OBJECTIVES: The nervous system modulates the immune response in many autoimmune syndromes by neurogenic inflammation. One of the pivotal mediators is nerve growth factor (NGF), which is known for its effects on neuronal survival and growth. There is considerable evidence that NGF acts as an important mediator of many immune responses. This article reviews the role of NGF in rheumatic diseases and strategies for potential therapeutic interventions.
METHODS: We conducted a database search using Medline and Medpilot. Eight hundred abstracts containing the keyword NGF and 1 of the following terms

Enthesopathy is the pathologic change of the insertion of tendons, ligaments and joint capsules on the bone. It is a cardinal feature of spondyloarthropathies (SpA), but it can occur in other rheumatic disease. Recent studies using magnetic resonance imaging (MRI) and ultrasonography (US) have demonstrated that enthesopathy can often be asymptomatic, in both the axial and peripheral skeleton. Therefore, a systematic US study of peripheral entheses could be useful in the diagnostic process of patients with rheumatic diseases, in particular SpA. Recently, power Doppler US (PDUS) has been proved

Antimalarials are long-standing drugs that have been used since the nineteenth century for the treatment of skin rashes and lesions in lupus and rheumatoid arthritis. In recent decades, their use in these disorders has been consolidated, and new mechanisms of action have been incorporated, broadening the therapeutic perspectives of these drugs. Antimalarials are the treatment of choice in mild and moderate manifestations of systemic lupus erythematosus and are established as part of combined therapy in rheumatoid arthritis. They have been shown to have beneficial effects on atherosclerosis, as

Medicine of V and IV centuries B.C. attested in the Corpus Hippocraticum ascribes all diseases to the rheuma, i.e. the flux of humours into the body. This flux produces not only the rise of cold, hoarseness, cough, reddening, dropsy, but also arthritis, sciatica, gout.

The realisation that the production of inflammatory cytokines in inflammatory rheumatic diseases may be induced by non-infectious endogenous signals has encouraged researchers to explore mechanisms of innate immunity and their contribution to the pathogenesis of these diseases. The nucleotide-binding and oligomerisation domain (NOD)-like receptors (NLRs) sense pathogens, products of damaged cells or endogenous metabolites and could potentially be involved in the initiation, amplification and progression of the inflammatory response in rheumatic diseases. NLRs are involved in the regulation of

High mobility group box chromosomal protein 1 (HMGB1) is originally identified as a DNA-binding protein that functions as a structural co-factor. HMGB1 is actively secreted by macrophage/monocytes via inflammatory stimuli. The extracellular HMGB-1 acts as a mediator of acute and chronic systematic inflammation. In this article we briefly review its role in rheumatic diseases: arthritis, polymyositis and dermatomyositis, lupus erythematosus and Sjogren's syndrome. Increased cytoplasmic expression and extracellular deposition of HMGB1 are found in the affected tissues of those diseases, especial

Raynaud's phenomenon (RP) is caused by a reversible spasm of the smallest arteries and the arterioles of the fingers and toes. Two forms of RP have been described: primary and secondary. Secondary RP is often present in patients with rheumatic diseases. This review presents the characteristics of the clinical pattern, the immunological profile and the capillaroscopic pattern in patients with primary and secondary RP in common rheumatic diseases. Attention is paid to standard examinations usually used in RP patients. Primary RP appears to be more common in women beginning usually at puberty. Th

First descriptions about cardiovascular manifestations (mostly myocarditis and endocarditis) in inflammatory rheumatic diseases were dated at the end of the 19(th) century. Inflammatory rheumatic diseases show an increased cardiovascular manifestation and mortality, but the reasons for this are unknown. This overview presents the current knowledge about cardiovascular manifestations in different inflammatory rheumatic diseases for an efficient differential diagnostic and clarification in the daily practice.

Collagen based vascular diseases with pulmonary involvement comprise rheumatoid arthritis, systemic lupus erythematosus, progressive systemic sclerosis, dermatomyositis and polymyositis, ankylosing spondylitis, Sjögren's syndrome, and mixed connective tissue diseases. The different characteristics of pulmonary involvement are described. In such circumstances, early recognition of lung involvement is of considerable significance and the relationship to the corresponding disease has to be made. Frequently unrecognised pulmonary involvement ends up as fibrosis with irreversible deficits in respir

Hepatic manifestations in autoimmune disease include chronic active hepatitis, primary biliary cirrhosis, primary sclerosing cholangitis, and nodular regenerative hyperplasia. These diseases are rare and may occur concomitantly or serially. Clinicians must be aware of the possibility of liver disease so that it can be treated as soon as possible.

PURPOSE OF REVIEW: Health outcomes in the pediatric rheumatic diseases have been a very active area of research in the past several years, with a significant number of published studies from the United States, Canada, and Europe. Although most studies have been in the area of juvenile idiopathic arthritis, there are increasing numbers of studies in juvenile dermatomyositis and juvenile systemic lupus erythematosus.
RECENT FINDINGS: These studies suggest that although there has been an improvement in overall outcomes, active disease persists in significant proportions of individuals into adult

The observation that systemic inflammatory rheumatic diseases such as rheumatoid arthritis (RA) are associated with a significantly increased rate of cardiovascular disease, which often occurs at a younger age than in the normal population, is particularly important given the increasing interest in the role of inflammation in atherogenesis in the general population. This review examines the accumulating evidence for accelerated atherogenesis of RA and updates the hypothesis that vasculitis plays a major role in this. Endothelial dysfunction (ECD), widely regarded as initial lesion in atherogen

The diagnosis and management of systemic rheumatic disease is predominantly based on clinical parameters. Nevertheless, specific laboratory diagnostic tests may often be critically helpful, for example, in ruling out septic and crystal-induced arthritis in the differential diagnosis of acute monoarthritis. In the evaluation of patients with polyarticular rheumatic disease syndromes, autoimmune testing may be helpful to corroborate the clinical diagnosis of such diverse illnesses as rheumatoid arthritis, systemic lupus erythematosus, Lyme disease, and anticardiolipin syndrome.

HCV-infection is an important infectious disease in rheumatology. It is the cause of mixed cryoglobulinemia and other rheumatic manifestations develop frequently during HCV-infection. These comprise: Sicca-syndrome, thromboembolic events associated with anti-cardiolipin antibodies and fibromyalgia. Also associated with HCV-infection is a non-erosive polyarthritis. This synovitis often fulfills the ACR-criteria for rheumatoid arthritis, but the disease course is different with frequent remissions and non-erosive joint involvement. The following autoantibodies are associated with HCV-infection:

Rheumatic diseases are one of the common groups of chronic diseases of childhood. They are multifactorial in origin and tend to involve multiple organ systems. Consequently management of these diseases requires the expertise of many health and allied health professionals. This review article focuses on the medical management of three of the relatively common diseases: juvenile rheumatoid arthritis (JRA), systemic lupus erythematozus (SLE) and dermatomyositis (DM).
