VIDIAC) model was recently proposed to grade glottic views during video-laryngoscopy. This study aimed to evaluate the diagnostic accuracy of the VIDIAC score in predicting first-pass nasal intubation success using the Touren video laryngoscope in patients undergoing head and neck surgery. METHODS: After ethi

Management of pulmonary aspiration due to undiagnosed achalasia during induction of general anesthesia - A case report.

BACKGROUND: Intraoperative pulmonary aspiration is a rare but potentially fatal complication associated with various risk factors. Preoperative recognition of these risk factors can prevent aspiration events during general anesthesia or facilitate prompt corrective measures in patients experiencing this complication. CASE: A 70-year-old female patient with hypertension underwent bilateral total knee arthroplasty under general anesthesia. Despite using a midnight nothing-per-oral (NPO) protocol, an unpredictable intraoperative aspiration event occurred during anesthesia induction. A detailed e

Efficacy and safety of esketamine for emergency endotracheal intubation in ICU patients: a double-blind, randomized controlled clinical trial.

Emergency endotracheal intubation in critically ill patients are dangerous procedures with a greater risk of severe hypotension The efficacy and safety of esketamine with sympathoexcitatory effects for rapid sequence induction in critically ill patients remain unclear. In this prospective double-blinded randomized controlled trial, adult patients were randomly assigned to receive either esketamine or midazolam/sufentanil admixture for induction. The primary outcomes were the effects of induction with esketamine or midazolam/sufentanil admixture on hemodynamic responses (heart rate (HR) and mea

Apnoeic oxygenation for emergency anaesthesia of pre-hospital trauma patients.

BACKGROUND: Efficient and timely airway management is universally recognised as a priority for major trauma patients, a proportion of whom require emergency intubation in the pre-hospital setting. Adverse events occur more commonly in emergency airway management, and hypoxia is relatively frequent. The aim of this study was to establish whether passive apnoeic oxygenation was effective in reducing the incidence of desaturation during pre-hospital emergency anaesthesia. METHODS: A prospective before-after study was performed to compare patients receiving standard care and those receiving addit

Does the aerosol box accommodate the double-lumen tube intubation?

The aerosol box was widely used to shield healthcare providers from exposure to COVID-19 during single-lumen intubation procedures. However, it has not previously been evaluated for its use in double-lumen tube intubations. This report presents the case of a 25-year-old COVID-19-positive male with a fever who required an emergency thoracotomy for a mediastinal abscess. During the rapid-sequence induction of general anesthesia, an attempt to use the aerosol box for double-lumen tube intubation was made. The attempt faced unique challenges due to the aerosol box's restrictive dimensions and the

The Effect of Propofol on Peri-Induction Hemodynamics and Resuscitation in Operative Penetrating Trauma.

BACKGROUND: Hemorrhaging trauma patients may be disproportionately affected by choice of induction agent during rapid sequence intubation (RSI). Etomidate, ketamine, and propofol are safe in the trauma population-at-large but have not been assessed in patients with ongoing hemorrhage. We hypothesize that in hemorrhaging patients with penetrating injury, propofol deleteriously affects peri-induction hypotension compared to etomidate and ketamine. METHODS: Retrospective cohort study. Primary outcome was the effect of induction agent on peri-induction systolic blood pressure. Secondary outcomes

Gastric tubes and airway management in patients at risk of aspiration: history, current concepts, and proposal of an algorithm.

Rapid sequence induction and intubation (RSII) and awake tracheal intubation are commonly used anesthetic techniques in patients at risk of pulmonary aspiration of gastric or esophageal contents. Some of these patients may have a gastric tube (GT) placed preoperatively. Currently, there are no guidelines regarding which patient should have a GT placed before anesthetic induction. Furthermore, clinicians are not in agreement as to whether to keep a GT in situ, or to partially or completely withdraw it before anesthetic induction. In this review we provide a historical perspective of the use of

Anaesthesia and orphan disease: airway and anaesthetic management in Huntington's disease.

We present a case that highlights the issues surrounding the delivery of a safe general anaesthetic to a patient with Huntington's disease (HD) and bulbar dysfunction. In the case of a 46-year-old patient undergoing laparoscopic percutaneous endoscopic gastrostomy tube insertion, we discuss the rationale behind our chosen method and anaesthetic agents as well as airway issues specific to HD. In a patient whose condition would not allow for an awake fibreoptic intubation, we opted for a modified rapid sequence induction. Special considerations were made with regard to muscle relaxation given th

Evaluation of induction practices for general anesthesia in patients with obesity: A French nationwide online survey.

BACKGROUND: Obesity is a growing public health issue associated with increased peri-induction risk during general anesthesia, particularly airway-related complications. Substantial variability in national and international guidelines may contribute to heterogeneous clinical practice. This study aimed to describe anesthetic induction practices in patients with obesity in France and to explore the influence of body mass index and obesity-related comorbidities on the decision for or against rapid sequence induction. METHODS: This nationwide declarative survey used an anonymous online questionnai

Nasotracheal intubation-extubation-intubation and asleep-awake-asleep anesthesia technique for deep brain stimulation.

BACKGROUND: The asleep-awake-asleep (AAA) technique and laryngeal mask airway (LMA) is a common general anesthesia technique for deep brain stimulation (DBS) surgery. However, the LMA is not always the ideal artificial airway. In this report, we presented our experiences with nasotracheal intubation-extubation-intubation (IEI) and AAA techniques in DBS surgery for Parkinson's disease (PD) patients to meet the needs of surgery and ensure patients' safety and comfort. CASE PRESENTATION: Three PD patients scheduled for DBS surgery had to receive general anesthesia for various reasons. For the fi
