Significance of venous anastomosis in fingertip replantation.

Adequate venous outflow is the most important factor for successful fingertip replantation. The authors have attempted venous anastomosis in all cases of fingertip replantation to overcome postoperative congestion. In this article, the significance of venous repair for fingertip replantation is described from the authors' results of 64 complete fingertip amputations in 55 consecutive patients, which were replanted from January of 1996 to June of 2001. The overall survival rate was 86 percent. Of the 44 replantations in zone I, 37 survived, and the success rate was 84 percent. Of the 20 replant

[Bone growth after finger replantation in childhood].

Following replantation in childhood, growth disorders of the affected epiphysis can influence function and aesthetic appearance of the hand. In a retrospective study long-term results of replantation in the upper limb in childhood were analysed with respect to factors influencing further growth. 22 patients with 29 replanted fingers were reviewed clinically and radiologically after an average interval of ten years. An average bone growth of 93% compared to the contralateral non-injured side was found. With the epiphysis affected, bone growth was reduced to 86% of the contralateral side. Analys

Replantation of avulsed teeth: considerations and complications.

Avulsion of teeth is a traumatic dental injury which can be managed by replantation. Important considerations for successful replantations are highlighted. Several sequelae to replantation are discussed in relation to the clinical features observed in a patient seen 6 years after replantation of two avulsed upper incisors. Many of these complications might have been avoided if the patient had returned to the clinic for root canal therapy shortly after the teeth was replanted.

Microvascular replantation of a completely avulsed scalp.

Total scalp avulsion is an unusual injury. To obtain good functional and cosmetic results, immediate revascularization using microsurgical techniques and replacement of the scalp in the correct anatomical position is the procedure of choice. This article describes a case report of successful replantation of a totally avulsed scalp from a 26-year-old male. Bilateral superficial temporal vessel anastomoses were performed using reversed autologous long saphenous vein grafts on the right side and direct repair on the left. Points of technique are discussed and the literature is reviewed.

Single artery replantation of totally avulsed scalp.

The treatment of choice for the avulsed scalp is microvascular replantation. After replantation there are numerous arterial anastomoses, with venous congestion. Usually an effort to establish inflow through two arterial anastomoses is made. We elected to anastomose one artery and multiple veins in each of our patients. We present three consecutive successful replantations of total scalp avulsion, using only one arterial anastomosis, and multiple veins. Surprisingly, two of the patients regained their ability to elevate their eyebrows actively.

Replantation of severed fingers: two unique cases.

Two cases of amputated digits are reported. One involved a period of ischemia at freezing temperatures, and the other at unusually high temperatures. Both fingers survived upon replantation. The implications of extreme temperature upon ischemic tissues are discussed.

Digital replantation. A ten-year retrospective study.

Eight hundred and two digits (592 complete amputation and 210 incomplete amputations) of 549 patients were replanted. The male to female ratio was 3:1 and the age range 1-63 years. 728 digits survived and 74 digits failed. The overall survival rate was 90.8%. Special varieties of complicated amputation were encountered in this series: (1) distal segment replantation, 139 digits, 131 digits (94%) survived; (2) digital replantation of children, 45 cases, 78 digits, average age 3.8 years, the youngest being 1 year old, 68 digits survived (88.7%); (3) rotational avulsion amputation of thumb, 26 di

Microsurgical replantation of two totally avulsed scalps.

We describe our experience with two patients in whom total avulsion of the scalp occurred. Successful replantation was carried out by microvascular surgery, with 90 and 95% survival of the scalp. Normal hair regrowth and ability to raise the eyebrows was achieved in both cases. A single superficial temporal artery was anastomosed, providing excellent perfusion of the entire scalp. Microvascular replantation should be attempted with all scalp avulsions, regardless of appearance and mechanism of injury. The patient must be transferred to a medical center with comprehensive microvascular surgery

Venous flaps in digital revascularization and replantation.

