Comparison of the effectiveness of uvulopalatopharyngoplasty and laser palatoplasty for snoring.

We present the surgical management of 51 patients with a primary diagnosis of snoring. Patients with sleep apnoea syndrome were eliminated from the study. Four procedures were used: the classical uvulopalatopharyngoplasty, a limited uvulopalatoplasty, laser palatoplasty with tonsillectomy; and laser palatoplasty alone. Fifty-one of the 67 patients (76.1%) responded to a telephone questionnaire with a mean follow-up period of 18.2 months, 78.4% of patients reported improved or absent snoring. There was almost equal patient satisfaction between the conventional technique and the laser technique.

Snoring, apnoea and ENT symptoms in the paediatric community.

This study was designed to determine the prevalence of snoring and assess the extent of associated ENT symptoms in children up to 10 years of age. A questionnaire was presented to the parents of 245 children chosen at random from the General Practice list of the town of Frome, Somerset, UK. The prevalence of snoring was found to be 27%. This increased to 47% in the presence of an upper respiratory infection. Snoring was found to be significantly (P < 0.001) associated with a history of poor hearing, restless sleep, and having a cold. Less significant (P < 0.01) associations occurred with paren

Short- and long-term outcomes of uvulopalatopharyngoplasty for snoring.

Uvulopalatopharyngoplasty is a well established and highly successful operation in the treatment of snoring. However, most published data are based on relatively short-term follow-up results. Anecdotal cases of late recurrence of snoring after an initially successful surgical result have been reported but few formal studies have been performed to determine the true magnitude of this problem. We compared the short-term and long-term results on our patients and found the late recurrence rate after a minimum follow-up period of 12 months (range: 12-84 months, mean 31.3 months) to be in the region

Effectiveness of musculus palatopharyngeus resection in uvulopalatopharyngoplasty for snoring.

Sixty-eight patients with annoying snoring were included in a prospective randomized control study. Thirty-two (group 1) were subjected to classical uvulopalatopharyngoplasty (UPPP) technique; 36 (group 2) underwent a modified "extended UPPP technique" comprising the bilateral resection of the palatopharyngeal muscles. All patients were free of obstructive sleep apnea syndrome (OSAS) as documented by an overnight polysomnography. The two groups were homogeneous for age, preoperative and postoperative body mass index, and duration of follow-up. We recorded the subjective preoperative and postop

Treatment of snoring--with and without carbon dioxide laser.

Thirty patients with bedpartners complaining of their heavy snoring which had resulted in separate bedrooms were treated surgically. Three different methods were used, each group consisted of 10 patients. In group A conventional scalpel excision was used for uvulopalatopharyngoplasty including tonsillectomy. In groups B and C, patients with small tonsils were selected and therefore the tonsils were saved. Group B was operated with carbon dioxide laser under a microscope and using micromanipulator. The same kind of excision was used as in group A but the tonsils were saved. Group C was operated

Systematic review of the different aspects of primary snoring.

Primary snoring, also known as simple or non-apnoeic snoring, is regarded as the first stage of sleep disordered breathing without severe medical consequences for the snorer and co-sleeper. Although it is a highly prevalent phenomenon in the general population, our knowledge is limited because of the lack of a consensus on terminology. This systematic review of the aspects used in the definitions of simple/primary snoring was conducted to obtain an inventory of current practices and compare these definitions with the conceptual definition of the American Academy of Sleep Medicine. An evidence-based approach to the management of snoring in adults.

BACKGROUND: Snoring is frequently encountered by the otolaryngologist. Given its significant impact on quality of life and that it is a symptom of sleep-related breathing disorders, diagnosis and treatment are of major importance. In particular, the diagnosis should aim to distinguish between simple snoring and obstructive sleep apnoea. This article aims to provide a systematic, concise and evidence-based method of managing the adult patient with snoring. METHOD: This review was based on a literature search last undertaken on 30 June 2014. The MEDLINE, EMBASE and Cochrane databases were searc

Efficiency of radiofrequency assisted uvulopalatopharyngoplasty in the treatment of snoring.

Snoring is an acoustic phenomenon, which is a consequence of vibrations of the soft tissue caused by partial obstruction of the upper breathing pathway while sleeping. It is estimated that 20% of men and 5% of women between 30 and 35 years of age snore and this rate increases to 50% of men and 30% of women over 60 years of age. The aim of this study was to evaluate the efficiency of radiofrequency assisted uvulopalatopharyngoplasty in the treatment of snoring by comparing snoring index and other polysomnography values before and after surgical treatment. The study included 22 patients (men 87%

[S1 guideline on the "diagnosis and treatment of snoring in adults"].

