JAK2 R683S Mutation Resulting in Dual Diagnoses of Chronic Eosinophilic Leukemia and Myelodysplastic/Myeloproliferative Overlap Syndrome.

A 66-year-old male presented with hypereosinophilia, thrombocytosis, extensive thrombosis refractory to direct oral anticoagulant therapy, and evidence of end-organ damage, including rash, splenic infarcts, and pulmonary infiltrates. Bone marrow biopsy revealed myeloid malignancy consistent with both chronic eosinophilic leukemia and myelodysplastic/myeloproliferative neoplasms (MDS/MPN) with SF3B1 mutation and thrombocytosis. Next-generation sequencing of the patient's eosinophils and neutrophil compartments revealed pathologic variants in EZH2 and SF3B1 in addition to a noncanonical JAK2 R68

A Large Gastrointestinal Tumor With Cystic Degeneration Initially Mistaken for Gastric Splenosis: A Case Report.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract and usually present as submucosal gastric lesions. Atypical morphologies, such as exophytic or cystic growth, are uncommon and can lead to lesions being mistaken for hepatic, pancreatic, or splenic origin. We report a case of a 63-year-old woman referred for evaluation of unexplained weight loss. Imaging revealed a 4 cm lobulated lesion adjacent to the spleen with peripheral cystic changes. The mass enhanced similarly to splenic tissue and was initially interpreted as splenosis. Fina

Laparoscopic distal pancreatectomy without needle aspiration before resection for giant mucinous cell neoplasms.

Laparoscopic resection of large mucinous cystic neoplasms (MCN) has recently been reported. However, in most reports, needle aspiration of the cyst contents was performed before resection and can cause dissemination. Here, we report two patients with giant MCN: a 26-year-old woman with a 23-cm MCN and a 41-year-old woman with an 18-cm MCN. The MCN were successfully resected without aspiration by laparoscopic surgery. CT revealed no tumor involvement of the origins of the splenic artery and vein in either case. In case 1, we performed hand-assisted laparoscopic surgery while dissecting around t

Extramedullary hemopoiesis and littoral cell angioma of the spleen: our experience and review.

Littoral cell angioma (LCA) is a rare primary vascular neoplasm of the spleen. A 54-year-old man was referred to our emergency department for abdominal pain. A CT scan showed multiple round hypodense lesions of various sizes throughout the spleen. The spleen was increased in volume. An MRI confirmed the lesion with a suspect of multiple angiomas vs. amartomas. The haematologists excluded any haematological disease. After a collegial discussion, we decided to perform laparotomic splenectomy. Histologically, the multiple lesions consisted in anastomosing vascular channels lined by plump cells. T

Non-Pharmacologic Management of Splenomegaly for Patients with Myelofibrosis: Is There Any Role for Splenectomy or Splenic Radiation in 2020?

PURPOSE OF REVIEW: Myelofibrosis is a chronic myeloproliferative neoplasm which can lead to massive splenomegaly. Currently approved medical therapies do not improve splenomegaly in all patients and effects are not sustained. Thus, spleen-directed therapies (i.e., splenectomy and splenic irradiation) have been used in some cases to palliate the signs and symptoms of the disease. Here, we critically review the literature regarding palliative splenectomy and splenic irradiation in myelofibrosis, and discuss their position in the current treatment landscape. RECENT FINDINGS: Retrospective studie

Canine Non-Angiogenic, Non-Myogenic Splenic Stromal Sarcoma: a Retrospective Clinicopathological Analysis and Investigation of Podoplanin as a Marker of Tumour Histogenesis.

Splenic stromal sarcomas are rarely reported tumours that were previously grouped as non-angiomatous, non-lymphomatous mesenchymal neoplasms of the canine spleen. Highly variable survival times have been reported probably due to their heterogeneous nature. The purpose of this study was to assess the outcome and prognostic factors in dogs with splenic stromal sarcoma after treatment by splenectomy. Clinical data were collected retrospectively and histopathology was reviewed for 47 patients. Histological classification, based on morphology in haematoxylin and eosin-stained sections, in conjuncti

A Rare Case of Splenunculus Buried within the Stomach.