This is a report on 15 patients who underwent replantation/revascularization of a single digit with a substantial dorsal soft tissue defect. The dorsal defect was covered with a venous flap, a free flap that has only venous inflow and outflow. Postoperatively, the venous flaps were warm, pink, and appeared to exhibit a blanch and refill phenomenon, clinically resembling capillary filling. The flaps from the dorsal aspect of an uninjured digit had a survival rate of 100 percent, with no partial necrosis, while the flaps from a forearm or dorsal foot donor site failed. The advantages of using ve

Lower leg replantation in children: railroad amputation.

Replantation in the lower leg, although technically feasible, still remains a complicated clinical issue in terms of the decision-making process. In children, a limb salvage or replant procedure would seem preferable; however, in crush amputations from railroad injury, a very severe trauma, difficulties in treatment should be thoroughly weighed against prosthetic possibilities: for example, only a fair or good functional outcome would justify an attempt to replant a severed shin. Unfortunately, the outcome in most cases is unpredictable at the time of initial evaluation, and even during the re

Problems and complications encountered in replantation surgery.

The surgical techniques involved in a discipline as comparatively new and complex as replantation might be expected to result in a significant rate of complications. However, serious complications related to replantation surgery remain relatively infrequent. Meticulous attention to detail helps to avoid predicaments, minimizes potential problems, and allows the surgeon to correct the occasional complication that arises.

[Use of volar veins in finger replantation].

The number and the location of the venous anastomosis in finger replantation are discussed. The principle: 'two veins for one artery' has been given up in favour to do only one venous anastomosis. A vein on the palmar side should be preferred. Here the anastomosis is protected by a thicker layer of subcutanous tissue. In addition this technique allows to do the total vascular reconstruction without turning the hand. In a number of 84 finger replantations there was a failure rate of 9,5% by using only one vein for replantation. In 17 finger replantations using one volar vein we lost only one fi

Digital replantation and amputation--comparison of function.

Clearer indications regarding when to replant an amputated digit can be obtained by comparing the functional capabilities of two similar groups of patients, one of which has had amputations and one of which has had replantations. This paper reports such a comparative study. Function was assessed by asking the patient to perform 10 tests designed to cover a broad spectrum of everyday activities, and grip strength was compared to that in a normal hand. Grip strength was appreciably greater in patients with replanted thumbs and multiple digits, and less so in those with replanted single digits. B

Success Rates of Finger Revascularization and Replantation.

BACKGROUND: Revascularization surgery has been reported to have a higher success rate than replantation due to sufficient venous return. However, in complex cases, success depends on a wide range of indications. This study aimed to investigate success rates in cohorts that included severe cases. METHODS: This single-center, noninterventional, retrospective cohort study included 292 patients (349 digits) who underwent revascularization or replantation at our institution between January 2000 and December 2022. Sex, age, smoking history, comorbidities, affected digit, amputation level, complete

Posttraumatic Penile Replantation with Minimal Skin Necrosis.

Penile amputation is a surgical emergency where practical and timely perioperative management is crucial for ensuring a successful outcome. Tenuous viability of penile and scrotal skin has been well described in the literature, with a putative mechanism attributed to the transection of distal branches of the external pudendal artery. Although the perforasomes critical to penile replantation have been debated, this case report details a patient who successfully recovered sensation and function with minimal necrosis after penile replantation. Surgically, this was facilitated by intentional drain

Replantation versus transplantation: Where do we stand?

PURPOSE: Despite advances in extremity trauma care and reconstructive microsurgery, management of the traumatic amputations remains a challenge. The majority of patients will forever experience some level of disability even with replantation or advanced prosthetics. The goal of this article is to familiarize hand and reconstructive surgeons with the current state of upper extremity transplantation, so they better can educate their amputee patients regarding this as an option following limb loss. METHODS: Current literature, in addition to the international registry on hand and composite tissu
