What family physicians need in their doctor's bag.

Our objective was to determine what family physicians need in their doctor's bag for urgent and non-urgent house calls. In a prospective survey, family physicians completed a structured report form after consecutive house calls, recording what equipment they needed for each. Thirteen doctors from two urban practices, making a total of 111 house calls, took part. Frequency of reported need of diagnostic equipment, therapeutic agents and administrative supplies was measured. Items required more than 70% of the time were: stethoscope; sphygmomanometer; patient's medical chart; and prescription pa

Acquisition and Classification of Lung Sounds for Improving the Efficacy of Auscultation Diagnosis of Pulmonary Diseases.

PURPOSE: Lung diseases are the third leading cause of death worldwide. Stethoscope-based auscultation is the most commonly used, non-invasive, inexpensive, and primary diagnostic approach for assessing lung conditions. However, the manual auscultation-based diagnosis procedure is prone to error, and its accuracy is dependent on the physician's experience and hearing capacity. Moreover, the stethoscope recording is vulnerable to different noises that can mask the important features of lung sounds which may lead to misdiagnosis. In this paper, a method for the acquisition of lung sound signals a

Airway auscultation. A new method of confirming tracheal intubation.

Unrecognised accidental oesophageal intubation remains an important cause of morbidity and mortality in anaesthetic practice. We have evaluated a new method of distinguishing tracheal from oesophageal intubation using a simple adaptation of an ordinary stethoscope which is inserted into the patient end of the breathing system. We call this technique 'airway auscultation'. Characteristic sounds are heard with the stethoscope during inflation and deflation which allow the observer to diagnose the position of the tube. When the tube is in the trachea loud breath sounds are heard. In contrast, whe

Method for Classifying Spectrograms of Acoustic Signals from Swallowing of Individuals with and without Dysphagia Using a Convolutional Neural Network.

Dysphagia is highly prevalent and may lead to important clinical complications. Although traditional diagnostic methods are effective, they have limitations, including high cost and patient discomfort. Cervical auscultation is a non-invasive technique for monitoring swallowing sounds and may support dysphagia assessment. To present a method for analyzing and classifying acoustic signals obtained through cervical auscultation of individuals with and without dysphagia using a machine learning approach. Cross-sectional study including swallowing assessments from individuals aged 18 years or older

[Objective tinnitus].

Objective tinnitus is a known but rare otologic symptom. This is a case report about a six-year-old boy, who was referred to a hearing clinic for further investigation of an audible clicking sound. He experienced clicking sounds, and his parents were able to hear the sounds when listening nearby. This objective tinnitus is caused by myoclonus of either the m. tensor tympani or the m. stapedius. We discuss methods to evaluate objective tinnitus including the clinical use of a stethoscope and also the possibility of using impedance measuring in patients with objective tinnitus.

Assessment of children for acute respiratory infections in hospital outpatients in Tanzania: what drives good practice?

Respiratory infections cause significant mortality in developing countries but are frequently undiagnosed. Reasons for this are unclear. We observed 1,081 outpatient consultations with patients less than five years of age in Tanzania. In 554 patients with cough or difficulty breathing, the absolute percentages examined were 5% for respiratory rate counted, 14% chest exposed, and 25% stethoscope used. Decisions to conduct particular examinations did appear to follow clinical logic, with odds ratios of 4.28 for counting respiratory rate (95% confidence interval [CI]=1.75-10.47), 2.57 for exposin

Segmentation of Radar-Recorded Heart Sound Signals Using Bidirectional LSTM Networks.

Sounds caused by the action of the heart reflect both its health as well as deficiencies and are examined by physicians since antiquity. Pathologies of the valves, e.g. insufficiencies and stenosis, cardiac effusion, arrhythmia, inflammation of the surrounding tissue and other diagnosis can be reached by experienced physicians. However, practice is needed to assess the findings correctly. Furthermore, stethoscopes do not allow for long-term monitoring of a patient. Recently, radar technology has shown the ability to perform continuous touchless and thereby burden-free heart sound measurements.

Design of an Auscultation System for Phonoangiography and Monitoring of Carotid Artery Diseases.

