One-and-a-half syndrome in ischaemic locked-in state: a clinico-pathological study.

Five patients with "locked-in" syndrome and dysconjugate palsy of horizontal gaze were studied. In all cases internuclear ophthalmoplegia due to dysfunction or destruction of the median longitudinal fasciculus was combined with an ipsilateral gaze palsy, producing the "one-and-a-half" syndrome. Clinical and electro-oculographic examination suggested involvement of the paramedian pontine reticular formation when all ipsilateral saccades were abolished, when exotropia of the contralateral eye was present, and when vestibular stimulation showed full conjugate deviation to the damaged side. Restoration of the corticoreticular pathway following shunt operation for hydrocephalus in a stroke patient.

RATIONALE: We report on a stroke patient who showed restoration of discontinued corticoreticular pathways (CRPs) on serial diffusion tensor tractography (DTT) concurrent with recovery of gait disturbance following shunt operation for hydrocephalus. PATIENT CONCERNS: A 67-year-old female patient underwent stereotactic drainage for management of intraventricular hemorrhage due to a rupture of the left posterior communicating artery. DIAGNOSES: After 4 weeks from onset, the patient exhibited quadriparesis with more severe weakness in the proximal muscles and could not even stand or walk. She un

Topochemistry of Internuclear and Intranuclear Interneurons of the Vasomotor Area in the Medulla Oblongata of Hypertensive Rats.

Immunohistochemical examination with the antiserum against neuronal NO synthase and cystathionine β-synthase was used to study the following two pools of interneurons in Wistar rats at various periods after the development of renovascular hypertension: intranuclear interneurons (lying in the projection of the solitary nucleus, reticular gigantocellular nucleus, and parvocellular nucleus) and 2 groups of internuclear interneurons (small interneurons, area 50-300 μ(2); and large interneurons, area above 350 μ(2)). Intranuclear and internuclear interneurons probably play a role in the central mec

Neonatal-onset brainstem reticular reflex myoclonus following a prenatal brain insult: generalized myoclonic jerk and a brainstem lesion.

Brainstem reticular reflex myoclonus (BRRM) is characterized by sudden, generalized, shock-like movements that can be elicited by sensory stimulation. We present a boy, born after 35 weeks gestation, who was diagnosed with neonatal-onset BRRM. Within 1 hr of birth, the patient showed spasticity and generalized clonic movements of all limbs elicited with tactile stimulation anywhere on the body. Surface electromyography showed co-contraction of agonistic and antagonistic muscles, revealing that his generalized clonic movements were tremulous myoclonus in nature. [Sudden loss of consciousness: clinical presentation and pathophysiologic mechanisms].

Most cases of sudden and temporary loss of consciousness [syncope] are caused by hypoperfusion of the formatio reticularis. more rarely by primary neurologic or metabolic disorders. The most common etiology is vasodepressor (vasovagal) syncope, which is caused by peripheral vasodilation due to acute withdrawal of the efferent sympathetic tone, while the parasympathetic outpour is increased at the same time. Although the efferent limb of the baroreflex manifests in a rather uniform way, the afferent parasympathetic limb is very variable, leading to a variety of clinical presentations and trigge

Lesion of pontine micturition area in rat causes a severe kidney syndrome.

Bilateral symmetric lesions of a small strip in the dorsal pontine reticular formation between the locus coeruleus and the laterodorsal tegmental nucleus caused a severe disturbance of kidney and urinary bladder function beginning with haematuria, micturition paralysis and incontinence on the second postoperative day. Thirteen rats with these lesions were killed on the sixth postoperative day, dissectioned and their kidneys were pathomorphologically investigated. Their urinary bladders were extremely expanded and filled with bloody urine. All kidneys showed a picture of hydronephrosis and were

The effects of local intraparenchymal pentobarbital on intracranial hypertension following experimental subarachnoid hemorrhage.

Barbiturates are often utilized clinically in circumstances in which elevated intracranial pressure is expected. In this study, the mechanism of action of barbiturates was examined in dogs with intracranial hypertension induced by injecting autogenous incubated blood into the chiasmatic cistern. Intracranial pressure and systemic blood pressure were continuously monitored. A single bilateral administration of powdered pentobarbital (2 mg and 0.4 mg) in experimental animals and solid d-glucose (2 mg) in control animals was given into the posterior hypothalamus, pontine reticular formation, or m

Tonic, phasic and cortical arousal in Gilles de la Tourette's syndrome.

This study explored the hypothesis that Gilles de la Tourette's syndrome involves a disturbance in arousal modulation. The experimental group consisted of 20 unmedicated subjects with the Gilles de la Tourette's syndrome, and the control group of 20 subjects with chronic medical illnesses (haemophilia, von Willebrandt's disease and diabetes). There were differences between groups in the manner in which log conductance level changed over time during sound and light habituation experiments involving mild levels of stimulation with the Gilles de la Tourette group showing less change in arousal le

Sodium valproate and brainstem energetics.

