Sternoclavicular joint septic arthritis and mediastinitis. A case report and review of the literature.

Septic arthritis of the sternoclavicular joint is rare. Its causes have been reported to include immuno-compromizing diseases, intravenous drug abuse, fractures of the clavicle or catheterization of the subclavian vein. We report a case of septic arthritis of the SCJ in a diabetic patient following periarticular injection of steroids in the ipsilateral shoulder, as this route of infection has not been documented, to our knowledge, in the literature to date. We review the literature regarding epidemiology and methods of surgical treatment that have been proposed, and present our own surgical ex

Atraumatic posterior dislocation of the sternoclavicular joint. A case report and literature review.

Atraumatic spontaneous posterior dislocation of the sternoclavicular joint (SCJ) is an extremely rare event. Only three recorded cases exist in the literature, and they are poorly documented and without roentgenographic confirmation. Spontaneous posterior dislocation of the SCJ occurred in an active 50-year-old woman without any known underlying pathology. Closed reduction attempted five days later was unsuccessful, and the patient was treated conservatively with short-term modification of activity. The patient is asymptomatic and followed carefully one year postdislocation. A review of the li

Primary osteoarthritis of the sternoclavicular joint: surgical management using the sternal docking technique.

BACKGROUND: Dislocation of the sternoclavicular joint (SCJ) is the most common SCJ condition reported to be managed surgically. However, primary SCJ osteoarthritis (OA) is substantially more common. There are few reports in the literature on the outcome of surgical management of primary SCJ OA. We have successfully adopted sternal docking allograft reconstruction for SCJ instability and have now expanded the technique to patients with primary SCJ OA. This is our first report on the outcome of the sternal docking technique specifically for patients with primary SCJ OA. METHODS: Between 2012 an

Posterior Sternoclavicular Joint Dislocations in Children and Adolescents: Missed Diagnoses and Management Outcomes.

OBJECTIVE: To evaluate pediatric and adolescent patients with posterior sternoclavicular (SC) joint dislocations, identify the prevalence of missed diagnoses at initial presentation, and determine injury features that may aid in earlier recognition. METHODS: We retrospectively reviewed patients presenting to our institution with CT-confirmed posterior SC joint dislocations between October 2015 and November 2024. Anterior dislocations, subluxations, and cases without confirmatory imaging were excluded. Demographics, injury characteristics, diagnosis timelines, management, and outcomes were rec

Unveiling the Rare: Sternoclavicular Joint Brucellosis Osteomyelitis-A Case Report and Literature Review.

Brucellosis, an endemic zoonotic infection, often presents with musculoskeletal complications, but sternoclavicular joint (SCJ) osteomyelitis is exceptionally rare. We report a case of a 49-year-old male with SCJ osteomyelitis secondary to brucellosis, emphasizing the diagnostic challenges and management. Advanced imaging and serology were pivotal in confirming osteomyelitis, underscoring the need for clinical suspicion in endemic regions. The patient achieved full recovery with a combination of antibiotics. This case highlights the importance of differentiating osteomyelitis from arthritis in

Intracardic migration of Kirschner wire from the right sternoclavicular joint: a case report.

BACKGROUND: Migration of wires and pins within the heart is an uncommon complication. Intracardic migration of Kirschner wire can cause several complications. CASE PRESENTATION: A 55-year-old male patient was admitted to the emergency service with dyspnea, stabbing chest pain. The patient's medical history showed that he had undergone a fixation operation using Kirschner wire and plate for treatment of the right sternoclavicular joint dislocation about 5 months prior. Chest computerized tomography revealed a metallic foreign body locating in the pericardium between the aorta and the right ven

Cutibacterium acnes sternoclavicular joint osteomyelitis in an otherwise healthy 55-year-old man.

Sternoclavicular joint osteomyelitis is extremely rare, with only 225 reported cases in the last 45 years. We present an unusual case in an otherwise healthy 55-year-old man with a history of well-controlled type 2 diabetes mellitus and hypertension. He presented to the emergency department after a week of left knee pain that worsened to full-body joint pain with left sternoclavicular swelling. He was started on antibiotics with multiple washouts of the left knee and treated for septic arthritis. By MRI and CT, he was found to have left sternoclavicular joint osteomyelitis and abscess and unde

Operative treatment of the painful sternoclavicular joint: a new technique using interpositional arthroplasty.

