MRI and Ultrasound of the Thoracolumbar Fascia in the Setting of Degenerative Spinal Diseases.

Background and Objectives: The thoracolumbar fascia (TLF) has been implicated in low back pain, but imaging-based characterization in degenerative lumbar disorders, particularly in surgical cohorts, remains limited. To describe MRI-derived and US-derived TLF thickness estimates in a heterogeneous degenerative lumbar surgical cohort and explore preliminary imaging patterns and associations with selected clinical variables. Materials and Methods: In this prospective single-center cohort, adults scheduled for elective lumbar surgery underwent preoperative US (short- and long-axis at L3) and revie

Diagnostic and Prognostic Value of Serum Neurofilament Light Chain in Canine Spinal Cord Diseases.

This study evaluated serum neurofilament light chain (NfL) as a biomarker for spinal cord diseases in dogs, including 46 healthy dogs and 76 with conditions, such as intervertebral disc herniation (IVDH), syringomyelia (SM), fibrocartilaginous embolism (FCE), and acute non-compressive nucleus pulposus extrusion (ANNPE). There was a significant difference in serum NfL levels between healthy dogs (12.55 pg/mL) and those with spinal cord diseases (91.10 pg/mL; p < 0.0001). The NfL level in dogs with SM (50.7 pg/mL) was significantly lower than that in dogs with IVDH (99.3 pg/mL; p = 0.012) and th

Anesthesia outcomes of pregnant women with spinal diseases: a single-center case-series study.

BACKGROUND: Neuraxial anesthesia is widely used as the most effective and standard method in obstetric anesthesia. However, there is a concern that neuraxial anesthesia may be technically difficult or ineffective in pregnant women with spinal disease. Therefore, this study aimed to investigate the implementation rate of neuraxial anesthesia among pregnant women with spinal diseases and their success rate at our institution. METHODS: The subjects of this study were pregnant patients who delivered at Juntendo University Nerima Hospital between April 2017 and December 2020. After obtaining ethic…

Motor imagery in stroke patients, or plegic patients with spinal cord or peripheral diseases.

OBJECTIVES: When motor imagery (MI) is impaired in stroke patients, it is not clear, whether this is caused by the central lesion with a disruption of networks or this may be due to inactivity/lack of practice following hemiparesis. To answer this question, we investigated MI in two groups of patients: stroke patients and patients with no central lesion, who suffered high-grade tetraparesis caused by myopathy or spinal muscular atrophy. MATERIALS AND METHODS: The first study measured MI in 31 sub-acute and chronic stroke patients with hand paresis. We used self-assessment questionnaires [Kina

[Stress myelography. A new functional study technic in diseases of the lumbar spinal canal].

To optimize functional diagnostics in lumbar syndromes a new myelographic technique was developed termed "loading myelography". During the procedure the patient stands with a 10 kg. weight on his outstretched arms. Based on the law of leverage the load exercised on the vertebral column is more than two and a half times of one-half of the body weight. The author tested the efficacy of the method in 119 patients suffering from disc prolapse, spinal canal stenosis, spondylolisthesis or arachnitis. The results of the conventional myelogram compared with myelography under load conditions demonstrat

Impact of Preoperative Total Knee Arthroplasty on Radiological and Clinical Outcomes of Spinal Fusion for Concurrent Knee Osteoarthritis and Degenerative Lumbar Spinal Diseases.

Concurrent knee osteoarthritis (KOA) and degenerative lumbar spinal disease (LSD) has increased, but the total knee arthroplasty (TKA) effect on degenerative LSD remains unclear. The aim of this study was to retrospectively analyze to compare radiological and clinical outcomes between spinal fusion only and preoperative TKA with spinal fusion for the patients with concurrent KOA and degenerative LSD. A total of 72 patients with concurrent KOA and degenerative LSDs who underwent spinal fusion at less than three levels were divided in two groups: non-TKA group (n = 50) and preoperative TKA group

Spinal hydatid disease with epidural extension and spinal cord compression: A radiology case report.

Spinal hydatid disease is rare, destructive parasitic infection of the spine. The disease often presents with nonspecific symptoms. that overlaps with spinal tuberculosis, pyogenic infection, and metastatic malignancy. We report a case of 65-year-old man with thoracic spine involvement of Hydatid disease. Magnetic resonance imaging demonstrated destructive vertebral disease with a characteristic cystic morphology. The imaging pattern favoured hydatid cyst as the most likely diagnosis. Correlation with epidemiologic history and serology was used to confirm the diagnosis and guide management. Du

RISKS AND BENEFITS OF SPINAL FUSION SURGERY FOR DEGENERATIVE SPINE DISEASES: SYSTEMATIC REVIEW OF LITERATURE.

