HRSA's evidence-based tele-emergency network grant program: Multi-site prospective cohort analysis across six rural emergency department telemedicine networks.

BACKGROUND: The Health Resources and Services Administration (HRSA), Federal Office of Rural Health Policy (FORHP) funded the Evidence-Based Tele-Emergency Network Grant Program (EB TNGP) to serve the dual purpose of providing telehealth services in rural emergency departments (teleED) and systematically collecting data to inform the telehealth evidence base. This provided a unique opportunity to examine trends across multiple teleED networks and examine heterogeneity in processes and outcomes. METHOD AND FINDINGS: Six health systems received funding from HRSA under the EB TNGP to implement t

Trans*Forming Access and Care in Rural Areas: A Community-Engaged Approach.

Research indicates that rural transgender and gender diverse (TGD) populations have a greater need for health services when compared with their urban counterparts, face unique barriers to accessing services, and have health disparities that are less researched than urban TGD populations. Therefore, the primary aim of this mixed-methods study (n = 24) was to increase research on the health care needs of TGD people in a rural Appalachian American context. This study was guided by a community-engaged model utilizing a community advisory board of TGD people and supportive parents of TGD children.

Cell dynamics and immune response to BLV infection: a unifying model.

INTRODUCTION: In countries such as South Africa with a high prevalence of HIV and TB policy directives support program integration. Operational research suggests this is desirable, at least for increasing coverage of HIV and TB services, but warns that implementation models must take local health service infrastructure into account. METHODS: A program evaluation of HIV and TB prevention and therapeutic services was performed at facility level in two purposefully selected districts in South Africa - one deep rural and an urban district - in order to describe integration and how it is implement

Mental Health of First Responders in Rural Australia: A Qualitative Exploration of Intersecting Individual, Organisational and Contextual Factors on Psychological Wellbeing.

To explore the factors shaping mental health experiences among rural and remote first responders and to examine how organisational and contextual conditions influence psychological wellbeing. First responders are routinely exposed to traumatic events and are at elevated risk of mental health difficulties. In rural and remote settings, geographic isolation, limited workforce capacity, and reduced access to specialised services may compound these risks. However, less is known about how workplace culture and service structures interact with trauma exposure in these contexts. A participatory actio

The Surgical and Anesthesia Workforce and Provision of Surgical Services in Rural Communities: A Mixed-Methods Examination.

PURPOSE: Rural-urban disparities in the surgical and anesthesia workforce exist. This mixed-methods study describes the distribution of the surgical and anesthesia workforce and qualitatively explores how such workforce and other factors influence rural hospitals' provision of surgical services. METHODS: We calculated provider counts by county from the Area Health Resource File. Using American Hospital Association survey data, we sampled rural hospitals, stratified by critical access status and state policies. We conducted qualitative semistructured interviews with administrators at 16 hospit

Beyond the classroom: The transformative experience of short rural immersion programs for health professional students: A narrative review.

BACKGROUND: Rural areas house nearly half the global population yet face a significant shortage of skilled health professionals, exacerbating health inequities. Short-term rural immersion programs offer a cost-effective approach to exposing health professional students to the unique challenges of rural healthcare. Transformative learning theory is well suited to examining how these programs foster critical reflection and perspective shifts in students. OBJECTIVES: To explore the potentially transformative impact of short-term (<6 weeks) rural immersion programs on health professional students

Barriers to accessing specialist care for older people with chronic obstructive pulmonary disease in rural New Zealand.

INTRODUCTION: Access to health care for people living in rural areas presents many challenges. For people who live with chronic obstructive pulmonary disease (COPD), the ability to access specialist health services facilitates improved health outcomes, however many barriers to accessing specialist health care have been identified. This paper reports on the challenges people living with COPD in rural New Zealand (NZ) face in accessing specialist health care services. METHODS: Nine people living with COPD in a small NZ rural town were interviewed in 2007. FINDINGS: Data analysis, using a gener

Assessment of Oral Health Disparities and Barriers to Care among Underserved Populations in Urban and Rural Settings.

INTRODUCTION: Oral health disparities persist between urban and rural populations, particularly among underserved communities. Geographic isolation, financial constraints, and lack of access to dental services contribute significantly to these disparities. Rural populations often encounter greater barriers in accessing healthcare facilities, whereas urban underserved groups face challenges such as overburdened public health systems. This study aims to assess and compare the disparities in oral health outcomes and the barriers to care faced by underserved populations in both urban and rural set

Multidimensional poverty and health: an analysis of multidimensional poverty and its influencing factors among rural residents in Western China from a health ecology perspective.

BACKGROUND: Although absolute poverty in rural China has been systematically eradicated, rural residents continue to suffer deprivation in terms of education, healthcare and living conditions, which has become an urgent obstacle to China's goal of rural revitalisation and common prosperity. In addition, the impact of health on multidimensional poverty remains unclear. METHODS: This study constructs four periods of balanced panel data on the basis of baseline survey data from 2012 and follow-up survey data from 2015, 2019 and 2022 to conduct a longitudinal study of multidimensional poverty amo

"We Only Escort Women to the Health Facility": Traditional birth attendants and the performance of indicator-driven care in rural Tanzania.

Global health regimes have reversed the role of traditional birth attendants (TBAs) from partners in the fight against maternal and child mortality to barriers to indicator-driven care. In rural Tanzania, widespread fear of government surveillance and punishment for non-compliant individuals and organizations that climaxed during the presidency of the late John Magufuli (2015-2021) put increased pressure on TBAs. To negotiate tensions, TBAs adopted the position that they only escort women to health facilities for birth. We argue that TBAs' insistence on this only both occludes and protects the

The Delivery of Kidney Care in Rural or Sparsely Populated Settings.

Social determinants of health and limited access to medical care contribute to underdiagnosis and late presentation of chronic kidney disease (CKD). Rural areas face unique challenges in providing optimal care for patients with CKD due to poverty, limited health care access, and geographical barriers. This review addresses the challenges of caring for rural patients with CKD and discuss potential strategies to increase health care equity. Furthermore, we examine rural disparities in patients with CKD, patients with kidney failure on in-center or home dialysis, and patients with kidney transpla

Feasibility issues impacting optimal levels of maternity care in rural communities: implementing the Rural Birth Index in British Columbia.

INTRODUCTION: The continued attrition of maternity services across rural communities in high resource countries demands a rigorous, systematic approach to determining population level need, including a clear understanding of feasibility issues that may constrain achieving and sustaining recommended levels of services. The Rural Birth Index (RBI) proposes a robust and objective methodology to determine such need along with attention to the feasibility of implementation. BACKGROUND: Predictions of appropriate levels of maternity care in rural communities require consideration of the feasibility

Development and validation of an indicator system for the modernization of rural public sports services in China.

BACKGROUND: Rural public sports services are increasingly important for promoting physical activity, reducing rural health inequities, and strengthening community-based public service governance. However, existing studies have mainly examined service supply, satisfaction, or policy narratives, and have paid less attention to how modernization can be measured as a systemic and cross-sectoral process. Objective: This study develops and empirically validates an indicator system for the modernization of rural public sports services in China. METHODS: Drawing on structural functionalism, new publi
