Cardiac and renal effects of a transjugular intrahepatic portosystemic shunt in cirrhosis.

Refractory ascites and recurrent variceal bleeding are among the serious complications of portal hypertension and cirrhosis for which a transjugular intrahepatic portosystemic shunt (TIPS) can be used. Cirrhotic patients have varying degrees of haemodynamic derangement, mainly characterized by peripheral arterial vasodilatation, central underfilling and activation of several vasoactive systems. These changes affect the heart, the lungs and the kidneys in particular. The cardiac effects of TIPS are immediate and are related to the redirection of blood from the splanchnic circulation into the sy

In vivo use of human umbilical vessels and the ductus venosus arantii.

Clinical use of umbilical vessels in the neonate is commonplace. In the adult, surgical reopening of the umbilical vein is feasible in the majority of patients, providing direct access to the portal circulation. Umbilical vein catheterization allows for portal manometry in the intact, unanesthetized state. Prolonged catheterization has served to facilitate physiologic and pharmacologic studies hitherto unavailable. Selective splanchnic catheterization with hepatoportography permits roentgenographic studies, important in the evaluation of patients with hepatobiliary diseases. Clinically, the um

Hepatorenal syndrome.

Hepatorenal syndrome is a common complication in patients with advanced cirrhosis and ascites characterized not only by renal failure but also by marked alterations in systemic hemodynamics and vasoactive systems. Pentafurcated Celiac Trunk.

BACKGROUND: Commonly, but not exclusively, the celiac trunk (CT) trifurcates into the left gastric (LGA), common hepatic (CHA) and splenic (SA) arteries. Additional branches of the CT are scarcely reported in the literature. Less than ten reports were found presenting patterns of pentafurcation of the CT (pCT), all being resulted after anatomic dissections. METHOD: We hereby report such a rare pCT, which was found on the computed tomography angiograms of a 71-year-old female patient. RESULTS: From that pCT were branching off three collateral branches, two ascending and one descending, and tw

[The role of nitric oxide in hyperdynamic circulation in portal hypertensive rats].

OBJECTIVE: To evaluate the role of nitric oxide (NO) in hyper-hemodynamics in portal hypertension, we investigated the effects of nitric oxide synthase inhibitor L-NMMA with or without L-arginine, the substrate for enzyme NO synthase on the systemic and splanchnic hemodynamics in portal hypertensive rats. METHOD: Ninety-seven male Sprague-Dawley rats were divided into four groups: intrahepatic portal hypertension (IHPH, n = 23) by injection of CCl4, prehepatic portal hypertension (PHPH, n = 26) by stenosis of the portal vein, end-to-side portacaval shunt (PCS, n = 23), and sham-operated contr

Effects of endoscopic variceal sclerotherapy on azygos vein blood flow and systemic haemodynamics.

The present study was designed to determine the systemic haemodynamic effects of obliterating oesophageal varices by endoscopic sclerotherapy. We evaluated systemic and splanchnic haemodynamics before and after the first course of sclerotherapy in cirrhotic patients. The baseline cardiac index was significantly correlated with baseline azygos vein blood flow (r = 0.64; P < 0.01) and the azygos vein blood flow and cardiac index significantly decreased (-33% and -16%, respectively; P < 0.01) following sclerotherapy. The systemic vascular resistance index was also increased significantly (+ 20%;

Splanchnic, Thoracoabdominal, and Cerebral Blood Flow Volumes in Healthy Children and Young Adults in Fasting and Postprandial States: Determining Reference Ranges by Using Phase-Contrast MR Imaging.

Purpose To estimate reference ranges for blood flow volume (BFV) in major splanchnic, thoracoabdominal, and neck vessels by using phase-contrast magnetic resonance (MR) imaging in children and young adults in fasting and postprandial states. Materials and Methods In this institutional research ethics board-approved prospective study, healthy volunteers underwent phase-contrast MR imaging in a fasting state and again after a standardized meal. BFV values were reported as medians and ranges, and postmeal to premeal BFV ratios were calculated. BFVs in volunteers divided into two groups according

Variation at the Angiotensin-converting enzyme and endothelial nitric oxide synthase genes is associated with the risk of esophageal varices among patients with alcoholic cirrhosis.

