Is there an association between age at first words and speech sound disorders among 4- to 5-year-old children? An epidemiological cross-sectional study based on parental reports.

OBJECTIVE: To investigate the role of the period of emergence of the first words and its interactions with other risk factors in predicting the development of speech sound disorder (SSD) among 4- to 5-year-old children. METHODS: After 373 children underwent otolaryngology and speech pathology examinations, their parents answered a questionnaire about potential risk factors for speech impairment. The presence of SSD was identified by a speech pathologist who administered Fanzago's Articulation Test to each child. Multivariate logistic analysis was used to explore the relationships between vari

Exploring the Overlap Between Dyslexia and Speech Sound Production Deficits.

PURPOSE: Children with dyslexia have speech production deficits in a variety of spoken language contexts. In this article, we discuss the nature of speech production errors in children with dyslexia, including those who have a history of speech sound disorder and those who do not, to familiarize speech-language pathologists with speech production-specific risk factors that may help predict or identify dyslexia in young children. METHOD: In this tutorial, we discuss the role of a phonological deficit in children with dyslexia and how this may manifest as speech production errors, sometimes in

Family pedigrees of children with suspected childhood apraxia of speech.

UNLABELLED: Forty-two children (29 boys and 13 girls), ages 3-10 years, were referred from the caseloads of clinical speech-language pathologists for suspected childhood apraxia of speech (CAS). According to results from tests of speech and oral motor skills, 22 children met criteria for CAS, including a severely limited consonant and vowel repertoire, difficulty sequencing syllables, and inconsistent and unusual errors. Family pedigrees for these children were constructed through parent interviews and direct testing of nuclear family members. Familial aggregation for speech-sound and language

Articulatory disorders in speech among Finnish-speaking students according to age, sex, and speech therapy.

A group of Finnish-speaking students (n = 451) was examined clinically to study occurrence of articulatory disorders in speech, controlling for the possible effects of age, sex, and previous speech therapy. Distortions of the /s/ sound were found in 16%, of the /r/ sound in 3%, and of the /l/, /n/, or /d/ sounds in about 1% of the subjects; the rarest faultily produced sounds were usually combined with other articulatory disorders. According to fitted log-linear functions, age and sex were not related to prevalence of articulatory disorders in speech among young adults, while there was a tende

Apraxia of Speech in the Spontaneous Speech of Nonfluent/Agrammatic Primary Progressive Aphasia.

BACKGROUND: Apraxia of speech (AOS) is a core feature of nonfluent/agrammatic primary progressive aphasia (naPPA), but its precise characteristics and the prevalence of AOS features in spontaneous speech are debated. OBJECTIVE: To assess the frequency of features of AOS in the spontaneous, connected speech of individuals with naPPA and to evaluate whether these features are associated with an underlying motor disorder such as corticobasal syndrome or progressive supranuclear palsy. METHODS: We examined features of AOS in 30 patients with naPPA using a picture description task. We compared th

Bridging the Gap Between Speech and Language: Using Multimodal Treatment in a Child With Apraxia.

Childhood apraxia of speech is a neurologic speech sound disorder in which children have difficulty constructing words and sounds due to poor motor planning and coordination of the articulators required for speech sound production. We report the case of a 3-year-old boy strongly suspected to have childhood apraxia of speech at 18 months of age who used multimodal communication to facilitate language development throughout his work with a speech language pathologist. In 18 months of an intensive structured program, he exhibited atypical rapid improvement, progressing from having no intelligible

Physician Knowledge and Comfort Identifying Pediatric Speech Sound Development Concerns.

OBJECTIVE: Physicians who provide pediatric care play an important role in early identification of developmental communication concerns, yet little is known about their comfort, knowledge, and screening practices related to pediatric speech sound disorders (SSDs). This study examined physicians' perspectives on universal screening, comfort screening speech sound concerns relative to language concerns, characteristics associated with comfort, self-rated knowledge across child linguistic profiles, speech sound screening tool use, and perceived training needs. METHODS: U.S.-licensed physicians w

Red flags for speech impairment: who should we refer to speech therapy?

BACKGROUND AND OBJECTIVE: Speech disorders are a common presenting concern to paediatricians. Yet evidence to guide detection and speech sound therapy referral of cases at risk for persistent speech disorder is limited, with no normative English speech data published for over two decades. Here we describe speech development in a large, contemporaneous normative sample, to help clinicians identify errors of disordered speech from developmental errors (which are of lesser concern). We also examine differences in speech development over 20 years. METHODS: Children aged 2 years 0 months-12 years

Pairing Speech Sounds With Vagus Nerve Stimulation Drives Stimulus-specific Cortical Plasticity.

