The emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has led to an unprecedented global healthcare crisis. While SARS-CoV-2-associated COVID-19 affects primarily the respiratory system, patients with COVID-19 frequently develop extrapulmonary manifestations. Notably, changes in the hematological system, including lymphocytopenia, neutrophilia and significant abnormalities of hemostatic markers, were observed early in the pandemic. Hematological manifestations have since been recognized as important parameters in the pathophysiology of SARS-CoV-2 and in the management o

Obesity is known to be a major risk factor of a whole range of cardiovascular, metabolic and respiratory disorders. The pattern of regional fat distribution plays an important role in the pre-disposition of obese subjects to respiratory complications. Obesity is responsible for important changes in respiratory function both during spontaneous breathing as well as during general anesthesia and mechanical ventilation. The most characteristic abnormalities consist of decreased functional residual capacity, reduced expiratory reserve volume, decreased compliance and increased resistance of the res

Obesity, particularly severe central obesity, affects respiratory physiology both at rest and during exercise. Reductions in expiratory reserve volume, functional residual capacity, respiratory system compliance and impaired respiratory system mechanics produce a restrictive ventilatory defect. Low functional residual capacity and reductions in expiratory reserve volume increase the risk of expiratory flow limitation and airway closure during quiet breathing. Consequently, obesity may cause expiratory flow limitation and the development of intrinsic positive end expiratory pressure, especially

BACKGROUND: Primary ciliary dyskinesia (PCD) is an inherited autosomal-recessive disorder of impaired mucociliary clearance characterized by chronic respiratory diseases, otolaryngological diseases, central nervous system abnormalities, reproductive system abnormalities, and cardiac function abnormalities. General anesthesia in these patients is associated with a higher incidence of respiratory complications than in patients without the disease.
CASE SUMMARY: A 16-year-old male patient was referred to the emergency room complaining of right ankle pain due to distal tibiofibular fracture. Thre

Posterior laryngeal clefts (PLCs) are described in the literature as rare laryngeal abnormalities. The authors believe type I clefts are much more common than previously reported. In two busy pediatric tertiary care centers, such clefts are the second most common congenital laryngeal finding at rigid endoscopy, second only to laryngomalacia. PLCs frequently present with symptomatology that can be attributed to other common disease processes and are often undiagnosed unless the surgeon maintains a high index of suspicion and specifically examines the posterior glottis by palpation during microl

BACKGROUND: Congenital pulmonary airway malformations (CPAMs) are rare developmental anomalies of the lower respiratory tract. Although their pulmonary and postnatal respiratory implications are well recognized, the possible coexistence of laryngotracheal abnormalities remains poorly investigated. Given the developmental continuity between the upper and lower airways, clarifying this relationship may have diagnostic, embryological, and clinical relevance.
OBJECTIVE: To map the available evidence on laryngotracheal anomalies or dimensional alterations of the upper airway in patients with CPAM

Communicating bronchopulmonary foregut malformation (CBPFM) and congenital tracheal stenosis (CTS) are difficult developmental disorders especially when they are presented simultaneously in a patient. The authors report a case of a newborn boy born at 37 weeks of gestation weighing 2356 g with CBPFM (right esophageal lung) and long segment CTS. Staged surgical repair (by-force endotracheal intubation for securing the airway followed by bronchotracheal anastomosis for CBPFM, tracheostomy with handmade, length-adjustable tracheostomy tube, and slide tracheoplasty) was performed. He has been heal

Background: Meconium peritonitis (MP) is an infrequent prenatal complication. Association between MP and pulmonary hypoplasia has never been reported. Case Report: A female infant with antenatally diagnosed MP and ascites was delivered at 36 gestational weeks. She died shortly after birth due to pulmonary insufficiency. Autopsy confirmed the presence of MP and ascites, and additionally revealed intestinal malrotation, volvulus, necrosis and perforation, and pulmonary hypoplasia. Congenital cytomegalovirus (CMV) infection was also noted. This was an unexpected finding as the mother was tested n

This review addresses how anomalous cardiovascular anatomy imparts consequences to the airway, respiratory system mechanics, pulmonary vascular system, and lymphatic system. Abnormal formation or enlargement of great vessels can compress airways and cause large and small airway obstructions. Alterations in pulmonary blood flow associated with congenital heart disease (CHD) can cause abnormalities in pulmonary mechanics and limitation of exercise. CHD can lead to pulmonary arterial hypertension. Lymphatic abnormalities associated with CHD can cause pulmonary edema, chylothorax, or plastic bronc

We studied prospectively 26 young patients with achondroplasia to test two hypotheses: that respiratory problems may be the result of occult spinal cord compression, and that achondroplastic patients with cord compression might have occult respiratory abnormalities. Respiratory abnormalities were present in 85%, the majority caused by a primary problem of the pulmonary system, such as small thoracic cage or obstructed airway. Three patients had hypoxemia, recurrent cyanotic spells, and episodes of respiratory distress explainable only by cervicomedullary cord compression; in each patient, resp

A 35-year-old woman with a high fever had a cystic lesion of the right lung including fluid and air. She was diagnosed with an infected bronchial cyst caused by congenital bronchial atresia with partial anomalous pulmonary venous return. She underwent a right upper lobectomy successfully. Patients with congenital bronchial atresia often have recurrent pulmonary infections with various imaging findings. The aberrant vein may relate to interruption of the bronchus in a patient with partial anomalous pulmonary venous return. Although segmental resection is recommended for this benign disease, lob

The occurrence of abnormalities of the kidney, urinary tract and other organ systems was reviewed in 13,775 autopsies. Forty-seven percent of 427 autopsies (60% of those under 18) with congenital abnormality of the kidney and urinary tract were found to have an associated abnormality other organ systems. Abnormalities of the cardiovascular (CV) system were most commonly associated with those of the kidney and urinary tract (25%), followed by the gastrointestinal (GI) tract (18%), central nervous system (10%), skeletal (9%), respiratory (8%), facial (7%), reproductive (5%), and chromosome and a

Inhalation cough challenge is a useful tool in the physiological and pharmacological investigation of the cough reflex. There are three types of challenge used, capsaicin and the vanilloids, organic acids, and distilled water challenge. In addition, challenge may be administered in a single dose or cumulative dose fashion. The correct combination of inhalation technology and tussive stimulus produces a sensitive and specific method for estimating the cough reflex.

Endoscopy remains the gold standard for assessing dynamic airway abnormalities in pediatric patients, but its invasiveness and variable interoperator agreement have driven interest in imaging-based alternatives. Four-dimensional computed tomography (CT) now enables objective quantification of tracheal collapse during free breathing, while photon-counting CT achieves effective doses approaching chest radiography, fundamentally altering the risk-benefit calculus for serial monitoring. MRI approaches-from real-time cine for obstructive sleep apnea localization to retrospectively gated techniques

Congenital diaphragmatic hernia (CDH) is a common birth defect that can be subdivided into several different types depending on the anatomical structures affected. Several studies have demonstrated that right-sided CDH (R-CDH) and left-sided CDH (L-CDH) in humans are associated with different clinical presentations and outcomes. We hypothesized that R-CDH and L-CDH are phenotypically and pathogenically distinct, and that the frequency of associated anomalies would differ among patients with R-CDH compared to patients with L-CDH. Using population-based data from ongoing studies in the Californi
