Reverse Shoulder Arthroplasty to Treat Proximal Humerus Fracture Sequelae: A Review.

While several proximal humerus fractures treated nonsurgically reach satisfactory outcomes, some become symptomatic malunions or nonunions with pain and dysfunction. When joint-preserving options such as malunion or nonunion repair are not optimal because of poor remaining bone stock or glenohumeral arthritis, shoulder arthroplasty is a good option. Because of the semiconstrained design of reverse shoulder arthroplasty, it is effective at improving function when there is notable bony deformity or a torn rotator cuff. Clinical studies have demonstrated reliable outcomes, and a classification sy

The biomechanical effects of acromial fracture angulation in reverse total shoulder arthroplasty.

BACKGROUND: The biomechanical changes and treatment guidelines on acromial fracture after reverse shoulder arthroplasty (RSA) are still not well understood. The purpose of our study was to analyze the biomechanical changes with respect to acromial fracture angulation in RSA. METHODS: RSA was performed on 9 fresh-frozen cadaveric shoulders. An acromial osteotomy was performed on the plane extending from the glenoid surface to simulate an acromion fracture. Four conditions of acromial fracture inferior angulation were evaluated (0°, 10°, 20°, and 30° angulation). The middle deltoid muscle loadi

Anterior Shoulder Dislocation With an Ipsilateral Humeral Shaft Fracture: A Case Report.

Anterior shoulder dislocation is the most common type of shoulder dislocation but if accompanied by an ipsilateral humeral shaft fracture, it becomes extremely rare. There was no clear approach for dealing with these cases. We would like to present a case of a 17-year-old medically free male who was brought to the emergency department by ambulance after a road traffic accident. The patient was conscious, alert, and oriented. His Glasgow Coma Scale (GCS) was 15/15. He had multiple bruises all over his body with obvious swelling in his right arm with an inability to move the arm. There was tende

Treatment of unusual locked posterior fracture-dislocation of the shoulder: a case series.

BACKGROUND: Locked posterior fracture-dislocation of the shoulder (LPFDS) is a very rare injury that occurs predominantly in young patients following high-energy trauma. The long-term outcome of the treatment of this injury is often poor. This study sought to present the characteristics of injury, discuss the pathological anatomy, and to report the treatment outcomes of our case series. METHODS: Between January 2012 and May 2018, a total of 234 patients who underwent surgical treatment for proximal humerus fractures were reviewed. Among them, six patients (mean age, 54.7 years; range, 35-76 y

Acromial fracture after reverse total shoulder arthroplasty: a systematic review.

BACKGROUND: The reverse total shoulder arthroplasty has become the most common method of arthroplasty of the shoulder. The complication of acromial or scapular stress fracture deserves consideration to describe incidence and determine whether prosthetic design or patient factors act as risk factors. METHODS: A systematic review of the literature was performed including the EMBASE, Medline and the Cochrane Library in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. RESULTS: The search returned 565 articles. After exclusion, 25 papers remained. In

Reverse shoulder arthroplasty for type 1 sequelae of a fracture of the proximal humerus.

AIMS: The aim of this study was to analyze the results of reverse shoulder arthroplasty (RSA) in patients with type 1 sequelae of a fracture of the proximal humerus in association with rotator cuff deficiency or severe stiffness of the shoulder. PATIENTS AND METHODS: A total of 38 patients were included: 28 women and ten men. Their mean age at the time of arthroplasty was 73 years (54 to 91). Before the RSA, 18 patients had been treated with open reduction and internal fixation following a fracture. A total of 22 patients had a rotator cuff tear and 11 had severe stiffness of the shoulder wit

Delayed union of a scapular fracture-an unusual cause of persistent shoulder pain.

INTRODUCTION: Scapular fractures in athletes are rare, although they have been reported in football and baseball players. Most scapular fractures heal with nonoperative management; delayed union is rarely a problem. CASE SUMMARY: A 15-yr-old male fell onto his posterior shoulder after he had been pushed into the boards and then fell to the ice during a hockey check. He was diagnosed with a rotator cuff injury at an outside facility. Initial radiographs were negative. The patient was not immobilized, and he continued to play hockey despite intermittent pain. Seven months later, he presented to

The success of closed reduction in acute locked posterior fracture-dislocations of the shoulder.

We retrospectively reviewed the results of closed treatment in 7 patients with acute locked posterior fracture-dislocations of the shoulder. Shoulders were reduced with patients under general anesthesia and were splinted in neutral rotation for 6 weeks. Follow-up averaged 46 months (range, 24-88 months). All shoulders were reduced within 14 days. Humeral head defects ranged from 18% to 32% of the articular surface, and all patients had documented posterior instability under anesthesia after relocation. Of the 7 patients, 6 had a stable shoulder. The mean American Shoulder and Elbow Surgeons sh

Bilateral anterior dislocation of the shoulders with proximal humeral fractures: a case report.

Bilateral simultaneous anterior dislocation of the shoulders with bilateral 3-part fracture of the proximal humeri is unusual. A 42-year-old man presented with pain and restriction of movement on both shoulders. He was injured by a heavy object falling over his back while he was leaning forward holding an overhead bar. His arms were abducted and externally rotated. The injury was not correctly diagnosed, and the patient was treated with repeated manipulations and splintage for 2 weeks. Radiological examination revealed bilateral anterior dislocation of the shoulders with displaced 3-part fract

Spiral fracture of the radius: an unusual case of shoulder dystocia-associated morbidity.

