INTRODUCTION AND IMPORTANCE: Kawasaki disease (KD) is an acute febrile systemic vasculitis that predominantly affects small to medium sized vessels and mostly occurs in children below 5 years of age. The morbidity and mortality mostly occur due to cardiac involvement.
CASE PRESENTATION: The authors present a case of a 5-year-old male child from hilly region of Nepal who presented with fever for 7 days along with strawberry tongue and non-exudative conjunctivitis without rashes, extremity changes or lymphadenopathy. A suspicion of incomplete KD (IKD) was made. The notable investigation finding

Mental illness is not uncommon in women of childbearing age. Pregnancy, delivery and the postpartum state could precipitate mental illness in predisposed women or worsen psychiatric symptoms in those known to have mental ill-health. Maternal mental illness exposes the infant to neglect, abuse or outright harm. We aimed to report a case of a neonate born to a mentally ill mother and to highlight the challenges of management of the mother-infant dyad in a resource-constrained- setting. The hospital records of the mother and that of her baby were summarized and the literature was reviewed to cont

BACKGROUND: Clients' needs and their satisfaction are priority issues in studying quality in reproductive health services. However, only 24-37% of Ethiopian women were informed about the available family planning methods and their utilization. High national total fertility rate (4.8) and low contraceptive prevalence rate (29%) might be attributed to these and other reasons. This study assessed the level of client-centeredness of family planning services and their determinants in public health facilities of Bahir Dar Town, Northwest Ethiopia.
METHODS: Exit interviews of 326 consecutively enrol

BACKGROUND: Several studies have been published in the western literature on the incidence, clinical course and outcome of patients with ARDS. However, there is limited data about ARDS in the tropics and moreover, the etiology and outcomes of ARDS in tropical countries are different from those of western nations. In tropical countries like India, resources are limited and costs of therapy play a major role in deciding treatment options. This prospective observational study was designed to analyze the clinical profile, outcomes and predictors of mortality of patients with ARDS in an urban tropi

In recent years, most of the focus on improving the quality of paediatric care in low-income countries has been on improving primary care using the Integrated Management of Childhood Illness, and improving triage and emergency treatment in hospitals aimed at reducing deaths in the first 24 hours. There has been little attention paid to improving the quality of care for children with chronic or complex diseases. Children with complicated forms of tuberculosis (TB), including central nervous system and chronic pulmonary TB, provide examples of acute and chronic multisystem paediatric illnesses t

BACKGROUND: The diagnosis and management of acutely dyspneic patients in resource-limited developing world settings poses a particular challenge. Focused cardiopulmonary ultrasound (CPUS) may assist in the emergency diagnosis and management of patients with acute dyspnea by identifying left ventricular systolic dysfunction, pericardial effusion, interstitial pulmonary edema, and pleural effusion. We sought to assess the accuracy of emergency providers performing CPUS after a training intervention in a limited-resource setting; a secondary objective was to assess the ability of CPUS to affect c

Subclavian artery (SA) aneurysms (SCAA) are relatively uncommon but carry a significant risk of rupture, thrombosis and embolism if left untreated. We describe the etiology, mode of presentation and outcome of surgical management of 10 patients presenting with SCAA between January 1990 and December 2010. The mean age was 43.7 years. There were seven men and three women. Five patients had aneurysms due to trauma, four had it due to thoracic outlet syndrome and one had it secondary to atherosclerosis. Five patients presented with ischemic symptoms of the distal upper extremity, while the rest pr

AIM: We describe our experience of developing a modified vacuum-assisted closure (VAC) dressing for open abdomens.
BACKGROUND: We see a high volume of trauma in our department. Massive delays in presentation of patients with acute abdomen are common. Closure at initial laparotomy is not possible in many cases, either because the patient has or will develop abdominal compartment syndrome, or because several re-look laparotomies will be required. A significant proportion of our patients who have undergone laparotomy therefore spend some of their stay in hospital with an open abdomen. The manage

BACKGROUND: Infant and young children medulloblastoma poses significant treatment challenges due to high recurrence rates and the severe neurodevelopmental risks associated with craniospinal irradiation (CSI). Outcome data from low-middle-income countries (LMICs) remain scarce.
METHODS: A retrospective chart review was conducted for children (age <5 years) diagnosed with medulloblastoma at King Hussein Cancer Center, Jordan, between 2010 and 2023.
RESULTS: Twenty-seven patients were identified. The median age was 2 years (range, 0.5-4.3 years) with 78% males, and over half (52%) presented wi

