Comorbid rectus abdominis diastasis is a risk factor for recurrence of umbilical hernia in Japanese patients.

INTRODUCTION: Rectus abdominis diastasis (RAD) is characterized by thinning and widening of the linea alba and laxity of the ventral abdominal muscle. RAD, when coexistent with umbilical hernia, is considered a risk factor for hernia recurrence. We investigated the impact of comorbid RAD in patients with umbilical hernia who had undergone hernia repair. METHODS: We enrolled 30 patients who had undergone umbilical hernia repair using either a laparoscopic or anterior approach between April 2006 and May 2018. We diagnosed RAD according to preoperative CT. The outcomes of patients with umbilical

Effects of exercise on diastasis of the rectus abdominis muscle in the antenatal and postnatal periods: a systematic review.

BACKGROUND: Diastasis of the rectus abdominis muscle (DRAM) is common during and after pregnancy, and has been related to lumbopelvic instability and pelvic floor weakness. Women with DRAM are commonly referred to physiotherapists for conservative management, but little is known about the effectiveness of such strategies. OBJECTIVES: To determine if non-surgical interventions (such as exercise) prevent or reduce DRAM. DATA SOURCES: EMBASE, Medline, CINAHL, PUBMED, AMED and PEDro were searched. STUDY SELECTION/ELIGIBILITY: Studies of all designs that included any non-surgical interventions t

Vulvar neoplasm and reconstruction of a lesion with a vertical rectus abdominis myocutaneous flap: case report.

A 41-year-old nulliparous woman was admitted to the Institute of Gynecology and Obstetrics in 2007 with a painless tumor mass invading the whole vulvoperineal and gluteal region. Pathohistological biopsy revealed FIGO Stage II squamocellular invasive carcinoma, but clinically it was Stage IVa verucal vulvar carcinoma because of malignant infiltration to the distal part of the vagina and bilateral gluteal region. The patient underwent radical vulvectomy with bilateral inguinofemoral lymphadenectomy and partial vaginectomy. Reconstruction was performed after three weeks using a distally based ve

Supercharging the transverse rectus abdominis musculocutaneous flap: breast reconstruction for the overweight and obese population.

BACKGROUND: Autologous breast reconstruction with the transverse rectus abdominis musculocutaneous (TRAM) flap is traditionally based on either the superior epigastric vessels (pedicled) or the deep inferior system (free). In the overweight and obese population, both techniques have been shown to have increased complications of the reconstructed breast. Another alternative is supercharging the flap by anastamosing the deep inferior epigastric vessels to either the internal mammary or thoracodorsal systems. We present a single surgeon's experience with unilateral TRAM reconstructions supercharg

Spontaneous hematoma of the rectus abdominis muscle: a rare cause of acute abdominal pain in the elderly.

A rectus abdominis sheath hematoma (RSH) is uncommon. It may mimic other acute abdominal disorders. The underlying conditions are trauma, coagulation disorders, or anticoagulant therapy, complications related to operations, subcutaneous injections to the abdominal wall, although it can also develop spontaneously. Acute abdominal pain and a palpable mass after muscular strain such as coughing, sneezing, and twisting were features highly suggestive of RSH. The diagnostic means of choice is computerized tomography. The treatment is usually conservative, but surgery may be needed in cases with lar

Effects of postural changes and vestibular lesions on diaphragm and rectus abdominis activity in awake cats.

Changes in posture can affect the resting length of the diaphragm, requiring alterations in the activity of both the abdominal muscles and the diaphragm to maintain stable ventilation. To determine the role of the vestibular system in regulating respiratory muscle discharges during postural changes, spontaneous diaphragm and rectus abdominis activity and modulation of the firing of these muscles during nose-up and ear-down tilt were compared before and after removal of labyrinthine inputs in awake cats. In vestibular-intact animals, nose-up and ear-down tilts from the prone position altered re

[Free rectus abdominis muscle perforating artery flaps for reconstruction of the head and neck defects].

During the past eight years, tissue defects of the head and neck region in a total of 45 patients were repaired with free rectus abdominis muscle perforating artery flaps. These flaps are subclassified into (1) reduced musculocutaneous flaps, (2) thin reduced musculocutaneous flaps, (3) paraumbilical perforator-based flaps, and (4) thin paraumbilical perforator-based flaps. The advantages of these flaps are as follows. Since the flaps involve no or only a small portion of the rectus abdominis muscle, (1) the muscle can be left intact on the abdominal wall, and (2) a thin flap can be easily cre

An invasive granular cell tumor originating from the left rectus abdominis muscle: a case report.

BACKGROUND: Granular cell tumors (GCTs) are rare soft tissue neoplasms, with invasive forms being exceptionally uncommon in the abdominal wall. This case highlights the diagnostic and therapeutic challenges of managing an invasive GCT in an atypical location. CASE PRESENTATION: A 50-year-old male presented with a painless, enlarging mass in the left rectus abdominis, with no history of trauma or systemic symptoms. Imaging revealed an exophytic mass with ill-defined borders. Histopathological examination confirmed an invasive granular cell tumor. The patient underwent complete surgical resecti

Gluteal and Sacral Defect Reconstruction Using a Transabdominal Oblique Rectus Abdominis Myocutaneous Flap: Literature Review.