Cerebrovascular diseases such as stenosis of the carotid artery are accountable for about 1 million death per year across Europe. Diagnostic tools like US, angiography or MRI require specific hardware and highly depend on the experience of the examining clinician. In contrast auscultation with a stethoscope can be used to screen for bruits - audible vascular sounds associated with turbulent blood flow. Dynamical changes in the flow due to pathological narrowing of the vessel can indicate the need for additional diagnostic investigations. A reliable auscultation setup is prerequisite to ensure

Real-World Evaluation of the Eko Electronic Teleauscultation System.

Heart murmur evaluation is the most common cause of referral to cardiology, and auscultation of heart sounds with a stethoscope remains a key component of the initial cardiovascular exam. Adoption of telecardiology has been limited by challenges in teleauscultation. We set out to compare in-person auscultatory findings with heart sounds recorded by the Core stethoscope (Eko, Berkeley, CA) in patients with normal heart sounds, innocent heart murmurs, and a variety of pathologic findings. Our study demonstrates that Eko recordings had a high percent of agreement with in-person auscultation findi

A simplified form of cardiotocography for antenatal fetal assessment.

Antenatal cardiotocography has become the primary method of evaluation of fetal wellbeing, and the relationship between the presence of fetal heart rate accelerations in response to fetal movement and subsequent good fetal outcome has been demonstrated. However, in areas where electronic monitors are few or not available it would be useful if such accelerations could be demonstrated using the Pinard stethoscope. A prospective study involving 200 women with a singleton pregnancy of more than 34 weeks gestation was performed at Harare Maternity Hospital, Harare, Zimbabwe, when a 6 min electronic

Temporal changes in the presence and intensity of innocent cardiac murmurs in clinically healthy Cairn terrier and Dachshund puppies between 4 and 8 weeks of age.

BACKGROUND: Spontaneous week-to-week variation in the presence and intensity of innocent cardiac murmurs in individual puppies is unknown. METHODS: Sixty privately owned, clinically healthy Cairn terrier and Dachshund puppies between 4 and 8 weeks of age were included. All dogs underwent weekly cardiac auscultation at the breeders' home by a veterinary cardiology specialist using an acoustic stethoscope. On each occasion, a phonocardiogram was recorded with an electronic stethoscope. Furthermore, all dogs were auscultated once at a first opinion veterinary practise and once at the authors' in

Investigating into segmentation methods for diagnosis of respiratory diseases using adventitious respiratory sounds.

Respiratory condition has received a great amount of attention nowadays since respiratory diseases recently become the globally leading causes of death. Traditionally, stethoscope is applied in early diagnosis but it requires clinician with extensive training experience to provide accurate diagnosis. Accordingly, a subjective and fast diagnosing solution of respiratory diseases is highly demanded. Adventitious respiratory sounds (ARSs), such as crackle, are mainly concerned during diagnosis since they are indication of various respiratory diseases. Therefore, the characteristics of crackle are

An objective measure of signal quality for pediatric lung auscultations.

A stethoscope is a ubiquitous tool used to 'listen' to sounds from the chest in order to assess lung and heart conditions. With advances in health technologies including digital devices and new wearable sensors, access to these sounds is becoming easier and abundant; yet proper measures of signal quality do not exist. In this work, we develop an objective quality metric of lung sounds based on low-level and high-level features in order to independently assess the integrity of the signal in presence of interference from ambient sounds and other distortions. The proposed metric outlines a mappin

MYSTETH-home-based heart monitoring.

INTRODUCTION: The MySteth is an intelligent medical tool designed for cardiac disease screening, utilizing either a stethoscope or smartphone to record heart sounds. Normal heart sounds in healthy individuals consist of "lub" and "dub" noises, while murmurs-additional sounds during heartbeats-can indicate cardiac anomalies such as valve dysfunctions and rapid blood flow, categorized as systolic or diastolic. METHOD: MySteth was developed and tested using heart sounds recorded via smartphone and digital stethoscope. For ensuring the clinical validity of the data, all heart sound samples were m
