Hernia is an exceedingly common pathology, to which inguinal hernias are frequently diagnosed. Though this entity is regularly seen, in pregnancy a different diagnosis must be excluded: round ligament varicocele (RLV). Right-sided round ligament (RSRL) is a rare congenital anomaly that is strongly associated with a number of intrahepatic vascular anomalies. Round ligament varicosity (RLV) is rare and almost all cases are pregnant women. RLV appears as a unilateral or bilateral groin swelling. Pain and tenderness may present. Clinical evaluation is inadequate for exact diagnosis because inguinal hernia has similar findings. Ultrasonography (US) is essential when a groin swelling is detected in a pregnant woman. We present gray scale US and colour Doppler US findings of a 32-week pregnant woman with bilateral RLVs at the inguinal canal, parauterine area and in the myometrium.

We report on a 49-year-old woman with cysts of the round ligament causing a nonreducible mass of the groin. Cysts of the round ligament are rare. Physical findings were similar to those of an inguinal hernia. Such cysts are usually not considered in the differential diagnosis of groin hernias and might be identified only at the time of herniorraphy.

We report the case of a 36-year-old woman who presented with signs and symptoms of an irreducible inguinal hernia. Surgical exploration revealed a mesothelial cyst of the round ligament of the uterus. Mesothelial cysts of the round ligament are rare lesions, frequently masquerading as inguinal hernias, and should be included in the differential diagnosis of any inguinal mass. Round ligament varicosities (RLVs) are rare, and only occur in pregnancy. The swelling due to RLVs mimics an inguinal hernia and generally resolves spontaneously after delivery. Distinguishing between varicosities and hernias is critical to avoid unnecessary surgeries on pregnant women. We aimed to determine the significance of RLVs during pregnancy and to review and describe their clinical and sonographic characteristics. All patients were diagnosed by gray scale and color Doppler ultrasonography, managed conservatively, and RLVs regressed spontaneously postpartum.

3 pregnant women, aged 34, 28, and 39 years respectively, presented with an inguinal swelling. In one patient the swelling was bilateral and painful. Reposition was possible and ultrasound examination revealed round ligament varicosities, the most common cause of an inguinal swelling during pregnancy. Expectative treatment is advised as the condition usually disappears after parturition. It is concluded that in every pregnant woman with an inguinal swelling, sonography with doppler imaging must be performed to confirm the diagnosis of round ligament varicosities to avoid unnecessary surgery.

Left-sided gallbladder, a rare congenital anomaly, is defined as a gallbladder located to the left of the round ligament. A 42 year-old female underwent a cholecystectomy for gallstone disease. At laparotomy, we found that the round ligament was located to the right of its normal position and was continuous with the right anterior branch of the portal vein.
