BurstDR™ spinal cord stimulation for chemotherapy-induced peripheral neuropathy.

BACKGROUND: Chemotherapy-induced peripheral neuropathy (CIPN) is one of the most common persistent adverse effects of chemotherapy affecting cancer survivors. Other than the use of duloxetine, no other intervention has a proven benefit for these patients. Here, a patient with multiple myeloma who developed CIPN experienced a significant reduction in their symptoms after the implantation of a spinal cord stimulator. CASE DESCRIPTION: A 64-year-old male with a history of multiple myeloma presented with chronic pain 4 years after chemotherapy, followed by an autologous stem cell transplant. As t

Improvement of Post-sympathectomy Raynaud's Syndrome With Spinal Cord Stimulation.

Raynaud's phenomenon is a vascular disorder, characterized by vasospasm-induced discoloration, numbness, and pain in the extremities. While pharmacological treatments and sympathectomy are commonly employed, many patients experience symptom recurrence, and effective therapies for refractory cases remain limited. This case study presents a 60-year-old male with severe Raynaud's symptoms, including fingertip necrosis, unresponsive to pharmacotherapy and endoscopic thoracic sympathectomy. Despite initial symptom relief, the patient's condition worsened, leading to finger necrosis. Spinal cord sti

Weight Trends After Spinal Cord Stimulation Therapy for Chronic Pain.

OBJECTIVES: Spinal cord stimulation (SCS) therapy is an effective treatment for chronic pain, particularly in conditions such as postlaminectomy syndrome and complex regional pain syndrome (CRPS). Rare case reports described significant weight loss in patients who underwent dorsal column SCS therapy for chronic pain. Recently, neuromodulation for obesity has become a novel field for research. We aimed to investigate weight trends among patients treated with SCS for chronic pain. MATERIALS AND METHODS: We conducted a retrospective chart review in 342 patients treated with SCS or dorsal root ga

Percutaneous high cervical spinal cord stimulation for refractory trigeminal neuralgia.

BACKGROUND: Trigeminal neuralgia (TN) is a debilitating pain that affects the dermatomes associated with the trigeminal nerve (V1, V2, and V3). Unfortunately, many medical treatments and surgical procedures fail to sufficiently modulate the pain associated with this condition. CASE DESCRIPTION: This study presents two extreme cases of refractory TN (RTN) that progressed to atypical facial pain and describes successful mitigation of the neuralgia of said cases by percutaneous implantation of upper cervical spinal cord stimulation (SCS). The SCS was designed to target the descending spinal trig

The Role of Spinal Cord Stimulation in Axial Back Pain.

Chronic axial low back pain (LBP) is one of the most common ailments in the United States, with a significant impact on quality of life and function. Multimodal therapy is often utilized for pain relief, including opioid pain medication. Current indications for spinal cord stimulation include chronic neuropathic conditions, such as failed back surgery syndrome, radiculopathies, complex regional pain syndrome types I and II, postherpetic neuralgia, and peripheral diabetic neuropathies. While current lead placements perform remarkably when used for their appropriate indications, there is no spec

Cervical Spinal Cord Stimulation for Trigeminal Neuralgia: a Narrative Review.

BACKGROUND: Trigeminal neuralgia (TN) is a chronic neuropathic pain condition affecting one or more divisions of the fifth cranial (trigeminal) nerve. TN is defined by recurrent unilateral electric shock-like pain that is abrupt in both onset and termination. The pain is triggered by innocuous sensory stimuli and is classified as either classic TN, related to vascular compression; secondary TN, due to a tumor along the trigeminal nerve or an underlying disease like multiple sclerosis; or idiopathic TN. Among the various therapies available for TN, carbamazepine remains the first-line treatment

Spinal cord stimulation in complex regional pain syndrome type 2.

INTRODUCTION: Spinal cord stimulation (SCS) may be used to treat complex regional pain syndrome (CRPS) when other treatments fail. This study aimed to describe the effects of SCS for CRPS with known nerve injury, i.e. CRPS type 2 (CRPS-2), on pain reduction and opioid use. METHODS: Data from the outpatient clinic were retrieved from the Neurizon Neuromodulation Database. The patient records of 16 patients treated with SCS at Aalborg University Hospital, Denmark, who met the Budapest (2003) criteria for CRPS-2 and completed six- and 12+-month follow-up were included. In Excel, paired t-tests c

Allergic Dermatitis to Spinal Cord Stimulation Device: A Case Report.

