Severe coagulopathy after a bite from a 'harmless' snake (Rhabdophis subminiatus).

European physicians may occasionally be confronted with cases of snake venom poisoning. The increasing interest in snakes as pets has unfortunately resulted in the importation of a number of venomous snakes. We here report the third known case in the medical literature of a patient who was exposed to severe coagulopathy after being bitten by a 'harmless' ringsnake (Rhabdophis subminiatus). The prolonged bleeding diathesis and the complete depletion of fibrinogen observed in our patient are consistent with the presence of a factor X activator in the venom, as has been described in a murine mode

Clinicopathological study of acute renal failure following viperine snake bite.

Fifty patients of acute renal failure following Viperine snake bite were studied. Oliguria (100%), local swelling (48%) and bleeding tendencies (42%) were the predominant clinical features encountered. Of the 25 patients in whom detailed coagulation studies were done, 24 patients had disseminated intravascular coagulation (DIC) and 1 had primary fibrinolysis. DIC was commoner with Russell's viper bite (62%) in comparison to Echis carinatus bites (40%). Renal histology obtained in 29 cases revealed tubular necrosis (35%), cortical necrosis (24%) tubular degeneration (17%) and glomerular changes

An epidemiological study of the occurrence of habu snake bite on the Amami Islands, Japan.

This study was carried out in order to estimate the risk of habu snake bite on the Amami Islands in relation to meteorological and human behavioural factors. A time-series analysis of the seasonal variation of habu snake bite was also carried out in order to clarify the possible mechanism of the epidemiological features of habu bite. It was found that the risk of habu bite has been decreasing over a long period of time on both the islands of Amamioshima and Tokunoshima. In general, the risk is higher on Tokunoshima for males and on Amamioshima it is higher for females. There was a significant

Clinical features therapy and epidemiology of tiger snake bite in dogs and cats.

Snake bite was diagnosed in 125 dogs and 115 cats over 10 years. Young sporting dogs and young cats were mainly affected. More dogs (48%) were seen in contact with tiger snakes than cats (7%). One hundred and four (84%) dogs and 89 (76%) cats were bitten in the warmer months of the year (October to March). As the incidence rose in September/October, dogs were bitten on days when the temperature was near 20 degrees C or over. The commonest presenting signs were dilated pupils and absences of pupillary light reflex. Dyspnoea, hypothermia, hindleg ataxia and glycosuria were common features in cat

A study of the current knowledge base in treating snake bite amongst doctors in the high-risk countries of India and Pakistan: does snake bite treatment training reflect local requirements?

The call for greater production of better quality anti-snake venom (ASV) is a major thrust in the effort to reduce snake bite mortality. However, snake bite mortality has many causes and these should also be addressed. A key feature of efficient ASV usage is ensuring that doctors are trained to administer ASV only when it is required and in amounts that are necessary to neutralize venom. The need for better snake bite management training has been referred to, but little attention has been paid to how effectively medical education actually prepares doctors to treat snake bite. The objective of

Snake venoms in science and clinical medicine. 1. Russell's viper: biology, venom and treatment of bites.

Russell's viper, Vipera russelli (Shaw), is distributed erratically in 10 south Asian countries and is a leading cause of fatal snake bite in Pakistan, India, Bangladesh, Sri Lanka, Burma and Thailand. In Burma it has been the 5th most important cause of death. Its venom is of great interest to laboratory scientists and clinicians. The precoagulant activity of the venom was used by Macfarlane and others to elucidate the human clotting cascade. Up to 70% of the protein content is phospholipase A2, present in the form of at least 7 isoenzymes. Possible clinical effects of the enzyme include haem

The Role of Procalcitonin in Predicting Necessity of Antivenom Administration and Clinical Severity in Snake Bites.

One of the most important steps for preventing deaths due to snake bites is to administer snake antivenom to the eligible patients in a swift manner. In our study, we aimed to investigate whether procalcitonin is useful for predicting the clinical severity and the necessity of antivenom therapy at the early stages in patients presenting with snake bite. A total of 78 patients over the age of 18 who applied to the emergency department within the first 24 hours were included in this retrospective cross-sectional study. Age and sex of patients, severity of snake bites, total antivenom vials admin

Clinical profile & complications of neurotoxic snake bite & comparison of two regimens of polyvalent anti-snake venom in its treatment.

BACKGROUND & OBJECTIVES: The optimal anti-snake venom (ASV) dose required to treat neurotoxic snake envenomation is not known. Low-dose ASV (national protocol: maximum dose 200 ml) may be as efficacious as the conventional regimen (100 ml six hourly till all symptoms disappear), but a direct comparison of the regimens is not available. The aim of this study was to test the efficacy of low-dose ASV regimen against the conventional high-dose regimen. METHODS: The clinical profile of 51 patients with neurotoxic snake envenomation was studied. Patients were treated with either the national protoc

Low dose versus high dose anti-snake venom therapy in the treatment of haematotoxic snake bite in South India.