Esophageal varices are a frequent complication among patients with liver cirrhosis. Nitric oxide and other vasoactive molecules regulate the vascular tone in both the liver microcirculation and the systemic and splanchnic circulation. Several genes that encode proteins involved in the maintenance of vascular tone, such as the endothelial-constitutive nitric oxide synthase (ecNOS), the angiotensinogen (AGT), the angiotensin-converting enzyme (ACE), and the angiotensin II receptor type 1 (AT1R) are polymorphic, and these polymorphisms have been associated with several cardiovascular diseases. Portal vein thrombosis: When to treat and how?

Portal vein thrombosis is an unusual thrombotic condition not frequently seen in the general population; however, it has a higher prevalence in special circumstances such as in liver cirrhosis and hepatic or pancreatic malignancy. It also can be associated with significant morbidity and mortality. In this review, we discuss the current data available to guide therapy in the setting of different associated co-morbidities, hypercoagulable states, and associated thrombosis of the remaining splanchnic circulation. We discuss indications for anticoagulation, including the choice of anticoagulants,

Protein S deficiency: imaging findings.

Protein S deficiency is an autosomal-dominant inherited disorder of coagulation associated with spontaneous and recurrent venous thrombosis. Approximately 5% of patients with deep venous thrombosis of the lower extremities who are less than 45 years old have a deficiency of protein S. Patients frequently have spontaneous and recurrent deep venous thrombosis and pulmonary embolism during young adulthood. Thrombosis occurs less commonly in the splanchnic and cerebral veins. Arterial thrombosis is rare but is seen in the cerebral circulation. Radiologists should include protein S deficiency in th

[Hepatorenal syndrome].

BACKGROUND: The association of renal failure with terminal liver insufficiency is referred to as hepatorenal syndrome (HRS). It is characterized by impaired renal function together with abnormalities in arterial circulation and activity of endogenous vasoactive system. In the pathogenesis of HRS, vasodilatation in the splanchnic circulation plays a major role. This leads to underfilling in arterial circulation in this area with subsequent vasoconstriction in renal arteries leading to lower renal circulation and functional renal failure. One of the most difficult tasks in clinical evaluation of

[Splanchnic vein thrombosis].

Splanchnic vein thrombosis (SVT) represents an unusual manifestation of venous thromboembolism. The etiological factors for SVT can be divided into local and systemic, frequently found concurrently. SVT can be the first presenting symptom in myeloproliferative neoplasms. SVT puts the patients affected in jeopardy of developing the intestinal infarction, impairing the liver function and portal hypertension development with the risk of potentially life-threatening gastrointestinal bleeding. The current guidelines emphasise the role of anticoagulation in acute splanchnic thrombosis. Considering t

[Diagnosis and therapy of hepatorenal syndrome].

Hepatorenal syndrome (HRS) is defined as severe vasoconstriction of renal cortical vessels with critical impairment of glomerular filtration rate, initiated by deteriorating liver function. In addition, systemic vasodilation, predominantly of splanchnic vessels is present. Although the urinary status is basically normal, during the course of HRS kidneys are loosing their ability to concentrate and at an urinary production rate of less than 100 ml/day the urinary osmolarity and Na(+)-concentration drops significantly. Because HRS can not be improved by conventional medical treatment regimens, i

Altered Dynamics in Splanchnic Amino Acid Extraction During Early Sepsis Recovery in Pigs: Whole Body Compared with Interorgan Balance Assessments.

BACKGROUND: Establishing the metabolic fate of nutrients from food intake is crucial for interpreting the benefits of food interventions. Using stable isotopes labeled amino acids (AA) such as L-phenylalanine (Phe) during feeding allows for the determination of first-pass splanchnic extraction (SPE) on a whole-body level (WbSPE). The relationship between this measure and the total fraction of food-derived Phe retained by the splanchnic area has not been validated. OBJECTIVE: Therefore, we compared WbSPE with the enteral-derived Phe and actual PheSPE, measured by splanchnic-trans-organ fluxes

Hepatosplanchnic perfusion changes following isolated abdominal trauma and surgery: A scoping review of evidence dominated by pneumoperitoneum models.

The hepatosplanchnic vascular response to isolated abdominal organ injury remains poorly defined. The aim of this review was to synthesize the available evidence on macrovascular hepatosplanchnic flow and organ-specific microvascular perfusion following isolated abdominal injury or surgery. PubMed/MEDLINE was searched through 11 May 2026 for human and animal studies reporting visceral hemodynamic changes after abdominal trauma, experimental injury, or surgery. Systematic-review methods included structured study selection, data extraction, and design-specific risk-of-bias assessment within a PR