Degenerative spine diseases are a common cause of low back pain and neurological dysfunction, often associated with aging, and may require surgical interventions when conservative treatment is ineffective. This study aims to evaluate the risks and benefits of minimally invasive spinal fusion surgery (MIS) compared to open surgery (OS) for the treatment of these conditions. Through a systematic review, 83 studies were analyzed, of which 10 were included in the meta-analysis. The methodology followed PRISMA guidelines, with searches in PubMed, Scopus, Web of Science, Medline Ovid, and SciELO dat

Causal relationships between dietary factors and spinal diseases: a univariable and multivariable Mendelian randomization study.

BACKGROUND: Spinal diseases and their associated symptoms are prevalent across all age groups, and their incidence severely affects countless individuals' quality of life. The role of daily habits in the progression of these diseases is increasingly emphasized in research. Moreover, there are reports suggesting associations between dietary factors and the onset of spinal diseases. However, the exact causal relationship between dietary factors and spinal diseases has not been fully elucidated. METHODS: We obtained GWAS data on 16 dietary intake and 187 dietary likings from the UK Biobank, and …

Exploring synthetic controls in rare diseases with a proof of concept in spinal cord injury.

BACKGROUND: Successfully completing clinical trials for rare and heterogeneous disorders, like spinal cord injuries (SCI), remains challenging, thereby reducing the ability to test and translate promising preclinical findings. We propose synthetic controls, derived from data-driven predictions of recovery in patients undergoing standard treatments, to mitigate these challenges, in particular related to patient recruitment. METHODS: Based on data from the European Multicenter Study about Spinal Cord Injury (EMSCI) and the Sygen trial, we construct synthetic controls from personalized predictio

The impact of routine whole spine MRI screening in the evaluation of spinal degenerative diseases.

PURPOSE: Magnetic resonance imaging (MRI) of the spine is a sensitive investigation, which not only provides detailed images of the spinal column but also adjacent spinal regions and para-vertebral organ systems. Such incidental findings (IF) can be asymptomatic but significant. The efficacy of whole spine T2 sagittal screening in providing additional information has been demonstrated in several spinal diseases but its routine use in patients with spinal degenerative diseases has not been studied. METHODOLOGY: A review of 1486 consecutive T2w whole spine screening MRI performed for cervical, …

A study and analysis of spinal canal diseases in the Black Lion Hospital by myelography.

Sixty seven lumbar and thoracic myelographic examinations were performed during the period February 1987 to September 1988. Fourteen patients (21%) had normal myelography while 53 (79%) patients had pathological findings. Spinal canal tumours accounted for the majority of complete CSF blocks (76.7%) Seventy-five per cent of the intradural tumours showed no abnormality on plain films of the vertebral column, whereas 75% of the extradural tumours had pathological findings. Dorsal (central) disc herniation was more frequent (70%) than lateral herniation with almost equal distribution at L5-S1 and

Protective effects of sarcopenia-related traits on spinal degenerative diseases: A large prospective cohort study.

BACKGROUND: Paraspinal muscle degeneration has been implicated in the pathogenesis of spinal degenerative diseases. However, the impact of systemic muscle status, particularly sarcopenia, on SDD development remains poorly understood. METHODS: Multiple regression models, smooth curve fitting and threshold effect analyses were employed to evaluate associations between appendicular lean mass (ALM) and grip strength with SDDs using UK Biobank data (n = 490,116 for ALM; n = 497,300 for grip strength). Two-sample Using summary statistics from the UK Biobank and FinnGen consortium, the study perform

Outcome of the L5-S1 segment after posterior instrumented spinal surgery in degenerative lumbar diseases.

BACKGROUND: Posterior decompression, instrumentation, and posterolateral fusion are surgical procedures for the treatment of degenerative lumbar diseases. Solid fusion usually causes adjacent problems. This study investigated the clinical outcome and radiographic fate of the L5-S1 segment in patients who underwent posterior instrumented surgery for degenerative lumbar diseases. METHODS: From January 1999 to December 2000, 181 patients (average age 59.4 years, range 45-79 years) underwent posterior decompression, posterior instrumentation, and posterolateral fusion for degenerative lumbar dise