Most of the studies on the appropriate dose of anti-snake venom (ASV) are from tertiary hospitals and the guidelines are unclear. Our observational study compared the outcomes of two prevalent treatment regimes for haematotoxic snake bite in a secondary care hospital in South India. The time to normalisation of whole blood clotting time, mortality and complications were not different between the groups. The average dose of ASV required in the low and high dose groups were 106 mL and 246 mL, respectively. Consequently, patients who received low dose ASV incurred approximately 50% less expense.

Coluber Constrictor Bite to the Eye: A Novel Case Report of a Wild Snake Bite to the Eye in North America and Review of Literature.

Snake bites involving the eye are an uncommon cause of ocular trauma; herein, we present one of the few known instances of a snake bite directly to the globe, specifically by the way of a Coluber constrictor, also known as the "Black Racer." In this case report, we describe a nine-year-old girl who presented immediately following a snake bite to the right eye. The patient's vision was unaffected despite visualized puncture wounds through the conjunctiva with associated near-total subconjunctival hemorrhage. The patient was taken to the operating room emergently for globe exploration. Circumfer

Two case reports of deadly Himalayan bites with tertiary level care: A snake and a possible scorpion.

In the Himalayan region, there is a prevalence of unknown bites (not much data except media) including snakes with high range of mortality among victims because hilly terrain leads to delay in transportation and delayed initiation of proper treatment due to lack of developed tertiary care centers. These bites can present from local hypersensitivity reactions to neurological, cardiological, respiratory, hematological, musculoskeletal, and renal manifestations. We highlight two cases that presented with delayed and varied manifestations, recovered but delayed with dedicated supportive care. A 25

Venomous snake bites in Lao PDR: a retrospective study of 21 snakebite victims in a provincial hospital.

This is a retrospective case series report of 21 snakebite victims admitted to a provincial hospital 80 km north of the capital city Vientiane between January 2007 and June 2008. There were 13 Malayan Pit Viper (MPV) and 6 Green Pit Viper (GPV) bites. Two patients could not identify the snake, but developed severe coagulopathy and were therefore most likely bitten by a MPV or GPV. Seven of 13 patients bitten by the MPV and 3 of 6 patients with GPV bites developed a coagulopathy; 2 patients bitten by a MPV had severe local signs of envenomation. The supply of antivenom was limited, thus only 6

Successful Management of a Chronic Pedal Ulcer Secondary to Snake Bite: A Case Report.

Chronic ulceration following snake-bite envenoming is a severely disabling and therapeutic challenge, often refractory to multiple treatment modalities. We report the successful management of a non-healing pedal ulcer, 30 years in duration, on the left lower limb of a 53-year-old man, secondary to a viper bite. The ulcer had a three-decade history of recurrence despite various treatments, including a failed surgical skin graft 15 years earlier. Upon presentation, extensive workup ruled out vascular insufficiency, infection, osteomyelitis, and malignancy. A wound biopsy confirmed chronic inflam

Process intensification for production of Adenovirus-like particles (ADDomer) as snake-bite therapy.

Snakebite envenomation causes over 140,000 deaths annually, with the burden disproportionately affecting low-resource regions. Conventional antivenoms (AVs), derived from hyperimmunized equines, are expensive, have limited cross-species efficacy, and pose scalability challenges. Adenovirus-like particles (ADDomer) produced using insect cell-baculovirus vector system (IC-BEVS), emerged as a promising alternative platform for antivenom development. However, current ADDomer production relies on batch-mode processes, which inherently limit yield and scalability. In this study, we aimed to overcome

Acute Acquired Hemophilia A Following Snake Bite: A Case Report and Clinical Insights.

BACKGROUND Coagulopathies can manifest on a spectrum, from minor mucosal bleeding to life-threatening hemorrhage. Minor cases can be discovered in the setting of known risk factors, such as malignancy, old age, immunosuppression. However, acquired hemophilia A diagnosed after a snake bite is of lesser-known incidence and can present in a more acute, potentially life- or limb-threatening fashion. To properly diagnose this coagulopathy, one must be familiar with the related signs, symptoms, and laboratory findings so that swift diagnosis can follow. Diagnosis is key for early proper management,

Pituitary dysfunction in survivors of Russell's viper snake bite envenomation: A prospective study.

PURPOSE: Endocrinal insufficiency caused by vasculotoxic snake envenomation is under-recognized and is mostly confined to a specific geographic area. We conducted a prospective study to determine the prevalence and pattern of pituitary-target gland insufficiencies caused by snake envenomation. MATERIALS AND METHODS: The hormonal evaluation of patients who had suffered from vasculotoxic snake envenomation was done at baseline and at 6 months of follow-up. Those patients with a documented hormonal insufficiency underwent magnetic resonance imaging (MRI) of the hypothalamo-pituitary area. The se

Envenoming by Crotalid Snake Chinese Moccasin Agkistrodon Acutus Bite - A Case Report.

Although the bites caused by snakes from former Agkistrodon family in the areas of occurrence are not rare and even have certain epidemiologic importance, in case of envenoming by Deinagkistrodon acutus the clinical studies and case reports are very sporadic. This case report describes the envenoming of a private snake breeder bitten by young Chinese moccasin Deinagkistrodon acutus to the thumb of his left hand. He sought for a medical help immediately after snakebite. Patient presented with a local oedema on the affected limb, extending up to the half of the forearm. Laboratory examinations s
