Human Keratinocyte Response to Superantigens.

Staphylococcus aureus and Streptococcus pyogenes are significant human pathogens, causing infections at multiple body sites, including across the skin. Both are organisms that cause human diseases and secrete superantigens, including toxic shock syndrome toxin-1 (TSST-1), staphylococcal enterotoxins (SEs), and streptococcal pyrogenic exotoxins (SPEs). On the skin, human keratinocytes represent the first cell type to encounter these superantigens. We employed transcriptome sequencing (RNA-seq) to evaluate the human primary keratinocyte response to both TSST-1 and staphylococcal enterotoxin B (S

Skin microflora and bacterial infections of the skin.

The skin is a milieu for controlled bacterial growth. Skin supports the growth of commensal bacteria, which protect the host from pathogenic bacteria. Environmental and local factors, host immunity, and organism adherence and virulence are intricately related to cutaneous infection. Resident gram-positive bacteria include Staphylococcus, Micrococcus, and Corynebacterium sp. Staphylococcus aureus and Streptococcus pyogenes are notoriously pathogenic in the skin. In order for bacteria to be pathogenic, they must be able to adhere to, grow on, and invade the host. Bacteria possess numerous virule

A Huge Thoracic Aortic Aneurysm as a Rare Complication of Hyperimmunoglobulin E Syndrome.

Hyperimmunoglobulin E syndromes (HIES) are a heterogeneous group of primary immunodeficiencies sharing manifestations including recurrent lung infection, staphylococcal skin abscesses, eczema, and significantly raised serum levels of immunoglobulin E. Arterial aneurysms are among the rare vascular manifestations of HIES that may lead to devastating outcomes. We present a case of a 17-year-old girl who had a known history of HIES since childhood. She manifested a recurrent chest infection and skin rash, and it was discovered that she had a large thoracic descending aortic aneurysm, which requir

Recognition of Staphylococcus aureus by the innate immune system.

The gram-positive bacterium Staphylococcus aureus is a major pathogen responsible for a variety of diseases ranging from minor skin infections to life-threatening conditions such as sepsis. Cell wall-associated and secreted proteins (e.g., protein A, hemolysins, and phenol-soluble modulin) and cell wall components (e.g., peptidoglycan and alanylated lipoteichoic acid) have been shown to be inflammatory, and these staphylococcal components may contribute to sepsis. On the host side, many host factors have been implicated in the innate detection of staphylococcal components. One class of pattern

Fluctuating lesion of the scalp after a journey to the tropics: a case of furunculoid myiasis.

Fluctuating lesions or furuncles of the scalp occur frequently in dermatological practice. This clinical condition is often caused by gram positive bacteria (e.g. staphylococcal or streptococcal skin infection) or fungal infection (e.g. Kerion celsi). However, a rare diagnosis such as myiasis might be considered, especially if a journey to an endemic area is reported. Herein, we present a case of furunculoid myiasis of the scalp and review the pathogenesis and therapeutic options to treat this condition.

A Twenty-Three-Year-Old Man with a Chronic Anterior Chest Wall Wound, Eosinophilia, and Elevated IgE-Job's Syndrome.

A 23-year-old man presented with a chronic anterior chest wall wound. Previously he had a split thickness skin graft in the area in where the wound bed had become infected, developing a thick purulent drainage. The infected skin graft was excised. Histologic examination of the failed graft revealed skin with surface ulceration, focal abscess formation, deep penetrating acute and chronic inflammation with numerous eosinophils, and granulomatous changes demonstrating a foreign body-type reaction to fungal hyphae (highlighted by periodic acid-Schiff staining, Figures 1A-E). The patient's past med

Antibody-mediated protection against Staphylococcus aureus dermonecrosis and sepsis by a whole cell vaccine.

Staphylococcus aureus is a very important human pathogen that causes significant morbidity and mortality worldwide. Several vaccine clinical trials based on generating antibody against staphylococcal surface polysaccharides or proteins have been unsuccessful. A killed whole cell lysate preparation (SaWCA) was made by lysing a USA 300 strain with lysostaphin followed by sonication and harvest of the supernatant fraction. Immunization with SaWCA and cholera toxin (CT) generated robust IL-17A but relatively modest antibody responses, and provided protection in the skin abscess but not in the derm

Skin dermatoses in the returning traveller: a practical guide.

