Management of Self-Injurious Behavior, Aggression, and Psychogenic Non-Epileptic Seizures in Patients With Tuberous Sclerosis: A Case Report With a Review of Literature.

Tuberous sclerosis complex (TSC) is a neurocutaneous disorder that affects multiple systems. TSC encompasses neurobehavioral abnormalities that are considered less sensitive and specific to the disease. Autism spectrum disorder, attention deficit disorder, anxiety, mood disorders, and self-injurious behavior (SIB) are neurobehavioral manifestations associated with tuberous sclerosis. Among them, SIB is poorly described and studied. We present a case report and a brief review of the literature, which offers us insight into the pathological mechanism that explains associated SIB in TSC patients

Evidence that lack of brain dopamine during development can increase the susceptibility for aggression and self-injurious behavior by influencing D1-dopamine receptor function.

1. Lesch-Nyhan disease has a defined neurological lesion that is accompanied by abnormal motor function, aggression and self-injurious behavior. 2. The dopamine deficiency in Lesch-Nyhan disease has been modelled by destroying dopamine-containing neurons in neonatal rats with 6-hydroxydopamine. 3. Because D1-dopamine antagonists will block self-injurious behavior induced by L-DOPA in neonatal-6-OHDA-lesioned rats, D1-dopamine antagonists are proposed as a potential therapy for aggression and self-injurious behavior in patients with these symptoms. 4. The determination that the drug SCH-12679,

Domestic Violence and Self-Injurious Thoughts and Behaviors Among Adults.

Background: Exposure to domestic violence is associated with self-injurious thoughts and behaviors, including suicidal and nonsuicidal self-injury. However, an updated comprehensive review of the literature is unavailable. Aims: This systematic review aimed to describe the strength of the association between domestic violence exposure and self-injurious thoughts and behaviors among the adult population based on global literature. It also aimed to identify mediating and moderating factors and discuss whether the association varies according to the form of violence, sex, and between high-income

Narcotic-Seeking Behavior and Self-Injury: A Report of Three Cases.

Patients addicted to prescription opiates have found innovative ways to reliably obtain their desired prescription opiates at a time when such prescriptions are restricted due to the opioid crisis. Instead of turning to the black market, some patients deceive their health care providers and malinger to create or enhance severe chronic pain conditions that require medically necessary treatment with prescription opiates. Such sophisticated narcotic-seeking patients set up situations by which they become severely and chronically injured through natural or iatrogenic means. This article reports 3

Typology of Self-injurious Behaviors Admitted to a Tertiary Care Hospital.

BACKGROUND: If we can classify self-injurious behaviors without a priori assumptions about related variables, it could contribute to the nosology of self-injurious behaviors with etiological and management underpinnings. METHODS: A cross-sectional study was conducted among 90 adult subjects consecutively admitted to the medical and surgical wards of a tertiary care center. In addition to socio-demographic and relevant clinical variables, intent, lethality, impulsivity, social support, and stressful life events were measured. K-means cluster analysis was used to delineate groups. RESULTS: Hie

Behavioral measures of impulsivity and compulsivity in adolescents with nonsuicidal self-injury.

BACKGROUND: Nonsuicidal self-injury (NSSI) is prevalent among adolescents and research is needed to clarify the mechanisms which contribute to the behavior. Here, the authors relate behavioral neurocognitive measures of impulsivity and compulsivity to repetitive and sporadic NSSI in a community sample of adolescents. METHODS: Computerized laboratory tasks (Affective Go/No-Go, Cambridge Gambling Task, and Probabilistic Reversal Task) were used to evaluate cognitive performance. Participants were adolescents aged 15 to 17 with (n = 50) and without (n = 190) NSSI history, sampled from the ROOTS

An investigation of self-injurious behaviors in adults with severe intellectual disabilities.

