Comparison of blood gases during transport using two methods of ventilatory support.

Transportation of critically ill patients requiring ventilatory support represents a common, yet difficult, problem faced by clinicians. We examined 28 patients requiring transport in a prospective, randomized fashion, comparing manual ventilation with ventilation provided by a transport ventilator. Patients were ventilated to their destination with one method and returned with the alternate method. After manual ventilation, all patients showed a marked respiratory alkalosis (pH increased from 7.39 to 7.51 and PaCO2 decreased from 39 to 30 torr). After ventilation with the transport ventilator

Inhaled Epoprostenol to Facilitate Safe Transport in Legionnaires' Disease.

Hypoxemic patients often desaturate further with movement and transport. While inhaled epoprostenol does not improve mortality, improving oxygenation allows for transport of severely hypoxemic patients to tertiary care centers with a related improvement in mortality rates. Extracorporeal membrane oxygenation (ECMO) use is increasing in frequency for patients with refractory hypoxemia, and with increasing regionalization of care, safe transport of hypoxemic patients only becomes more important. In this series, four cases are presented of young patients with severe hypoxemic respiratory failure

Extracorporeal membrane oxygenation transport after traumatic aortic valve injury.

Traumatic injury to the aortic valve is an uncommon clinical entity. Rarer still is the transport of such a patient using extracorporeal membrane oxygenation (ECMO) to a specialized ECMO center for definitive repair. We present a case of traumatic rupture of the aortic valve complicated by severe acute respiratory distress syndrome with interhospital transport using ECMO and subsequent aortic valve replacement.

Oxygen consumption-oxygen delivery dependency in adult respiratory distress syndrome.

Considerable attention has been directed toward the study of oxygen transport principles in the adult respiratory distress syndrome (ARDS). In particular, concerns have been voiced regarding disturbed tissue oxygen extraction creating a pathologic dependency of oxygen consumption on oxygen delivery. This article discusses oxygen transport principles, data relevant to the oxygen extraction controversy, and implications regarding the pathogenesis and potential therapies for ARDS.

Effect of Inflatable Warming Blanket on Preventing Hypothermia in Neonates after Cesarean Section.

OBJECTIVE: To evaluate the effectiveness of an inflatable heating blanket in preventing hypothermia during hospital transport of neonates following cesarean section, and to assess its impact on vital signs and parental satisfaction. METHODS: Newborns delivered via cesarean section between June 2023 and October 2023 were randomly assigned to two groups. The control group infants were transported with the mother on a transport bed without additional warming. In the observation group, newborns were placed on an inflatable warming blanket before transport with the mother. The study included 96 ne

Global Survey of Extracorporeal Membrane Oxygenation Transport Programs: Variability in Team Composition, Training, and Capabilities.

Extracorporeal membrane oxygenation (ECMO) is a form of cardiopulmonary support associated with improved survival in severe respiratory failure and refractory cardiac arrest. Extracorporeal membrane oxygenation is highly specialized and resource-intensive; high-volume ECMO centers demonstrate improved outcomes, supporting the creation of regionalized care models. As such, the role of critical care transport medicine (CCTM) is vital. Given limited prior analysis, we aimed to descriptively review ECMO transport processes globally. A 27 item survey addressing team composition, training, and trans

Airway Gland Structure and Function.

Submucosal glands contribute to airway surface liquid (ASL), a film that protects all airway surfaces. Glandular mucus comprises electrolytes, water, the gel-forming mucin MUC5B, and hundreds of different proteins with diverse protective functions. Gland volume per unit area of mucosal surface correlates positively with impaction rate of inhaled particles. In human main bronchi, the volume of the glands is ∼ 50 times that of surface goblet cells, but the glands diminish in size and frequency distally. ASL and its trapped particles are removed from the airways by mucociliary transport. Airway g

[Consensus of Chinese experts on diagnosis and treatment processes of acute myocardial infarction in the context of prevention and control of COVID-19 (first edition)].

The SARS-CoV-2 epidemic starting in Wuhan in December, 2019 has spread rapidly throughout the nation. The control measures to contain the epidemic also produced influences on the transport and treatment process of patients with acute myocardial infarction (AMI), and adjustments in the management of the patients need to be made at this particular time. AMI is characterized by an acute onset with potentially fatal consequence, a short optimal treatment window, and frequent complications including respiratory infections and respiratory and circulatory failure, for which active on-site treatment i

Mode of Transport and Patient Status Upon Arrival at a Tertiary Pediatric Emergency Department in a Developing Country: A Prospective Observational Study.

Introduction In children with acute illness or injury, timely access to healthcare is crucial for optimal outcomes, including early identification, prompt stabilization, and appropriate referral. Prehospital transport practices often represent a weak link in the system and are frequently underreported. Enhancing these practices could greatly benefit pediatric care. This study aims to analyze the mode of transport and condition upon arrival of children at our hospital's pediatric emergency department. Methods This hospital-based prospective observational study was conducted from August 2019 to

[When the lungs are flooding, pumps are needed--epithelial Na-channel in respiratory insufficiency].

When the lungs are flooding, pumps are needed - epithelial Na-channel in respiratory insuffiency For a newborn in order to adapt to breathing air, the lungs must gain a sufficient size and respiratory surface area, and the pulmonary circulation and interstitial lymph circulation must be functional. Any disturbance in these events will result in a chronic pulmonary disease. Transportation of sodium ions from the alveolar side of the mucous membrane into the interstitial and blood space has turned out to be an important factor in the airway surface liquid volume regulation. Deficiency of transep

Adverse Events During Intrahospital Transport of Critically Ill Children: A Systematic Review.

Intrahospital transport of a critically ill patient is often required to achieve a diagnostic and/or therapeutic objective. However, clinicians who recommend a procedure that requires transport are often not fully aware of the risks of transport. Clinicians involved in the care of critically ill children may therefore benefit from a clear enumeration of adverse events that have occurred during transport, risk factors for those events, and guidance for event prevention. The objective of this review was to collect all published harm and adverse events that occurred in critically ill children in

Should a Portable Ventilator Be Used in All In-Hospital Transports?

Movement of the mechanically ventilated patient may be for a routine procedure or medical emergency. The risks of transport seem manageable, but the memory of a respiratory-related catastrophe still gives many practitioners pause. The risk/benefit ratio of transport must be assessed before movement. During transport of the ventilated patients, should we always use a transport ventilator? What is the risk of using manual ventilation? How are PEEP and FIO2 altered? Is there an impact on the ability to trigger during manual ventilation? Is hyperventilation and hypoventilation a common problem? Do

Interhospital Transport System for Critically Ill Patients: Mobile Extracorporeal Membrane Oxygenation without a Ventilator.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) has been successfully used as a method for the interhospital transportation of critically ill patients. In South Korea, a well-established ECMO interhospital transport system is lacking due to limited resources. We developed a simplified ECMO transport system without mechanical ventilation for use by public emergency medical services. METHODS: Eighteen patients utilized our ECMO transport system from December 2011 to September 2015. We retrospectively analyzed the indications for ECMO, the patient status during transport, and the patient