Skin problems are common in returning travellers, despite the fact that vaccination, insect repellent techniques, adequate clothing and footwear are all ways to prevent skin disease. When travelling to unusual areas, the traveller can still expect to pick up diseases that are common across the world, such as lice, scabies, and staphylococcal or streptococcal infections. What can differ are the different arthropods and the diseases they can spread. Given the plethora of dermatoses in returning travellers, a methodological approach is necessary. We attempted such an approach by creating a practi

[Hyper-IgE syndrome].

The Hyper-IgE-syndrome (Job-, Buckley-) is characterized by recurrent staphylococcal infections of the skin, the ears and the lungs, by an eczematoid dermatitis from early infancy on, and by extreme elevation of serum IgE. The inconstantly found decreased chemotaxis of the polymorphonuclear leukocytes seems to be a secondary sign of a so far unknown deficiency possibly of the T-cell mediated immunity. Thus, therapy is restricted to antibiotic and surgical treatment. Many patients have a typical coarse face and some involvement of the bones (osteoporosis, craniosynostosis).

Preadmission Application of 2% Chlorhexidine Gluconate (CHG): Enhancing Patient Compliance While Maximizing Skin Surface Concentrations.

OBJECTIVE: Surgical site infections (SSIs) are responsible for significant morbidity and mortality. Preadmission skin antisepsis, while controversial, has gained acceptance as a strategy for reducing the risk of SSI. In this study, we analyze the benefit of an electronic alert system for enhancing compliance to preadmission application of 2% chlorhexidine gluconate (CHG). DESIGN, SETTING, AND PARTICIPANTS: Following informed consent, 100 healthy volunteers in an academic, tertiary care medical center were randomized to 5 chlorhexidine gluconate (CHG) skin application groups: 1, 2, 3, 4, or 5

Atopic dermatitis and Ritter's disease: differential diagnosis by means of total and specific serum IgE.

Atopic dermatitis and Ritter's disease in infants and children frequently present diagnostic as well as therapeutic problems. Both conditions are characterized by generalized exfoliative dermatitis, and are prone to recurrent infections with Staphylococcus. However, in Ritter's disease, it has been shown that these are specifically coagulase-positive Staphylococcus phage Group 2 organisms. This combination of exfoliative dermatitis, with lysis and separation of the skin, associated with infection has been aptly named the staphylococcal scalded skin syndrome. An unusual case is reported herein

Staphylococcal toxin-induced T cell proliferation in atopic eczema correlates with increased use of superantigen-reactive Vbeta-chains in cutaneous lymphocyte-associated antigen (CLA)-positive lymphocytes.

Staphylococcal superantigens have been implicated in the pathogenesis of atopic dermatitis (AD). This may occur through superantigenic activation of T lymphocytes and their subsequent induction of the skin homing receptor CLA on activated cells. We investigated the proliferative responses of peripheral blood mononuclear cells (PBMC) from 10 patients with an infective exacerbation of AD and six normal controls to the staphylococcal superantigens, staphylococcal enterotoxin A and B (SEA, SEB) and toxic shock syndrome toxin-1 (TSST-1), and the mitogens phytohaemagglutinin (PHA) and concanavalin A

Inborn errors of immunity with susceptibility to S. aureus infections.

Staphylococcus aureus (S. aureus) is a significant human pathogen, in particular in patients with an underlying medical condition. It is equipped with a large variety of virulence factors enabling both colonization and invasive disease. The spectrum of manifestation is broad, ranging from superficial skin infections to life-threatening conditions like pneumonia and sepsis. As a major cause of healthcare-associated infections, there is a great need in understanding staphylococcal immunity and defense mechanisms. Patients with inborn errors of immunity (IEI) frequently present with pathological

Phages for treatment of Staphylococcus aureus infection.

Combating multi-drug resistant bacterial infections should be a universal urgency. The gram- positive Staphylococcus aureus (S. aureus) bacteria are generally harmless; healthy people frequently have them on their skin and nose. These bacteria, for the most part, produce no difficulties or only minor skin diseases. Antibiotics and cleansing of the affected region are usually the treatments of choice. S. aureus can become virulent causing serious infections that may lead to pustules to sepsis or death. Normally, it is thought that antibiotics may solve problems concerning bacterial infection; b