Self-injurious behavior (SIB) is commonly observed among individuals with intellectual disability (ID) living in state-run supports and services centers. Specific examples of SIB include poking oneself in the eye; harming oneself by hitting, scratching, or pinching; and pica (i.e., swallowing objects causing bodily harm). Previous research has focused on SIB in individuals with ID more generally without focusing on specific levels of ID or taking into account other important personal variables. This study examined 45 adults with severe ID living in two large state-run facilities in the Southea

Alexithymia as a mediator between childhood trauma and self-injurious behaviors.

OBJECTIVE: The aim of this study was to test whether alexithymia mediates the relationship between childhood maltreatment and self-injurious behaviors (SIB) in college women. METHOD: The sample was comprised of 100 female undergraduate students. Measures were the Childhood Trauma Questionnaire [D. Bernstein, L. Fink, Manual for the Childhood Trauma Questionnaire, The Psychological Corporation, New York, 1998], the Toronto Alexithymia Scale-20 [Journal of Psychosomatic Research 38 (1994) 23; Journal of Psychosomatic Research 38 (1994) 33], and the Self-Injurious Behaviors Questionnaire which a

Early development of stereotyped and self-injurious behaviors: I. Incidence.

From five programs with 457 eligible children, 39 children younger than 40 months who had developmental disabilities were studied. Video sampling, observation notes, parent reports, staff reports, and clinical records were the data sources. Twenty-one children, or 4.6% of the total number in the program, exhibited SIBs. Virtually all children in the sample who were motorically capable of it showed body-rocking in at least one position. The SIBs were directed mainly toward the head, and body-rocking occurred mainly in four-point and seated position.

Self-injury by people with mental retardation: a compulsive behavior hypothesis.

Self-injury is a significant problem for many individuals with developmental disabilities, particularly those with severe or profound mental retardation. Many hypotheses have been suggested to account for self-injury, but none has been comprehensive. In this paper hypotheses suggesting psychological, behavioral, physiological, or neurochemical factors as causes of self-injury were critically reviewed. A compulsive behavior hypothesis was then introduced, which allows for alternative interpretations of some existing data and suggests several readily testable predictions.

Predictors of treatment non-response for depression in a sample of female adolescents with risk-taking and self-injurious behavior - A one year follow-up.

OBJECTIVE: Depressive disorders (DD) are highly prevalent among adolescents. While up to 60 % do not respond to treatment, evidence on predictors of treatment non-response in this age group is mixed, impeding meaningful clinical implications. Drawing on a consecutive clinical cohort of adolescents with risk taking and self-injurious behavior, the present study aimed to identify predictors of treatment non-response for female DD in a naturalistic one year follow-up. METHODS: The sample comprised female adolescents with verified DD (n = 152). Patients underwent assessments at baseline (T0) and

Non-extraction solution for self-injurious behavior in children with Lesch-Nyhan syndrome: a retrospective case series with up to 14 years of follow-up.

Lesch-Nyhan syndrome (LNS) is a rare X-linked disorder characterized by hyperuricemia, neurologic impairment, and severe self-injurious behavior (SIB), frequently involving the oral tissues. Dental management of oral SIB remains challenging, and irreversible tooth extraction is still commonly employed. This retrospective case series describes four unrelated patients with LNS (aged 2.5 to 16 years) treated over a 20-year period using a customized dual-laminate thermoplastic intraoral appliance. The primary outcome was cessation of oral self-injury; secondary outcomes included tissue healing, ap

Self-Inflicted Head Injury in Behavioral Variant Frontotemporal Dementia with Compulsive Behaviors: A Case Report.

Here we present the case of a 56-year-old right-handed White male who developed osteomyelitis and empyema after repetitive compulsive excoriation rituals. His recent history included profound personality changes, apathy, loss of empathy, limited insight, behavioral agitation, and episodic memory loss. In addition to these progressive behavioral deficits, he had significant difficulties with executive functioning, leading to the loss of his job and inability to independently perform instrumental activities of daily living. Brain MRI showed asymmetric enlargement of the right lateral ventricle a

Frequency of body focused repetitive behaviors and comparison to self-injurious behaviors in patients with tic disorders.

