Evidence of acute diaphragmatic fatigue in a "natural" condition. The diaphragm during labor.

Acute diaphragmatic fatigue has been experimentally shown to occur in normal healthy subjects and in patients with chronic obstructive pulmonary disease by asking them to modify their pattern of breathing or to breathe against high inspiratory resistances. During the expulsive period of labor women are asked periodically to make strong expulsive efforts and to sustain them isometrically for many seconds; this is likely to produce "natural" diaphragmatic fatigue. To investigate whether this was the case, six women were studied in the delivery room from the moment of the rupture of the amnion un

Innervation of pelvic viscera in the rat. Evoked potentials in nerves to bladder and penis (clitoris).

In 29 rats, responses evoked by pelvic and hypogastric nerve stimulation were recorded from postganglionic nerves to bladder and penis (clitoris). Responses to pelvic nerve stimulation had nonsynapsing and synapsing components. The nonsynapsing component was relatively large in main nerve to penis and small in lateral nerve to penis and nerves to bladder. Pelvic nerve fiber synapsing on pelvic ganglion neurons to bladder had a large subliminal fringe, while fibers synapsing on neurons supplying penis (clitoris) had a small subliminal fringe. Recruitment was greater in nerves to bladder and lat

The Neurophysiological Features Associated with Unilateral Spatial Neglect Recovery: A Scoping Review.

The purpose of this scoping review is to provide updated information on the neural basis and neurophysiological features associated with unilateral spatial neglect (USN) recovery. We applied the Preferred Reporting Systems for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) framework and identified 16 relevant papers from the databases. Critical appraisal was performed by two independent reviewers using a standardized appraisal instrument developed by the PRISMA-ScR. We identified and categorized investigation methods for the neural basis and neurophysiological

Recruitment of cardiac parasympathetic activity: effects of clonidine on cardiac vagal motoneurones, pressure lability, and cardiac baroreflex slope in rats.

BACKGROUND: Association of low cardiac vagal activity and poor outcome is demonstrated in the cardiology setting. This has not been addressed in the postoperative setting. Cardiac vagal motoneurones (CVMs) in the brain stem generate sinus arrhythmia. They may reduce blood pressure (BP) variability ('pressure lability'). An alpha-2 agonist, clonidine, was administered to assess whether cardiac vagal activity could be recruited from a very low baseline activity, increase the sensitivity of the cardiac baroreflex and sinus arrhythmia, and reduce the pressure lability. METHODS: In ventilated anae

Scapular muscle recruitment patterns: trapezius muscle latency with and without impingement symptoms.

BACKGROUND: Altered muscle activity in the scapular muscles is commonly believed to be a factor contributing to shoulder impingement syndrome. However, one important measure of the muscular coordination in the scapular muscles, the timing of the temporal recruitment pattern, is undetermined. PURPOSE: To evaluate the timing of trapezius muscle activity in response to an unexpected arm movement in athletes with impingement and in normal control subjects. STUDY DESIGN: Prospective cohort study. METHODS: Muscle latency times were measured in all three parts of the trapezius muscle and in the mi

Motoneuron pool excitability of hemiplegic patients: assessing recovery stages by using H-reflex and M response.

OBJECTIVES: To compare the excitability of the motoneuron pools of both the spastic and the unimpaired sides of patients with hemiplegia with a new method by using H-reflexes and M responses. The method determines the ratio of the developmental slope of the H-reflex (Hslp) to the slope of the M response (Mslp). We also examined the relation between the Brunnstrom stages and the Hslp/Mslp. DESIGN: Experiment. SETTING: Electrophysiologic experimental laboratory in a Japanese medical school. PATIENTS: Fifteen hemiplegic patients (9 men, 6 women; age range, 48-71yr; mean, 60yr) with spasticity

Can simultaneous bilateral movement involve the undamaged hemisphere in reconstruction of neural networks damaged by stroke?

Normalization of upper limb movement remains a difficult problem for a significant subpopulation of hemiplegic stroke patients. Clinical observations prompted investigation of a novel approach using simultaneous identical bilateral movements performed independently. We briefly report 12 controlled single-case experiments using multiple-baseline designs across three separate grasp/reach activities. Unilateral performance tests with the hemiplegic arm using the bilaterally trained actions demonstrated clinically and statistically significant improvements in movement patterns. These improvements

Detection of fatigue in the isometric electrical activation of paralyzed hand muscles of persons with tetraplegia.

Paralyzed muscle fatigue is the eventual depression of force due to either prolonged or repetitive electrical stimulation of motor units. The robustness and safety of future functional electrical stimulation (FES) systems will rely on their ability to detect the onset of muscle fatigue. The relative degree of muscle activation can be estimated by monitoring the M-wave. The aim of this study was to test a proposed method of quantitative fatigue assessment that detects muscle force output and its corresponding M-wave measured concurrently. The detection of force and M-wave concurrently allows an

Endoneural selective stimulating using wire-microelectrode arrays.

In acute experiments eight 5- to 24-wire-microelectrode arrays were inserted into the common peroneal nerve of the rat, to investigate whether the electrodes could selectively stimulate motor units of the extensor digitorum longus (EDL) muscle. Twitch-force-recruitment curves were measured from the EDL for each array electrode. The curves were plotted on a double-logarithmic scale and parameterized by the low-force slope (which represents the power p in the power-law relationship of force F versus stimulus current I, or F approximately I(p)) and the threshold current. The slopes and threshold

Action Observation Treatment for Upper Limb Rehabilitation in Patients With Stroke: Protocol for a Multicenter Randomized Controlled Trial.