BACKGROUND: Spinal cord stimulation is a safe and efficacious treatment for chronic pain. While there are limited reports in the literature, contact allergy related to these synthetic devices can occur, with nickel being the most common offending agent. CASE REPORT: We present a 66-year-old woman with chronic lower back pain, failing conservative and surgical treatment measures, who underwent successful permanent spinal cord stimulator (SCS) implantation. Following placement, she developed a rash overlying the implantation site, diagnosed by dermatology as allergic dermatitis. She attempted t

Synergetic efficacy of simultaneous DRG- and traditional spinal cord stimulation.

BACKGROUND: Dorsal root ganglion stimulation has established its role in chronic pain states and is commonly used as an alternative treatment to traditional spinal cord stimulation. Due to its approach, DRG stimulation is preferably used in pain conditions affecting a small area or a distinct nerve root. In selected patients, a combination of both techniques might be useful. METHODS: We report a series of five patients with chronic pain treated with DRG stimulation and traditional spinal cord stimulation from 2011 to 2018. Pain was reported on the VAS scale at the baseline, before and 12 mont

Neuromonitoring for Spinal Cord Stimulation Lead Placement Under General Anesthesia.

Spinal cord stimulation (SCS) is a common therapeutic technique for treating medically refractory neuropathic back and other limb pain syndromes. SCS has historically been performed using a sedative anesthetic technique where the patient is awakened at various times during a surgical procedure to evaluate the location of the stimulator lead. This technique has potential complications, and thus other methods that allow the use of a general anesthetic have been developed. There are two primary methods for placing leads under general anesthesia, based on 1) compound muscle action potentials and 2

Rate of Complications Following Spinal Cord Stimulation Paddle Electrode Removal.

OBJECTIVE: Spinal cord stimulation (SCS) is a safe, reversible surgical treatment for complex regional pain syndrome and failed back surgery syndrome refractory to conventional medical management. Paddle electrodes are routinely used for the permanent implant because of the reduced risk of migration, lower energy requirements, and expanded coverage options. The risks associated with paddle lead removal are not well defined in the literature. METHODS: We retrospectively reviewed the outcomes of all patients at the Cleveland Clinic who underwent removal of SCS paddle electrodes between 2009 and

A review of spinal cord stimulation systems for chronic pain.

Spinal cord stimulation (SCS) applications and technologies are fast advancing. New SCS technologies are being used increasingly in the clinical environment, but often there is a lag period between the clinical application and the publishing of high-quality evidence on safety and efficacy. Recent developments will undoubtedly expand the applicability of SCS, allowing more effective and individualized treatment for patients, and may have the potential to salvage patients who have previously failed neuromodulation. Already, high-level evidence exists for the safety, efficacy, and cost-effectiven

Spinal Cord Stimulation for Refractory Neuropathic Pain of Neuralgic Amyotrophy.

The aim of this paper was to report the effect of temporary and chronic spinal cord stimulation for refractory neuropathic pain in neuralgic amyotrophy (NA). A 35-year-old female presented with two-months history of a severe, relentless neuropathic pain of the left shoulder, forearm, palm, and fingers. The neuropathic pain was refractory to various medical treatments, including nonsteroidal anti-inflammatory drugs, opiates, epidural and stellate ganglion blocks, and typically unrelenting. The diagnosis of NA was made with the characteristic clinical history and magnetic resonance imaging. The

Is spinal cord stimulation safe? A review of its complications.

OBJECTIVE: We aim to evaluate the complications of spinal cord stimulation (SCS). METHODS: This article is a nonsystematic review of literature about the safety of SCS. The complications of this technique are described, focusing the analysis in their prevention, diagnosis, and treatment. RESULTS: Electrode migration is the most common complication of SCS and it is more frequent with percutaneous electrodes than with paddle-type ones. Lead migration may be solved by reprogramming the stimulator, but if it fails, surgical repositioning is mandatory. Several anchoring techniques are described i

Spinal cord stimulation: Current applications for treatment of chronic pain.

Spinal cord stimulation (SCS) is thought to relieve chronic intractable pain by stimulating nerve fibers in the spinal cord. The resulting impulses in the fibers may inhibit the conduction of pain signals to the brain, according to the pain gate theory proposed by Melzack and Wall in 1965 and the sensation of pain is thus blocked. Although SCS may reduce pain, it will not eliminate it. After a period of concern about safety and efficacy, SCS is now regaining popularity among pain specialists for the treatment of chronic pain. The sympatholytic effect of SCS is one of its most interesting thera

Oscillating field stimulation in the treatment of spinal cord injury.

The application of electrical current to injured tissue is known to promote healing. The use of this modality in healing the injured spinal cord to promote neurologic recovery has been introduced as a potential treatment for patients who previously had minimal hope of recovery. In in vitro and in vivo experiments, neural regeneration has been seen to occur, especially when an oscillating field is used. With this modality, an electrical current is applied in which the polarity changes direction on a periodic basis, preventing the "die-back" phenomenon of severed neural pathways. This mechanism