The majority of patients with chronic tic disorders suffer from psychiatric comorbidities, most frequently attention-deficit/hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD), depression, and anxiety. While characteristics of these psychiatric disorders are widely known, other comorbidities including self-injurious behaviors (SIB) and body focused repetitive behaviors (BFRB) are still under-investigated. Currently, there is a controversial discussion whether BFRB and SIB belong to the same spectrum because of clinical similarities or they represent distinct entities. In this s

An open-label prospective pilot trial of nucleus accumbens deep brain stimulation for children with autism spectrum disorder and severe, refractory self-injurious behavior: study protocol.

BACKGROUND: Children and youth with autism spectrum disorder (ASD) may manifest self-injurious behaviors (SIB) that may become severe and refractory with limited pharmacologic or behavioral treatment options. Here, we present the protocol of a prospective, mixed-methods study to assess the safety and efficacy of deep brain stimulation (DBS) of the nucleus accumbens (NAcc) for children and youth with ASD and severe, refractory SIB. METHODS: This is a prospective, single-center, single-cohort, open-label, non-randomized pilot trial of 6 patients. Participants will be recruited through specializ

The Persistence of Self-injurious and Aggressive Behavior in Males with Fragile X Syndrome Over 8 Years: A Longitudinal Study of Prevalence and Predictive Risk Markers.

Self-injurious and aggressive behaviors are common in fragile X syndrome (FXS). However, little is known about the persistence of these behaviors and associated risk markers. We established the prevalence and persistence of self-injurious and aggressive behaviors over eight years in males with FXS, and associations with risk markers. Results showed 77% and 69% persistence rates for self-injurious and aggressive behavior, respectively. Baseline levels of repetitive behavior predicted persistent self-injurious behavior. Chronological age, impulsivity and overactivity were associated with persist

Sexual Orientation and Exposure to Close Others' Self-Injurious Thoughts and Behaviors.

IMPORTANCE: Sexual minority young adults experience disproportionately high rates of self-injurious thoughts and behaviors (SITB), including suicidal ideation, suicide attempts, and nonsuicidal self-injury (NSSI). While exposure to close others' SITB (eg, a friend or family member's suicide attempt) is a known risk factor for SITB, its role in sexual orientation disparities in SITB remains underexplored. OBJECTIVE: To examine whether exposure to close others' SITB is associated with sexual orientation disparities in SITB. DESIGN, SETTING, AND PARTICIPANTS: This cohort study assessed data fro

Profile of childhood trauma subtypes associated with self-injurious thoughts and behaviors.

OBJECTIVE: Childhood trauma has been identified as a risk factor for self-injurious thoughts and behaviors (STBs), but the roles of different types of childhood trauma have not been clarified. The current study aimed to explore the association between different childhood trauma experiences and STB profiles. METHOD: The current study utilized data from a cross-sectional survey of 89,281 Chinese university and college students (Mage = 19.6). Participants were classified into one of six STBs groups, including individuals who are not suicidal or having nonsuicidal self-injury (NS) behavior, indiv

Non-suicidal Self-injurious Thoughts and Behaviors Among Adolescent Inpatients.

Non-suicidal self-injury (NSSI) is a serious public health concern that typically onsets during early adolescence. Adolescents (N = 980, ages 12-19 years) admitted for acute, residential psychiatric treatment completed baseline clinical interviews assessing mental disorders and questionnaires measuring demographics, early life adversity, and symptom severity. Prevalence rates of NSSI for lifetime (thoughts: 78%; behaviors: 72%), past year (thoughts: 74%; behaviors: 65%), and past month (thoughts: 68%; behaviors: 51%) were high. Although effect sizes were modest, the presence of a lifetime depr