The effect of the anticonvulsant sodium valproate on cerebral brainstem energy metabolism has been investigated. Stupor and coma were produced in mice by the intraperitoneal injection of sodium valproate at a dose of 600 mg/kg. Glucose, glycogen, ATP, and phosphocreatine were measured in small tissue samples from the ascending reticular activating system. Levels of all metabolites were either normal or elevated in precoma and comatose mice as compared to controls. These data are consistent with the concept that sodium valproate does not have a primary action through depletion of high energy ph

Experimental pain and neuropeptides.

Noxious stimulus-induced changes in substance P (SP) release in the dorsal horn of the spinal cord and met-enkephalin release in the brain stem were investigated. For this purpose, a specially devised push-pull cannula system was used. Noxious stimuli were applied to the skin of the hind paw of rabbits and the lumbar dorsal horn was perfused using a push-pull cannula. Substance P concentration in the perfusate was assayed by radioimmunoassay. Noxious mechanical stimuli, but not thermal stimuli, increased the release of immunoreactive substance P (iSP). Innocuous stimuli did not affect the rele

The nucleus reticularis dorsalis: a region sensitive to physostigmine.

Microinjection of physostigmine (3 micrograms) into the nucleus reticularis dorsalis of the rat produced hypertension. Little or no increases in blood pressure were found at the other areas of the medullary reticular formation. The pressor effect following injection of physostigmine into the nucleus reticularis dorsalis was sensitive to atropine (1 microgram) but resistant to hexamethonium (5 micrograms), similarly injected. Atropine (1 microgram) bilaterally injected into the nucleus decreased pressor responses to intravenous physostigmine (100 micrograms/kg). These data may indicate that the

Pontine gaze paresis due to traumatic craniocervical hyperextension. Report of two cases.

Pontine gaze paresis is frequently due to tumor of the brain stem. Occasionally it may be caused by inflammation or ischemia. Two cases are reported, each with pontine gaze paresis and other signs of lower brain-stem injury, basal skull fractures, and second cervical vertebra fractures. This pattern of injuries is believed to be the result of craniocervical hyperextension with stretch injury to the brain stem at the junction of the medulla and pons.

Involvement of the cholinergic mechanism in depression of the caudate spindle.

Involvement of the cholinergic and catecholaminergic mechanisms in the caudate spindle recorded from the anterior and posterior sigmoid gyri was examined in cats. Physostigmine (0.01 to 0.1 mg/kg i.v.) abolished the appearance of the caudate spindle. These inhibitory effects were antagonized by the administration of atropine (0.25 to 1 mg/kg i.v.). The caudate spindle was inhibited by high frequency stimulation of the mesencephalic reticular formation: This inhibitory effect was antagonized by atropine. On the other hand, L-DOPA (25 to 50 mg/kg, i.v.), L-DOPA + MAO inhibitor and methamphetamin

[Electrical activity of the cortex and subcortical structures during the development of hypoxia due to blood loss].

Previous experiments on dogs showed that in several cases, along with the development of hypoxia, polymorphous delta-waves changed into synchronous oscillations, also of delta range, the so-called standard slow complexes (SSC). The new experimental data showed discharges of neuronal pool during SSC to be present in the caudate nucleus, lateral and medial thalamus, hypothalamus, hypocampus, n. amygdale, and mesencephalic reticular formation. The most frequent neuronal pulse activity was connected with the second half of the SSC negative phase (P is less than 0.01). Discharge flashes of the neur

Hippocampal mediation of raphe lesion- and PCPA-induced hyperactivity in the rat.

These experiments provide a direct test of the hypothesis that median raphe lesion- or PCPA-induced hyperactivity in the rat is mediated specifically by the hippocampus. Previous studies had shown that a marked depletion of hippocampal serotonin accompanied median lesion-induced hyperactivity. In the present studies, aspiration of the anterodorsal hippocampus of adult male rats prior to median raphe lesions or PCPA administration abolished the ability of both of these treatments to produce locomotor hyperactivity in animals chronically housed in tilt cages. Control lesions of the overlying dor

Medullary relay neurons in the carotid-body chemoreceptor pathway of cats.

Central terminations of the carotid body chemoreceptor afferents were localized by recording field potentials and unit activity evoked by electrical stimulation of the carotid sinus nerve, and unit activity during chemical stimulation of the chemoreceptors. Areas where evoked responses with short latency could be recorded and which contained neurons that responded to carotid body excitation were located in two regions of the medulla, about the level of the obex: a dorsal region which included the nucleus tractus solitarius and the reticular formation just below this nucleus; and a ventrolatera
