A comparison of self-reported emotional and trauma-related concerns among sexually abused children with and without sexual behavior problems.

Numerous studies document concomitant features of sexual behavior problems (SBPs) among children 12 years of age or younger, but rarely does research involve child self-report assessments. This study provides the most comprehensive examination to date of self-reported concerns among children with SBP, using a large sample (N = 392) of clinically referred participants who reported sexual abuse histories. Children between the ages of 8 and 12 were categorized as demonstrating SBP (n = 203) or not demonstrating SBP (n = 189) as determined by scores on the Child Sexual Behavior Inventory. Children

Sexual trauma and interest in mobile health among women with mental illness in Ghana.

Sexual violence against vulnerable populations is common worldwide. Many survivors of sexual assault experience long-term mental health difficulties. This study evaluated sexual violence exposure among women with mental illness in Ghana and examined their readiness to engage in mobile health interventions. We surveyed 200 women receiving inpatient or outpatient treatment at a large psychiatric hospital. Survey results indicated that 41.0 % reported having experienced sexual violence in the past. Over two-thirds of respondents had a high probability of PTSD (68.4 %) and these proportions were h

Associations among military sexual trauma, positive alcohol expectancies, and coping behaviors in female veterans.

OBJECTIVE: Military sexual trauma (MST) is prevalent among female veterans and is associated with deleterious health outcomes. Adaptive coping strategies (e.g., emotional support) are associated with more positive outcomes, while maladaptive strategies (e.g., substance use) are associated with greater impairment. However, research on factors that influence specific coping strategy use is limited. For women with a history of MST, expectancies about the effects of alcohol may enhance the use of maladaptive and reduce the use of adaptive strategies. The present study tested this hypothesis. Exploring Risk for Sexual Recidivism and Treatment Responsivity Through the Lens of Early Trauma.

Adverse childhood experiences (ACE) are common in the histories of individuals who have sexually offended. Many risk factors for sexual recidivism resemble symptoms of early trauma, and early trauma may present a responsivity barrier to engagement in offense-focused treatment. Using the ACE scale, the current study aimed to (i) examine relationships between ACE scores and static and dynamic risk assessment scores, (ii) examine whether ACE scores differ between treatment completers versus noncompleters, and finally (iii) examine whether ACE scores predict treatment noncompletion. ACE scores wer

Military Sexual Trauma: Gender, Military Cultures, and the Medicalization of Abuse in Contemporary America.

Sexual violence is a serious problem within armed services. This article explores intra-service rape in branches of the U.S. military from the 1990s to the present. The article begins by establishing the parameters of the crisis of sexual abuse within the U.S. armed services. Second, it explores systematic failures to recognize forms of suffering. Victim-survivors in the military are vulnerable to military-specific obstacles to reporting their abuse and being believed. Attention is paid to differences by gender and sexual orientation. Third, it analyses the medicalization of suffering in the m

Sexual trauma history is associated with reduced orbitofrontal network strength in substance-dependent women.

AIM: Substance use disorders (SUDs) are highly comorbid with post-traumatic stress disorder (PTSD). PTSD-SUD comorbidity is associated with greater functional impairments and relapse risk. Women with SUDs experience markedly higher rates of trauma and PTSD compared to men with SUDs, particularly due to sexual and domestic abuse. Despite the strong association between trauma exposure and SUDs, the neurobiological correlates are understudied, particularly among females with SUDs. However, there is indication of abnormal somatic and interoceptive processing in women with PTSD. The present study e

Children's Coping and Perceptions of Coping Efficacy After Sexual Abuse: Links to Trauma Symptoms.

Despite an emphasis on coping following childhood sexual abuse (CSA) to reduce trauma-related symptoms, very few studies have researched the associations between sexually abused children's coping and trauma-related difficulties, and perceived coping efficacy has been largely overlooked. The current study investigated whether children's use and perceived efficacy of avoidant, internalized, angry, and active/social coping strategies were associated with caregiver- and child-reported posttraumatic stress symptoms (PTSS), and caregiver-reported internalizing and externalizing symptoms among 202 se

Effects of trauma and PTSD on self-reported physical functioning in sexual minority women.

OBJECTIVE: Sexual minority women (lesbians, bisexual women, and women who partner with women) experience high rates of trauma exposure, are more likely to develop posttraumatic stress disorder (PTSD), and have high rates of physical health problems compared with heterosexual women. The present study tested whether PTSD may be the mechanism through which trauma exposure affects self-reported physical functioning in this population. METHOD: In a sample of 857 sexual minority women, we examined the association between trauma exposure and worsening physical functioning (measured using the 12-item

Assailant identity and self-reported nondisclosure of military sexual trauma in partnered women veterans.

OBJECTIVE: Department of Veterans Affairs estimates of military sexual trauma (MST) suggest 27% of female veterans have experienced MST. However, Department of Defense data (Department of Defense, 2014) show that a subgroup of active-duty women do not report sexual assaults to a military authority, suggesting barriers to disclosure exist among military samples. No study of female veterans has examined rates of nondisclosure among those with previous screens for MST; these data could inform screening efforts and establishment of safe havens for candid disclosures. METHOD: Using an explanatory

Sexual Coercion, Trauma, and Sex Work in Justice-Involved Women with Substance Use Disorders.

Sexual victimization is strongly associated with mental health problems, traumatic responses, earlier onset of substance use and criminal justice involvement. It is well known that multiple forms of sexual victimization and aggression have been experienced by most incarcerated women. Two hundred women with a substance use disorder and involvement with the criminal justice system within the previous two years (parole, probation, arrest, or incarceration) were recruited from multiple sites in metropolitan Chicago. We examined whether empowerment moderates the relationship between trauma symptoms

Sexual risk, substance use, mental health, and trauma experiences of gang-involved homeless youth.