BACKGROUND: Individuals with communication disorders, such as aphasia, exhibit weak auditory cortex responses to speech sounds and language impairments. Previous studies have demonstrated that pairing vagus nerve stimulation (VNS) with tones or tone trains can enhance both the spectral and temporal processing of sounds in auditory cortex, and can be used to reverse pathological primary auditory cortex (A1) plasticity in a rodent model of chronic tinnitus. OBJECTIVE/HYPOTHESIS: We predicted that pairing VNS with speech sounds would strengthen the A1 response to the paired speech sounds. METHO

Exploring Australian speech-language pathologists' use and perceptions ofnon-speech oral motor exercises.

PURPOSE: To explore Australian speech-language pathologists' use of non-speech oral motor exercises, and rationales for using/not using non-speech oral motor exercises in clinical practice. METHODS: A total of 124 speech-language pathologists practising in Australia, working with paediatric and/or adult clients with speech sound difficulties, completed an online survey. RESULTS: The majority of speech-language pathologists reported that they did not use non-speech oral motor exercises when working with paediatric or adult clients with speech sound difficulties. However, more than half of the

Speech sound skills, language comprehension, and early reading development in poor readers.

This retrospective study investigates two questions: (a) whether speech sound difficulties, reported by parents looking back on their children's early speech sound skills and concurrently at ages 7-8, can predict language comprehension and early reading challenges in children identified as poor readers and (b) whether there is a relationship between the type of speech errors and language comprehension and early reading skills in these children. Two hundred twenty-eight children identified as poor readers were assessed on reading and language comprehension. The findings revealed that children w

Prevalence of articulatory disorders of different sounds in a group of Finnish first-graders.

Occurrence and type of articulatory speech disorders were studied in 157 7-year-old first-graders found to have articulatory disorders at school and referred for further treatment to the Department of Phoniatrics, University Central Hospital. The control group consisted of all 130 first-graders in two elementary schools in Kuopio. About one third of the control group had articulatory speech disorders, one fifth of them expressing need for speech therapy. In both groups distortions of the /s/-sound were the most common disorders, distortions of the /r/-sound being rather uncommon and those of t

Speech paired vagus nerve stimulation restores neural sound processing in a rat model of autism.

INTRODUCTION: Prenatal exposure to valproic acid (VPA) is a common environmental cause of autism spectrum disorder (ASD) and often leads to expressive and receptive language impairments. Similar communication difficulties among individuals with ASD are often linked to abnormal subcortical and cortical sound processing. Rodents prenatally exposed to VPA exhibit degraded cortical responses to speech and an impaired ability to behaviorally discriminate speech sounds. METHODS: We sought to determine whether sound processing could be restored with paired vagus nerve stimulation (VNS). In a first e

Disentangling Crowdfunding from Fraudfunding.

BACKGROUND: Adenoid hypertrophy alters the posture of the oro-facial structures by obstructing the upper airways and restricting nasal breathing. These postural changes may eventually have an impact on facial skeletal development and dental occlusion. The postural adjustments may negatively impact a child's life by affecting a number of speech-production factors. OBJECTIVE: This study aimed to detect the speech sound disorders in school aged Egyptian children with adenoid hypertrophy and if they were related to structure changes in the airway that caused by adenoid hypertrophy. METHODOLOGY:

Gradient speech change during intervention for school-aged children and adults with cleft palate ± lip.

Gradient speech change, where speech sound production develops in a broadly step-wise fashion towards the standard adult form, is a well-recognised phenomenon in children developing typical speech, but is much less studied in speakers with developmental speech sound disorders. Instrumental techniques, such as electropalatography (EPG), may be useful for identifying gradient speech change and may supplement phonetic transcription in important ways. This study investigated whether gradient speech change occurred in six participants with cleft palate ± lip undergoing intervention within a usage-b

Temporal plasticity in auditory cortex improves neural discrimination of speech sounds.

BACKGROUND: Many individuals with language learning impairments exhibit temporal processing deficits and degraded neural responses to speech sounds. Auditory training can improve both the neural and behavioral deficits, though significant deficits remain. Recent evidence suggests that vagus nerve stimulation (VNS) paired with rehabilitative therapies enhances both cortical plasticity and recovery of normal function. OBJECTIVE/HYPOTHESIS: We predicted that pairing VNS with rapid tone trains would enhance the primary auditory cortex (A1) response to unpaired novel speech sounds. METHODS: VNS w