BACKGROUND: Over 9 million people are living with type 1 diabetes globally, with a disproportionate burden in resource-limited settings where access to care remains constrained. Individual interventions have shown modest benefits, but coordinated intervention clusters could achieve greater health impact. We aimed to evaluate the cost-effectiveness of intervention clusters for type 1 diabetes management in resource-limited settings.
METHODS: We conducted a systematic review (PRISMA 2020) searching PubMed/MEDLINE, Embase, Cochrane Central Register of Controlled Trials, Web of Science Core Colle

BACKGROUND: Neonatal sepsis continues to remain a serious threat, with multiple maternal and neonatal risk factors. Understanding these determinants will empower clinicians in comparable settings to apply targeted interventions, ultimately reducing newborn morbidity and mortality.
AIM: To assess maternal and infant risk factors, the aim is to develop evidence-based strategies for preventing neonatal sepsis.
METHODS: A total of 453 neonates and their mothers were enrolled, of which 146 were cases. Clinical, demographic, and obstetric data were analyzed including maternal infection history, de

BACKGROUND: Sub-Saharan Africa suffers the greatest impact of malaria, with the 2024 Health Organization (WHO )report stating that the region represents 94% of global cases and 95% of deaths. Challenges in malaria elimination stem from weak health systems and limitations of traditional diagnostic methods like microscopy and malaria Rapid Diagnostic Tests (mRDTs), which result in missed diagnoses, delays in treatment, and preventable fatalities in resource-limited settings. This paper addresses these diagnostic limitations by developing and systematically evaluating a machine learning (ML) fram

BACKGROUND: Artificial intelligence (AI) has the potential to transform chest radiography interpretation by enhancing diagnostic accuracy, identifying subtle findings, reducing errors, and helping prioritize patient care. Although chest radiography remains a cost-effective and widely used imaging tool, its effectiveness is limited by overlapping anatomy and variability in clinical expertise. Integrating AI can help overcome some of these challenges, especially in resource-constrained settings. However, robust validation in real-world clinical contexts is essential before widespread implementat

This paper provides a guideline for establishing medical skills training clinics aimed at better preparing future physicians with essential clinical and surgical skills, particularly in underdeveloped regions where high mortality rates are often linked to insufficient hands-on training. In some areas, physicians are forced to perform procedures on live patients without prior practical experience. This study examines simulation kits developed by a student-run surgical organization at a Caribbean medical school and discusses the outcomes of their clinical skills clinics. Results show that system

OBJECTIVE: Haploidentical (haplo-) hematopoietic stem cell transplantation (HSCT) has been a standard treatment for hematological malignancies for decades. However, it remains unreimbursable in Thailand due to resource constraints. Only one-fifth of the patients suitable for HSCT in our center had matched donors. Since October 2020, haplo-HSCT has been initiated for patients without matched donors using hospital funding, as it is not reimbursed by the national health policy. This cohort study aimed to demonstrate the clinical outcomes, identify problems, manage complications, adjust the protoc

PURPOSE: The high cost of gastrostomy tubes (G-tubes) limits their use and availability in resource-limited settings, where Foley urinary catheters are commonly used as low-cost alternatives for pediatric feeding. However, when used as feeding tubes, Foley catheters are prone to complications due to inadequate securement. This study aims to clinically validate a novel, low-cost securement device designed to improve the safety and functionality of Foley catheters as a pediatric gastrostomy securement system in low-resource settings.
METHODS: We developed a securement device for pediatric feedi

BACKGROUND: Common variable immunodeficiency (CVID) is the most common, symptomatic inborn error of immunity (IEI) worldwide. CVID diagnosis requires lymphocyte subset analysis by flow cytometry to delineate risk for autoimmune and inflammatory (AI) disease, known as complicated CVID. In resource-limited settings, reduced access to flow cytometry limits CVID diagnostics.
OBJECTIVES: We investigated the utility of serum cytokine testing, as compared to standard-of-care flow cytometry, for the diagnosis of AI disease in IEI and CVID.
METHODS: We performed a retrospective review of patients wit
