Does bony morphology of the shoulder affect rotator cuff retear rates? A 24-year retrospective cohort of rotator cuff repairs.

BACKGROUND: Rotator cuff tears can result in pain which can be debilitating. The etiology of failed repair or retears is multifactorial. Multiple risks factors were proposed, with critical shoulder angle (CSA) being one of the factors of emerging interest. Other radiological parameters including glenoid inclination (GI), lateral acromial angle (LAA), and acromial index (AI) were also suggested for a possible correlation. METHODS: A retrospective review of 397 rotator cuff repairs on January 1, 2000 to December 31, 2023 recruited from two major hospitals was carried out. Primary outcome measur

Associations of normal fasting glucose levels and of insulin resistance with degenerative rotator cuff tear : Normoglycemia and rotator cuff tear.

BACKGROUND: The upper normoglycemic range has been proposed as a risk factor for degenerative rotator cuff tendon tear (RCT), and insulin resistance has been suggested as a risk factor for tendinopathy. However, no research has established their association with degenerative RCT in the general population. This study aimed to determine whether fasting glucose levels and insulin resistance are risk factors for degenerative RCT in the normoglycemic population and identify the risk range for fasting glucose. METHODS: This study included 418 normoglycemic participants from a rural cohort. Particip

Intense focused ultrasound stimulation of the rotator cuff: evaluation of the source of pain in rotator cuff tears and tendinopathy.

The objective of this preliminary study was to evaluate the ability of individual 0.1-s long pulses of intense focused ultrasound (iFU) emitted with a carrier frequency of 2 MHz to evoke diagnostic sensations when applied to patients whose shoulders have rotator cuff tears or tendinopathy. Patients were adults with painful shoulders and clinical and imaging findings consistent with rotator cuff disease. iFU stimulation of the shoulder was performed using B-mode ultrasound coupled with a focused ultrasound transducer that allowed image-guided delivery of precisely localized pulses of energy to

Reverse Shoulder Arthroplasty with Virtual Implant Positioning System in the Management of Massive Rotator Cuff Tear with Rotator Cuff Arthropathy.

INTRODUCTION: Massive irreparable rotator cuff tears associated with cuff tear arthropathy present a challenging clinical condition in elderly patients because of pain, loss of shoulder function, and progressive degenerative changes. Reverse shoulder arthroplasty (RSA) has emerged as the preferred surgical treatment by restoring shoulder biomechanics through medialization and inferiorization of the glenohumeral center of rotation. Recent advances, including computed tomography (CT)-based virtual implant positioning (VIP) systems, have further improved implant positioning accuracy and surgical

Leptin receptor signaling mediates the distinct tendon-bone interface reconstruction in rotator cuff tears and osteoporosis-comorbid rotator cuff tears.

BACKGROUND: The intact tendon-bone interface (TBI) consists of four histological layers-tendon, fibrocartilage, calcified fibrocartilage, and bone-that gradually merge into each other, making complete structural restoration after injury challenging. Osteoporosis poses a significant risk for rotator cuff tears (RCTs) and re-tears after arthroscopic rotator cuff repair (ARCR). Activating Leptin receptor (Lepr) mediated Stat3 signaling transduction facilitates the transcription of Runx2 and Sox9, respectively, and promotes osteogenesis and chondrogenesis. MATERIALS AND METHODS: Sixty-five female

Does a critical rotator cuff tear stage exist?: a biomechanical study of rotator cuff tear progression in human cadaver shoulders.

BACKGROUND: It is unknown at which stage of rotator cuff tear the biomechanical environment is altered. The purpose of this study was to determine if a critical rotator cuff tear stage exists that alters glenohumeral joint biomechanics throughout the rotational range of shoulder motion, and to evaluate the biomechanical effect of parascapular muscle-loading. METHODS: Eight cadaver shoulders were used with a custom testing system. Four progressive rotator cuff tear stages were investigated on the basis of footprint anatomy. Three muscle-loading conditions were examined: rotator cuff only; rota

PGE2 Ameliorates Aging-Aggravated Rotator Cuff Muscle Atrophy.

BACKGROUND: The aging-related escalation of muscle degeneration impacts the structure and function of rotator cuff muscles, contributing to spontaneous and tear-induced muscle atrophy. This study investigated how prostaglandin E2 (PGE2), a regulator of muscle regeneration, influences muscular structure and mitochondrial function in aged mice by using SW033291 to inhibit PGE2 degradation, revealing potential therapeutic pathways for mitigating rotator cuff muscle deterioration. METHODS: A total of 20 young (5 to 6-month-old) and 100 aged (18 to 20-month-old) female C57BL/6J mice were divided i

Anchorless Transosseous Rotator Cuff Repair: A Technical Note.

BACKGROUND: Transosseous rotator cuff repair provides robust fixation and broad footprint compression without the risk of foreign body reaction that may be seen with suture anchors. We present our technique for anchorless transosseous repair using a modern device to efficiently create bone tunnels and assist in suture passage. INDICATIONS: Tears of the supraspinatus tendon, with or without extension to the infraspinatus, in patients with acute or chronic tears, with good bone quality, and who fail appropriate nonoperative management. TECHNIQUE: The beach-chair position with an articulating a

Rip-Stop Fixation Constructs for Rotator Cuff Repair.