BACKGROUND: Accessory spleen, or splenunculus, is a congenital anomaly resulting from failure of splenic anlage fusion during embryonic development. It is most commonly located near the splenic hilum or pancreatic tail and rarely occurs within the stomach wall. Gastric accessory spleen is a rare entity that may mimic submucosal neoplasms such as gastrointestinal stromal tumour, posing a diagnostic challenge. CASE PRESENTATION: We report the case of a 37-year-old male presenting with a 3-month history of epigastric pain, dyspepsia, and nausea. Endoscopy revealed a 2.5-3 cm submucosal mass in t

Acute intestinal obstruction and splenic hemorrhage due to metastatic choriocarcinoma. A case report.

Choriocarcinomas are rare malignancies which occasionally cause acute abdominal symptoms, mainly in the form of hemorrhage. We here describe a patient with metastatic choriocarcinoma presenting as acute intestinal obstruction and splenic hemorrhage. This seems to be the first report in the literature of a choriocarcinoma causing invagination of the small intestine. The abdominal manifestations of the neoplasm are briefly discussed.

Leiomyosarcoma of the splenic vein.

Leiomyosarcomas arising from the wall of blood vessels are rare and aggressive neoplasm. We report a case of a previously healthy 66-year-old woman who presented with intermittent abdominal pain, progressive constipation, and weight loss. Abdominal computed tomography showed a 12 cm solid heterogeneous tumor in the tail of the pancreas. The patient subsequently underwent surgical resection of the pancreatic mass. Surprisingly, histological and immunohistochemical analyses revealed leiomyosarcoma arising from the smooth muscle of the splenic vein. After surgery, she received adjuvant chemothera

Hemangioma of the spleen: presentation, diagnosis, and management.

Splenic hemangioma is a rare disorder but remains the most common benign neoplasm of the spleen. It often has a latent clinical picture; however, spontaneous rupture has been reported to occur in as many as 25% of this patient population.1 Treatment most often consists of splenectomy. This report reviews an 8-year experience with splenic hemangioma at Mayo Clinic. Thirty-two patients were identified with SH during the 8-year study period. The average age was 63 years (range 23 to 94 years) with 17 women and 15 men. Six patients presented with symptoms potentially related to the SH. The remaind

Algorithmic Approach to the Splenic Lesion Based on Radiologic-Pathologic Correlation.

Splenic lesions are commonly discovered incidentally at imaging, without clinical signs or symptoms that may aid in diagnosis. As such, the differential diagnosis and subsequent management are based primarily on imaging characteristics. Much has been written about the myriad pathologic conditions that can occur in the spleen; however, there is little guidance on the approach to an incidental splenic mass. Applying an approach frequently used in imaging to the splenic mass-based on the number and consistency of lesions and refined by supplementary imaging features-allows formulation of a useful

Splenic Lesions in Dogs: A Retrospective Epidemiological and Pathological Study.

Splenic neoplastic and non-neoplastic lesions are frequently diagnosed in dogs and often exhibit overlapping clinical signs and gross morphological features, rendering histopathological examination essential for definitive diagnosis, therapeutic planning, and prognostic assessment. In Colombia, epidemiological data regarding these lesions remain limited, whereas international studies report considerable heterogeneity in the relative frequencies of neoplastic and non-neoplastic conditions. This study aimed to characterize the histopathological spectrum of splenic lesions in dogs and provide upd

Warshaw Technique in Spleen-Preserving Pancreatectomy: A Case Series.

We present three cases of spleen-preserving total pancreatectomy using the Warshaw technique for neoplasms of the pancreatic head. The first case involved a 5 cm mucinous pancreatic neoplasm invading the middle colic artery. The second case featured a patient with a BMI of 40 and adenocarcinoma of the pancreatic head. The third case involved a serous cystadenoma in the pancreatic head in a patient with a history of gastric surgery for a gastric ulcer, requiring surgical intervention due to weight loss and abdominal symptoms. These cases highlight the surgical nuances and benefits of the Warsha

Atypical Presentation of a Splenic Abscess Secondary to Splenic Artery Thrombosis With Isolated Confusion: A Case Report.

