Successful treatment by exchange transfusion of a young infant with sodium nitroprusside poisoning.

Although sodium nitroprusside (SNP) is often used in pediatric intensive care units, cyanide toxicity can occur after SNP treatment. To treat SNP-induced cyanide poisoning, antidotes such as amyl nitrite, sodium nitrite, sodium thiosulfate, and hydroxycobalamin should be administered immediately after diagnosis. Here, we report the first case of a very young infant whose SNP-induced cyanide poisoning was successfully treated by exchange transfusion. The success of this alternative method may be related to the fact that exchange transfusion not only removes the cyanide from the blood but also a

Nitrite poisoning caused by food contaminated with cooling fluid.

Two fatal cases of nitrite poisoning are described. From the identical clinical picture of both patients (fulminant fall of blood pressure, remarkable cyanosis attributed to methemoglobinemia, and "nasi" vomit), food poisoning seemed to be the most likely cause. Laboratory findings revealed that the food had been contaminated during transportation in a van by a leaking cooling fluid which contained sodium nitrite as anticorrosive agent.

Cyanide poisoning successfully treated without 'therapeutic methemoglobin levels'.

A 24-year-old woman ingested an unknown amount of potassium cyanide in a suicide attempt. Coma and metabolic acidosis developed. Administration of the Lilly Cyanide Antidote kit (Eli Lilly and Co, Indianapolis) resulted in prompt resolution of symptoms and full recovery. Whole blood cyanide level was 13 micrograms/mL approximately one hour after ingestion. The highest measured methemoglobin level after sodium nitrite administration was 9.2%, demonstrating that attaining a "therapeutic methemoglobin level" of 25% is unnecessary to insure a satisfactory clinical outcome. Because severe hypotensi

Pharmacological treatment of inhalation injury after nuclear or radiological incidents: The Chinese and German approach.

Inhalation injury is often associated with burns and significantly increases morbidity and mortality. The main toxic components of fire smoke are carbon monoxide, hydrogen cyanide, and irritants. In the case of an incident at a nuclear power plant or recycling facility associated with fire, smoke may also contain radioactive material. Medical treatments may vary in different countries, and in this paper, we discuss the similarities and differences in the treatments between China and Germany. Carbon monoxide poisoning is treated by 100% oxygen administration and, if available, hyperbaric oxygen

A simple protocol for the synthesis of dinitrosyl iron complexes with glutathione: EPR, optical, chromatographic and biological characterization of reaction products.

The diamagnetic binuclear form of dinitrosyl iron complexes (B-DNIC) with glutathione can be easily synthesized in the air at ambient temperature. The synthetic protocol includes consecutive addition to distilled water of glutathione, which decreases the pH of the test solution to 4.0, a bivalent iron salt (e.g., ferrous sulphate) and sodium nitrite at the molar ratio of 2:1:1, with a subsequent increase in pH to neutral values. Under these conditions, the amount of B-DNIC formed is limited by initial nitrite concentration. In the novel procedure, 20mM glutathione, 10mM ferrous sulfate and 10m

[Induction of tumours by different endogenously formed N-nitroso-ureas in the hooded rat (author's transl)].

By means of application of methyl-, ethyl-, n-propyl-, iso-propyl- and n-butylurea together with sodium nitrite the formation of corresponding carcinogenic N-nitroso compounds is demonstrated. The transplacental action of endogenously formed ethylnitrosourea led to a characteristic spectrum of tumours in the offspring comprising mainly neurogenic neoplasms of the brain and Nn. trigemini. The tumour incidence is especially high after application of precursors (ethylurea and nitrite) with the food. In postnatal experiments with methylurea and nitrite, there were induced mostly neurogenic tumours

Effects of inhalation of low-dose nitrite or carbon monoxide on post-reperfusion mitochondrial function and tissue injury in hemorrhagic shock swine.

INTRODUCTION: Tissue reperfusion following hemorrhagic shock may paradoxically cause tissue injury and organ dysfunction by mitochondrial free radical expression. Both nitrite and carbon monoxide (CO) may protect from this reperfusion injury by limiting mitochondrial free radial production. We explored the effects of very small doses of inhaled nitrite and CO on tissue injury in a porcine model of hemorrhagic shock. METHODS: Twenty pigs (mean wt. 30.6 kg, range 27.2 to 36.4 kg) had microdialysis catheters inserted in muscle, peritoneum, and liver to measure lactate, pyruvate, glucose, glycero

In Vivo Studies on the Interaction Between Orally Administered Nitrite and Omeprazole: Beyond Proton-Catalyzed S-Nitrosation.

