Spinal fractures and pseudoarthrosis complicating ankylosing spondylitis: MRI manifestation and clinical significance.

PURPOSE: To analyze magnetic resonance (MR) patterns of fractures and pseudoarthrosis of the ankylosing spondylitic spine, and related changes in the dura and adjacent soft tissue. MATERIALS AND METHODS: Sixteen patients with radiographically evident fractures or pseudoarthrosis of the spine were included. Each underwent MR studies. Ten patients among them underwent surgical operations. RESULTS: Both transdiscal (n = 12) and transvertebral (n = 4) fractures were identified. The levels were located from T9 to L3. Five of 16 patients had pseudoarthrosis. The fractures or pseudoarthrosis had tw

Hip fracture specialists facilitate low-dose spinal anaesthesia in fractured neck of femur surgery.

Fractured neck of femur is a common but potentially devastating complication of frailty. In other surgical specialities, there is an inverse relationship between surgical experience and duration of surgery; however, this has not been quantified in hip trauma. In perioperative hip fracture care, prolonged surgery may be associated with increased morbidity and significantly impacts on the conduct of anaesthesia. Specifically, low-dose spinal anaesthesia, which is associated with improved haemodynamic stability, cannot be used if surgery is likely to be prolonged. We studied the duration of hip f

The Importance of Instrumentation Length in Ankylosing Spinal Disorders and Thoracolumbar Fractures.

Background/Objectives: Ankylosing Spinal Disorders (ASDs) encompass a heterogeneous group of rheumatic diseases characterized by progressive ankylosis of the axial skeleton, including Ankylosing Spondylitis (AS), Diffuse Idiopathic Skeletal Hyperostosis (DISH), and Non-Radiographic Axial Spondyloarthritis (nr-AxSpA). Spinal ankylosis profoundly alters the biomechanical properties of the vertebral column, transforming it into a rigid long-bone equivalent and dramatically increasing fracture risk even after low-energy trauma. Once a fracture occurs, the long lever arm created by the ankylosed se

[Do socioeconomic factors influence treatment choice for thoracic and lumbar spinal fractures?].

Thoracic and lumbar fractures are common injuries that can be treated either surgically or non-surgically. This study investigated if socioeconomic and demographic factors influence treatment choices in Sweden using data from the Swedish Fracture Register and other national health registers. Results showed no significant association between socioeconomic factors and treatment choices. However, patients age 50-69 years were less likely to undergo surgery, probably because of factors such as osteoporosis and increased comorbidity. The study suggests that Swedish healthcare primarily bases treatm

Symptoms of sacral fractures after posterior spinal instrumented fusion: A case series.

BACKGROUND: Improvements in spinal fusion devices and techniques have enabled stronger spinal fusion, resulting in excellent clinical outcomes. Nevertheless, complications associated with implants, such as screw misalignment, screw lubrication, cage dislocation, and skin issues, might occur. This study aimed to investigate the characteristics and symptoms of sacral fractures after spinal instrumented fusion. METHODS: This case series retrospectively examined the medical records of eight patients (one man and seven women; mean age: 74 years) diagnosed with sacral fractures after undergoing pos

Risk factors for L5 pedicle fractures after single-level posterior spinal fusion.

BACKGROUND CONTEXT: Pedicle fractures are a rare but potentially devastating complication of posterior instrumented spinal fusion (PSF). Preoperative awareness of the possible risk factors may help prevent these fractures by modifying the surgical plan. However, the risk factors have not yet been identified. PURPOSE: To determine the preoperative parameters associated with postoperative L5 pedicle fracture after L4/5 PSF. STUDY DESIGN: Case control study. PATIENT SAMPLE: Patients undergoing L4/5 PSF at a single academic institution between 2014 and 2020. OUTCOME MEASURES: Occurrence of pos

[Adjacent segment degeneration following spinal fusion of vertebral fractures in paraplegic patients].

The effects of adjacent segment degeneration (ASD) after spinal fusion of vertebral fractures have previously not been demonstrated in patients with trauma-related paraplegia. The aim of this study was to evaluate the role of ASD in patients with paraplegia caused by vertebral fractures and to observe whether there is a difference between unilateral or combined spinal fusion in long-term results regarding the degeneration of cranial or caudal adjacent spinal segments. A total of 111 paraplegic patients with an average age of 45 years who underwent spinal fusion of vertebral fractures were obse

Neonatal spinal muscular atrophy type 1 with bone fractures and heart defect.

The authors present the case of an infant girl with severe generalized weakness, multiple bone fractures, and heart defect. She needed mechanical ventilation from birth. Radiographs showed mid-diaphyseal fractures of both humeri and of the right femur as well as generalized osteopenia. Electroneuromyography showed spontaneous fibrillations at rest with no active movements. Motor response to a stimulus could not be registered. A systolic heart murmur was detected, and echocardiography showed a large atrial septal defect and an additional membrane in the left atrium. DNA analysis confirmed the d

Functional outcomes after surgery for spinal fractures: return to work and activity.

OBJECT OF STUDY: The literature regarding surgical treatment's impact on patient function after spinal fracture is sparse. Some authors have speculated that operative injury--the dissection of paraspinous muscle tissue, damage to spinal motion segments, implantation of spinal devices--may impair functional recovery in spine trauma patients. Nonoperative care has produced satisfactory results in some hands, but results are difficult to reproduce, treatment is resource-intensive, and functional outcomes are poorly documented. This study reports return to work and functional recovery in a 5-year

Fall-related fractures in persons with spinal cord impairment: a descriptive analysis.

Falls are a significant cause of injury, disability, and death in the elderly, but little is known about the risk of wheelchair-related falls. The purpose of this study is to describe the incidence, etiology, location of fracture, treatment, and health care utilization of fall-related fractures in persons with spinal cord impairment (SCI). A retrospective review of 45 medical records of patients with SCI who sustained fractures, nonconcomitant with the onset of their initial injury, was completed at a Veterans Health Administration (VHA) SCI service over a 10-year period. Of the 24 veterans wh

[Magnetic resonance tomography examination of thoracolumbar spinal fractures after fixateur interne stabilization].

To analyse the possible injuries of vertebral segments, especially the disc, after unstable thoracolumbar fractures stabilised with AO internal fixator, we performed magnetic resonance imaging (MRI) of the traumatised region after implant removal. There were two aspects of disc degeneration (DD):(1) biochemical changes and (2) structural damage. MRI detects biochemical processes as one aspect of DD that is often small even in the presence of greater structural damage of the nucleus pulposus caused by fracture. None of the patients presented with structural failure of the anulus fibrosus, which

Functional morphology of the spinal canal after endplate, wedge, and burst fractures.

Changes in the canal diameter during physiological motions are important considerations in the treatment of patients who have a burst fracture with the presence of bony fragments, but without neurologic deficit. In this in vitro study, the changes in the soft-canal diameter of the thoracolumbar region, when intact and after different fractures, was investigated under several different loading conditions. The soft-canal diameter was clearly identified on the lateral radiographs by attaching a series of steel balls to the posterior longitudinal ligament and ligamentum flavum in the midsagittal p
