[Rectal goblet cell carcinoid. Primary tumor or metastasis?].

Goblet cell carcinoids are biphasic neoplasms of the gastrointestinal tract composed of a glandular and neuroendocrine differentiation. Typically, goblet cell carcinoids are localized in the vermiform appendix. We report the case of a 60-year-old female patient with the diagnosis of a 1.2-cm rectal goblet cell carcinoid tumor discovered during prophylactic proctocolonoscopy. Because of the known aggressive behavior of this entity, a rectosigmoidectomy was performed. The preoperative staging revealed neither local nor systemic spread. After 8 months, the patient is in good health. As a primary

An Incidental Discovery of Rectal Leiomyoma in an Asymptomatic Patient: A Case Report.

Leiomyomas are benign smooth muscle tumors predominantly found in the uterine wall, with rare occurrences in the gastrointestinal tract. Here, we report a rare case of a leiomyoma that was incidentally found in the rectum of an asymptomatic male who underwent a routine screening colonoscopy. Histopathological examination confirmed the diagnosis of leiomyoma. This case emphasizes the significance of routine colonoscopic screenings in the early detection of asymptomatic colorectal lesions and the pivotal role of Immunohistochemistry in differentiating rectal leiomyomas from other mesenchymal neo

Transanal Endoscopic Microsurgical Submucosal Dissection: An Efficient Treatment Option for Giant Superficial Neoplastic Lesions of the Rectum.

INTRODUCTION: The resection of giant superficial neoplastic lesions of the rectum (>5 cm) is challenging even for experienced specialists. Endoscopic mucosal resection, endoscopic submucosal dissection (ESD), and transanal endoscopic microsurgery (TEM) have all been used for the treatment of such tumors. However, because of their individual disadvantages, the ideal technique for the treatment of these lesions has yet to be determined. Transanal endoscopic microsurgical submucosal dissection (TEM-ESD) is a recently developed hybrid technique that combines the advantages of conventional TEM and

Efficacy of endoscopic surveillance in the detection of local recurrence after radical rectal cancer surgery is limited? A retrospective study.

BACKGROUND: Rectal cancer, one of most common neoplasms, is characterized by an overall survival rate exceeding 60%. Nonetheless, local recurrence (LR) following surgery for rectal cancer remains a formidable clinical problem. The aim of this study was to assess the value of postoperative endoscopic surveillance (PES) for the early detection of LR in rectal cancer after radical anterior resection with sigmoid-rectal anastomosis. METHODS: We performed an anterior resection in 228 patients with stages I‑III rectal cancer who had undergone surgery from 2001 to 2008 in the Oncology Center in Bydg

A rectal gastrointestinal stromal tumor case accompanied by elevated CEA mimicking rectal carcinoma on 18F-FDG PET/CT.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasm arising from gastrointestinal tract and can be benign or malignant. Rectal GISTs are rare and have poor prognosis. We here reported an older male who presented with features of distending discomfort in the rectum and pain in the anus due to a large rectal tumor. Physical examinations detected a mass in the rectum without blood staining on the gloved finger. Carcinoembryonic antigen (CEA) was found to be slightly elevated and the prostate-specific antigen level was normal. 18F-FDG PET/CT showed a soft tissue densit

Bowel cancer - guide for the GP.

BACKGROUND: Many patients will remain asymptomatic until the advanced stages of colorectal cancer and hence, will only be identified by means of a coordinated screening program. OBJECTIVE: This article outlines the risk assessment, early detection primary prevention and management of colorectal cancer. DISCUSSION: It is vital that general practitioners recognise the enormous variation in the patterns of clinical presentation of colorectal neoplasia such as rectal bleeding, iron deficiency anaemia, change in bowel habit, and unexplained weight loss. Any patient over 40 years of age presenting

Lymphogranuloma Venereum-Associated Proctitis Mimicking a Malignant Rectal Neoplasia: Searching for Diagnosis.

BACKGROUND: Chlamydia trachomatis-lymphogranuloma venereum (LGV) is a sexually transmitted infection (STI) and an uncommon cause of proctitis. The diagnosis requires a high index of clinical suspicion, since the clinical, imaging, endoscopic, and histological findings can mimic multiple benign or malignant conditions like inflammatory bowel disease and rectal neoplasms. CASE PRESENTATION: We present the case of a 48-year-old Caucasian male with no significant previous medical history who was admitted due to the suspicion of a rectal neoplasia. He underwent an abdominopelvic computed tomograph

Metastatic Oncocytic Renal Neoplasm Presenting as Gastrointestinal Bleeding.

Renal cancer (RC) is known for its diverse clinical presentations and unpredictable behavior. While it often metastasizes to common sites such as the lymph nodes, lungs, bones, and liver, its potential to metastasize to the gastrointestinal tract (GIT) is relatively rare. We report an unusual case of an 87-year-old male patient with a history of metastatic oncocytic renal neoplasm who presented with intermittent rectal bleeding. Colonoscopy revealed a bleeding mass in the sigmoid colon. Biopsy confirmed metastatic renal neoplasm, consistent with prior pathology. Immunohistochemistry was positi

Does peritoneal perforation affect short- and long-term outcomes after transanal endoscopic microsurgery?

