Intralesional sclerotherapy in the management of hemangiomas of the glans penis.

PURPOSE: We studied the role of sclerotherapy in the management of glans penis hemangiomas. MATERIALS AND METHODS: Four patients with hemangiomas of the glans penis were treated on an outpatient basis with intralesional injections of 30% hypertonic saline. RESULTS: In all 4 patients the lesions regressed and all were satisfied with the outcome. CONCLUSIONS: Intralesional sclerotherapy is a cost-effective modality for the treatment of glans penis hemangiomas, especially in developing countries and at peripheral hospitals where expensive laser equipment may not be available.

Sclerotherapy versus banding in the treatment of variceal bleeding.

Endoscopic sclerotherapy has been the mainstay in the management of esophageal variceal bleeding to control acute bleeding and decrease recurrent bleeding. Endoscopic variceal ligation is a new technique that is equally effective in the control of acute bleeding but achieves obliteration of varices in fewer treatment sessions with presumably less cost, results in a lower rebleeding rate, has fewer complications, and is associated with reduced mortality. Combination therapy with both endoscopic variceal ligation and endoscopic sclerotherapy appears to have no clear advantage over variceal ligat…

[Endoscopic ligation and sclerotherapy in the treatment of esophageal varices].

Endoscopic variceal ligation (EVL) and esophageal variceal sclerotherapy (EVS) are both used in the treatment of esophageal varices. Sclerotherapy is widely used with success, however it is associated with multiple local and systemic complications. To overwhelm the complications of sclerosis, Stiegmann in 1986 proposed the elastic band variceal ligation which demonstrated to be efficient as sclerotherapy though with minor side effects. Therefore, the Authors decided to apply a therapeutical protocol with EVL for the primary prevention of variceal esophageal bleeding. Preliminary results are he…

Sclerotherapy in bleeding gastric varices of hepatic schistosomiasis.

We report the results of sclerotherapy in 20 patients with bleeding gastric varices due to hepatic schistosomiasis. In an endemic area, patients with hepatic schistosomiasis, and bleeding gastric varices seen on endoscopy to be inferior extension of esophageal varices, were treated with emergency endoscopic injection just proximal to the cardia. Hemostasis was achieved in 17. Obliteration of varices was achieved in all patients with sclerotherapy, combined with surgery. Thirteen patients who had not been operated on in the past and consented to surgery underwent esophagogastric devascularizati…

[Obstructive intramural hematoma of the esophagus after sclerotherapy].

A case of obstructive intramural hematoma of the esophagus post variceal sclerotherapy is reported in a 44-year-old man who presented retrosternal pain and complete dysphagia. Endoscopy showed a bluish submucosal mass occupying the esophageal lumen. Diagnosis was confirmed by Computed Tomography. A favorable outcome was observed with symptomatic treatment.

Sclerotherapy of testicular hydroceles with 3% aqueous phenol.

With the evolution of minimally invasive approaches in medicine, phenol has regained its popularity for the sclerotherapy of testicular hydroceles. Together with reported efficiency and safety of 2.5% phenol in the literature, the recently proved safety of 3% phenol in esophageal variceal sclerotherapy has led us to perform a prospective study to lessen the number of sessions. Sclerotherapy with 3% aqueous phenol was applied on an ambulatory basis to 23 patients with 31 hydroceles, who were over 40 years old and who had no fertility problems. The over-all cure rate was 96% with an average foll…

Sclerotherapy with or without octreotide for acute variceal bleeding.

BACKGROUND: Sclerotherapy is considered the most effective way to stop bleeding from esophageal varices, but acute variceal bleeding is still associated with a high risk of rebleeding and death. We compared sclerotherapy alone with sclerotherapy and octreotide to control acute variceal bleeding and prevent early rebleeding in patients with cirrhosis. METHODS: In a double-blind, prospective trial, 199 patients with cirrhosis and acute variceal bleeding who underwent emergency sclerotherapy were randomly assigned to receive a continuous infusion of octreotide (25 micrograms per hour) or placebo…

Intramural haematoma of the oesophagus complicating sclerotherapy for varices.

A patient is described who developed an intramural haematoma of the oesophagus shortly after variceal endoscopic sclerotherapy. This unusual complication is best imaged by computed tomography or nuclear magnetic resonance and needs conservative treatment. Resolution usually occurs spontaneously and may result in eradication of the varices.

Endoscopic injection sclerotherapy for esophageal varices in the elderly.