Due to the frequency of this phenomenon and the often considerable distress caused to the affected person, competent advice, diagnosis and treatment of snoring in adults is of particular importance. The aim of this guideline is to promote high-quality medical care for patients affected by this problem. According to the three-level concept of the AWMF, it corresponds to an S1 guideline. Prior to any therapeutic intervention, relevant sleep medical history, clinical examination, as well as a mandatory objective diagnostic measure are performed. Snoring is only treated if the patient asks for it.

Role of snoring and daytime sleepiness in occupational accidents.

To establish whether snoring and excessive daytime sleepiness (EDS), the main symptoms of obstructive sleep apnea syndrome, influence the risk of occupational accidents a population-based, prospective study was performed. In 1984, 2,874 men aged 30-64 answered questions on snoring and EDS. Ten years later 2,009 (73.8% of the survivors) responded to a follow-up questionnaire including work-related questions and potential confounders. Information on occupational accidents during 1985-1994 was obtained from national register data. A total of 345 occupational accidents were reported by 247 of the

A new method for evaluating and reporting the severity of snoring.

We propose a new questionnaire-based method for evaluating and reporting the severity of snoring, the snoring scale score (SSS), which provides an assessment of the loudness, frequency and periodicity of snoring sound. Ninety-nine patients with snoring problems were assessed and the results of 19 patients who underwent surgery to reduce snoring were reported using the SSS and the linear analogue score (LAS) at six weeks and six months after surgery. All patients reported reduction of snoring on both the SSS and LAS after surgery although minor discrepancies of reporting were found in six patie

Observation of the mechanism of snoring using sleep nasendoscopy.

We have performed sleep nasendoscopy on 54 adult snorers in whom obstructive sleep apnoea had been excluded by an overnight sleep study. The purpose of the study was to identify the site or sites of noise production in each case. This was successfully achieved in 50 of the 54 and 70% showed palatal flutter snoring only. In a further 20%, palatal flutter snoring was combined with evidence of noise generation at another site. The second site was supraglottic in 10%, tonsillar in 8% and tongue base in 2%. The tongue base was also the sole site of noise generation in 8% and the epiglottis was the

[Surgical treatment of snoring and of sleep apnea syndrome].

For ten years, uvulo-palato-pharyngoplasty has become the preferred surgical treatment for snoring. Techniques, variations, postoperative followings, complications, results are reported. For sleep apnea syndrome, other surgical procedures can also be considered: tracheotomy, adenoido-tonsillectomy, nasal surgery, hyo-tongue surgery, maxillomandibular surgery. In children, adenoido-tonsillectomy is usually the adequate treatment.

Is there a relationship between the degree of nasal obstruction and snoring?

The possible connection between snoring and nasal obstruction was studied in three groups of adult patients. In group 1, comprising 112 unselected patients, 36% snored, and 50% of the snorers suffered from nasal obstruction. The other two groups consisted of selected patients: group 2 (n = 41) and group 3 (n = 40) all of whom suffered from nasal obstruction. In group 2 nasal airway resistance was normal both before and after decongestant treatment, and in group 3 nasal airway resistance was pathologically high even after decongestant treatment. No difference was found between groups 2 and 3 re

[Experience with surgical therapy in snoring and sleep apnea].

Knowledge of the connection between habitual snoring diseases and obstructive sleep apnoea has stimulated research into sleep disturbance in patients at high risk of cardiovascular disease. Any disturbance of nasal ventilation leads to a higher resistance to breathing and may cause cardiovascular complications in the long run. Anatomical and functional conditions that lead to intermittent incomplete blockage or complete occlusion of the upper respiratory tract must be detected and surgically eliminated, if possible. Surgical procedures for treating snoring and obstructive sleep apnoea may be s

Is snoring a cause of vascular disease? An epidemiological review.

Eight studies that examined the relation between snoring and vascular disease were identified. The prevalence of habitual snoring, measured by questionnaire or interview, varied from 3% to 29% of adults and was dependent on age, sex, obesity, and smoking habit. In men, habitual snoring was associated with hypertension and ischaemic heart disease, with adjusted relative risks in the range 1.3-2.0. For women, only one study provided adjusted estimates of relative risk, which were 2.8 for hypertension and 1.2 for angina. Adequately adjusted relative risks for cerebrovascular disease have not been
