Total myelography in the evaluation of lumbar discs. With the presentation of three cases of thoracic neoplasms simulating nerve root lesions.

It has been our practice to perform total myelography in all cases of lumbar disc disease including the various syndromes of spinal stenosis, because experience has indicated that myelographic abnormalities in the cervical and thoracic areas could have clinical importance despite the presence of gross changes in the lumbar region. Failure to establish a definite diagnosis by restricted lumbar myelography makes it mandatory to evaluate all levels of the spinal axis. Recently, 3 patients with thoracic spinal cord tumors presented with primary signs and symptoms of lumbar spine disorders. Evidenc

Leptomeningeal metastases: evaluation by gadolinium enhanced spinal magnetic resonance imaging.

BACKGROUND: Leptomeningeal metastases are common in patients with metastatic systemic cancer or certain primary brain tumors. They may be unsuspected clinically and may be missed by cerebrospinal fluid (CSF) cytology. We undertook a retrospective study of the diagnostic value of gadolinium enhanced spinal MR imaging in patients with known or at high risk for leptomeningeal metastases (LM). MATERIAL AND METHODS: Ninety-six gadolinium enhanced MR examinations of the whole spine were performed in 61 patients (26 primary central nervous system tumors, 20 solid tumors and 15 lymphoproliferative ne

Myelopathy caused by a histiocytic sarcoma in a cat.

An eight-year-old, female spayed, domestic shorthair cat presented with a three-week history of progressive general proprioceptive ataxia and upper motor neuron paresis of the hindlimbs. Computed tomography revealed a mediastinal mass invading the vertebral canal with the T1 spinal nerve and roots, causing extramedullary compression of the cranial thoracic spinal cord. Histopathological and immunohistochemical studies of the mass during postmortem examination disclosed a neoplasm, later determined to be a poorly differentiated histiocytic sarcoma. Feline histiocytic tumours are rare, with only

Pediatric Extramedullary Epidural Spinal Teratomas: A Case Report and Review of the Literature.

BACKGROUND: Teratomas in the pediatric population are most commonly found in the sacrococcygeal region. Pediatric intraspinal teratomas, however, are an exceedingly rare central nervous system (CNS) neoplasm. The clinical presentation of these intraspinal neoplasms can vary significantly and thus can be difficult to identify in infants less than one year of age where verbal expression and motor development are still lacking. Case Description. A 7-month-old, previously healthy male presented with a thoracic scoliosis and an asymptomatic right midupper thoracic spinal prominence present since bi

Multidisciplinary management of primary tumors of the vertebral column.

Primary spinal neoplasms are rare tumors that can lead to significant morbidity secondary to local bone destruction and invasion into adjacent neurological and vascular structures. These tumors represent a clinical challenge to even the most experienced physicians and require a multidisciplinary approach to ensure optimal patient outcomes. This review will discuss the most common primary bone tumors and focus on recent surgical, medical, and radiation treatment advances.

Cryosurgery re-visited for the removal and destruction of brain, spinal and orbital tumours.

Advances in neuroimaging and cryosurgical techniques have prompted us to re-evaluate the potential of cryosurgical techniques for the removal and the destruction of various neoplasms. We have used cryosurgical instrumentation to remove tumours in the brain, spine and orbit in 71 patients without complications. Cryosurgery was used to facilitate removal and extraction in 64 and to destroy residual neoplasms when removal was incomplete in 7. Intraoperative real time ultrasonic imaging permitted precise delimitation of tumours from surrounding tissues and allowed monitoring during the production

Spinal hemangioma mimicking a dumbbell-shaped schwannoma: Case report and review of the literature.

Spinal hemangiomas are common, benign vascular lesions that involve the bony portion of vertebral bodies and are generally asymptomatic. Rarely, they can become aggressive and present with predominantly epidural extension, mimicking other neoplasms. We present the case of a fifty-one year old woman who presented with myelopathy and was discovered to have a large mass causing epidural spinal cord compression, thought to be due to a peripheral nerve sheath tumor. She underwent surgery for tumor debulking. Intraoperatively, the mass was found to be mostly epidural with minimal bone involvement. F

Neurologic disease with pregnancy and considerations for the obstetric anesthesiologist.

Women with neurologic conditions present a challenge during pregnancy and in the peripartum period. Given the low prevalence of these diseases during pregnancy, most management decisions are guided by retrospective reviews and case reports. This article reviews current literature for some of the more common or complex neurologic conditions affecting pregnancy with special consideration for anesthetic management. In particular, epilepsy; multiple sclerosis; primary intracranial hypertension; secondary intracranial hypertension-Arnold-Chiari malformations and intracranial neoplasms; spinal cord

[Preoperative devascularization of a vertebral metastasis with a spinal artery at the same level].

An hypervascular and lytic thoracic (Th10) vertebral metastasis from kidney neoplasm causing cord compression was successfully resected after embolization. Because the anterior spinal artery arose at the same level, arterial embolization was only partial and completed by percutaneous vertebroplasty using Nbutyl cyanoacrylate.

Collision tumor comprised of chronic lymphocytic leukemia and myxopapillary ependymoma.