This study presents the results of a new surgical technique, resection of the medial end of the clavicle and interposition of the sternal head of the sternocleidomastoid muscle, that was used for the treatment of 17 painful sternoclavicular (SC) joints, 14 of which were available for review. The etiology of the pain was primary or posttraumatic degenerative arthritis in 10, osteitis condensans in 2, and SC joint infection in 2. The results of surgery, evaluated with the Rockwood SC joint rating scale and examination of the patients, were determined at a mean follow-up of 46 months. Results wer

Bipolar fracture of clavicle mimicking fracture dislocation of sternoclavicular joint: A case report.

INTRODUCTION AND IMPORTANCE: Bipolar fractures involving segmental fractures of the lateral and proximal clavicles are exceptionally rare, with only isolated cases documented in the literature. Such fractures may easily be overlooked during the initial presentation. CASE PRESENTATION: We present the case of a 35-year-old male with deformation in the middle segment of the clavicle following a road traffic accident (RTA). On radiography, the injury was initially thought to be a lateral clavicle fracture combined with sternoclavicular joint dislocation but was later changed to a bipolar clavicle

Minimally Invasive Surgery for Sternoclavicular Joint Infection with Osteomyelitis, Large Abscesses, and Mediastinitis.

BACKGROUND: Sternoclavicular joint infections require en bloc resection for radical cure; however, this aggressive procedure may result in multiple adverse events. Therefore, performing minimally invasive surgery is desirable. In this report, we describe a case of sternoclavicular joint infection complicated by osteomyelitis, large abscesses, and mediastinitis that was successfully treated with incision and drainage. Case Presentation. A 42-year-old man with no medical history presented to our hospital with complaints of painful swelling in the left chest wall and acute dyspnea. Computed tomog

Asymmetrical bilateral sternoclavicular joint dislocation combined with bilateral clavicular fracture: A case report.

RATIONALE: Asymmetrical bilateral sternoclavicular joint (SCJ) dislocation consists of posterior SCJ dislocation on one side and anterior SCJ dislocation on the other side. This is an extremely rare injury and only a few cases have been reported in the literature. If not been diagnosed timely and accurately, asymmetrical bilateral SCJ dislocation can be life-threatening. PATIENTS CONCERNS: We experienced a patient who has a life-threatening posterior dislocation of right SCJ and anterior dislocation on the left SCJ combined with bilateral clavicular fracture after a traffic accident. Bilateral septic arthritis of the sternoclavicular joint complicating infective endocarditis: a case report.

BACKGROUND: Septic arthritis is an infectious disease that commonly affects weight-bearing or proximal joints such as the knee and the hip. The sternoclavicular joint is an unusual site of this entity. It usually occurs in patients with diabetes mellitus, intravenous drug abusers, or those with rheumatoid arthritis. Analysis of the previous literature showed few articles and these described essentially cases of unilateral presentation. CASE PRESENTATION: We report a rare case of a bilateral septic arthritis of the sternoclavicular joint sustained by a 71-year-old Tunisian woman whose medical

Clinical outcome after reconstruction for sternoclavicular joint instability using a sternocleidomastoid tendon graft.

BACKGROUND: Anterior instability of the sternoclavicular joint is uncommon and usually follows a benign course, although symptomatic patients may require surgical intervention. The optimal treatment for symptomatic instability of the sternoclavicular joint remains unclear. The aim of this study was to evaluate the clinical outcome after reconstruction of the sternoclavicular joint with use of a sternocleidomastoid tendon graft to treat chronic debilitating anterior instability of the sternoclavicular joint. METHODS: Thirty-two patients underwent surgical reconstruction of the sternoclavicular

Posterior dislocation of sternoclavicular joint encroaching on ipsilateral vessels in 2 pediatric patients.

Dislocation of the sternoclavicular joint (DSCJ) with posterior displacement of the clavicle is uncommon in children. This can lead to ipsilateral compression of the great vessels. Diagnosis may not be apparent on routine radiographs. Axial computed tomographic (CT) scan is the imaging of choice for diagnosis. Reconstruction and 3-dimensional (3D) views on CT scan may be helpful to define the anatomy before surgical reduction. We present 2 patients who had traumatic posterior DSCJ with compromise to their vascular structures. Patient 1 was a 13-year-old boy with posterior DSCJ on his left side