In arthroscopic rotator cuff repair, poor tendon quality, medially based tears, lateral tendon loss, or limited tendon mobility can all preclude the use of double-row suture constructs, presenting a challenge in achieving secure fixation and tendon-to-bone healing. Rip-stop suture configurations can be used in these settings to improve resistance to tissue cutout and provide enhanced biomechanical characteristics compared with standard single-row repairs. The load-sharing rip-stop technique uses 2 double-loaded medial suture anchors, which are placed adjacent to the articular margin, and 1 rip

Arthroscopic Rotator Cuff Repair: How to Avoid Retear.

A healed rotator cuff repair results in a superior outcome for the patient compared with a non-healed repair. The surgeon can maximize the chance of a healed repair by knowing the end-point of each key step in the repair process and adhering to a few core principles. First, the rotator cuff tear pattern (e.g. crescent, L-tear, reverse L-tear, U-tear) must be recognized, starting with careful assessment of preoperative MRI but concluding with the arthroscopic assessment of tear edge mobility. Second, a low-tension, anatomic, and mechanically robust repair construct (e.g. linked, double row; loa

Factors Associated with Atraumatic Posterosuperior Rotator Cuff Tears.

BACKGROUND: Certain metabolic factors have been proposed as risk factors for a posterosuperior rotator cuff tear. Although metabolic syndrome is of increasing concern in industrialized societies, little information exists regarding its association with posterosuperior rotator cuff tears. The purpose of this study was to determine the risk factors for an atraumatic posterosuperior rotator cuff tear, including metabolic factors and metabolic syndrome. METHODS: This study involved 634 subjects (634 shoulders) drawn from a cohort of rural residents. Posterosuperior rotator cuff tear diagnoses wer

Rotator cuff tear and sarcopenia: are these related?

BACKGROUND: Sarcopenia is the loss of muscle mass and consequent loss of muscle function with aging. Its prevalence among the general population is 12% to 30% in those aged >60 years. We evaluated (1) the difference in the prevalence of sarcopenia between patients with rotator cuff tear and controls and (2) the sarcopenia severity according to the size of the rotator cuff tear. METHODS: Group 1 included 48 consecutive patients with chronic symptomatic full-thickness rotator cuff tears (mean age, 60.1 ± 6.5 years; range, 46-76 years), and group 2 included 48 age- and sex-matched patients. [Update on the postsurgical shoulder rotator cuff rehabilitation].

Primary care physicians play a crucial role in rehabilitation following rotator cuff surgery. This synthesis paper aims to address essential issues of postsurgical rehabilitation, based on recent recommendations. The rehabilitation comprises 4 phases, which are determined by the tendon healing process. For each phase, this article presents the stage of tendon healing, the allowed mechanical constraints, the exercise and physiotherapy modalities, the possible activities and the potential complications. Information and counseling to support the patient implication in the rehabilitation are also d

Early anchor displacement after arthroscopic rotator cuff repair.

PURPOSE: Arthroscopic rotator cuff repair is a success and became the mainstream method of reconstruction in recent years. Due to the technical development in shoulder surgery, novel suture anchors and high-strength sutures are widely used providing convincing fixation, however anchor pullout still remains a problem. The aim of our study was to observe early anchor movement on a large patient cohort, and identify clinical, biological and technical variables that can lead to this complication. METHODS: A total of 5,327 patients who had arthroscopic rotator cuff reconstruction for full thicknes

Rotator Cuff Tendinopathy: Navigating the Diagnosis-Management Conundrum.

Synopsis The hallmark characteristics of rotator cuff (RC) tendinopathy are pain and weakness, experienced most commonly during shoulder external rotation and elevation. Assessment is complicated by nonspecific clinical tests and the poor correlation between structural failure and symptoms. As such, diagnosis is best reached by exclusion of other potential sources of symptoms. Symptomatic incidence and prevalence data currently cannot be determined with confidence, primarily as a consequence of a lack of diagnostic accuracy, as well as the uncertainty as to the location of symptoms. Risk Factors for Infection After Rotator Cuff Repair.

PURPOSE: To identify risk factors for infection after rotator cuff repair. We hypothesized that patient characteristics and surgical technique would affect the rate of infection. METHODS: The records of 1,824 rotator cuff repairs performed by a single surgeon from 1995 to 2010 were reviewed retrospectively. Fourteen patients had an early deep postoperative wound infection that was treated with surgical irrigation and debridement. One hundred eighty-five control patients who were treated with rotator cuff repair and did not develop an infection were selected randomly for comparison and statist

Tears of the rotator cuff. Causes--diagnosis--treatment.

Rotator cuff ruptures are the most common degenerative tendon injury and occur mainly in older patients as multifactorial disorders manifesting the main symptoms of pain and restricted range of motion. Thorough clinical examination of the shoulder includes testing the function of the rotator cuff and leads to a tentative clinical diagnosis that is the prerequisite for diagnostic imaging procedures. Sonography of the shoulder gives rapid access to a very good sensitive overview of the rotator cuff. Conventional radiological imaging permits differential diagnosis since a reduced acromiohumeral i

MRI findings of rotator cuff myotendinous junction injury.

OBJECTIVE: The purpose of this article is to describe the MRI features of rotator cuff myotendinous junction injuries. MATERIALS AND METHODS: We retrospectively identified MRI cases with myotendinous junction injury of the rotator cuff muscles and reviewed clinical, imaging, and surgical records. MR images were reviewed independently by two musculoskeletal radiologists to grade myotendinous junction injuries (strain, partial tear, or complete tear) and to assess for concurrent tendon tears (partial or full thickness) and bone changes (fracture or contusion). RESULTS: The final study group co