Inorganic nitrite contributes to the nitrosation of biomolecules and exerts antioxidant effects. The proton pump inhibitor omeprazole has pro-oxidant effects, inhibits the formation of nitroso species in the stomach, and abrogates the blood pressure-lowering effects of orally administered nitrite. Here, we examine whether a two-week treatment with nitrite leads to tissue nitrosation that scales with local thiol concentrations and whether oral nitrite treatment can prevent the pro-oxidant effects of omeprazole. Male Sprague-Dawley rats received daily doses of omeprazole 10 mg/kg i.p. (or vehicl

Antidotal use of methemoglobin forming cyanide antagonists in concurrent carbon monoxide/cyanide intoxication.

An estimated 35% of all fire victims in the United States have toxicologically significant blood levels of CO and CN. However, the treatment of concurrent CO/CN intoxication has been paid scant attention. The suggestion has been made that these victims should be treated for CN poisoning. The current therapeutic management of CN poisoning in this country includes the utilization of two methemoglobin formers: amyl nitrite and sodium nitrite. This study was undertaken to determine if the administration of methemoglobin formers is advisable, as the victim is already suffering from O2 deprivation d

Survival and Outgrowth of Clostridium sporogenes Spores during Curing and Storage of Corned Beef with Reduced Levels of Sodium Chloride.

Brines for curing meats contain salt and nitrite levels (up to 21% and 900 ppm, respectively) that inhibit outgrowth of spores. Brines containing lowered levels of salt were examined for their effect on Clostridium sporogenes spore outgrowth in cured corned beef during curing and subsequent storage (20°C). Outgrowth of C. sporogenes spores was monitored in the corned beef and also in Botulism Assay Medium (BAM) under various levels of sodium chloride and nitrite and different pH values. NaCl concentrations of 2.5% or less in cured corned beef had little effect on initial rates of growth, but 3

[Features of ozone effect on the oxygen-dependent blood processes under hypercapnia conditions].

The aim of this work is to study of ozone effect on blood oxygen-dependent processes under hypercapnia conditions. The studied blood samples are pretreated with a hypercapnic gas mixture followed by the addition of ozonized isotonic sodium chloride solution (with an ozone concentration of 6 mg/l), as well as gaseous transmitters donors, nitroglycerin and sodium hydrosulfide. It has been established that hypercapnia enhanced the ozone effect on the blood oxygen transport function and was characterized by the oxyhemoglobin dissociation curve shift to the right, also increased hydrogen sulfide sy

An inquest into regulatory mechanism of caveolin by ischemic preconditioning against orchidectomy-challenged rat heart.

Lower level of testosterone in men is related to major risks of cardiovascular diseases. This risk may increase due to the opening of mitochondrial permeability transition pore (mPTP). The mPTP is also regulated by ischemic preconditioning (IPC) and a membrane protein known as caveolin. The cardioprotective effect of IPC is the most effective methodologies used in testosterone deficiency. Daidzein (DDZ) a caveolin inhibitor shows cardioprotective action. The experiment has been designed to evaluate the pretreated DDZ effect in IPC-mediated cardioprotective action in orchidectomy (OCZ)-challeng

Adrenergic receptors regulate macrophage secretion.

Macrophages have been shown to possess both alpha 2 and beta 2-adrenergic receptors which appear to antagonize each other. The ability of these two kinds of receptors to mediate macrophage secretion was tested, using alpha- and beta-antagonists, idazoxan and propranolol. Both antagonists could inhibit hydrogen peroxide and superoxide secretion, although the timing of the inhibition is different. However, only idazoxan has the ability to reduce nitrite accumulation, while propranolol fails to affect macrophage production. In addition, sodium nitrite was able to inhibit hydrogen peroxide and sup

Biomedical evaluation of a novel nitrogen oxides releasing wound dressing.

