Production of autoantibodies specific for the Fc region of autologous IgG, called rheumatoid factors (RF), is a characteristic finding in patients with rheumatoid arthritis (RA). Using a recently described technique for direct expansion of human T-lymphocytes isolated from small intestine biopsies, we have investigated the local cellular immune response in six patients with B-cell lymphomas of various subtypes. This article describes the development of a simple technique by which the numbers of surface immunoglobulin expressing cells can be analysed by dual fluorescence flow cytometry in samples of whole blood. Peripheral blood lymphocytes from some patients with rheumatoid vasculitis and giant cell arteritis were cytotoxic in vitro towards endothelial cells, but not fibroblasts. Use of cell surface markers and cell density showed that cytotoxicity correlated with numbers of HLA-DR bearing cells, but not with cells bearing CD3, the transferrin receptor, or less dense lymphocytes. We concluded that activated, cytotoxic lymphocytes were present in the peripheral blood of a sub-set of patients with vasculitis.