BACKGROUND: Collision tumors involving the co-occurrence of two morphologically and genomically distinct neoplasms in the same anatomical site are exceptionally rare in the central nervous system (CNS). CASE DESCRIPTION: We report a unique case of a CNS collision tumor comprising chronic lymphocytic leukemia and myxopapillary ependymoma in a 77-year-old male with acute neurological decline. Presumed to represent leukemic infiltration, urgent laminectomy was pursued for tissue diagnosis and spinal cord decompression, revealing the unexpected ependymal component. CONCLUSION: This case highligh

[Solid tumours of the vertebral column and spinal cord in children. Reasons and consequences of diagnostic delay].

INTRODUCTION: Primary or secondary tumours of spinal cord and vertebral column have various histological structure, various degree of malignancy and various growth dynamics. Such localisation of neoplastic disease creates problems, connected with the risk of irreversible damage of neurological functions. It is essential to make a proper diagnosis very early. AIM: An analysis of initial clinical symptoms and reasons for diagnostic delay in children with neoplasms of the vertebral column and spinal cord. This can facilitate and accelerate diagnostic and therapeutic process. MATERIAL AND METHOD

Atypical Teratoid Rhabdoid Tumor of the Cauda Equina in a Child: Report of a Very Unusual Case.

Atypical teratoid/rhabdoid tumors (AT/RT) are highly aggressive malignant primitive neoplasms that commonly occur in children younger than 2 years of age. The prognosis is generally dismal with a median survival time of <1 year. The majority of AT/RT occur in the posterior fossa and less frequently the supratentorium. Primary pediatric spinal AT/RT are exceedingly rare and only 15 cases have been reported to date. Here we report a very unusual case of primary spinal AT/RT extensively involving the spinal cord from T11 down to the cauda equina. In this patient, the tumor was highly aggressive a

[Primary central nervous system lymphoma with initial symptoms suggesting herpes encephalitis].

Primary central nervous system lymphoma (PCNSL) is rare neoplasm, affecting both immunocompetent and immunodeficient patients. It is usually seen as intracranial tumor, but it can often involve cerebrospinal meninges, eyeballs and spinal cord. Although PCNSL is sensitive both to radiotheraphy and chemotheraphy, it's recurrences are very frequent. Mean survival time does not exceed several months. We described a case of 43 year old patient with diagnosed PCNSL and discussed clinical signs, diagnostics and treatment of the neoplasm. In our case report we emphasized transient remission after trea

Novel Approach to Retro-Odontoid Pseudotumor Resection without Atlantoaxial Dislocation.

BACKGROUND Retro-odontoid pseudotumor is a fibrous or pannus mass behind the odontoid process, mainly caused by atlantoaxial instability, degeneration, or abnormal deposits behind the dens. Most scholars agree immobilization of the upper cervical spine can solve this problem; the mass can absorb spontaneously. However, retro-odontoid pseudotumor cases without atlantoaxial instability are limited, and it remains controversial whether the mass should be removed. CASE REPORT A 67-year-old man had intermittent neck pain for 10 years and numbness and weakness of the extremities for 1 month. Primary

Spinal Cord Compression as Initial Presentation in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common primary hepatic malignancy and aggressive neoplasms with high metastatic potential to the lung, regional lymph nodes, and bone. Metastatic spinal cord compression due to HCC is a rare, unusual initial presentation and is a neurosurgical emergency. We present two cases of HCC where spinal cord compression was the initial presentation before the diagnosis of HCC. Our first patient presented with bilateral flank pain. The biopsy of the chest wall showed HCC. However, a CT scan of the abdomen showed metastatic involvement of the T11 vertebra. He wa

Malignant spinal cord compression in cancer patients may be mimicked by a primary spinal cord tumour.

Although it is quite rare, second primary neoplasms in cancer patients may present with the signs and symptoms of malignant spinal cord compression. Primary spinal cord tumours in the cancer patients may be deceptive and considered as the recurrent first cancer. Therefore, it should be precisely differentiated and appropriately managed. We report such a case of intramedullary ependymoma of the cervical spinal cord mimicking metatstatic recurrent lymphoma and causing cord compression. A 50-year-old man developed intramedullary ependymoma of the cervical spinal cord 1.5 years following chemoradi

[It looks like a spinal tumor-Differential diagnosis].

BACKGROUND: Spinal space-occupying lesions frequently pose a diagnostic challenge, particularly when distinguishing malignant from nonmalignant abnormalities. The challenge for the radiologist is to reliably differentiate malignant from benign entities. OBJECTIVE: To identify imaging features that argue against a neoplastic disease and provide clues to relevant differential diagnoses. MATERIAL AND METHODS: A literature search was conducted in pubmed.gov to identify and characterize tumor-like spinal conditions. RESULTS: A total of 11 differential diagnoses were selected that can mimic a neo

Deciphering the enigmatic symptoms of Pancoast tumors: Navigating the complex landscape of pain management-A case report.

Pancoast tumors, also known as superior sulcus tumors, encompass a diverse spectrum of neoplasms that infiltrate the apex of the chest wall, yielding distinctive clinical presentations. One of the earliest signs of tumor growth is pain radiating to the upper limb, stemming from peripheral nerve involvement, which can mimic joint pain or spinal radicular irritation. In this case report, we present the clinical history of a 64-year-old female smoker who had previously been recommended for orthopedic elbow surgery due to epicondylitis. Subsequent to the development of additional symptoms and rigo