This study examined the associations of sexual risk behaviors, substance use, mental health, and trauma with varying levels of gang involvement in a sample of Los Angeles-based homeless youths. Data were collected from 505 homeless youths who self-reported various health information and whether they have ever identified as or been closely affiliated with a gang member. Multivariable logistic regression assessed associations of lifetime gang involvement with risk taking behaviors and negative health outcomes. Results revealed seventeen percent of youths have ever identified as a gang member and

Examining the associations between sex trade involvement, rape, and symptomatology of sexual abuse trauma.

The high prevalence of rape and sexual trauma symptomatology among women involved in street-based sex trades is well-established. Because prior research has lacked appropriate, non-sex trade involved comparison groups, it is unknown whether differences exist among similarly situated women who do and do not trade sex. This article explores experiences of childhood and adult rape and symptomatology of sexual abuse trauma among a community-based sample of 322 women who use methamphetamine in San Francisco, California, 61% of whom were involved in the sex trade. Study participants were recruited v

Women at war: understanding how women veterans cope with combat and military sexual trauma.

The wars in Iraq (Operation Iraqi Freedom, OIF) and Afghanistan (Operation Enduring Freedom, OEF) have engendered a growing population of US female veterans, with women now comprising 15% of active US duty military personnel. Women serving in the military come under direct fire and experience combat-related injuries and trauma, and are also often subject to in-service sexual assaults and sexual harassment. However, little is known regarding how women veterans cope with these combat and military sexual trauma experiences once they return from deployment. To better understand their experiences,

An evolving integrative treatment program for military sexual trauma (MST) and one veteran's experience.

Military sexual trauma (MST) increases the risk for Posttraumatic Stress Disorder (PTSD) and multiple other comorbidities, presenting substantial challenges for nurses and psychiatric and medical clinicians. A specialized VA Medical Center outpatient program is patterned after Herman's three-phased, empirically-supported, recovery treatments. We use a case example of a female veteran MST survivor to illustrate our treatment model. She presented to our program meeting diagnostic criteria for PTSD, Major Depressive Disorder, and a history of substance abuse. Post-treatment she demonstrated impro

Military-related sexual trauma among Veterans Health Administration patients returning from Afghanistan and Iraq.

We examined military-related sexual trauma among deployed Operation Enduring Freedom and Operation Iraqi Freedom veterans. Of 125 729 veterans who received Veterans Health Administration primary care or mental health services, 15.1% of the women and 0.7% of the men reported military sexual trauma when screened. Military sexual trauma was associated with increased odds of a mental disorder diagnosis, including posttraumatic stress disorder, other anxiety disorders, depression, and substance use disorders. Sexual trauma is an important postdeployment mental health issue in this population.

Symptoms of trauma and traumatic memory retrieval in adult survivors of childhood sexual abuse.

We examined posttraumatic stress symptoms and the memory retrieval process in 2 groups of adult survivors of childhood sexual abuse: 29 participants who reported having memories of their abuse prior to entering therapy (PM) and 13 who reported no memories of abuse prior to therapy (NPM). Participants were asked to indicate on checklists symptoms of constriction, hyperarousal, and intrusion experienced (a) prior to entering therapy and (b) during the surfacing of a memory while in therapy. Overall, the findings indicate that for both groups the same cluster of posttraumatic stress symptoms occu

Assailants' sexual dysfunction during rape: prevalence and relationship to genital trauma in female victims.

BACKGROUND: Partial or complete failure to maintain an erection sufficient for coitus is known to occur in a proportion of sexual assailants during the rape episode. OBJECTIVE: The purpose of this study was to determine whether the presence of coercion, physical violence, and genital injury associated with sexual assaults is influenced by the assailant's erectile impotence. METHODS: This prospective, observational study evaluated consecutive female patients presenting to a free-standing urban sexual assault clinic during a 3-year study period. Sexual assault victims presenting directly to fo

Specific PTSD symptom clusters mediate the association of military sexual trauma severity and sexual function and satisfaction in female service members/veterans.

BACKGROUND: Sexual satisfaction and function are vastly understudied in female service members/veterans (SM/Vs). Military sexual trauma (MST) is associated with poorer sexual satisfaction and function, but the mechanism through which MST relates to sexual satisfaction and function is unknown. Posttraumatic stress disorder (PTSD) is one of the most frequent diagnoses following MST, and those with poorer sexual satisfaction and function experience higher PTSD symptoms, particularly numbing and anhedonia symptom clusters. In this study, we examined which symptom clusters (re-experiencing, avoidan

Trauma symptoms, sexual behaviors, and substance abuse: correlates of childhood sexual abuse and HIV risks among men who have sex with men.

Childhood sexual abuse is associated with high-risk sexual behavior in men who have sex with men. This study examined psychological and behavioral correlates of HIV risk behavior associated with childhood sexual abuse in a sample of men who have sex with men. Men attending a large gay pride event (N = 647) completed anonymous surveys that assessed demographic characteristics, childhood sexual abuse history, symptoms of dissociation and trauma-related anxiety, borderline personality characteristics, substance use, and sexual risk behavior. Results indicated that men who have a history of childh

Co-rumination Goals Among Emerging Adult Women in the US Coping With Sexual Trauma.

Sexual violence (SV) victimization is a common occurrence for women and severely impacts mental health, making effective coping essential. Co-rumination with friends is one way SV survivors cope, yet their specific goals for repeatedly discussing their trauma remain unexplored. This study, guided by the transactional theory of stress and coping, analyzed open-ended responses from 100 college women (ages 18-25) who experienced attempted or completed rape at age 14 or later. Thematic analysis revealed five co-rumination goals: Unloading, Validation and Clarity, Help, Not Feeling Alone, and Preve