Splenic abscess is a rare but life-threatening condition whose diagnosis is frequently delayed because of its nonspecific presentation. Development of a splenic abscess following splenic infarction secondary to splenic artery thrombosis is exceptional, particularly when revealing an underlying myeloproliferative neoplasm. An 85-year-old woman presented to the emergency department after three weeks of intermittent confusion without fever, abdominal pain, or gastrointestinal symptoms. While neurological investigations were unrevealing, thoracoabdominal computed tomography unexpectedly demonstrat

Incidental Finding of Appendiceal Mucinous Neoplasm After Trauma: A Case Report.

Appendiceal neoplasms are uncommon; most are identified by pathologic examination after appendectomy for presumed appendicitis or as an incidental finding. When found incidentally, patients are typically asymptomatic. If the neoplasm has perforated, patients may present with symptoms that mimic acute appendicitis. In advanced disease, patients may have systemic symptoms associated with peritoneal disease, including abdominal distension, weight loss, and diffuse abdominal pain. Because of their rarity, as well as rapidly evolving research on the subject, the nomenclature of appendiceal mucinous

Fine-needle aspiration of intrapancreatic accessory spleen: cytomorphologic features and differential diagnosis.

BACKGROUND: Intrapancreatic accessory spleen (IPAS) is a rare benign lesion of the pancreas that frequently clinically and radiographically mimics a solid neoplasm. Very rarely, epidermoid cysts may form in IPAS and be mistaken for a cystic neoplasm of the pancreas on radiographic imaging. IPAS and epidermoid cyst involving intrapancreatic cyst (ECIPAS) are benign, and, if recognized, do not require surgical intervention. There are few reports of the cytopathologic features of IPAS diagnosed by fine-needle aspiration (FNA). METHODS: Here we report a series of 6 cases of endoscopic ultrasound

Elevated serum CA 19-9 level associated with a splenic cyst: which is the actual clinical management? Review of the literature.

Splenic cysts are relatively rare entities. The differential diagnosis for these lesions includes parasite infections, results of previous trauma or infarction, congenital forms, primitive splenic neoplasm or cystic metastasis. They can be either symptomatic, causing mainly abdominal pain, or asymptomatic, thus being diagnosed as in incidental finding during radiological examination for other clinical reasons: among these a raised serum level of CA 19-9 can be a case. It has been demonstrated that epidermoid and mesothelial congenital cyst can be associated with a pathological level of this tu

Upper gastrointestinal bleeding related to an eroded gastrosplenic collateral lately after splenic artery embolization.

BACKGROUND: Upper gastrointestinal, non-variceal haemorrhage can be related to various etiologies, including peptic ulcer, neoplasm, gastritis, Dieulafoy lesions and other, rare underlying diseases. Here, we describe another, yet unreported etiology of gastric bleeding. CASE PRESENTATION: A 49-year-old man presented with melena; gastroscopy revealed blood in the stomach without active bleeding source. Computed tomography angiography demonstrated a cluster of enlarged gastrosplenic arterial collaterals in the gastric wall and coils in the splenic artery, related to an embolization procedure 30

A rare case of spontaneous splenic rupture complicated by hemophilia A.

Splenic lymphangiomas are benign neoplasms resulting from congenital malformations of lymphatic channels manifesting as cystic lesions, occurring mostly in childhood. This process usually involves additional sites in a diffuse or multifocal fashion but although rare, can also present isolated to the spleen. The clinical picture varies from asymptomatic identified incidentally to nonspecific symptoms from compression of adjacent organs. Spontaneous rupture of these lesions can lead to hemoperitoneum, acute abdomen and hemorrhagic shock. We present the case of a 33-year-old male who required urg

Cyclin-D1 rearrangement as a secondary event in the large cell transformation of splenic marginal zone lymphoma with a TP53 deletion.

IGH::CCND1 translocation has been implicated as a preliminary finding in mantle cell lymphoma (MCL) and plasma cell myeloma, with cyclin D1 over-expression by immunohistochemistry stain, without gene rearrangement, reported in other hematologic neoplasms. CCND1 rearrangement has been reported infrequently as a secondary event in high-grade B-cell lymphomas. We present the case of an elderly man who was found to have a splenic marginal zone lymphoma with a heterozygous TP53 deletion. Years later at the time of relapse, the patient acquired a CCND1 rearrangement, in addition to a persistent TP53