BACKGROUND: The most common neonatal complications associated with shoulder dystocia include transient brachial plexus palsy, clavicular fracture, and humeral fracture. Fracture of the fetal radius has not been previously reported. CASE: We encountered a shoulder dystocia with the fetal head in the right occiput anterior position that necessitated the McRoberts maneuver, suprapubic pressure, the Wood and Rubin maneuvers, and extraction of the posterior fetal arm to effect delivery. The 4610-g infant experienced a spiral fracture of the right (anterior) radius and a fracture of the left (poste

Triple "E" syndrome: bilateral locked posterior fracture dislocation of the shoulders.

Bilateral locked posterior fracture dislocation of the shoulders is one of the least common injuries of the shoulder, and this injury has been suggested to be pathognomonic of seizures when diagnosed in the absence of trauma. The authors present a case of idiopathic bilateral locked posterior fracture dislocations of the shoulder, along with a review of the medical literature. The authors also present the "triple E syndrome," describing the possible etiologies of this injury: epilepsy (or any convulsive seizure), electrocution, or extreme trauma.

[Fractures of the scapula and injuries of the acromioclavicular joint. The traumatized shoulder and its sequelae].

Fractures of the scapula are rare and mainly result from a severe direct trauma. Most fractures may be treated non-operatively, only severely displaced and unstable fractures of the neck and/or the clavicula, unstable intraarticular fractures of the glenoid or open fractures (especially in combination with neurovascular lesions) should be fixed operatively. A dislocated fracture of the clavicula or glenoid should be approached first ventrally. Displaced fractures of the neck and body may also require an open reduction and fixation through a dorsal approach. Careful preoperative planning and an

Posterior fracture-dislocation of the shoulder. A superior subacromial approach for open reduction.

We describe 11 cases of posterior fracture-dislocation of the shoulder that required open reduction and fixation. Difficulties with access through anterior approaches led us to use the superior subacromial approach. This is an extension of the approach often used to expose the rotator cuff; the joint is opened by splitting the supraspinatus tendon 5 mm behind the cuff interval. The glenoid, proximal humerus and any fracture fragments can be seen from above, allowing reduction of the dislocation and osteosynthesis to be performed with minimal risk of damage to the humeral head and its blood sup

Posterior shoulder dislocation with ipsilateral humeral shaft fracture. A case report.

A 41-year-old man incurred the unusual combination of subacromial posterior dislocation of the shoulder and ipsilateral humeral shaft fracture. The importance of inspection of joints proximal to long bone fractures cannot be overemphasized. The transscapular view is easily adapted to the diagnosis of this injury. When recognized early, closed reduction is the preferred treatment; open reduction, as in the present case, is reserved for chronic dislocation.

Reverse Total Shoulder Arthroplasty for Failed Osteosynthesis in Proximal Humerus Fractures: Outcomes and Challenges.

Background Proximal humerus fractures (PHF) are among the most common humeral fractures, particularly affecting elderly individuals, often following low-energy trauma. While conservative treatment is often preferred in geriatric patients, surgical management remains indicated for displaced fractures. Reverse total shoulder arthroplasty (RTSA) has increasingly been used as a primary treatment option for complex PHF, especially Neer three and four-part patterns. However, in the setting of failed osteosynthesis, there is a lack of evidence regarding the role and outcomes of RTSA. Methods A retros

Reverse shoulder arthroplasty in proximal humerus fractures: What is the optimal neck-shaft-angle?

PURPOSE: Limited evidence exists for functional outcomes of neck-shaft-angle (NSA) in reverse shoulder arthroplasty (RSA) for proximal humerus fractures (PHFs). We evaluated the functional outcomes of 155° versus 145° NSA RSA, to determine superiority for acute PHFs. METHOD: Retrospective analysis of 66-patients with displaced 3/4-part, head-split or fracture-dislocation PHFs, managed with acute RSA, within 4-weeks of injury at single trauma unit. 33-patients underwent 155° NSA RSA cemented stem; 33-patients 145° NSA RSA uncemented stem. All had same glenoid components with equivocal humeral s

Outcomes of intraoperative greater tuberosity fractures in reverse shoulder arthroplasty: a matched cohort study.

BACKGROUND: Although uncommon, intraoperative greater tuberosity fractures (IGTFs) can occur during reverse total shoulder arthroplasty (rTSA) and may undermine clinical results. Our purpose was to determine, for both primary and revision rTSA, the 1) incidence of IGTF, 2) surgical steps when IGTF most often occurred, 3) healing rate of IGTF (assessed radiographically), and 4) clinical results (range of motion [ROM], pain, satisfaction, and patient-reported outcome measures [PROMs]) in patients with and without IGTF. METHODS: We used our institution's shoulder database to retrospectively iden

Acromial stress fractures and reactions after reverse total shoulder arthroplasty: a case-control study.

BACKGROUND: Acromial stress fractures can occur after reverse total shoulder arthroplasty (rTSA). We performed this study to assess the incidence, risk factors, characteristics, and outcome of acromial stress fractures and reactions after rTSA. METHODS: We determined the incidence of acromial stress fractures and reactions in a cohort of patients who underwent rTSA, and assessed risk factors using a case-control design. Each patient who developed an acromial stress fracture or reaction after rTSA (case) was matched by date of rTSA with 2 patients who did not develop acromial stress fractures/