BACKGROUND: Peritoneal perforation (PP) is frequently reported as a complication of transanal endoscopic microsurgery (TEM). Nevertheless, these concerns have only rarely been addressed in the literature, with no mention of the long-term oncologic consequences of PP. METHODS: A prospective database was analyzed with the intent to evaluate the influence of PP on the short- and long-term outcomes for patients undergoing TEM. RESULTS: Peritoneal perforation occurred in 28 (5.8%) of 481 patients who underwent TEM for a rectal neoplasm. The conversion rate to abdominal surgery was 10.7% (3/28). A

Stratified Mucin-producing Lesions of the Anus: Insights into an Emerging Histologic Type of HPV-driven Anal Neoplasia.

Primary anal cancers are rare and typically driven by high-risk human papillomavirus (HPV) infection. Though squamous cell carcinoma is most common, a spectrum of HPV-related nonsquamous anogenital neoplasms with similarities to cervical stratified mucin-producing carcinoma has been reported. In this study, we mined our institutional archives to characterize the clinicopathologic features of this emerging entity. Six cases were identified from the files at 2 institutions, including 4 cases of invasive stratified mucin-producing carcinoma and 2 stratified mucin-producing intraepithelial lesions

Analysis of the anatomical characteristics of the pelvis in Koreans to aid in development of a NOTES platform.

Transanal endoscopic microsurgery (TEM) is a minimally invasive technique affording full-thickness resection of rectal tumors and can also be used as a platform for transrectal access to the peritoneal cavity for NOTES (natural orifice transluminal endoscopic surgery) procedures. The authors investigated the anatomical characteristics of the pelvis in Koreans to develop an ergonomically designed NOTES platform. A total of 256 patients (156 men and 100 women) who underwent pelvic magnetic resonance imaging for evaluating rectal neoplasms were enrolled for analysis. The authors retrospectively r

[Neuroendocrine neoplasms of the appendix and colorectum].

Appropriate diagnosis and treatment of neuroendocrine neoplasms (NENs) of the appendix and colorectum requires a detailed knowledge of their proper classification according to the updated WHO and TNM systems. The WHO classification distinguishes well differentiated NEN, the neuroendocrine tumors (G1 and G2 NETs), from the poorly differentiated carcinomas (G3 NECs). While NETs are common in the appendix and rectum, NECs occur predominantly in the colon. G1 appendiceal and rectal NETs of 1 cm in size or below that do not invade either the muscular wall or vessels bear almost no metastatic risk a

Bleeding jejunal leiomyoma: a new approach.

Leiomyomas represent one of the most common benign neoplasms of the small intestine. Most of these are asymptomatic and are found incidentally at surgery or autopsy; however, leiomyomas may present with prominent signs and symptoms. Bleeding is the most common clinical problem and is occasionally life-threatening. We report the case of a 90-yr-old man who presented with rectal bleeding that became massive within hours, necessitating blood transfusion. An arteriogram revealed significant hemorrhage from the jejunal branch of the superior mesenteric artery (thought to be arteriovenous malformati

Neuroendocrine neoplasm with metastasis to the thyroid: a case report and literature review.

Thyroid cancer can be divided into two types according to its cellular origin, i.e., malignant tumors originating from thyroid cells and cancers that metastasize to the thyroid from other sites, the latter of which are, clinically rare. This article reports the diagnosis and treatment of a rectal neuroendocrine neoplasm metastasis to the thyroid. No similar cases have been reported before. This case suggests that when evaluating thyroid tumors, clinicians should not only carefully identify the clinical features of the tumor but also pay special attention to the patient's history of tumors, esp

The clinical and pathological features of four cases of feline intestinal haemangiosarcoma are described. All cases were in domestic shorthaired cats and the mean age of the animals (n=3) was 13 years. The tumours originated in the colon, small intestine, ileocaecocolic junction or rectum. The rectal tumour was juxtaposed with an adjacent mast cell neoplasm. Metastasis to mesenteric lymph node occurred in two cases, and in one of these cats there was also abdominal seeding. The histopathological appearance was of a spindle cell neoplasm with vascular differentiation in each case. Immunohistoch

Bilio-Cecal Stent Migration Presenting as Massive Rectal Bleeding.

Endoscopic biliary stenting is a well-established intervention for the treatment of biliary, hepatic, and pancreatic disorders. The common indications include strictures, neoplasms, stones, infections, and bile leaks. Stents can be occluded, predisposing patients to ascending cholangitis and biliary sepsis. Distal stent migration is another known complication of endoscopic stenting and is usually spontaneous. Bowel perforation, abscesses, bleeding, and pancreatitis are rare complications of distal stent migration and are usually limited to the duodenum. Herein, we describe an extremely rare ca