A group of 1127 patients who underwent sclerotherapy for esophageal varices were compared for outcome in terms of age: over and under age 70 years. Esophageal varices were completely eradicated in 81 of 110 patients > 70 years (73.6%) and in 791 of 1017 patients < 70 years (77.8%). Gastrointestinal bleeding after sclerotherapy occurred in 9 patients > 70 years (8.2%) and in 84 of those < 70 years (8.3%). Complications occurred in 16 patients > 70 years (14.5%) and in 141 < 70 years (13.9%). Liver failure and hepatoma accounted for more than 80% of the causes of death in both groups (80.3% vers…

Percutaneous aspiration and ethanol sclerotherapy for thyroid cysts.

Benign thyroid cysts often recur after aspiration; the effectiveness of tetracycline instillation in the case of recurrence has been questioned. We, therefore, tested the efficacy of percutaneous ethanol injection in 20 patients with "pure" cyst relapsing after aspiration. After evacuation, 95% ethanol was instilled under sonographic guidance and re-aspirated 5 min later. The procedure was performed twice for larger cysts. Follow-up studies were carried out after 1, 3, 6, and 12 months. In case of recurrence at 1 month, patients (n = 5) were submitted to a second session. A slight burning sens

[Pleural empyema as a complication of the sclerotherapy of esophageal varices].

We present three patients developing pleural empyema after sclerotherapy of esophagic varixes. In all of them, the presence of persistent fever syndrome following the sclerotherapy, along with radiological images of pleural overflow, were the starting point of the patient's study. In the emergency esophagogrames, the extravasation of contrast outside the esophagus could not be confirmed. The treatment, which must be started as soon as possible, was based in conservative measures (drainage, antibiotics and a proper nutrition), given the poor general condition of these patients. Despite all thes…

Octreotide infusion or emergency sclerotherapy for variceal haemorrhage.

To compare octreotide with injection sclerotherapy in the treatment of acute variceal haemorrhage, patients admitted with gastrointestinal bleeding and oesophageal varices confirmed by endoscopy were randomised to receive either emergency sclerotherapy with 3% sodium tetradecyl sulphate or octreotide (50 micrograms intravenous bolus plus 50 micrograms per h intravenous infusion for 48 h). At the end of the study period (48 h), the octreotide group also had sclerotherapy to obliterate the varices. 100 patients were recruited. Demographic features including the aetiology of portal hypertension a…

Percutaneous treatment of hepatic cysts by aspiration and sclerotherapy.

We treated 35 patients who had hepatic cysts (30 congenital cysts, 5 hydatid cysts) with percutaneous puncture and sclerotherapy. After puncture and drainage of the cyst, a 95% alcohol solution was instilled as sclerosing agent into the cystic cavity. In all the patients, cyst puncture and drainage was successful. Follow-up in all cases was at least 12 months. In three uncooperative patients, cysts recurred due to incomplete sclerosis of the lining epithelium of the cyst wall. No major complications were encountered in all cases. All congenital cysts were treated on an outpatient basis. Patien…

Endoscopic sclerotherapy for control of bleeding varices in children.

Thirty-three children with esophageal varices due to portal hypertension underwent injection sclerotherapy over a period of 6 yr. Thirty-one completed the sclerotherapy course, and the varices were eradicated in all. In nine, the procedure was performed as an emergency because of continued bleeding and, in each case, a gastric fundal varix was the source of the blood loss. Sclerotherapy successfully controlled the bleeding in four of these, whereas five required surgical underrunning of the fundal varix. After surgery, these five continued sclerotherapy until the esophageal varices were eradic…

[Experimental surgical model of hydrocele for the use of sclerotherapy].

Sclerosant therapy for different pathological conditions (testicular hydrocele, pneumothorax) is a little known and scantily used procedure in the clinical setting. We have developed a surgical procedure in the dog that permits reproducing adult hydrocele. This experimental model is useful for morphometric assessment of the sclerosant effects of the commonly used agents (magnesium silicate hydrate, carbolic acid, ethanolamine oleate, hydroxytetracycline chlorhydrate and quinacrine chlorhydrate). Our results show that the degree of sclerosis achieved with quinacrine and hydrotetracycline is sig

Alcohol sclerotherapy of non-parasitic cysts of the liver.

Between 1980 and 1987, nine patients with non-parasitic cysts of the liver were treated with computed tomography-guided percutaneous punctur … dditional surgical treatment due to residual and multiple cysts. Computed tomography-guided alcohol sclerotherapy of non-parasitic liver cysts appears to be a safe and effective initial therapy.