Chronic wounds are a major cause for both suffering and economical losses. Management of chronic non-healing wounds requires multipronged approach. They are polymicrobial and agonizing for the patient due to associated pain. Moist dressing providing antimicrobial action is a highly desirable chronic wound management option. Here we report a hydrogel based dressing that possesses the antimicrobial properties of acidified sodium nitrite and the homeostatic property of a hydrogel. The dressing was developed by combining citric acid cross-linked cotton gauze and sodium nitrite loaded gelatin. The

A critical role for gastric xanthine oxidoreductase in the formation of S-nitrosothiols and blood pressure responses to nitrite in rats.

Nitrite and nitrate bioactivation to nitric oxide (NO) in the enterosalivary cycle of nitrate offers an enormous therapeutic potential for cardiovascular and metabolic diseases and involves xanthine oxidoreductase (XOR), an enzyme widely expressed in the gastrointestinal system. XOR has nitrite reductase activity, reducing nitrite to NO, especially under low pH conditions. Here, we hypothesized that XOR activity critically interacts with acidic gastric pH to cause the blood pressure responses to oral nitrite. To test this hypothesis, rats pretreated with the XOR inhibitor allopurinol (100 mg/k

Clinical features and management of cyanide poisoning.

The pathophysiology, clinical features, and management of cyanide toxicity are reviewed and sources of cyanide are listed. Cyanide is a deadly poison that is found in many foods and household and industrial products, including some that are readily available. Cyanide binds with cytochrome oxidase, the enzyme responsible for oxidative phosphorylation, and paralyzes cellular respiration. Because the tissues cannot use oxygen that is delivered, aerobic metabolism ceases. The signs and symptoms of cyanide poisoning reflect the extent of cellular hypoxia. Manifestations may include respiratory abno

Severe Methemoglobinemia Due to Accidental Ingestion of Nitrite-Containing Nail Polish Remover: A Case Report.

BACKGROUND Methemoglobinemia is an uncommon hemoglobinopathy caused by oxidation of heme iron from its ferrous (Fe²⁺) state to the ferric (Fe³⁺) state, resulting in functional anemia that does not improve with supplemental oxygen. "Poppers" is a slang term for recreational drugs marketed as nail polish removers or room deodorizers, often sold under brand names such as "Jungle Juice," which may cause methemoglobinemia due to their nitrite content. CASE REPORT A 67-year-old White woman accidentally ingested "Jungle Juice" nail polish remover containing isobutyl nitrite, resulting in severe methe

Treatment with nitrite prevents reactive oxygen species generation in the corpora cavernosa and restores intracavernosal pressure in hypertensive rats.

Hypertension is a risk factor for erectile dysfunction (ED) and both conditions are associated with oxidative stress. Given that nitrite is described to display antioxidant effects, we hypothesized that treatment with nitrite would exert antioxidant effects attenuating both reactive oxygen species (ROS) generation in the corpora cavernosa (CC) and ED induced by hypertension. Two kidney, one clip (2K1C) hypertension was induced in male Wistar rats. Treatment with sodium nitrite (15 mg/kg/day, p.o., gavage) was initiated two weeks after surgery to induce hypertension and maintained for four week

Chemical Synthesis of Proteins Using an o-Nitrobenzyl Group as a Robust Temporary Protective Group for N-Terminal Cysteine Protection.

We report here a robust and practical strategy for chemical protein synthesis using an o-nitrobenzyl group as a temporary protective group for an N-terminal cysteine residue of intermediate hydrazide fragments. By reinvestigating the photoremoval of an o-nitrobenzyl group, we establish a robust and reliable strategy for its quantitative photodeprotection. The o-nitrobenzyl group is completely stable to oxidative NaNO2 treatment and has been applied to the convergent chemical synthesis of programmed death ligand 1 fragment, providing a practical avenue for hydrazide-based native chemical ligati

Purification of Vibrio fischeri nitrite reductase and its characterization as a hexaheme c-type cytochrome.

Dissimilatory nitrite reductase was isolated from anaerobically nitrate-grown Vibrio fischeri cells and purified to electrophoretic homogeneity. The enzyme catalyzes the six-electron reduction of nitrite to ammonia. Upon sodium dodecyl sulfate-polyacrylamide gel electrophoresis, under either nonreducing or reducing conditions, the purified nitrite reductase migrated as a single protein band of Mr 57,000. Gel filtration chromatography revealed a native molecular weight of 58,000, indicating the enzyme as isolated to be present in the monomeric form. Purified nitrite reductase exhibited typical