Reconstruction of large gluteal and sacral defects after tumor resection is challenging, especially in patients with previous surgery or radiation. Local flaps may be unreliable in such cases. The vertical and oblique rectus abdominis myocutaneous (ORAM) flaps provide well-vascularized tissue with enough reach for coverage. The transabdominal route is also used for deep or high gluteal defects. This article aims to present a complex case of gluteal defect reconstruction using a transabdominal ORAM flap and to review the relevant literature on similar techniques. A 28-year-old man with recurren

Precision medicine exploration of postpartum rectus abdominis muscle separation: from basic research to clinical practice.

Postpartum diastasis recti abdominis (PDRA), characterized by pathological separation of the rectus abdominis muscles, affects 30%-60% of women, with many cases persisting beyond 6 months postpartum and having significant impacts on musculoskeletal function and quality of life. This review synthesizes recent advancements across molecular pathogenesis, precision diagnostics, and therapeutic innovations. Key findings reveal that PDRA pathophysiology involves pregnancy-induced hormonal fluctuations, extracellular matrix remodeling, and mechanical overloading, necessitating individualized manageme

Pectoralis Major and Abdominis Rectus Musculocutaneous Flap Reconstruction for Sternal Wound Closure in Ectopia Cordis.

Ectopia cordis is a rare congenital condition resulting in extrathoracic positioning of the heart. The severity of presentation may vary with partial or complete displacement through sternal or diaphragmatic defects. Operative management is typically required, but due to the rarity of the condition, no standard exists for optimal closure. Reconstruction techniques can include compound local flap placement including pectoralis major, rectus abdominis, or latissimus dorsi flaps as well as synthetic material placement, autologous bone, and cartilage grafts to correct sternal deformity. This case

Subcutaneous Onlay Laparoscopic Approach for small ventral hernias associated with divarication of rectus abdominis muscle.

BACKGROUND: Ventral hernias when associated with divarication of rectus abdominis if repaired without concomitant repair of the divarication is plagued by higher rates of recurrences. Out of the many options available for the repair, Subcutaneous Onlay Laparoscopic Approach (SCOLA) with concurrent plication of the divarication is gaining popularity. METHODS: All patients with a ventral hernia associated with divarication of recti were included in this prospective study. The calculated sample size was 34 with a confidence level of 95% and a margin of error of 5%. The patient demographics, oper

Deep Inferior Epigastric Perforators Topography for "Island Transverse Rectus Abdominis Musculocutaneous Flap" in Breast Reconstruction.

Background  The Island transverse rectus abdominis musculocutaneous (TRAM) flap is well vascularized with very reliable blood flow, because all perforators of the zone I are included when it is harvested. The number of perforators, topographic mapping, and their relationship with reconstructed outcomes were investigated. Methods  Fifty patients with Island TRAM breast reconstruction from September 2021 to August 2022 were investigated. The zone I was divided into a total of eight sections. Under the loupe magnification, all perforators larger than 0.5 mm in zone I were counted with fine dissec

Extended vertical rectus abdominis myocutaneous flap for reconstruction of large pelviperineal defects following oncologic resection.

BACKGROUND AND OBJECTIVES: Extended vertical rectus abdominis myocutaneous (eVRAM) flap has been proposed for reconstruction of large pelviperineal defects where traditional VRAM flap is insufficient. We present our experience with eVRAM flap for pelviperineal reconstruction following oncologic resection. METHODS: A retrospective study was conducted, including all the patients who underwent reconstruction with eVRAM flap after complex pelvic resection, between 2012 and 2020. EVRAM flap was indicated when traditional VRAM was considered deficient to cover or reach the skin defect or to fill th

Exercises, Tubigrip and taping: can they reduce rectus abdominis diastasis measured three weeks post-partum?

BACKGROUND: Rectus abdominis diastasis is regarded as a risk factor for abdominal muscle dysfunction and reduced quality of life postpartum. It is thought that specific exercises and additional physical support might reduce the diastasis, with a need to establish efficacy in doing so. OBJECTIVES: Determine the effect of four abdominal exercises as well as Tubigrip or taping in reducing rectus abdominis diastases three weeks postpartum. DESIGN: Cross-sectional repeated measures comparison. METHODS: 32 women undertook a single session of ultrasound imaging. Ultrasound measurements of inter-re

Deep Inferior Epigastric Perforator Versus Free Transverse Rectus Abdominis Myocutaneous Flap: Complications and Resource Utilization.

INTRODUCTION: Abdominal based breast reconstruction exists in a continuum from pedicled transverse rectus abdominis myocutaneous (TRAM) flap to deep inferior epigastric perforator (DIEP) free flap. DIEP flap has the advantage of complete rectus abdominis sparing during harvest, thus decreasing donor site morbidity. Aim of this study is to determine whether the surgical advantages of the DIEP flap impact postoperative outcomes versus the free TRAM flap (fTRAM). METHODS: We reviewed the Nationwide Inpatient Sample database (2010-2011) for all cases of DIEP and fTRAM breast reconstruction. Inclu