BACKGROUND: In the last few years, new noninvasive strategies have emerged as rehabilitative treatments for patients with stroke. Action observation treatment (AOT) is a rehabilitation approach based on the properties of the mirror neuron system with a positive impact on modifying cortical activation patterns and improving the upper limb kinematics. AOT involves the dynamic process of observing purposeful actions with the intention of imitating and then practicing those actions. In recent years, several clinical studies suggested the effectiveness of AOT in patients with stroke to improve moto

Neurophysiological findings in conus medullaris infarction-a case report and systematic review of the literature.

INTRODUCTION: Conus medullaris infarction (CMI) is a rare vascular phenomenon that has been scarcely reported in the literature. While previous studies have described the clinical and radiological features of CMI, little attention has been paid to its associated neurophysiological findings. METHODS: We present a case of idiopathic CMI and its neurophysiological findings, then present our findings from a systematic review of other reports of CMI with neurophysiological features found via PubMed search. RESULTS: Nine articles describing ten cases of CMI with associated neurophysiological data

Motor unit number estimation via MScanFit MUNE in spinal muscular atrophy.

INTRODUCTION/AIMS: MScanFit MUNE (MScanFit) is a novel tool to derive motor unit number estimates (MUNEs) from compound muscle action potential (CMAP) scans. Few studies have explored its utility in 5q spinal muscular atrophy (SMA5q) patients, assessing only the abductor pollicis brevis (APB) muscle. We aimed to assess different distal muscles in pediatric and adult SMA5q patients, further evaluating clinical-electrophysiological correlations. METHODS: We analyzed MScanFit parameters reflecting the extent of denervation (MUNE; N50) and parameters of collateral reinnervation in APB, abductor d

Comparison between surface electrodes and ultrasound monitoring to measure TMS evoked muscle contraction.

INTRODUCTION: Transcranial magnetic stimulation (TMS) is widely used to explore cortical physiology in health and disease. Surface electromyography (sEMG) is appropriate for superficial muscles, but cannot be applied easily to less accessible muscles. Muscle ultrasound (mUS) may provide an elegant solution to this problem, but fundamental questions remain. We explore the relationship between TMS evoked muscle potentials and TMS evoked muscle contractions measured with mUS. METHODS: In 10 participants, we performed a TMS recruitment curve, simultaneously measuring motor evoked potentials (MEPs

Motor unit number index: A potential electrophysiological biomarker for pediatric spinal muscular atrophy.

INTRODUCTION/AIMS: In adult spinal muscular atrophy (SMA), the motor unit number index (MUNIX) has been shown to be an useful electrophysiological biomarker. This study evaluated the feasibility and the clinical relevance of using the MUNIX technique for patients with pediatric SMA (Ped-SMA) and correlated MUNIX results with clinical scores. METHODS: Fourteen patients with type II Ped-SMA (11 females; median age 11 y [interquartile range (IQR), 4.8-17 y]) and 14 controls (nine females; median age 10.75 y [IQR, 6.5-13.4 y]) were enrolled and matched by sex, age, height, weight, and body mass i

The severity of motor dysfunctions and urinary dysfunction is not correlated in multiple system atrophy.

OBJECTIVE: Although it is well known that patients with multiple system atrophy (MSA) cerebellar dominant type (MSA-C) show severe autonomic dysfunction, the relationship between autonomic and motor dysfunction remains uncertain. Previously we reported that severe urinary voiding dysfunction is useful in differential diagnosis of MSA and other diseases. Herein, we aimed to clarify the relationship between the severity of motor dysfunctions and urinary dysfunction. METHOD: This study is a retrospective review of 46 patients with MSA-C diagnosed according to Gilman's second consensus criteria.

Do additional inputs change maximal voluntary motor unit firing rates after spinal cord injury?

BACKGROUND: Motor unit firing frequencies are low during maximal voluntary contractions (MVCs) of human thenar muscles impaired by cervical spinal cord injury (SCI). OBJECTIVE: This study aimed to examine whether thenar motor unit firing frequencies increase when driven by both maximal voluntary drive and other concurrent inputs compared with an MVC alone. METHODS: Motor unit firing rates, force, and surface electromyographic activity (EMG) were compared across 2 contractions: (a) MVC alone and (b) MVC combined with another input (combination contraction). Other inputs (conditions) included

A prolongation of the postspike afterhyperpolarization following spike trains can partly explain the lower firing rates at derecruitment than those at recruitment.

The original motivation for this study was the observation in previous human experiments that the motor neuron firing frequency (recorded from the motor units in the EMG) was lower at derecruitment than that at recruitment, with slow linearly varying voluntary contractions. Are the lower firing rates at derecruitment correlated with a change in the postspike afterhyperpolarization (AHP) after preceding spike trains? This question was investigated by intracellular recordings from cat motor neurons in both unanesthetized and anesthetized preparations. The firing frequencies at recruitment and de

Spinal neuronal inhibition and EEG synchrony by electrical stimulation in subcortical forebrain regions of the cat.

In cats anaesthetized with sodium pentobarbital and 70% N2O, single lumbar dorsal horn neurons were excited by controlled noxious radiant heating of glabrous hindpaw skin. The EEG was recorded from the pericruciate cortex and posterior lateral gyrus. Subcortical forebrain sites where electrical stimulation inhibited dorsal horn neuronal heat-evoked responses contralaterally were identified by mapping the caudate nucleus, internal capsule, septum, nucleus accumbens and basal forebrain regions. Inhibitory sites were mainly located in the ventral forebrain (ventral septum, diagonal band, basal fo
